| 1 | Chronic Liver Failure-Sequential Organ Failure Assessment is better than the Asia-Pacific Association for the Study of Liver criteria for defining acute-on-chronic liver failure and predicting outcome显示文摘AIM:To compare the utility of the Chronic Liver FailureSequential Organ Failure Assessment(CLIF-SOFA)and Asia-Pacific Association for the Study of Liver(APASL)definitions of acute-on-chronic liver failure(ACLF)in predicting short-term prognosis of patients with ACLF.METHODS:Consecutive patients of cirrhosis with acute decompensation were prospectively included.They were grouped into ACLF and no ACLF groups as per CLIF-SOFA and APASL criteria.Patients were followed up for 3 mo from inclusion or mortality whichever was earlier.Mortality at 28-d and 90-d was compared between no ACLF and ACLF groups as per both criteria.Mortality was also compared between different grades of ACLF as per CLIF-SOFA criteria.Prognostic scores like CLIF-SOFA,Acute Physiology and ChronicHealth Evaluation(APACHE)-Ⅱ,Child-Pugh and Model for End-Stage Liver Disease(MELD)scores were evaluated for their ability to predict 28-d mortality using area under receiver operating curves(AUROC).RESULTS:Of 50 patients,38 had ACLF as per CLIFSOFA and 19 as per APASL criteria.Males(86%)were predominant,alcoholic liver disease(68%)was the most common etiology of cirrhosis,sepsis(66%)was the most common cause of acute decompensation while infection(66%)was the most common precipitant of acute decompensation.The 28-d mortality in no ACLF and ACLF groups was 8.3%and 47.4%(P=0.018)as per CLIF-SOFA and 39%and 37%(P=0.895)as per APASL criteria.The 28-d mortality in patients with no ACLF(n=12),ACLF grade 1(n=11),ACLF grade 2(n=14)and ACLF grade 3(n=13)as per CLIF-SOFA criteria was 8.3%,18.2%,42.9%and76.9%(χ2 for trend,P=0.002)and 90-d mortality was 16.7%,27.3%,78.6%and 100%(χ2 for trend,P<0.0001)respectively.Patients with prior decompensation had similar 28-d and 90-d mortality(39.3%and 53.6%)as patients without prior decompensation(36.4%and 63.6%)(P=NS).AUROCs for 28-d mortality were 0.795,0.787,0.739 and 0.710 for CLIF-SOFA,APACHE-Ⅱ,Child-Pugh and MELD scores respectively.On multivariate analysis of these scores,CLIF-SOFA was the only significant independent predictor of mortality with an odds ratio 1.538(95%CI:1.078-2.194).CONCLUSION:CLIF-SOFA criteria is better than APASL criteria to classify patients into ACLF based on their prognosis.CLIF-SOFA score is the best predictor of short-term mortality. | Radha K Dhiman Swastik Agrawal Tarana Gupta Ajay Duseja Yogesh Chawla | 2014 | World Journal of Gastroenterology2014,20,40: | 26 |

| 4 | Clinical Validation of Global Coagulation Tests to Guide Blood Component Transfusions in Cirrhosis and ACLF显示文摘Background and Aims:Patients with cirrhosis and acuteon-chronic liver failure(ACLF)may have bleeding complications and need for invasive procedures.Point-of-care(POC)coagulation tests like thromboelastography(TEG)and Sonoclot may be better for guiding patient management than the standard coagulation tests(SCTs),like prothrombin time,platelet count and international normalized ratio.Methods:We prospectively compared and validated the POC tests and SCTs in 70 persons with ACLF and 72 persons with decompensated cirrhosis who had clinical bleeding and checked for episodes of re-bleeding and transfusion requirements.We assessed pre-procedure requirement of blood components when correction was done based on an SCT or POC strategy.Results:Episodes of bleeding were seen in 45%and 28%of ACLF and cirrhosis patient,respectively(p=0.036),with the major site of bleeding being gastrointestinal(31%and 16%,respectively).Platelet counts correlated with TEG-maximum amplitude in cirrhosis(p=0.045)and prothrombin time correlated positively with TEG-reaction(R)time(p=0.032),TEG-Clot kinetics(K)time(p=0.042),Son-activated clotting time(p=0.038)and negatively with clot rate(p=0.043)in ACLF,making these correctable target variables in POC transfusion algorithms.Of 223 procedures,transfusion of fresh frozen plasma and platelet concentrate was reduced by 25%(p=0.035)and 20.8%(p=0.045)by using a POC strategy in 76 patients.Correction of deranged Son-activated clotting time and TEG-reaction time was noted in 68%and 72%after 24 h of fresh frozen plasma transfusion in ACLF and 85%and 80%in cirrhosis,respectively.Conclusions:Our study clinically validates that POC tests can better detect coagulation defects and transfusion thresholds in ACLF and cirrhosis,whereas use of conventional tests appear to be less suitable in patients with clinical bleeding.Trial Registration:NCT04332484. | Madhumita Premkumar Rohit Mehtani Smita Divyaveer Kamal Kajal Anand VKulkarni Syed Ahmed Harmanpreet Kaur Harpreet Kaur Radhakrishna Dhiman Ajay Duseja Arka De | 2021 | Journal of Clinical and Translational Hepatology2021,9,2: | 7 |
