| 1 | Comparison of the effects of acute and chronic psychological stress on metabolic features in rats显示文摘This study was aimed to compare the effects of acute and chronic psychological stress on metabolic factors. Forty-two male Wistar rats were divided into control and stressed groups. Stress was applied by a communication box acutely (1 d) and chronically (15 and 30 d). Blood sampling was carried out by retro-orbital-puncture method. The plasma levels of glucose, cholesterol, triglyceride, insulin, and corticosterone were measured. In addition, feed and water intake, latency to eat and drink, adrenal and body weights were determined. Acute and chronic psychological stress did not significantly change basal plasma corticosterone levels. However, immediately (1 min) after acute exposure to stress, plasma corticosterone level increased compared to that before stress exposure. Acute stress increased plasma insulin levels significantly. Fifteen days of stress exposure resulted in plasma glucose increase. Chronic stress significantly increased feed intake, latency to eat, and adrenal weight compared to acute stress. The body weights of both control and stressed groups increased markedly during the experiment. Homeostasis model assessment of insulin resistance (HOMA-IR) index did not change significantly in the stressed group. | 11 | The comparison of insulin and uric acid levels in adolescents with and without metabolic syndrome显示文摘BACKGROUND and AIM:The prevalence of metabolic syndrome (MS)increased in recent years in both adolescents and children groups.The aim of the study is evaluating the relationship between insulin and uric acid (UA)level in MS in adolescents MATERIALS and METHODS:we studied 120adolescence aged 10to 19in two groups:control group without metabolic syndrome and case group with metabolic syndrome.The Criteria ofATP III was considered as a diagnosis factor for metabolic syndrome. DISCUSSION:Various studies have been conducted in various populations to evaluate the relationship between UA level and MS in adolescents.Abdominal obesity,low HDL,hypertriglyceridemia and hypertension are associated with high UA level.In their analysis,the MS OR in UA level≤4.9,4.9-5.8and≥5.8mg/dl was 1,2.53and 9.03,respectively,which were higher than our findings in current study.Hyperinsulinemia caused by insulin resistance is one of the complications associated with MS, which puts individuals at risk of diabetes and cardiovascular events. RESULTS:Uric acid level in the Case group was significantly higher than the control group (p =0.0001,43.84-1,4vs.4.1-4-1 mg/dl,respectively).Insulin level was significantly higher in the case group in compare to the control group (p =0.008,9.84- 5.3vs.12.24-6μU/ml,respectively). | 12 | Invasive candidiasis in critical care setting, updated recommendations from “Invasive Fungal Infections-Clinical Forum”, Iran显示文摘Invasive candidiasis(IC) bears a high risk of morbidity and mortality in the intensive care units(ICU). With the current advances in critical care and the use of widespectrum antibiotics, invasive fungal infections(IFIs) and IC in particular, have turned into a growing concern in the ICU. Further to blood cultures, some auxil-iary laboratory tests and biomarkers are developed to enable an earlier detection of infection, however these test are neither consistently available nor validated in our setting. On the other hand, patients' clinical status and local epidemiology data may justify the empiric antifungal approach using the proper antifungal option. The clinical approach to the management of IC in febrile, non-neutropenic critically ill patients has been defined in available international guidelines; nevertheless such recommendations need to be customized when applied to our local practice. Over the past three years, Iranian experts from intensive care and infectious diseases disciplines have tried to draw a consensus on the management of IFI with a particular focus on IC in the ICU. The established IFI-clinical forum(IFI-CF), comprising the scientific leaders in the field, has recently come up with and updated recommendation on the same(June 2014).
