| 1 | Multimodality treatment in hepatocellular carcinoma patients with tumor thrombi in portal vein显示文摘AIM To compare the therapeutic effect andsignificances of multimodality treatment forhepatocellular carcinoma (HCC) with tumorthrombi in portal vein (PVTT).METHODS HCC patients (n = 147) with tumortrombi in the main portal vein or the first branchof portal vein were divided into four groups bythe several therapeutic methods. There wereconservative treatment group in 18 out ofpatients (group A); and hepatic artery ligation(HAL) and/or hepatic artery infusion (HAl)group in 18 patients (group B), in whompostoberative chemoembolization was doneperiodically; group of removal of HCC with PVTTin 79 (group C) and group of transcatheterhepatic arterial chemoembolization (TACE) orHAl and/or portal vein infusion (PVI) afteroperation in 32 (group D).RESULTS The median survival period was 12months in our series and the 1-, 3-, and 5-yearsurvival rates were 44.3%, 24.5% and 15.2%,respectively. The median survival times were 2,5, 12 and 16 months in group A, B, C and D,respectively. The 1-, 3- and 5-year survival rateswere 5.6%, 0% and 0% in group A; 22.2%,5.6% and 0% in group B; 53.9%, 26.9% and16.6% in group C; 79.3%, 38.9% and 26.8% ingroup D, respectively. | 2 | APC and K-ras gene mutation in aberrant crypt foci of human colon显示文摘AIM To study the genetic alteration in ACF andto define the possibility that ACF may be a veryearly morphological lesion with molecularchanges, and to explore the relationshipbetween ACF and colorectal adenoma evencarcinoma.METHODS DNA from 35 CRC, 15 adenomas, 34ACF and 10 normal mucus was isolated by meansof microdissection. Direct gene sequencing of K-ras gene including codon 12, 13 and 61 as well asthe mutation cluster region (MCR) of APC genewas performed.RESULTS K-ras gene mutation frequency inACF, adenoma and carcinoma was 17.6% (6/34), 13.3% (2/ 15), and 14.3% (5/ 35)respectively, showing no difference ( P > 0.05)in K-fas gene mutation among three pathologicprocedures. The K-ras gene mutation inadenoma, carcinoma and 4 ACF restricted incodon 12 (GGT→GAT), but the other 2 mutationsfrom ACF located in codon 13 (GGC→GAC). K-res gene mutation was found more frequently inolder patients and patients with polypoidcancer. No mutation in codon 61 was found in thethree tissue types. Mutation rate of APO gene inadenoma and carcinoma was 22.9% (8/35) and26.7% (4/ 15), which was higher than ACF(2.9%) (P < 0.05). APC gene mutation incarcinoma was not correlated with age ofpatients, location, size and differentiation oftumor.CONCLUSION ACF might be a very earlymorphological lesion in the tumorogenesis ofcolorectal tumor. The morphological feature andgene mutation status was different in ACF andadenoma. ACF is possibly putative'microadenoma' that might be the precursor ofadenoma. | 8 | Population pharmacokinetics of the anti-PD-1 antibody camrelizumab in patients with multiple tumor types and model-informed dosing strategy显示文摘Camrelizumab,a programmed cell death 1(PD-1)inhibitor,has been approved for the treatment of patients with relapsed or refractory classical Hodgkin lymphoma,nasopharyngeal cancer and non-small cell lung cancer.The aim of this study was to perform a population pharmacokinetic(PK)analysis of camrelizumab to quantify the impact of patient characteristics and to investigate the appropriateness of a flat dose in the dosing regimen.A total of 3092 camrelizumab concentrations from 133 patients in four clinical trials with advanced melanoma,relapsed or refractory classical Hodgkin lymphoma and other solid tumor types were analyzed using nonlinear mixed effects modeling.The PKs of camrelizumab were properly described using a two-compartment model with parallel linear and nonlinear clearance.Then,covariate model building was conducted using stepwise forward addition and backward elimination.The results showed that baseline albumin had significant effects on linear clearance,while actual body weight affected intercompartmental clearance.However,their impacts were limited,and no dose adjustments were required.The final model was further evaluated by goodness-of-fit plots,bootstrap procedures,and visual predictive checks and showed satisfactory model performance.Moreover,dosing regimens of 200 mg every 2 weeks and 3 mg/kg every 2 weeks provided similar exposure distributions by model-based Monte Carlo simulation.The population analyses demonstrated that patient characteristics have no clinically meaningful impact on the PKs of camrelizumab and present evidence for no advantage of either the flat dose or weight-based dose regimen for most patients with advanced solid tumors. | Chen-yu Wang Chang-cheng Sheng Guang-li Ma Da Xu Xiao-qin Liu Yu-ya Wang Li Zhang Chuan-liang Cui Bing-he Xu Yu-qin Song Jun Zhu Zheng Jiao | 2021 | Acta Pharmacologica Sinica2021,42,8: | 3 |
