| 1 | Gut microbiota role in irritable bowel syndrome:Newtherapeutic strategies显示文摘In the last decade the impressive expansion of our knowledge of the vast microbial community that resides in the human intestine, the gut microbiota, has provided support to the concept that a disturbed intestinal ecology might promote development and maintenance of symptoms in irritable bowel syndrome(IBS). As a correlate, manipulation of gut microbiota represents a new strategy for the treatment of this multifactorial disease. A number of attempts have been made to modulate the gut bacterial composition, following the idea that expansion of bacterial species considered as beneficial(Lactobacilli and Bifidobacteria) associated with the reduction of those considered harmful(Clostridium, Escherichia coli, Salmonella, Shigella and Pseudomonas) should attenuate IBS symptoms. In this conceptual framework, probiotics appear an attractive option in terms of both efficacy and safety, while prebiotics, synbiotics and antibiotics still need confirmation. | 3 | MR with Gd-EOB-DTPA in assessment of liver nodules in cirrhotic patients显示文摘To date the imaging diagnosis of liver lesions is based mainly on the identification of vascular features, which are typical of overt hepatocellular carcinoma(HCC), but the hepatocarcinogenesis is a complex and multistep event during which, a spectrum of nodules develop within the liver parenchyma, including benign small and large regenerative nodule(RN), low-grade dysplastic nodule(LGDN), high-grade dysplastic nodule(HGDN), early HCC, and well differentiated HCC. These nodules may be characterised not only on the basis of their respective different blood supplies, but also on their different hepatocyte function. Recently, in liver imaging the introduction of hepatobiliary magnetic resonance imaging contrast agent offered the clinicians the possibility to obtain, at once, information not only related to the vascular changes of liver nodules but also information on hepatocyte function. For this reasons this new approach becomes the most relevant diagnostic clue for differentiating low-risk nodules(LGDN-RN) from highrisk nodules(HGDN/early HCC or overt HCC) and consequently new diagnostic algorithms for HCC have been proposed. The use of hepatobiliary contrast agents is constantly increasing and gradually changing the standard of diagnosis of HCC. The main purpose of this review is to underline the added value of Gd-EOB-DTPA in early-stage diagnoses of HCC. | 4 | 环巴尔喀什-西准噶尔成矿省矿床类型、成矿系统和跨境成矿带对接显示文摘环巴尔喀什-西准噶尔成矿省地处中亚成矿域核心区,古生代构造和岩浆活动强烈,成矿作用丰富多样,发育许多大型-超大型乃至世界级的金属矿床,包括斑岩型铜矿床、斑岩-石英脉-云英岩型钨钼矿床、矽卡岩型铜(多金属)矿床、火山成因块状硫化物型(VMS)多金属矿床、浅成低温热液型金矿床、石英脉-蚀变岩型中温热液金矿床、与花岗岩有关的Be-U矿床、岩浆熔离型铜镍硫化物矿床和豆荚状铬铁矿等,这些矿床集中分布,形成多处成矿带,包括哈萨克斯坦的扎尔玛-萨吾尔、波谢库尔-成吉斯和北巴尔喀什等成矿带以及新疆西准噶尔的萨吾尔、谢米斯台-沙尔布提和巴尔鲁克-克拉玛依等成矿带。哈萨克斯坦包含大型-超大型和世界级金属矿床的成矿带向东是否延入新疆西准噶尔?能否实现新疆西准噶尔找矿重大突破?都是备受关注的重大地质找矿问题。本文在前人研究并结合作者工作基础上,根据成矿带的成矿构造环境、矿床类型、成矿特点和成矿时代,总结出成矿省至少发育九类成矿系统,即(1)奥陶纪-志留纪岛弧斑岩型Cu-Au成矿系统;(2)奥陶纪岛弧VMS型多金属成矿系统;(3)泥盆纪岛弧岩浆熔离型铜镍硫化物成矿系统;(4)泥盆纪与蛇绿岩有关的豆荚状铬铁矿成矿系统;(5)早石炭世岛弧斑岩-浅成低温热液型Cu-Au成矿系统;(6)石炭纪岛弧斑岩型-矽卡岩型Cu-Mo-Au成矿系统;(7)晚石炭世弧后盆地与花岗岩有关的Be-U成矿系统;(8)早二叠世岛弧或岛弧和陆缘弧过渡弧斑岩-石英脉-云英岩型Mo-W成矿系统;(9)早二叠世岛弧石英脉-蚀变岩型中温热液金成矿系统。对比研究发现境内外相邻成矿带具有相同或相似的成矿系统,二者可以对接,新疆西准噶尔三条成矿带分别是哈萨克斯坦三条成矿带的东延部分,构成了成矿省北部的扎尔玛-萨吾尔Cu-Au成矿带、中部的波谢库尔-成吉斯-谢米斯台Cu-Au-Be-U多金属成矿带和南部的北巴尔喀什-克拉玛依Cu-Mo-W-Au-Cr成矿带。新疆西准噶尔具有形成大型-超大型矿床的成矿系统和成矿条件,有望实现找矿勘探的更大突破。 | 申萍 周涛发 袁峰 潘鸿迪 王居里 SEITMURATOVA Eleonora | 2015 | 岩石学报2015,31,2: | 34 |

