1Intraoperative ERCP : 它在 laparoscopic 胆囊炎的时代有什么角色?显示文摘 In the treatment of patients with symptomatic cholelithiasis and choledocholithiasis (CBDS) detected during intraoperative cholangiography (IOC),or when the preoperative study of a patient at intermediate risk for CBDS cannot be completed due to the lack of imaging techniques required for confirmation,or if they are available and yield contradictory radiological and clinical results,patients can be treated using intraoperative endoscopic retrograde cholangiopancreatography (ERCP) during the laparoscopic treatment or postoperative ERCP if the IOC finds CBDS.The choice of treatment depends on the level of experience and availability of each option at each hospital.Intraoperative ERCP has the advantage of being a single-stage treatment and has a significant success rate,an easy learning curve,low morbidity involving a shorter hospital stay and lower costs than the two-stage treatments (postoperative and preoperative ERCP).Intraoperative ERCP is also a good salvage treatment when preoperative ERCP fails or when total laparoscopic management also fails.Luis R Rábago Alejandro Ortega Inmaculada Chico David Collado Ana Olivares Jose Luis Castro Elvira Quintanilla 2011World Journal of Gastrointestinal Endoscopy2011,3,12:13
2Pancreatic and pulmonary mast cells activation during experimental acute pancreatitis显示文摘AIM:To study the activation of pancreatic and pulmonary mast cells and the effect of mast cell inhibition on the activation of peritoneal and alveolar macrophages during acute pancreatitis.METHODS:Pancreatitis was induced by intraductal infusion of 5% sodium taurodeoxycholate in rats.The mast cell inhibitor cromolyn was administered intraperitoneally(i.p.) 30 min before pancreatitis induction.The pancreatic and pulmonary tissue damage was evaluated histologically and mast cells and their state of activation were evaluated.Peritoneal and alveolar macrophages were obtained and the expression of tumor necrosis factor α was determined.Myeloperoxidase activity was measured to evaluate the effect of mast cell inhibition on the progression of the inflammatory process.Finally,the effect of plasma on cultured mast cells or macrophages was evaluated in vitro.RESULTS:The mast cell stabilizer signif icantly reduced inflammation in the pancreas and lung and the activation of alveolar macrophages but had no effect on peritoneal macrophages.Mast cell degranulation was observed in the pancreas during pancreatitis but no changes were observed in the lung.Plasma from rats with pancreatitis could activate alveolar macrophages but did not induce degranulation of mast cells in vitro.CONCLUSION:Pancreatic mast cells play an important role in triggering the local and systemic inflammatory response in the early stages of acute pancreatitis.In contrast,lung mast cells are not directly involved in the inflammatory response related to pancreatic damage.Inmaculada Lopez-Font Sabrina Gea-Sorlí Enrique de-Madaria Luis M Gutiérrez Miguel Pérez-Mateo Daniel Closa 2010World Journal of Gastroenterology2010,16,27:10
3Z-line examination by the PillCam^(TM) SB:Prospective comparison of three ingestion protocols显示文摘AIM:To evaluate the Z-line visualization by the PillCamTM SB2 using three different ingestion protocols. METHODS:Ninety consecutive patients undergoing small bowel capsule endoscopy(SBCE)between January and May 2008 were included in the study. They swallowed the capsule in the standing(Group A= 30),supine(Group B=30)and right supine positions (Group C=30).Baseline patient characteristics, difficulties in capsule ingestion,esophageal transit times(ETT)and Z-line visualization were noted. RESULTS:No significant differences were found between the groups with regard to baseline patient characteristics,ingestion difficulties and complete SB examinations(P>0.05).At least 1 frame of the Z-line was detected in 15.8%,46.7%and 90%of patients in groups A,B and C,respectively(P<0.001).The average number of Z-line images was 0.21±0.53,3.23 ±6.59 and 5.53±7.55 and the mean%of the Z-linedetected was 71.3,25.1 and 8.3,in groups A,B and C, respectively(both P<0.001).ETT times were longer in the supine group followed by the right supine and the standing groups(median of 237 s vs 64 s and 39 s, respectively;P<0.001). 