| 1 | 雷公藤多苷对寻常型银屑病患者外周血Th17/Treg 相关细胞因子的影响显示文摘目的观察雷公藤多昔治疗前后寻常型银屑病患者血清辅助性T细胞17(Thl7)/调节性T细胞(Treg)相关细胞因子白介素-17(1L-17),白介素-23(IL-23)和白介素-10(IL-10)的含量,探讨雷公藤多昔治疗寻常型银屑病的作用机制。方法对蚌埠医学院第一附属医院皮肤科2016年12月—2017年6月就诊的41例寻常型银屑病患者采用口服雷公藤多昔片治疗,每日3次,每次20mg,疗程8周,以银屑病皮损面积和严重程度(PASI)评分评价疗效;采用酶联免疫吸附法(ELISA)检测正常对照组及患者治疗前后血清中IL-17、L-23和IL-10水平变化。结果寻常型银屑病患者外周血Thl7相关细胞因子IL-17和IL-23水平显著高于正常对照组(均P<0.01);而Treg相关细胞因子IL-10水平显著低于正常对照组。雷公藤多昔治疗后IL-17和IL-23水平明显降低,与治疗前相比差异有统计学意义(均P<0.01);而IL-10水平明显升高,与治疗前相比差异也有统计学意义(P<0.01)。雷公藤多昔治疗后PASI评分与治疗前相比显著下降,差异有统计学意义(P<0.01).患者雷公藤多昔治疗前后PASI评分与IL-17和IL-23水平呈正相关性,而与IL-10呈负相关性(均P<0.01).结论雷公藤多昔治疗寻常型银屑病疗效显著,可能通过调节Thl7/Treg相关细胞因子水平发挥其治疗作用。 | TUEKAM TUEKAM Erna Gaelle 江从军 | 2019 | 中华全科医学2019，17，8： | 18 |

| 4 | Clinical characteristics and outcomes of patients with hepatic angiomyolipoma:A literature review显示文摘First reported in 1976,hepatic angiomyolipoma(HAML)is a rare mesenchymal liver tumor occurring mostly in middle-aged women.Diagnosis of the liver mass is often incidental on abdominal imaging due to the frequent absence of specific symptoms.Nearly 10%of HAMLs are associated with tuberous sclerosis complex.HAML contains variable proportions of blood vessels,smooth muscle cells and adipose tissue,which renders radiological diagnosis hazardous.Cells express positivity for HMB-45 and actin,thus these tumors are integrated into the group of perivascular epithelioid cell tumors.Typically,a HAML appears on magnetic resonance imaging(or computed tomography scan)as a hypervascular solid tumor with fatty areas and with washout,and can easily be misdiagnosed as other liver tumors,particularly hepatocellular carcinoma.The therapeutic strategy is not clearly defined,but surgical resection is indicated for symptomatic patients,for tumors showing an aggressive pattern(i.e.,changes in size on imaging or high proliferation activity and atypical epithelioid pattern on liver biopsy),for large(>5 cm)biopsy-proven HAML,and if doubts remain on imaging or histology.Conservative management may be justified in other conditions,since most cases follow a benign clinical course.In summary,the correct diagnosis of HAML is challenging on imaging and relies mainly on pathological findings. | Paul Calame Gaelle Tyrode Delphine Weil Verhoeven Sophie Felix Anne Julia Klompenhouwer Vincent Di Martino Eric Delabrousse Thierry Thevenot | 2021 | World Journal of Gastroenterology2021，27，19： | 5 |
| 5 | Adenoma and advanced neoplasia detection rates increase from 45 years of age显示文摘BACKGROUND Colonoscopy is considered a valid primary screening tool for colorectal cancer(CRC). The decreasing risk of CRC observed in patients undergoing colonoscopy is correlated with the adenoma detection rate(ADR). Due to the fact that screening programs usually start from the age of 50, very few data are available on the risk of adenoma between 40 and 49 years. However, the incidence of CRC is increasing in young populations and it is not uncommon in routine practice to detect adenomas or even advanced neoplasia during colonoscopy in patients under 50 years.AIM To compare the ADR and advanced neoplasia detection rate(ANDR) according to age in a large series of patients during routine colonoscopy.METHODS All consecutive patients who were scheduled for colonoscopy were included.Exclusion criteria were as follows: patients scheduled for partial colonoscopy or interventional colonoscopy(for stent insertion or stenosis dilation).Colonoscopies were performed in our unit by a team of 30 gastroenterologists in2016. We determined the ADR and ANDR in each age group in the whole population and in the population with an average risk of CRC(excluding patients with personal or family history of advanced adenoma or cancer).RESULTS6027 colonoscopies were performed in patients with a median age of 57 years(range, 15-96). The ADR and ANDR were 28.6% and 9.7%, respectively, in the whole population. When comparing patients aged 40-44(n = 382) and 45-49 years (n = 515), a strong increase in all parameters from 45 years was observed, with the ADR rising from 9.7% in patients aged 40-44 to 21.2% between 45 and 49(P <0.001) and the ANDR increasing from 3.1% in patients aged 40-44 to 6.4% in those aged 45-49 years(P < 0.03). With regard to patients aged 50-54(n = 849), a statistically significant increase in the ADR and ANDR was not observed between patients aged 45-49 and those aged 50-54 years.