| 8 | Immune response after photodynamic therapy increases anti-cancer and anti-bacterial effects显示文摘Photodynamic therapy(PDT) is a clinically approved procedure for treatment of cancer and infections. PDT involves systemic or topical administration of a photosensitizer(PS), followed by irradiation of the diseased area with light of a wavelength corresponding to an absorbance band of the PS. In the presence of oxygen, a photochemical reaction is initiated, leading to the generation of reactive oxygen species and cell death. Besides causing direct cytotoxic effects on illuminated tumor cells, PDT is known to cause damage to the tumor vasculature and induce the release of pro-inflammatory molecules. Pre-clinical and clinical studies have demonstrated that PDT is capable of affecting both the innate and adaptive arms of the immune system. Immune stimulatory properties of PDT may increase its beneficial effects giving the therapy wider potential to become more extensively used in clinical practice. Be-sides stimulating tumor-specific cytotoxic T-cells capable to destroy distant untreated tumor cells, PDT leads to development of anti-tumor memory immunity that can potentially prevent the recurrence of cancer. | 9 | Hypoxia inducible factor-1αaccumulation in steatotic liver preservation:Role of nitric oxide显示文摘AIM:To examine the relevance of hypoxia inducible factor(HIF-1)and nitric oxide(NO)on the preservation of fatty liver against cold ischemia-reperfusion injury(IRI). METHODS:We used an isolated perfused rat liver model and we evaluated HIF-1αin steatotic and non-steatotic livers preserved for 24 h at 4℃in University of Wisconsin and IGL-1 solutions,and then subjected to 2 h of normothermic reperfusion.After normoxic reperfusion,liver enzymes,bile production,bromosulfophthalein clearance,as well as HIF-1αand NO[endothelial NO synthase(eNOS)activity and nitrites/nitrates]were also measured.Other factors associated with the higher susceptibility of steatotic livers to IRI,such as mitochondrial damage and vascular resistance were evaluated. RESULTS:A significant increase in HIF-1αwas found in steatotic and non-steatotic livers preserved in IGL-1 after cold storage.Livers preserved in IGL-1 showed a significant attenuation of liver injury and improvement in liver function parameters.These benefits were enhanced by the addition of trimetazidine(an antiischemic drug),which induces NO and eNOS activation, to IGL-1 solution.In normoxic reperfusion,the presence of NO favors HIF-1αaccumulation,promoting also the activation of other cytoprotective genes,such as hemeoxygenase-1. | 10 | Re-Os geochronology of Cu and W-Mo deposits in the Balkhash metallogenic belt,Kazakhstan and its geological significance显示文摘<正>The Central Asian metallogenic domain(CAMD) is a multi-core metallogenic system controlled by boundary strike-slip fault systems.The Balkhash metallogenic belt in Kazakhstan,in which occur many large and super-large porphyritic Cu—Mo deposits and some quartz vein- and greisen-type W—Mo deposits,is a well-known porphyritic Cu—Mo metallogenic belt in the CAMD.In this paper 11 molybdenite samples from the western segment of the Balkhash metallogenic belt are selected for Re—Os compositional analyses and Re—Os isotopic dating.Molybdenites from the Borly porphyry Cu deposit and the three quartz vein-greisen W—Mo deposits—East Kounrad.Akshatau and Zhanet—all have relatively high Re contents(2712—2772μg/g for Borly and 2.267—31.50μg/g for the other three W—Mo deposits),and lower common Os contents(0.670—2.696 ng/g for Borly and 0.0051—0.056 ng/g for the other three).The molybdenites from the Borly porphyry Cu—Mo deposit and the East Kounrad,Zhanet,and Akshatau quartz vein- and greisen-type W—Mo deposits give average model Re—Os ages of 315.9 Ma,298.0 Ma,295.0 Ma,and 289.3 Ma respectively.Meanwhile,molybdenites from the East Kounrad,Zhanet,and Akshatau W—Mo deposits give a Re—Os isochron age of 297.9 