| 9 | Efficiency and safety evaluation of Baimai ointment for alleviating hypermyotonia following cerebral infarction显示文摘BACKGROUND: Baimai ointment has Shujin and Huoluo efficiency. It has been confirmed by basic pharmacodynamic experiments that Baimai ointment has some therapeutic effects on stroke. However,the detailed evaluations of Baimai ointment on clinically treating stroke patients for function recovery are still insufficient with little reports. OBJECTIVE: To observe the efficiency and safety of Baimai ointment on alleviating hypermyotonia following cerebral infarction DESIGN: A randomized,parallel and controlled experiment. SETTING: Department of Neurology,Beijing Aerospace General Hospital. PARTICIPANTS: A total of 120 patients with acute front-cycle cerebral infarction were selected from Department of Neurology,Beijing Aerospace General Hospital from July to December 2006 and met the diagnosis criteria modified by Forth National Seminar on Cerebrovascular Disease. The experiment had got confirmed consent from local ethic committee. All 120 patients with cerebral infarction were randomly divided into experimental group (n =80; 32 in the acute phase,32 in the recovery phase and 16 in the sequela phase) and control group (n =40; 16 in the acute phase,16 in the recovery phase and 8 in the sequela phase). METHODS: Within 4 weeks after the patients were enrolled in this study,no other traditional Chinese medicines containing similar components were adopted besides routine supporting therapy. The Baimai ointment being consisted of Jianghuang,Roudoukou,Gansong,Yangqishi,Gancao,She,shannai,Zanghuixiang,Zangchangpu,Huajiao and Jianhua was used by trial group (20 g/ampoule,Tibet Lingzhiqizheng Tibetan Drug Factory; batch number: 050403),2–3 times per day,the ointment was moderately coated traveling parts of Baimai. MAIN OUTCOME MEASURES: The evaluation of National Institutes of Health Stroke Scale (NIHSS),Modified Rankin Scale and Modified Ashworth Scale were performed before administration,2 and 4 weeks after administration. RESULTS: All 120 patients with acute front-cycle cerebral infarction were involved in the final analysis. ① Evaluation of therapeutic effects: After 4 weeks,the comparison of overall muscular tension and muscular tension in acute phase between the two groups (acute phase,chronic phase and recovery phase) demonstrated that the experimental group improved significantly (Z =–2.25,–2.06,P < 0.05). After 2 weeks,the change of muscular tension was not significantly different from baseline (P > 0.05). Whereas after 4 weeks,the change of muscular tension in acute phase was significantly different from baseline (P < 0.05); however,the change of muscular tension in recovery phase and sequelae phase was not significantly different from baseline (P > 0.05). After the treatment,the overall NIHSS and disability score of both groups did not change significantly (P > 0.05). ② Adverse reaction and side effect: There were two adverse events in experimental groups. One patient underwent petechia,and the symptom disappeared following stopping medication which did not reappear after remedication. The other patient complained of a pain coming from the place where the medication was used,and the symptom went away without treatment. Two cases of adverse events were not significantly correlated with the usage of medication,so both of which could not be confirmed to be adverse reactions. | 12 | Environmental and tectonic significance of Late Permian reefs in the Linxi and adjacent areas in Inner Mongolia of China显示文摘Many plateau-edge reefs and reef mounds of the Late Permian have been discovered in Linxi County and adjacent areas of Inner Mongolia, China. These reefs verify that