| 1 | Clostridium difficile causing acute renal failure: Case presentation and review显示文摘AIM: Clostridium difficile infection is primarily a nosocomial infection but asymptomatic carriers of Clostridium difficile can be found in up to 5% of the general population.Ampicillin, cephalosporins and clindamycin are the antibiotics that are most frequently associated with Clostridium difficile-associated diarrhea or colitis. Little is known about acute renal failure as a consequence of Clostridium difficile-associated diarrhea.METHODS: In this case report, we describe the course of Clostridium difficile-associated diarrhea in an 82-yearold patient developing acute renal failure. Stopping the offending agent and symptomatic therapy brought a rapid improvement of diarrhea and acute renal failure, full recovery was gained 18 d after admission. In a systematic review we looked for links between the two conditions.RESULTS: The link between Clostridium difficilr-associated diarrhea and acute renal failure in our patient was most likely volume depletion. | 2 | Co-expression of CDX2 and MUC2 in gastric carcinomas: Correlations with clinico-pathological parameters and prognosis显示文摘AIM: To evaluate the role of CDX2 homeobox protein as a predictor for cancer progression and prognosis as well as its correlation with MUC2 expression. CDX2 represents a transcription factor for various intestinal genes (including MUC2) and thus an important regulator of intestinal differentiation, which could previously be identified in gastric carcinomas and intestinal metaplasia.METHODS: Formalin-fixed and paraffin-embedded tissues from 190 gastric carcinoma patients were stained with monodonal antibodies recognizing CDX2 and MUC2, respectively. Immunoreactivity was evaluated semiquantitatively and statistical analyses including χ2 tests, uni- and multi-variate survival analyses were performed.RESULTS: CDX2 was mostly expressed in a nuclear or supranuclear pattern, whereas MUC2 showed an almost exclusive supranuclear reactivity. Both antigens were present in >80% of areas exhibiting intestinal metaplasia. An immunoreactivity in >5% of the tumor area was observed in 57% (CDX2) or in 21% (MUC2) of the carcinomas. The presence of both molecules did not correlate with WHO,significantly stronger vlUC2 expression in mucinous tumors). CDX2 correlated with a lower pT and pN stage in the subgroups of intestinal and stage I cancers and was associated with MUC2 positivity. A prognostic impact of CDX2 or MUC2 was not observed. CONCLUSION: CDX2 and MUC2 play an important role in the differentiation of normal, inflamed, and neoplastic gastric tissues. | 4 | Accuracy of triage strategies for human papillomavirus DNA-positive women in low-resource settings:A cross-sectional study in China显示文摘Objective: Care HPV is a human papillomavirus(HPV) DNA test for low-resource settings(LRS).This study assesses optimum triage strategies for care HPV-positive women in LRS.Methods: A total of 2,530 Chinese women were concurrently screened for cervical cancer with visual inspection with acetic acid(VIA),liquid-based cytology and HPV testing by physician-and self-collected care HPV,and physician-collected Hybrid Capture 2(HC2).Screen-positive women were referred to colposcopy with biopsy and endocervical curettage as necessary.HPV-positivity was defined as ≥1.0 relative light units/cutoff(RLU/CO) for both care HPV and HC2.Primary physician-HC2,physician-care HPV and self-care HPV and in sequential screening with cytology,VIA,or increased HPV test-positivity performance,stratified by age,were assessed for cervical intraepithelial neoplasia(CIN) grade 2/3 or worse(CIN2/3+) detection.Results: The sensitivities and specificities of primary HPV testing for CIN2+ were: 83.8%,88.1% for physiciancare HPV; 72.1%,88.2% for self-care HPV; and 97.1%,86.0% for HC2.Physician-care HPV test-positive women with VIA triage had a sensitivity of 30.9% for CIN2+ versus 80.9% with cytology triage.Self-care HPV testpositive women with VIA triage was 26.5% versus 66.2% with cytology triage.The sensitivity of HC2 test-positive women with VIA triage was 38.2% versus 92.6% with cytology triage.The sensitivity of physician-care HPV testing for CIN2+ decreased from 83.8% at ≥1.0 RLU/CO to 72.1% at ≥10.00 RLU/CO,while the sensitivity of selfcare HPV testing decreased from 72.1% at ≥1.0 RLU/CO to 32.4% at ≥10.00 RLU/CO; similar trends were seen with age-stratification.Conclusions: VIA and cytology triage improved specificity for CIN2/3 than no triage.Sensitivity with VIA triage was unsuitable for a mass-screening program.VIA provider training might improve this strategy.Cytology triage could be feasible where a high-quality cytology program exists.Triage of HPV test-positive women by increased test positivity cutoff adds another LRS triage option. | Margaret Wang Shangying Hu Shuang Zha o Wenhua Zhang Qinjing Pan Xun Zhang FengChen Jinxiu Han Junfei Ma Jennifer S.Smith Youlin Qiao Caihong Zhou Fanghui Zhao | 2017 | Chinese Journal of Cancer Research2017，29，6： | 6 |
| 5 | Solid-pseudopapillary tumor of the pancreatic tail显示文摘We report a case of the rare solid-pseudopapillary tumor of the pancreas. In contrast to other pancreatic tumors,the solid-pseudopapillary tumor has a favorable prognosis.The 60-year-old female patient we report on here was treated by left pancreatic resection combined with splenectomy for a non-metastasizing tumor of the pancreas. A solid-pseudopapillary tumor was found on histology. The patient had no signs of metastases at present.Since a microscopically invasive tumor growth is assumed,oncologically curative resection should be preferred vs the less radical enucleation. | 6 | The 4th World Landslide Forum: Landslide Research and Risk Reduction for Advancing the Culture of Living with Natural Hazards显示文摘The World Landslide Forum is a triennial mainstream conference that gathers together the scientific and technological community, policymakers, industry actors, public officials, and other stakeholders, who deal with the understanding and management of landslide disaster risk. The establishment of the ISDR-ICL Sendai Partnerships 2015–2025 for Global Promotion of Understanding and Reducing Landslide Disaster Risk in Sendai during the 2015 World Conference on Disaster Risk Reduction(WCDRR) enabled the landslide science and technology community support the implementation of the Sendai Framework for Disaster Risk Reduction 2015–2030 in order to prevent new and reduce existing disaster risk.The 4th World Landslide Forum(WLF4) was held in Ljubljana, Slovenia, from 29 May to 2 June 2017 and discussed the progress of landslide research and risk reduction for advancing the culture of living with natural hazards. A high-level panel composed of United Nations officials, international stakeholders, and national organizations sought to identify the best mechanisms to be developed by the community of the International Consortium on Landslides(ICL). The objective was to advance the implementation of the ISDR-ICL Sendai Partnerships,to achieve a better commitment among partners, and to provide substantive services to developing countries.