Ma,with an MSWD value of 0.97.Re-Os dating of the molybdenites indicates that Cu—W—Mo metallogenesis in the western Balkhash metallogenic belt occurred during Late Carboniferous to Early Permian(315.9—289.3 Ma),while the porphyry Cu—Mo deposits formed at—316 Ma,and the quartz vein-greisen W—Mo deposits formed at ~298 Ma.The Re—Os model and isochron ages thus suggest that Late Carboniferous porphyry granitoid and pegmatite magmatism took place during the late Hercynian movement.Compared to the Junggar-East Tianshan porphyry Cu metallogenic belt in northwestern China,the formation of the Cu—Mo metallogenesis in the Balkhash metallogenic belt occurred between that of the Tuwu-Yandong in East Tianshan and the Baogutu porphyry Cu deposits in West Junggar. | 11 | Extraintestinal manifestations in a large series of Italian inflammatory bowel disease patients显示文摘AIM:To investigate prevalence,type and time of onset of extraintestinal manifestations(EIMs)in a series of Italian inflammatory bowel disease(IBD)patients.METHODS:Data of 811 IBD consecutive patients,first referred to our Centre from 2000 to 2011,were retrospectively evaluated.RESULTS:Eight hundred and eleven IBD patients(437M,374 F)were studied:595 ulcerative colitis(UC)(73.4%)and 216 Crohn’s disease(CD)(26.6%).Among these,329(40.6%)showed EIMs:210 UC(35.3%)and119 CD(55.1%)(P<0.0001).Considering the time of the diagnosis of IBD,37 EIMs(11.2%)were developed before,229(69.6%)after and 63(19.2%)were simultaneous.The type of EIM were as follows:240 musculoskeletal(29.6%),in 72 CD patients and in 168 UC(P<0.0001);47 mucocutaneous(5.8%),in 26 CD and in21 UC(P=0.0049);26 ocular(3.2%),in 16 CD and in10 UC(CD 7.4%vs UC 1.7%,P=0.0093);6 hepatobiliary(0.8%);10 endocrinological(1.2%).In particular,with regards to the involvement of the musculoskeletal system,arthritis Type 1 was found in 41 CD(19%)and in 61 UC(10.2%)(P=0.0012)and Type 2 in 25 CD(11.6%)and in 100 UC(16.8%)(P=0.0012).CONCLUSION:Mucocutaneous manifestations,arthritis Type 1 and uveitis were significantly more frequent in CD than UC.The complications of the musculoskeletal system were the mostly observed ones,often with symptoms more severe than intestinal ones,confirming the need for close cooperation with rheumatologists. | Maddalena Zippi Claudia Corrado Roberta Pica Eleonora Veronica Avallone Claudio Cassieri Daniela De Nitto Paolo Paoluzi Piero Vernia | 2014 | World Journal of Gastroenterology2014,20,46: | 10 |

| 13 | Electrical storm: A clinical and electrophysiological overview显示文摘Electrical storm(ES) is a clinical condition characterized by three or more ventricular arrhythmia episodes leading to appropriate implantable cardioverterdefibrillator(ICD) therapies in a 24 h period. Mostly, arrhythmias responsible of ES are multiple morphologies of monomorphic ventricular tachycardia(VT), but polymorphic VT and ventricular fibrillation can also result in ES. Clinical presentation is very dramatic in most cases, strictly related to the cardiac disease that may worsen electrical and hemodynamic decompensation. Therefore ES management is challenging in the majority of cases and a high mortality is the rule both in the acute and in the long-term phases. Different underlying cardiomyopathies provide significant clues into the mechanism of ES, which can arise in the setting of structural arrhythmogenic cardiomyopathies or rarely in patients with inherited arrhythmic syndrome, impacting on pharmacological treatment, on ICD programming, and on the opportunity to apply strategies of catheter ablation. This latter has become a pivotal form of treatment due to its high efficacy in modifying the arrhythmogenic substrate and in achieving rhythm stability, aiming