| 5 | Genetic polymorphism in CD14 gene, a co-receptor of TLR4 associated with non-alcoholic fatty liver disease显示文摘AIM To evaluate the pathogenic role of toll-like receptor(TLR) gene polymorphisms in patients with nonalcoholic fatty liver disease(NAFLD).METHODS Two hundred and fifty subjects(NAFLD = 200, healthy volunteers = 50) underwent polymerase chain reaction and restriction fragment length polymorphism to assess one polymorphism in the toll-like receptor 2(TLR2) gene(A753G), two polymorphisms in the TLR4 gene(TLR4 Asp299 Gly and Thr399 Ile allele), and two polymorphisms in the cluster of differentiation 14(CD14)(C-159 T and C-550T) gene, a co-receptor of TLR4. Association of TLR gene polymorphisms with NAFLD and its severity was evaluated by genetic models of association.RESULTS On both multiplicative and recessive models of gene polymorphism association, there was significant association of CD14 C(-159) T polymorphism with NAFLD; patients with TT genotype had a 2.6 fold increased risk of developing NAFLD in comparison to CC genotype. There was no association of TLR2 Arg753 Gln, TLR4 Asp299 Gly, Thr399 Ile, and CD14 C(-550) T polymorphisms with NAFLD. | 6 | Frequency of primary iron overload and HFE gene mutations (C282Y,H63D and S65C) in chronic liver disease patients in north India显示文摘AIM:To identify the frequency of iron overload and study the three mutations in the HFE gene (C282Y,H63D,and S65C) in patients with chronic liver disorders (CLD) and controls. METHODS:To identify patients with iron overload (transferrin saturation > 45% in females and > 50% in males and serum ferritin > 1000 ng/mL) we evaluated 236 patients with CLD,including 59 with non-alcoholic steatohepatitis (NASH),22 with alcoholic liver disease (ALD),19 of cirrhosis due to viruses (HBV,HCV),and 136 with cryptogenic cirrhosis. Mutations of the HFE gene were analyzed by PCR-RE. hundred controls were screened for iron status and the mutations. RESULTS:Seventeen patients with CLD showed evidence of iron overload. Fifteen cases of iron overload had cryptogenic cirrhosis and two had ALD. None of the controls showed iron overload. We did not find any individual with 282Y or 65C either in the cases or in the controls. The prevalence of H63D heterozygosity was 12% in normal individuals,14.8% in 236 patients (16.9% in NASH,13.6% in ALD,26.3% in viral and 12.5% in cryptogenic cirrhosis) and the overall prevalence was 13.98%. Only two of the 17 patients with primary iron overload were heterozygous for H63D. | 7 | Are malaria elimination efforts on right track?An analysis of gains achieved and challenges ahead显示文摘Background:Malaria causes significant morbidity and mortality each year.In the past few years,the global malaria cases have been declining and many endem ic countries are heading towards malaria elimination.Nevertheless,reducing the number of cases seems to be easy than sustained elimination.Therefore to achieve the objective of com plete elimination and maintaining the elimination status,it is necessary to assess the gains made during the recent years.Main text:With inclining global support and World Health Organisation(WHO)efforts,the control programmes have been im plem ented effectively in many endemic countries.Given the aroused interest and investments into malaria elimination program m es at global level,the am bitious goal o f elimination appears feasible.Sustainable interventions have played a pivotal role in malaria contraction,however drug and insecticide resistance,social,dem ographic,cultural and behavioural beliefs and practices,and unreformed health infrastructure could drift back the progress attained so far.Ignoring such im peding factors coupled with certain region specific factors may jeopardise our ability to abide righteous track to achieve global elimination of malaria parasite.Although support beyond the territories is important,but well managed integrated vector managem ent approach at regional and country level using scrupulously selected area specific interventions targeting both vector and parasite along with the com m unity involvem ent is necessary.A brief incline in malaria during 2016 has raised fresh perturbation on whether elimination could be achieved on time or not.Conclusions:The intervention tools available currently can most likely reduce transmission but clearing of malaria epicentres from where the disease can flare up any time,is not possible without involving local population.Nevertheless maintaining zero malaria transmission and checks on malaria import in declared malaria free countries,and further speeding up of interventions to stop transmission in elimination countries is most desirable.Strong collaboration backed by adequate political and financial support am ong the countries with a common objective to eliminate malaria must be on top priority.The present review attempts to assess the progress gained in malaria elimination during the past few years and highlights some issues that could be important in successful malaria elimination. | Sunil Dhiman | 2019 | Infectious Diseases of Poverty2019,8,1: | 4 |