4Portal hypertensive colopathy is associated with portal hypertension severity in cirrhotic patients显示文摘AIM:To assess the prevalence of portal hypertension (PH) related colorectal lesions in liver transplant candidates, and to evaluate its association with the severity of PH.METHODS: Between October 2004 and December 2005, colonoscopy was performed in 92 cirrhotic liver transplant candidates. We described the lesions resulting from colorectal PH and their association with the grade of PH in 77 patients who underwent measurement of hepatic venous pressure gradient (HVPG).RESULTS: Mean age was 55 years and 80.7% of patients were men. The main etiology of cirrhosis was alcoholism (45.5%). Portal hypertensive colopathy (PHC) was found in 23.9%, colonic varices in 7.6% and polyps in 38% of patients (adenomatous type 65.2%). One asymptomatic patient had a well-differentiated adenocarcinoma. The manifestations of colorectal PH were not associated with the etiology of liver disease or with the Child-Pugh grade. Ninety percent of patients with colopathy presented with gastroesopha-geal varices (GEV), and 27.5% of patients with GEV presented with colopathy (P=0.12). A relationship between higher values of HVPG and presence of col-opathy was observed (19.9±6.2 mmHg vs 16.8±5.4 mmHg, P=0.045), but not with the grade of col-opathy (P=0.13). Preneoplastic polyps and neoplasm (P=0.02) and spontaneous bacterial peritonitis (P=0.006) were more prevalent in patients with colopathy. 5Management of primary achalasia: The role of endoscopy显示文摘Achalasia is an oesophageal motor disorder which leads to the functional obstruction of the lower oesophageal sphincter(LES) and is currently incurable. The main objective of all existing therapies is to achieve a reduction in the obstruction of the distal oesophagus in order to improve oesophageal transit, relieve the symptomatology, and prevent long-term complications. The most common treatments used are pneumatic dilation(PD) and laparoscopic Heller myotomy, which involves partial fundoplication with comparable shortterm success rates. The most economic non-surgical therapy is PD, with botulinum toxin injections reserved for patients with a higher surgical risk for whom the former treatment option is unsuitable. A new technology is peroral endoscopic myotomy, postulated as a possible non-invasive alternative to surgical myotomy. Other endoluminal treatments subject to research more recently include injecting ethanolamine into the LES and using a temporary self-expanding metallic stent. At present, there is not enough evidence permitting a routine recommendation of any of these three novel methods. Patients must undergo follow-up after treatment to guarantee that their symptoms are under control and to prevent complications. Most experts are in favour of some form of endoscopic follow-up, however no established guidelines exist in this respect. 6New perspectives in occult hepatitis C virus infection显示文摘Occult hepatitis C virus (HCV) infection, defined as the presence of HCV RNA in liver and in peripheral blood mononuclear cells (PBMCs) in the absence of detectable viral RNA in serum by standard assays, can be found in anti-HCV positive patients with normal serum levels of liver enzymes and in anti-HCV negative patients with persistently elevated liver enzymes of unknown etiology. Occult HCV infection is distributed worldwide and all HCV genotypes seem to be involved in this infection. Occult hepatitis C has been found not only in anti-HCV positive subjects with normal values of liver enzymes or in chronic hepatitis of unknown origin but also in several groups at risk for HCV infection such as hemodialysis patients or family members of patients with occult HCV. This occult infection has been reported also in healthy populations without evidence of liver disease. Occult HCV infection seems to be less aggressive than chronic hepatitis C although patients affected by occult HCV may develop liver cirrhosis and even hepatocellular carcinoma. Thus, anti-HCV negative patients with occult HCV may benefit from antiviral therapy with pegylated-interferon plus ribavirin. The persistence of very low levels of HCV RNA in serum and in PBMCs, along with the maintenance of specific T-cell responses against HCV-antigens observed during a long-term follow-up of patients with occult hepatitis C, indicate that occult HCV is a persistent infection that is not spontaneously eradicated. 