| 1 | Novel concepts in radiation-induced cardiovascular disease显示文摘Radiation-induced cardiovascular disease(RICVD) is the most common nonmalignant cause of morbidity and mortality among cancer survivors who have undergone mediastinal radiation therapy(RT).Cardiovascular complications include effusive or constrictive pericarditis,cardiomyopathy,valvular heart disease,and coronary/vascular disease.These are pathophysiologically distinct disease entities whose prevalence varies depending on the timing and extent of radiation exposure to the heart and great vessels.Although refinements in RT dosimetry and shielding will inevitably limit future cases of RICVD,the increasing number of long-term cancer survivors,including those treated with older higher-dose RT regimens,will ensure a steady flow of afflicted patients for the foreseeable future.Thus,there is a pressing need for enhanced understanding of the disease mechanisms,and improved detection methods and treatment strategies.Newly characterized mechanisms responsible for the establishment of chronic fibrosis,such as oxidative stress,inflammation and epigenetic modifications,are discussed and linked to potential treatments currently under study.Novel imaging modalities may serve as powerful screening tools in RICVD,and recent research and expert opinion advocating their use is introduced.Data arguing for the aggressive use of percutaneous interventions,such as transcutaneous valve replacement and drug-eluting stents,are examined and considered in the context of prior therapeutic approaches.RICVD and its treatment options are the subject of a rich and dynamic body of research,and patients who are at risk or suffering from this disease will benefit from the care of physicians with specialty expertise in the emerging field of cardiooncology. | Jason R Cuomo Gyanendra K Sharma Preston D Conger Neal L Weintraub | 2016 | World Journal of Cardiology2016,8,9: | 17 |
| 2 | Optimizing submerged arc welding using response surface methodology, regression analysis, and genetic algorithm显示文摘The weld quality is significantly affected by the weld parameters(arc voltage, welding current, nozzle to plate distance and welding speed) in the submerged arc welding(SAW). Bead-on-plate welds were performed on stainless steel plates by automated SAW machine. The experimental data were collected in accordance with the response surface methodology(RSM). In addition to RSM, the regression analysis was performed to set up inputeoutput relationships in the SAW process. It was found that weld parameters define the geometry of weld bead and determine the mechanical properties of the joint. The influence of the input variables on weld bead geometry is represented as graphs. It was found that an increment in voltage increases the bead width but decreases the bead height, whereas the current increment result-in an increment in bead height and no change in bead width. The bead width and height decrease with the increment in the welding speed. With an increment in the nozzle-to-plate distance, bead width decrease, but bead height increases. The value of bead hardness increases with the increment in current but the increment in voltage and travel speed does not have a significant influence on the bead hardness. The predictions from the mathematical model developed and the corresponding experimental results are having a fair agreement. | 3 | Fluoroscopy guided percutaneous renal access in prone position显示文摘Percutaneous nephrolithotomy is a very commonly done procedure for management of renal calculus disease. Establishing a good access is the first and probably themost crucial step of this procedure. A proper access is the gateway to success. However, this crucial step has the steepest learning curve for, in a fluoroscopy guided access, it involves visualizing a three dimensional anatomy on a two dimensional fluoroscopy screen. This review describes the anatomical basis of the renal access. It provides a literature review of all aspects of percutaneous renal access along with the advances that have taken place in this field over the years. The article describes a technique to determine the site of skin puncture, the angle and depth of puncture using a simple mathematical principle. It also reviews the common problems faced during the process of puncture and dilatation and describes the ways to overcome them. | 4 | Cochlear implantation outcomes in children with common cavity deformity;a retrospective study显示文摘Objective: A common cavity deformity(CCD) is a deformed inner ear in which the cochlea and vestibule are confluent forming a common rudimentary cystic cavity that results in profound hearing loss. There are few studies paying attention to common cavity. Our group is engrossed in observing the improvement of auditory and verbal abilities in children who have received cochlear implantation(CI), and comparing these targets between children