| 3 | Metformin does not improve survival in patients with hepatocellular carcinoma显示文摘AIM:To assess whether metformin,which has a chemopreventive effect in chronic liver disease,has any chemotherapeutic effect in hepatocellular carcinoma.METHODS:This was a retrospective study of 701 patients with newly diagnosed hepatocellular carcinoma(HCC)seen between January 2005 and June 2011 at Mayo Clinic,Rochester,Minnesota.This patient cohort was a part of the global HCC BRIDGE study,which is a large longitudinal study of HCC determining the realworld experience of HCC characteristics,management and patient outcomes.We defined significant metformin exposure as continuation of this agent at least 90d beyond diagnosis of HCC,and compared survival of diabetic patients on metformin to diabetic patients not on metformin and non-diabetics.RESULTS:Our cohort was 72.9%male,with a mean±SD age of 62.6±12.3 years.The most common etiologies of liver disease were hepatitis C(34%),alcoholic liver disease(29%),fatty liver disease(15%)and hepatitis B(9%).By univariate analysis,using diabetics not on metformin as the reference group,diabetic patients with HCC on metformin had no survival advantage,with a HR(95%CI)of 1.0(0.8-1.3).Non-diabetic HCC patients also did not appear to have a survival advantage as compared to diabetic HCC patients not on metformin,as demonstrated by a HR(95%CI)of1.1(0.7-1.7).Diabetics on metformin beyond 90 d after HCC diagnosis had a longer median survival at 34.2 mo,as compared to 25.5 mo among diabetic patients who were not on metformin or had discontinued metformin within 90 d after HCC diagnosis.This finding was likely due to potential survival bias among those who lived long enough to receive metformin.CONCLUSION:Although the literature suggests a chemotherapeutic effect in other malignancies,our study demonstrates no survival benefit to the use of metformin in diabetic patients with HCC. | Mamatha Bhat Roongruedee Chaiteerakij William S Harmsen Cathy D Schleck Ju Dong Yang Nasra H Giama Terry M Therneau Gregory J Gores Lewis R Roberts | 2014 | World Journal of Gastroenterology2014,20,42: | 11 |
| 4 | Lithological Discrimination of the Mafic-Ultramafic Complex, Huitongshan, Beishan, China: Using ASTER Data显示文摘The Beishan area has more than seventy mafic-ultramafic complexes sparsely distributed in the area and is of a big potential in mineral resources related to mafic-ultramafic intrusions. Many mafic-ultramafic intrusions which are mostly in small sizes have been omitted by previous works. This research takes Huitongshan as the study area, which is a major district for mafic-ultramafic occurrences in Beishan. Advanced spaceborne thermal emission and reflection radiometer(ASTER) data have been processed and interpreted for mapping the mafic-ultramafic complex. ASTER data were processed by different techniques that were selected based on image reflectance and laboratory emissivity spectra. The visible near-infrared(VNIR) and short wave infrared(SWIR) data were transformed using band ratios and minimum noise fraction(MNF), while the thermal infrared(TIR) data were processed using mafic index(MI) and principal components analysis(PCA). ASTER band ratios(6/8, 5/4, 2/1) in RGB image and MNF(1, 2, 4) in RGB image were powerful in distinguishing the subtle differences between the various rock units. PCA applied to all five bands of ASTER TIR imagery highlighted marked differences among the mafic rock units and was more effective than the MI in differentiating mafic-ultramafic rocks. Our results were consistent with information derived from local geological maps. Based on the remote sensing results and field inspection, eleven gabbroic intrusions and a pyroxenite occurrence were recognized for the first time. A new geologic map of the Huitongshan area was created by integrating the results of remote sensing, previous geological maps and field inspection. | 8 | Allelic Variation and Genetic Diversity at Glu-1 Loci in Chinese Wheat (Triticum aestivum L.) Germplasms显示文摘Wheat processing quality is greatly influenced by the seed proteins especially the high molecular weight glutenin subunit (HMW-GS) components, the low molecular weight glutenin subunit (LMW-GS) components and gliadin components. Genes encoding the HMW-GS and LMW-GS components were located on the long arms and the short arms of homoeologous group 1 chromosomes, respectively. HMW-GS components in 5 129 accessions of wheat germplasms were analyzed systematically, including 