8Alcohol use disorder and its impact on chronic hepatitis C virus and human immunodeficiency virus infections显示文摘Alcohol use disorder(AUD) and hepatitis C virus(HCV) infection frequently co-occur. AUD is associated with greater exposure to HCV infection, increased HCV infection persistence, and more extensive liver damage due to interactions between AUD and HCV on immune responses, cytotoxicity, and oxidative stress. Although AUD and HCV infection are associated with increased morbidity and mortality, HCV antiviral therapy is less commonly prescribed in individuals with both conditions. AUD is also common in human immunodeficiency virus(HIV) infection, which negatively impacts proper HIV care and adherence to antiretroviral therapy, and liver disease. In addition, AUD and HCV infection are also frequent within a proportion of patients with HIV infection, which negatively impacts liver disease. 9Improved Non-Covalent Biofunctionalization of Multi-Walled Carbon Nanotubes Using Carbohydrate Amphiphiles with a Butterfly-Like Polyaromatic Tail显示文摘We have developed an efficient strategy for the non-covalent functionalization of multi-walled carbon nanotubes(MWCNTs)which allows a biomimetic presentation of carbohydrates on their surface byπ-πstacking interactions.The strategy is based on the use of sugar-based amphiphiles functionalized with tetrabenzo[a,c,g,i]fluorene(Tbf),a polyaromatic compound with a topology that resembles a butterfly with open wings.The new carbohydrate-tethered Tbf amphiphiles have been synthesized in a straightforward manner using click chemistry.The reported method has been developed in order to improve the rather low ability of pyrene-based systems to exfoliate MWCNTs in water.By means of thermogravimetric analysis(TGA),ultraviolet(UV),infrared(IR),and fluorescence spectroscopies the interaction between MWCNTs and the Tbf group has been found to be stronger than those involving pyrene-based amphiphilic carbohydrates.The resulting aggregates with a multivalent sugar exposition on their surface are able to engage in specific ligand-lectin interactions similar to glycoconjugates on a cell membrane.Mohyeddin Assali Manuel Pernía Leal Inmaculada Fernández Pablo Romero-Gomez Rachid Baati Noureddine Khiar 2010Nano Research2010,3,11:3
10Effects of asymmetric dimethylarginine on renal arteries in portal hypertension and cirrhosis显示文摘AIM To evaluate the effects of asymmetric dimethylarginine(ADMA) in renal arteries from portal hypertensive and cirrhotic rats.METHODS Rat renal arteries from Sham(n = 15), pre-hepatic portal hypertension(PPVL; n = 15) and bile duct ligation and excision-induced cirrhosis(BDL; n = 15) were precontracted with norepinephrine, and additional contractions were induced with ADMA(10-6-10-3 mol/L), an endogenous inhibitor of nitric oxide(NO) synthase. Concentration-response curves to acetylcholine(1 × 10-9^(-3) × 10^(-6) mol/L) were determined in precontractedrenal artery segments with norepinephrine in the absence and in the presence of ADMA. Kidneys were collected to determine the protein expression and activity of dimethylarginine dimethylaminohydrolase(DDAH), an enzyme that catabolizes ADMA. RESULTS In renal arteries precontracted with norepinephrine, ADMA caused endothelium-dependent contractions. The pD 2 values to ADMA were similar in the Sham and PPVL groups(4.20 ± 0.08 and 4.11 ± 0.09, P > 0.05, respectively), but were lower than those of the BDL group(4.79 ± 0.16, P < 0.05). Acetylcholine-induced endothelium-dependent relaxation that did not differ, in terms of p D2 and maximal relaxation, among the 3 groups studied. Treatment with ADMA(3 × 10^(-4) mol/L) inhibited acetylcholine-induced relaxation in the 3 groups, but the inhibition was higher(P < 0.05) in the BDL group compared with that for the Sham and PPVL groups. The m RNA and protein expression of DDAH-1 were similar in kidneys from the three groups. Conversely, DDAH-2 expression was increased(P < 0.05) in PPVL and further enhanced(P < 0.05) in the BDL group. However, renal DDAH activity was significantly decreased in the BDL group. 