with common cavity and normal inner ear structure.Material and methods: A retrospective study was conducted in 12 patients with profound hearing loss that were divided into a common cavity group and a control group, six in each group matched in sex, age and time of implantation, based on inner ear structure. Categories of Auditory Performance(CAP) and speech intelligibility rating(SIR) scores and aided hearing thresholds were collected and compared between the two groups. All patients wore CI for more than 1 year at the Cochlear Center of Anhui Medical University from 2011 to 2015.Results: Postoperative CAP and SIR scores were higher than before operation in both groups(p < 0.05), although the scores were lower in the CCD group than in the control group(p < 0.05). | 5 | 胃癌腹膜微转移的临床研究显示文摘目的:研究胃癌患者腹膜微转移的相关因素及临床意义,为临床手术切除范围探寻理论依据。方法:应用CK19和CK20免疫组织化学方法,检测62例胃癌患者横结肠系膜前叶、胰腺被膜、网膜囊后壁和盆底腹膜组织,并与HE染色及腹腔灌洗液细胞学检查对比。结果:62例胃癌患者的腹膜组织常规病理HE染色均未见癌细胞,免疫组织化学证实腹膜微转移阳性率为43.55%(27/62),远高于腹腔灌洗液癌细胞的14.52%(9/62)。胃癌腹膜微转移与肿瘤的大小、浸润深度、临床分期、淋巴结转移有关(P<0.05),而与肿瘤的部位、患者年龄、性别无关(P>0.05)。结论:用免疫组化方法检测胃癌腹膜微转移,可为胃癌根治术术式的选择和术后综合治疗提供可靠依据。对于进展期胃癌患者,应常规腹膜取材检测微转移,多点取材有助于提高微转移检出率。 | 陈月光 何庆泗 孙国瑞 王琰珉 张朝阳 Gyanendra Swar | 2009 | 中国现代普通外科进展2009,12,6: | 3 |
| 6 | Prevalence and clinical characteristics associated with left atrial thrombus detection: Apixaban显示文摘BACKGROUND The prevalence of left atrial appendage(LAA) thrombus detection by transesophageal echocardiogram(TEE) in patients with non-valvular atrial fibrillation(AF) anticoagulated with apixaban is not well defined and identification of additional risk factors may help guide the selection process for pre-procedural TEE. The purpose of our study was to retrospectively analyze the prevalence of LAA thrombus detection by TEE in patients continuously anticoagulated with apixaban for ≥ 4 wk and evaluate for any cardiac risk factors or echocardiographic characteristics which may serve as predictors of thrombus formation.AIM To retrospectively analyze the prevalence of LAA thrombus detection by TEE in patients continuously anticoagulated with apixaban.METHODS Clinical and echocardiographic data for 820 consecutive patients with AF undergoing TEE at Augusta University Medical Center over a four-year period were retrospectively analyzed. All patients(apixaban: 226) with non-valvular AF and documented compliance with apixaban for ≥ 4 wk prior to index TEE were included.RESULTS Following ≥ 4 wk of continuous anticoagulation with apixaban, the prevalence ofLAA thrombus and LAA thrombus/dense spontaneous echocardiographic contrast was 3.1% and 6.6%, respectively. Persistent AF, left ventricular ejection fraction < 30%, severe LA dilation, and reduced LAA velocity were associated with thrombus formation. Following multivariate logistic regression, persistent AF(OR: 7.427; 95%CI: 1.02 to 53.92; P = 0.0474), and reduced LAA velocity(OR:1.086; 95%CI: 1.010 to 1.187; P = 0.0489) were identified as independent predictors of LAA thrombus. No Thrombi were detected in patients with a CHA2 DS2-VASc score ≤ 1.CONCLUSION Among patients with non-valvular AF and ≥ 4 wk of anticoagulation with apixaban, the prevalence of LAA thrombus detected by TEE was 3.1%. | 7 | Trans-umbilical endoscopic cholecystectomy with a water-jet hybrid-knife:A pilot animal study显示文摘AIM:To investigate the feasibility and safety of Natural orifice trans-umbilical endoscopic cholecystectomy with a water-jet hybrid-knife in a non-survival porcine model.METHODS:Pure natural orifice transluminal endoscopic surgery(NOTES) cholecystectomy was performed on three non-survival pigs,by transumbilical approach,using a water-jet hybrid-knife. Under general anesthesia,the following steps detailed the procedure:(1) incision of the umbilicus followed by the passage of a double-channel flexible endsocope through an overtube into the peritoneal cavity;(2) establishment of pneumoperitoneum;(3) abdominal exploration;(4)endoscopic cholecystectomy:dissection of the gallbladder performed using water jet equipment,ligation of the cystic artery and duct conducted using nylon loops;and(5) necropsy with macroscopic evaluation.RESULTS:Transumbilical endoscopic cholecystectomy was successfully completed in the first and third pig,with minor bleedings. The dissection times were 137and 42 min,respectively. The total operation times were 167 and 69 min,respectively. And the lengths of resected specimen were 6.5 and 6.1 cm,respectively.Instillation of the fluid into the gallbladder bed produced edematous,distended tissue making separation safe and easy. Reliable ligation using double nylon loops insured the safety of cutting between the loops.There were no intraoperative complications or hemodynamic instability. Uncontrolled introperative bleeding occurred in the second case,leading to the operation failure.CONCLUSION:Pure NOTES trans-umbilical cholecystectomy with a water-jet hybrid-knife appears to be feasible and safe.