3 459 landraces and 1 670 modern varieties. These accessions were chosen as candidate core collections to represent the genetic diversity of Chinese common wheat (Triticum aestivum ) germplasms documented and conserved in the National Gene Bank. These candidate core collections covered the 10 wheat production regions in China. In the whole country, the dominating alleles at the three loci are Glu-A1b (null), Glu-B1b (7 + 8), and Glu- D1a (2 + 12), respectively. The obvious difference between the land race and the modern variety is the dramatic frequency increase of alleles Glu-A1a (1), Glu-B1c (7 + 9), Glu-B1h (14 + 15), Glu-D1d (5 + 10) and allele cording 5 + 12 subunits in the later ones. In the whole view, there is minor difference on the genetic(allelic)richness between the landrace and the modern variety at Glu-1, which is 28 and 30 respectively. However, the genetic dispersion index (Simpson index) based on allelic variation and frequencies at Glu-A1, Glu-B1 and Glu-D1 suggested that the modern varieties had much higher genetic diversity than the landraces. This revealed that various isolating mechanisms (such as auto-gamous nature, low migration because of undeveloped transposition system) limited the gene flow and exchange between populations of the landraces, which led up to some genotypes localized in very small areas. Modern breeding has strongly promoted gene exchanges and introgression between populations and previous isolated populations. In the three loci, Glu-B1 has the highest genetic diversity, then Glu-D1, while Glu- A1 always keeps the lowest genetic diversity. In the landrace, the three regions with the highest allelic richness are Huanghuai Winter Wheat Region, Northwest Spring Wheat Region and Southwestern Winter Wheat Region. For the bred varieties, the highest allelic richness existed in Southwest Winter Wheat Region, Huanghuai Winter Wheat Region, Low & Middle Branch Winter Wheat Region of Yangtze River. Introduction and utilization of foreign varieties in cross breeding has had great effects on the allelic components and frequency of the three loci, which greatly affected the genetic dispersion index. | 9 | A draft sequence of the rice (Oryza sativa ssp. indica) genome显示文摘The sequence of the rice genome holds fundamental information for its biology, including physiology, genetics, development, and evolution, as well as information on many beneficial phenotypes of economic significance. Using a 'whole genome shotgun' approach, we have pro-duced a draft rice genome sequence of Oryza sativa ssp. in-dica, the major crop rice subspecies in China and many other regions of Asia. The draft genome sequence is constructed from over 4.3 million successful sequencing traces with an accumulative total length of 2214.9 Mb. The initial assembly of the non-redundant sequences reached 409.76 Mb in length, based on 3.30 million successful sequencing traces with a total length of 1797.4 Mb from an indica variant cultivar 93-11, giving an estimated coverage of 95.29% of the rice genome with an average base accuracy of higher than 99%. | 12 | Metabolic syndrome does not affect sustained virologic response of direct-acting antivirals while hepatitis C clearance improves hemoglobin A1c显示文摘AIM To determine whether successful treatment with direc-tacting antivirals(DAA) is associated with improvements in hemoglobin A1 c(HbA1 c) and if type 2 diabetes mellitus(T2 DM) or metabolic syndrome affects sustained virologic response(SVR).METHODS We performed a retrospective analysis of all hepatitis C virus(HCV) patients at the VA Greater Los Angeles Healthcare System treated with varying DAA therapy between 2014-2016. Separate multivariable logistic regression was performed to determine predictors of HbA1 c decrease ≥ 0.5 after DAA treatment and predictors of SVR 12-wk post treatment(SVR12).RESULTS A total of 1068 patients were treated with DAA therapy between 2014-2016. The presence of T2 DM or metabolic syndrome did not adversely affect SVR12. 