the Hinggan-Inner Mongolia Area was an open and unobstructed sea-trough in Late Permian. Facies and sequences of reef strata in Member 4 and Member 5 of the Linxi Formation were studied for information to reconstruct the closing process of the Linxi-Jiutai chasm in the sea-trough, i.e., to constrain subsidence, uplift, and final closing. Ages of the reef-bearing strata have been determined to be late and end of Late Permian, based on ranges or abundances of diverse fossils. Six reef-building organic assemblages were distinguished, representing different ecologic conditions, according to morphological functions and paleoecological adaptations of organisms including primarily sponges, bryozoans, and calcareous algae. Two types of shelf slopes, the gentle slope and the steep slope, have been interpreted for Member 4 and Member 5 of the Linxi Formation, based on the assemblages. | 13 | Impact of pre-oxidation on the formation of byproducts in algae-laden water disinfection:Insights from fluorescent and molecular weight显示文摘Pre-oxidation has been reported to be an effective way to remove algal cells in water, but the released algal organic matter (AOM) could be oxidized and lead to the increment in disinfection by-product (DBP) formation. The relationship between pre-oxidation and AOM-derived DBP formation needs to be approached more precisely. This study compared the impact of four pre-oxidants, ozone (O), chlorine dioxide (ClO), potassium permanganate(KMnO) and sodium hypochlorite (NaClO), on the formation of nitrogenous (N-) and carbonaceous (C-) DBPs in AOM chlorination. The characterization (fluorescent properties,molecular weight distribution and amino acids concentration) on AOM samples showed that the characterization properties variations after pre-oxidation were highly dependent on the oxidizing ability of oxidants. The disinfection experiments showed that Oincreased DBP formation most significantly, which was consistent with the result of characterization properties variations. Then canonical correspondent analysis (CCA) and Pearson’s correlation analysis were conducted based on the characterization data and DBP formation. CCA indicated that C-DBPs formation was highly dependent on fluorescent data. The formation of haloacetic acids (HAAs) had a positive correlation with aromatic protein-like component while trichloromethane (TCM) had a positive correlation with fulvic acid-like component.Pearson’s correlation analysis showed that low molecular weight fractions were favorable to form N-DBPs. A four-variable regression model (M30) with the correlation coefficient of 0.9816 and the root mean square errors of 0.061 was developed using a training set including 30 PBDEs. The correlation coefficient of 0.9841 and the root mean square errors of 0.054 between the values of RRT predicted by M30 and the RRT observed for 16 external PBDEs show a good predictive potential of M30. | 15 | Intranasal Immunization Using CTA1-DD as a Mucosal Adjuvant for an Inactivated Influenza Vaccine显示文摘Objective To evaluate the effect of intranasal immunization with CTA1-DD as mucosal adjuvant combined with H3N2 split vaccine. Methods Mice were immunized intranasally with PBS(negative control), or H3N2 split vaccine(3 μg/mouse) alone, or CTA1-DD(5 μg/mouse) alone, or H3N2 split vaccine(3 μg/mouse) plus CTA1-DD(5 μg/mouse). Positive control mice were immunized intramuscularly with H3N2 split vaccine(3 μg/mouse) and alum adjuvant. All the mice were immunized twice, two weeks apart. Then sera and mucosal lavages were collected. The specific HI titers, IgM, IgG, IgA, and IgG subtypes were examined by ELISA. IFN-γ and IL-4 were test by ELISpot. In addition, two weeks after the last immunization, surivival after H3N2 virus lethal challenge was measured. Results H3N2 split vaccine formulated with CTA1-DD could elicit higher Ig M, Ig G and hemagglutination inhibition titers in sera. Furthermore, using CTA1-DD as adjuvant significantly improved mucosal secretory Ig A titers in bronchoalveolar lavages and vaginal lavages. Meanwhile this mucosal adjuvant could enhance Th-1-type responses and induce protective hemagglutination inhibition titers. Notably, the addition of CTA1-DD to split vaccine provided 100% protection against lethal infection by the H3N2 virus.