14Reconstruction of lateral root formation through single-cell RNA sequencing reveals order of tissue in itiation显示文摘Postembryonic organogenesis is critical for plant development.Underground,lateral roots(LRs)form the bulk of mature root systems,yet the ontogeny of the LR primordium(LRP)is not clear.In this study,we performed the single-cell RNA sequencing through the first four stages of LR formation in Arabidopsis.Our analysis led to a model in which a single group of precursor cells,with a cell identity different from their pericycle origins,rapidly reprograms and splits into a mixed ground tissue/stem cell niche fate and a vascular precursor fate.The ground tissue and stem cell niche fates soon separate and a subset of more specialized vascular cells form sucrose transporting phloem cells that appear to connect to the primary root.We did not detect cells resembling epidermis or root cap,suggesting that outer tissues may form later,preceding LR emergence.At this stage,some remaining initial precursor cells form the primordium flanks,while the rest create a reservoir of pluripotent cells that are able to replace the LR if damaged.Laser ablation of the central and lateral LRP regions showed that remaining cells restart the sequence of tissue initiation to form a LR.Collectively,our study reveals an ontological hierarchy for LR formation with an early and sequential split of main root tissues and stem cells.Laura Serrano-Ron Pablo Perez-Garcia Alvaro Sanchez-Corrionero Inmaculada Gude Javier Cabrera Pui-Leng Ip Kenneth D.Birnbaum Miguel A.Moreno-Risueno 2021Molecular Plant2021,14,8:2

17Recovery of carbon monoxide from flue gases by reactive absorption in ionic liquid imidazolium chlorocuprate(I): Mass transfer coefficients显示文摘Recovery of carbon monoxide from flue gases by selective absorption of carbon monoxide in an imidazolium chlorocuprate(I) ionic liquid is considered in this work as an alternative to the use of molecular volatile solvents such as aromatic hydrocarbons. The present work evaluates the CO mass transfer rates from the gas phase to the ionic liquid solutions in the absence of chemical reaction. To that end, carbon dioxide was employed as an inert model gas and absorption experiments were performed to assess the influence of different process variables in a batch reactor with flat gas–liquid interface. The experimental mass transfer coefficients showed significant variation with temperature,(3.4–10.9) × 10-7m·s-1between 293 and 313 K; stirring speed,(10.2–33.1) × 10-7m·s-1between 100 and 300 r·min-1; and concentration of copper(I),(6.6–10.2) × 10-7m·s-1between 0.25 and 2 mol·L-1. 18Non-alcoholic fatty liver disease in patients with intestinal, pulmonary or skin diseases: Inflammatory cross-talk that needs a multidisciplinary approach显示文摘Non-alcoholic fatty liver disease(NAFLD)is currently considered the most common cause of liver disease.Its prevalence is increasing in parallel with the obesity and type 2 diabetes mellitus(DM2)epidemics in developed countries.Several recent studies have suggested that NAFLD may be the hepatic manifestation of a systemic inflammatory metabolic disease that also affects other organs,such as intestine,lungs,skin and vascular endothelium.It appears that local and systemic proinflammatory/anti-inflammatory cytokine imbalance,together with insulin resistance and changes in the intestinal microbiota,are pathogenic mechanisms shared by NAFLD and other comorbidities.NAFLD is more common in patients with extrahepatic diseases such as inflammatory bowel disease(IBD),obstructive syndrome apnea(OSA)and psoriasis than in the general population.Furthermore,there is evidence that this association has a negative impact on the severity of liver lesions.Specific risk characteristics for NAFLD have been identified in populations with IBD(i.e.age,obesity,DM2,previous bowel surgery,IBD evolution time,methotrexate treatment),OSA(i.e.obesity,DM2,OSA severity,increased transaminases)and psoriasis(i.e.age,metabolic factors,severe psoriasis,arthropathy,elevated transaminases,methotrexate treatment).These specific phenotypes might be used by gastroenterologists,pneumologists and dermatologists to create screening algorithms for NAFLD.Such algorithms should include non-invasive markers of fibrosis used in NAFLD to select subjects for referral to the hepatologist.Prospective,controlled studies in NAFLD patients with extrahepatic comorbidities are required to demonstrate a causal relationship and also that appropriate multidisciplinary management improves these patients’prognosis and survival.Mercedes Perez-Carreras Begoña Casis-Herce Raquel Rivera Inmaculada Fernandez Pilar Martinez-Montiel Victoria Villena 2021World Journal of Gastroenterology2021,27,41:2