at reducing recurrences of ventricular arrhythmia and at improving overall survival. | 14 | Chikungunya virus,epidemiology,clinics and phylogenesis:A review显示文摘Chikungunva virus is a mosquito-transmitted alphavirus that causes chikungunva fever,a febrile illness associated with severe arthralgia and rash.Chikungunva virus is transmitted by culicine mosquitoes:Chikungunya virus replicates in the skin,disseminates to liver,muscle,joints.lymphoid tissue and brain,presumably through the blood.Phylogenetic studies showed that the Indian Ocean and the Indian subcontinent epidemics were caused by two different introductions of distinct strains of East/Central/South African genotype of CHIKV.The paraphyletic grouping of African CHIK viruses supports the historical evidence that the virus was introduced into Asia from Africa.Phylogenetic analysis divided Chikungunva virus isolates into three distinct genotypes based on geographical origins:the first,the West Africa genotype,consisted of isolates from Senegal and Nigeria;the second contained strains from East/Central/South African genotype,while the third contained solely Asian.The most recent common ancestor for the recent epidemic,which ravaged Indian Ocean islands and Indian subcontinent in 2004- 2007.was found to date in 2002.Asian lineage dated about 1952 and exhibits similar spread patterns of the recent Indian Ocean outbreak lineage,with successive epidemics detected along an eastward path.Asian group splitted into two clades:an Indian lineage and a south east lineage.Outbreaks of Chikungunya virus fever in Asia have not been associated necessarily with outbreaks in Africa.Phylogenetic tools can reconstruct geographic spread of Chikungunva virus during the epidemics wave.The good management of patients with acute Chikungunva virus infection is essential for public health in susceptible areas with current Aedes spp activity. | Alessandra Lo Presti Alessia Lai Eleonora Cella Gianguglielmo Zehender Massimo Ciccozzi | 2014 | Asian Pacific Journal of Tropical Medicine2014,7,12: | 8 |
| 15 | Body weight, lifestyle, dietary habits and gastroesophageal reflux disease显示文摘While lifestyle modifications are currently used as firstline treatment for subjects with gastroesophageal reflux disease (GERD), the pathogenetic role of lifestyle factors and consequently, the efficacy of lifestyle measures is controversial. Our aim was to systematically review the pathogenetic link between overweight/ obesity, dietary habits, physical activity and GERD, and the beneficial effect of specific recommended changes, by means of the available literature from the 1999 to the present. Obesity, in particular, abdominal obesity, plays a key role in determining GERD symptoms and complications through mechanical and metabolic effects. Controlled weight loss (by diet or surgery) is effective in improving GERD symptoms. No definitive data exist regarding the role of diet and, in particular, of specific foods or drinks, in influencing GERD clinical manifestations. Moderate physical activity seems to be beneficial for GERD, while vigorous activity may be dangerous in predisposed individuals. In conclusion, being obese/overweight and GERD-specific symptoms and endoscopic features are related, and weight loss significantly improves GERD clinical-endoscopic manifestations. The role of dietary behavior, mainly in terms of specific dietary components, remains controversial. | 16 | From bench to bedside: Fecal calprotectin in inflammatory bowel diseases clinical setting显示文摘Fecal calprotectin(FC) has emerged as one of the most useful tools for clinical management of inflammatory bowel diseases(IBD). Many different methods of assessment