106 patients had both HCV and T2 DM. Within that cohort,patients who achieved SVR12 had lower mean HbA1 c pre treatment(7.35 vs 8.60,P = 0.02),and lower mean HbA1 c post-treatment compared to non-responders(6.55 vs 8.61,P = 0.01). The mean reduction in HbA1 c after treatment was greater for those who achieved SVR12 than for non-responders(0.79 vs 0.01,P = 0.03). In adjusted models,patients that achieved SVR12 were more likely to have a HbA1 c decrease of ≥ 0.5 than those that did not achieve SVR12(adjusted OR = 7.24,95%CI: 1.22-42.94). CONCLUSION In HCV patients with T2 DM,successful treatment with DAA was associated with a significant reduction in HbA1 c suggesting that DAA may have a role in improving insulin sensitivity. | 13 | Prognostic value of perioperative leukocyte count in resectable gastric cancer显示文摘AIM: To investigate the prognostic significance of perioperative leukopenia in patients with resected gastric cancer.METHODS: A total of 614 eligible gastric cancer patients who underwent curative D2 gastrectomy and adjuvant chemotherapy were enrolled in this study. The relationship between pre- and postoperative hematologic parameters and overall survival was assessed statistically, adjusted for known prognostic factors.RESULTS: The mean white blood cell count(WBC) significantly decreased after surgery, and 107/614(17.4%) patients developed p-leukopenia, which was defined as a preoperative WBC ≥ 4.0 × 109/L and postoperative WBC < 4.0 × 109/L, with an absolute decrease ≥ 0.5 × 109/L. The neutrophil count decreased significantly more than the lymphocyte count. P-leukopenia significantly correlated with poor tumor differentiation and preoperative WBC. A higher preoperative WBC and p-leukopenia were independent negative prognostic factors for survival [hazard ratio(HR) = 1.602, 95% confidence interval(CI): 1.185-2.165; P = 0.002, and HR = 1.478, 95%CI: 1.149-1.902; P = 0.002, respectively] after adjusting for histology, Borrmann type, p TNM stage, vascular or neural invasion, gastrectomy method, resection margins, chemotherapy regimens, and preoperative WBC count. The patients with both higher preoperative WBC and p- leukopenia had a worse prognosis compared to those with lower baseline WBC and no p-leukopenia(27.5 mo vs 57.3 mo, P < 0.001). described species of Eurotiomycetes are Mycocalicium hyaloparvicellulum(Mycocaliciaceae).Acarospora septentrionalis and Acarospora castaneocarpa(Acarosporaceae),Chapsa multicarpa and Fissurina carassensis(Graphidaceae),Sticta fuscotomentosa and Sticta subfilicinella(Lobariaceae)are newly introduced in class Lecanoromycetes.In class Pezizomycetes,Helvella pseudolacunosa and Helvella rugosa(Helvellaceae)are introduced as new species.The new families,Dendrominiaceae and Neoantrodiellaceae(Basidiomycota)are introduced together with a new genus Neoantrodiella(Neoantrodiellaceae),here based on both morphology coupled with molecular data.In the class Agaricomycetes,Agaricus pseudolangei,Agaricus haematinus,Agaricus atrodiscus and Agaricus exilissimus(Agaricaceae),Amanita melleialba,Amanita pseudosychnopyramis and Amanita subparvipantherina(Amanitaceae),Entoloma calabrum,Cora barbulata,Dictyonema gomezianum and Inocybe granulosa(Inocybaceae),Xerocomellus sarnarii(Boletaceae),Cantharellus eucalyptorum,Cantharellus nigrescens,Cantharellus tricolor and Cantharellus variabilicolor(Cantharellaceae),Cortinarius alboamarescens,Cortinarius brunneoalbus,Cortinarius ochroamarus,Cortinarius putorius and Cortinarius seidlii(Cortinariaceae),Hymenochaete micropora and Hymenochaete subporioides(Hymenochaetaceae),Xylodon ramicida(Schizoporaceae),Colospora andalasii(Polyporaceae),Russula guangxiensis and Russula hakkae(Russulaceae),Tremella dirinariae,Tremella graphidis and Tremella pyrenulae(Tremellaceae)are introduced.Four new combinations Neoantrodiella gypsea,Neoantrodiella thujae(Neoantrodiellaceae),Punctulariopsis cremeoalbida,Punctulariopsis efibulata(Punctulariaceae)are also introduced here for the introduced in the Zygomycota,while Neocallimastix cameroonii and Piromyces irregularis(Neocallimastigaceae)are introduced in the Neocallimastigomycota.Reference specimens or changes in classification | 18 | Tooth loss and alveolar remodeling in Sinosaurus triassicus (Dinosauria: Theropoda) from the Lower Jurassic strata of the Lufeng Basin, China显示文摘Pathological or traumatic loss of teeth often results in the resorption and remodeling of the affected alveoli in mammals. However, instances of alveolar remodeling in reptiles are rare. A remodeled alveolus in the maxilla of the Chinese theropod Sinosaurus (Lower Jurassic Lower Lufeng Formation) is the first confirmed example of such dental pathology in a dinosaur. Given the known relationship between feeding behavior and tooth damage in theropods (teeth with spalled enamel, tooth crowns embedded in bone) and the absence of dentary, maxillary, and premaxillary osteomyelitis, traumatic loss of a tooth is most likely the cause of alveolar remodeling.
