1Chronic hepatitis C virus infection:Prevalence of extrahepatic manifestations and association with cryoglobulinemia in Bulgarian patients显示文摘AIM: To assess the prevalence of extrahepatic manifestations in Bulgarian patients with chronic hepatitis C virus (HCV) infection and identify the clinical and biological manifestations associated with cryoglobulinemia. METHODS: The medical records of 136 chronically infected HCV patients were reviewed to assess the prevalence of extrahepatic manifestations. Association between cryoglobulin-positivity and other manifestations were identified using χ2 and Fisher’s exact test. Risk factors for the presence of extrahepatic manifestations were assessed by logistic regression analysis. RESULTS: Seventy six percent (104/136) of the patients had at least one extrahepatic manifestation. Clinical manifestations included fatigue (59.6%), kidney impairment (25.0%), type 2 diabetes (22.8%), paresthesia (19.9%), arthralgia (18.4%), palpable purpura (17.6%), lymphadenopathy (16.2%), pulmonary fi brosis (15.4%), thyroid dysfunction (14.7%), Raynaud’s phenomenon (11.8%), B-cell lymphoma (8.8%), sicca syndrome (6.6%), and lichen planus (5.9%). The biological manifestations included cryoglobulinproduction (37.5%), thrombocytopenia (31.6%), and autoantibodies: anti-nuclear (18.4%), anti-smooth muscle (16.9%), anti-neutrophil cytoplasm (13.2%) and anti-cardiolipin (8.8%). All extrahepatic manifestations showed an association with cryoglobulin-positivity, with the exception of thyroid dysfunction, sicca syndrome, and lichen planus. Risks factors for the presence of extrahepatic manifestations (univariate analysis) were: age ≥ 60 years, female gender, virus transmission by blood transfusions, longstanding infection (≥ 20 years), and extensive liver fi brosis. The most signifi cant risks factors (multivariate analysis) were longstanding infection and extensive liver fi brosis. CONCLUSION: We observed a high prevalence of extrahepatic manifestations in patients with chronic HCV infection. Most of these manifestations were associated with impaired lymphoproliferation and cryoglobulin production. 3Empagliflozin alleviates podocytopathy and enhances glomerular nephrin expression in db/db diabetic mice显示文摘BACKGROUND Modern guidelines recommend sodium-glucose cotransporter-2(SGLT2)inhibitors as the preferred antihyperglycemic agents for patients with type 2 diabetes and chronic kidney disease.However,the mechanisms underlying the renal protective effect of SGLT2 inhibitors are not fully understood.structure of podocytes and nephrin expression in glomeruli in db/db diabetic mice.METHODS We treated 8-wk-old male db/db mice with EMPA(10 mg/kg/d)or vehicle for 8 wk.Age-matched male db/+mice were included as non-diabetic controls.Parameters of body composition,glycemic and lipid control,and plasma concentrations of leptin,insulin and glucagon were assessed.We evaluated renal hypertrophy as kidney weight adjusted to lean mass,renal function as plasma levels of creatinine,and albuminuria as the urinary albumin-to-creatinine ratio(UACR).Renal structures were studied by light and transmission electron microscopy with a focus on mesangial volume and podocyte structure,respectively.Glomerular nephrin and transforming growth factor beta(TGF-β)were assessed by immunohistochemistry.RESULTS Severe obesity and hyperglycemia developed in db/db mice prior to the start of the experiment;increased plasma concentrations of fructosamine,glycated albumin,cholesterol,leptin,and insulin,and elevated UACR were detected.Mesangial expansion,glomerular basement membrane thickening,and increased area of TGF-βstaining in glomeruli were revealed in vehicle-treated mice.Podocytopathy was manifested by effacement of foot processes;nephrin-positive areas in glomeruli were reduced.EMPA decreased the levels of glucose,fructosamine and glycated albumin,UACR,kidney hypertrophy,mesangial expansion,glomerular basement membrane thickening,and glomerular TGF-βstaining,alleviated podocytopathy and restored glomerular staining of nephrin.CONCLUSION These data indicate that EMPA attenuates podocytopathy in experimental diabetic kidney disease.The anti-albuminuric effect of EMPA could be attributed to mitigation of podocyte injury and enhancement of nephrin expression.Vadim V Klimontov Anton I Korbut Iuliia S Taskaeva Nataliya P Bgatova Maksim V Dashkin Nikolai BOrlov Anna S Khotskina Evgenii L Zavyalov Thomas Klein 2020World Journal of Diabetes2020,11,12:2

11Influence of dust particles on spatial distributions of particles and fluxes in positive column of glow discharge显示文摘The research herein examined the results of numerical simulations of the positive column of a glow discharge in argon dusty plasma using COMSOL Multiphysics software under conditions similar to the project known as PK-4.Various scenarios dealing with formations of spatial distributions of densities and fluxes for charged particles were studied,and evaluations of the influence of dust particles on the discharge were obtained in a wide range of dust densities.Two extreme cases were distinguished:weak dust influence when the densities,fluxes and electric field profiles are not perturbed,and strong dust influence when all three density profiles(electrons,ions and charged dust)in the dust cloud are similar(parallel)to each other,resulting in all created charges in the dust cloud being lost inside the cloud.In such a case,the ambipolar field and the transport of charged particles are decreased in the dust cloud,and any ambipolar flux is almost absent within the cloud.田瑞焕 袁承勋 Dmitrii V BOGDANOV Evgeniy A BOGDANOV Anatoly A KUDRYAVTSEV 周忠祥 2019Plasma Science and Technology2019,21,11:0
12Liver transplantation in a patient with primary antiphospholipid syndrome and Budd-Chiari syndrome显示文摘The antiphospholipid syndrome(APS) is an acquired thrombophilic disorder in which autoantibodies are produced to a variety of phospholipids determinants of cell membranes or phospholipid binding proteins. There are few reports about association between antiphospholipid antibodies and development of BuddChiari syndrome(BCS). We report the case of BCS development in young Russian male with primary APS. The patient underwent orthotopic liver transplantation on August 26, 2012. At present time his state is good, the blood flow in the liver restored and its function is not impaired. We report about the first time the successful use of dabigatran etexilate for prolonged anticoagulation therapy in APS patient with BCS. 13Improving nutrition for the prevention of gestational diabetes:Current status and perspectives显示文摘Gestational diabetes mellitus(GDM)is a common complication of pregnancy and a serious public health problem.It carries significant risks of short-term and longterm adverse health effects for both mothers and their children.Risk factors,especially modifiable risk factors,must be considered to prevent GDM and its consequences.Observational studies have identified several nutritional and lifestyle factors associated with the risk of GDM.The results of intervention studies examining the effects of diet and lifestyle on the prevention of GDM are contradictory.Differences in the study populations,types and intensity of intervention,time frame of the intervention,and diagnostic criteria for GDM may explain the heterogeneity in the results of intervention studies.This review provides an overview of new diets and other factors that may help prevent GDM.The main results of epidemiological studies assessing the risk factors for GDM,as well as the results and methodological problems of intervention studies on the prevention of GDM and their meta-analyses,are discussed.In addition,the evidence that gene and lifestyle interactions influence the development of GDM,as well as prospects for increasing the effectiveness of interventions designed to prevent GDM,including new data on the possible uses of personalized diet therapy,are highlighted.Polina V Popova Evgenii A Pustozerov Aleksandra S Tkachuk Elena N Grineva 2021World Journal of Diabetes2021,12,9:0