have been developed and different cutoffs have been suggested for different clinical settings. We carried out a comprehensive literature review of the most relevant FC-related topics: the role of FC in discriminating between IBD and irritable bowel syndrome(IBS) and its use in managing IBD patients In patients with intestinal symptoms, due to the high negative predictive value a normal FC level reliably rules out active IBD. In IBD patients a correlation with both mucosal healing and histology was found, and there is increasing evidence that FC assessment can be helpful in monitoring disease activity and response to therapy as well as in predicting relapse, post-operative recurrence or pouchitis. Recently, its use in the context of a treat-to-target approach led to a better outcome than clinically-based therapy adjustment in patients with early Crohn's disease. In conclusion, FC measurement represents a cheap, safe and reliable test, easy to perform and with a good reproducibility. | 17 | Non-alcoholic fatty liver disease connections with fat-free tissues: A focus on bone and skeletal muscle显示文摘The estimates of global incidence and prevalence of non-alcoholic fatty liver disease(NAFLD) are worrisome, due to the parallel burden of obesity and its metabolic complications. Indeed, excess adiposity and insulin resistance represent two of the major risk factors for NAFLD; interestingly, in the last years a growing body of evidence tended to support a novel mechanistic perspective, in which the liver is at the center of a complex interplay involving organs and systems, other than adipose tissue and glucose homeostasis. Bone and the skeletal muscle are fat- free tissues which appeared to be independently associated with NAFLD in several cross-sectional studies. The deterioration of bone mineral density and lean body mass, leading to osteoporosis and sarcopenia, respectively, are age-related processes. The prevalence of NAFLD also increases with age. Beyond physiological aging, the three conditions share some common underlying mechanisms, and their elucidations could be of paramount importance to design more effective treatment strategies for the management of NAFLD. | 18 | Bile-acid-activated farnesoid X receptor regulates hydrogen sulfide production and hepatic microcirculation显示文摘AIM: To investigate whether the farnesoid X receptor (FXR) regulates expression of liver cystathionase (CSE), a gene involved in hydrogen sulfi de (H2S) generation. METHODS: The regulation of CSE expression in response to FXR ligands was evaluated in HepG2 cells and in wild-type and FXR null mice treated with 6-ethyl chenodeoxycholic acid (6E-CDCA), a synthetic FXR ligand. The analysis demonstrated an FXR responsive element in the 5'-flanking region of the human CSE gene. The function of this site was investigated by luciferase reporter assays, chromatin immunoprecipitation and electrophoretic mobility shift assays. Livers obtained from rats treated with carbon tetrachloride alone, or in combination with 6-ethyl chenodeoxycholic acid, were studied for hydrogen sulphide generation and portal pressure measurement. RESULTS: Liver expression of CSE is regulated by bile acids by means of an FXR-mediated mechanism. Western blotting, qualitative and quantitative polymerase chain reaction, as well as immunohistochemical analysis, showed that expression of CSE in HepG2 cells and in mice is induced by treatment with an FXR ligand. Administration of 6E-CDCA to carbon tetrachloride treated rats protected against the down-regulation of CSE expression, increased H2S generation, reduced portal pressure and attenuated the endothelial dysfunction of isolated and perfused cirrhotic rat livers.