| 9 | Socio-economic determinants for malaria transmission risk in an endemic primary health centre in Assam, India显示文摘Background:Malaria is a major cause of morbidity and mortality in Northeast India.As there is limited information available on the potential influence of socio-economic variables on malaria risk,the present study was conducted to assess the influence of demographic factors,the socio-economic status,and knowledge,awareness and education on malaria occurrence.Methods:Demographics,malaria knowledge and socio-economic variables were collected in four randomly selected health sub-centres of the Orang primary health centre in the Udalguri district,Assam and the association of malaria occurrence with different variables were analysed.The trend of malaria occurrence for different income groups,proximity to health centres and number of mosquito bites per day was also determined using the chi-square test.Relative risk(RR)for gender,house type,knowledge and use of bed nets was determined using Katz approximation.Results:Out of the 71 household heads interviewed,70.4%(50/71)were males.About half(54.9%,39/71)of the participants had a history of malaria in the last two years,of which 64.1%(25/39)were males,while 35.9%(14/39)were females(χ^(2)=5.13;p=0.02;RR=1.79).Of the total population surveyed,49.3%lived in bamboo houses and 35.2%lived at a distance of>3 km from the nearest health centre.The number of participants who had a history of malaria decreased with an increasing monthly income(p<0.0001).Malaria occurrence was higher among the households living in bamboo houses(69.2%),as compared to Kucha houses(20.5%)and Pucca houses(10.3%).No significant association was observed between education level and malaria occurrence(p=0.93).The participants who did not use bed nets regularly reported a high occurrence of malaria infection as compared to those who used bed nets everyday(p<0.0001).Conclusions:Lower income,house type,distance to health sub-centre,knowledge and awareness about malaria,number of mosquito bites per day and use of bed nets were positively associated with malaria occurrence.Increasing the number of health sub-centres close to rural areas,improving the economic status and increasing awareness about malaria prevention measures will thus help to reduce malaria-associated morbidities. | Kavita Yadav Sunil Dhiman Bipul Rabha PK Saikia Vijay Veer | 2014 | Infectious Diseases of Poverty2014,3,1: | 3 |

| 11 | Evaluation of functional outcome and stability of sutureless scleral tunnel fixated IOLs in children with ectopia lentis显示文摘AIM:To evaluate functional outcome of sutureless scleral tunnel intraocular lens(SSTIOL)in children with crystalline lens subluxation of more than 7 clock hours.METHODS:A prospective interventional study was conducted consisting of 45 eyes of 44 children in age group 6-18 y having>7 clock hours of lens subluxation who underwent lensectomy-vitrectomy followed by SSTIOL implantation.Primary outcome was improvement in best corrected visual acuity(BCVA)and secondary outcomes were assessment of intraocular lens(IOL)tilt using ultrasound biomicroscopy(UBM),mean change in astigmatism at last follow-up of 1 y and associated complications.RESULTS:The mean preoperative and postoperative BCVA was 1.05±0.28 and 0.64±0.45(log MAR)respectively(P=0.001)at last follow-up.The mean astigmatism preoperatively and postoperatively was-4.17±2.69 D and-1.86±1.25 D respectively(P=0.011).Significant IOL tilt(>5 degrees)was present in 5 cases.The mean percentage endothelial loss was 3.65%±1.92%.The most serious complication encountered was retinal detachment seen in 2 cases.CONCLUSION:SSTIOL implantation provides efficient visual rehabilitation in children provided there is stringent case selection.We recommend caution in children having white-to-white distance>12 mm and presence of peripheral retinal degenerations. | Anju Rastogi Prateek Kumar Shweta Dhiman Manisha Mishra Kamlesh AnAND Ankita Bhardwaj | 2020 | International Journal of Ophthalmology(English edition)2020,13,1: | 2 |

| 15 | Gallbladder cancer with tumor thrombus in the superior vena cava显示文摘BACKGROUND:Gastrointestinal cancers,especially pancreatobiliary cancers,are frequently associated with or are complicated by thromboembolic phenomena due to hypercoagulability and/or altered venous drainage,especially of the abdomen and lower limbs.This report describes an unusual and interesting case of gallbladder carcinoma developing a viable tumor thrombus in the superior vena cava(SVC)with resultant SVC obstruction,while on gefitinibbased anti-epidermal growth factor receptor(EGFR)therapy.METHODS:A 60-year-old woman was incidentally diagnosed to have gallbladder cancer on cholecystectomy.She had disease recurrence and received systemic chemotherapy followed by gefitinib-based anti-EGFR therapy.Subsequently,while on gefitinib-based therapy,she presented with clinical signs and symptoms suggestive of SVC thrombosis.RESULTS:A whole body PET scan revealed a metabolically active tumor thrombus in the SVC,besides other sites of metabolically active disease inclusive of the lung parenchyma, lymph nodes and abdomen.She was treated with antithrombotics and external beam radiotherapy directed to the SVC thrombus leading to symptomatic relief.She continues to survive on the day of writing this report.CONCLUSIONS:This rare complication,though theoretically possible,is unreported because of the short overall survival of advanced gallbladder cancer patients.This highlights that with the availability of better chemotherapeutic/biotherapeutic agents for increasing in the lifespan of cancer patients,we may come across such cases more frequently in the future. | Sandeep Batra Dinesh Chandra Doval Ullas Batra Pandalanghat Suresh Amit Dhiman Vineet Talwar | 2010 | Hepatobiliary & Pancreatic Diseases International2010,9,3: | 2 |
