| 1 | Endoscopic ultrasound guided radiofrequency ablation,for pancreatic cystic neoplasms and neuroendocrine tumors显示文摘AIM: To outline the feasibility, safety, adverse events and early results of endoscopic ultrasound(EUS)-radiofrequency ablation(RFA) in pancreatic neoplasms using a novel probe. METHODS: This is a multi-center, pilot safety feasibility study. The intervention described was radiofrequency ablation(RF) which was applied with an innovative monopolar RF probe(1.2 mm Habib EUS-RFA catheter) placed through a 19 or 22 gauge fine needle aspiration(FNA) needle once FNA was performed in patients with a tumor in the head of the pancreas. The HabibTM EUSRFA is a 1 Fr wire(0.33 mm, 0.013') with a working length of 190 cm, which can be inserted through the biopsy channel of an echoendoscope. RF power is applied to the electrode at the end of the wire to coagulate tissue in the liver and pancreas.RESULTS: Eight patients [median age of 65(range 27-82) years; 7 female and 1 male] were recruited in a prospective multicenter trial. Six had a pancreatic cysticneoplasm(four a mucinous cyst, one had intraductal papillary mucinous neoplasm and one a microcystic adenoma) and two had a neuroendocrine tumors(NET) in the head of pancreas. The mean size of the cystic neoplasm and NET were 36.5 mm(SD ± 17.9 mm) and 27.5 mm(SD ± 17.7 mm) respectively. The EUSRFA was successfully completed in all cases. Among the 6 patients with a cystic neoplasm, post procedure imaging in 3-6 mo showed complete resolution of the cysts in 2 cases, whilst in three more there was a 48.4% reduction [mean pre RF 38.8 mm(SD ± 21.7 mm) vs mean post RF 20 mm(SD ± 17.1 mm)] in size. In regards to the NET patients, there was a change in vascularity and central necrosis after EUS-RFA. No major complications were observed within 48 h of the procedure. Two patients had mild abdominal pain that resolved within 3 d. CONCLUSION: EUS-RFA of pancreatic neoplasms with a novel monopolar RF probe was well tolerated in all cases. Our preliminary data suggest that the procedure is straightforward and safe. | 3 | Biepicondylar fracture dislocation of the elbow joint concomitant with ulnar nerve injury显示文摘In this article, we present a case of humeral biepicondylar fracture dislocation concomitant with ulnar nerve injury in a seventeen year-old male patient. Physical examination of our patient in the emergency room revealed a painful, edematous and deformed-looking left elbow joint. Hypoesthesia of the little finger was also diagnosed on the left hand. Radiological assessment ended up with a posterior fracture dislocation of the elbow joint accompanied by intra-articular loose bodies. Open reduction-Internal fixation of the fracture dislocation and ulnar nerve exploration were performed under general anesthesia at the same session as surgical treatment of our patient. Physical therapy and rehabilitation protocol was implemented at the end of two weeks post-operatively. Union of the fracture lines, as well as the olecranon osteotomy site, was achieved at the end of four months post-operatively. Ulnar nerve function was fully restored without any sensory or motor loss. | 4 | An unusual case of incidental rupture of liver hemangioma during labor显示文摘Cavernous hemangiomas are the most common benign tumors of the liver and are found in about 2% of autopsy patients. 1,2 The vast majority of these tumors are small and asymptomatic; however, there have been a few reported cases in which these lesions led to spontaneous fatal hemorrhage. 1,2 Although liver hemangiomas occur in both sexes, most studies point to a female predominance; one study reported a ratio of female predominance of 4. 5: 1.3 It has been suggested that estrogen may be associated with the growth of liver hemangiomas, but the incidence of these lesions in pregnancy and the effect on them of the increased estrogen levels during pregnancy are unknown. 2 Symptomatic liver hemangiomas have been treated by steroids, radiation therapy, surgical resection and recently embolization, but surgeons may sometimes be confronted with intraabdominal hemorrhage originating from rupture of asymptomatic liver hemangiomas. | 9 | Modeling of Reversibleγ/αTransformations of Low Carbon Steels in the Intercritical Temperature Range显示文摘A phenomenological kinetic model has been developed for the prediction of non-isothermal reversible incomplete transformations in low-carbon hypoeutectoid steels. The theoretical basis of the proposed method has its origin in a possible extension of the traditional Austin-Rickett kinetic differential equation. To critically assess the applicability of the model, a number of experiments based on computer simulations have been performed to predict the austenite/ferrite proeutectoid transformation in the temperature range of Aei to Ae3 on plain carbon hypoeutectoid steels. A comparison with published experimental data has verified that the model developed is reasonable both quantitatively and with respect to well-established trends.
