3Thoracic impalement injury: A survivor with large metallic object in-situ显示文摘Impalement injuries, is a severe form of trauma, which are not common in civilian life. These injuries rarely occurs in major accidents. Abdomen, chest, limbs and perineum are often involved due to their large surface area. Thoracic impalement injury is usually a fatal injury, due to location of major vessels and heart in the thoracic cavity. These injuries are horrifying to site, but the patients who are lucky enough to make it to hospital, usually survive. Chances of survival are larger in right sided impalement injuries while central injuries are always died at the scene.Our patient, 25 years old male, was brought to the emergency room (ER) with large impaled metallic bar (about 2.5 feet long) in situ, in right sided chest. The patient was immediately shifted to operation room (OR) and was operated, his recovery was uneventful without any sequelae.Such patients should be treated and resuscitated according to advanced trauma life support (ATLS) protocols and operated without any delay for further investigations. Such operations are carried out by the most experienced surgeon team available. The impaled objects should not be processed if not necessary to avoid major hemorrhage and damage to vital structures, until the patient is in operation room. Large size and unusual position of impaled objects, makes the job difficult for surgeons/anesthetists.Although horrifying at scene, patients with thoracic impalement injuries are mostly young and healthy, and those who survive the pre-hospital phase are potentially manageable with proper resuscitation. 4Giardiasis in patients with dyspeptic symptoms显示文摘AIM: To investigate the prevalence of giardiasis in patients with dyspeptic symptoms.METHODS: Clinical records of consecutive patients who attended Gastroenterology Department at Aga Khan University Hospital from January 2000 to June 2003 and had esophagogastroduodenoscopy (EGD) with duodenal biopsies and international classification of diseases 9th revision with clinical modifications (ICD-9-CM) coded with giardiasis were studied.RESULTS: Two hundred and twenty patients fulfilled the above criteria. There were 44% (96/220) patients who were giardiasis positive, 72% (69/96) of them were males and 28% (27/96) of them were females.There were 65% (81/124) males and 35% (43/124)females who were giardiasis negative. The mean age of patients with giardiasis was 28±17 years, while that of giardiasis negative patients was 40±18 years (P<0.001).In patients with giardiasis, abdominal pain was present in 71% (68/96) of patients (P = 0.02) and diarrhea in 29% (28/96) (P = 0.005); duodenitis in 25% (24/96) on EGD (P = 0.006) and in 68% (65/96) on histopathology (P = 0.002).CONCLUSION: Giardiasis occurs significantly in young people with abdominal pain, while endoscopic duodenitis is seen in only 25% of giardiasis positive cases, which supports routine duodenal biopsy.Javed Yakoob Wasim Jafri Shahab Abid Nadim Jafri Saeed Hamid Hasnain Ali Shah Lubna Rizvi Muhammad Islam Hizbullah Shaikh 2005World Journal of Gastroenterology2005,11,42:0
5GIS-based assessment of land-agroforestry potentiality of Jharkhand State, India显示文摘Scaling-up agroforestry area in various forms is a scientific path towards achieving various sustainable development goals(SDGs),especially improving livelihood,reducing poverty,conserving environment and biodiversity,and transforming climate change.In this study,the effort was made to investigate the land potentiality for agroforestry at the district level in Jharkhand State,India by applying geographic information system(GIS)modeling technology using climate(temperature and precipitation),topography(slope and elevation),ecology(percent tree cover and normalized difference vegetation index(NDVI)),and social economics(poverty rate and tribal dominance)factors.The results revealed that six districts of Jharkhand State had agroforestry potential greater than 60.00%.The highest agroforestry suitability was found in Simdega District(78.20%),followed by Pakur(76.25%),West Singhbhum(72.70%),Dumka(68.84%),Sahibganj(64.63%),and Godda(63.43%)districts.Additionally,we identified 513 out of 32,620 villages of Jharkhand State potentially suitable(agroforestry suitability≥80.00%)for agroforestry with the objective of life improvement among marginalized society.Under the outside forest area,8.58%of the total geographical land of Jharkhand State was wasteland,much of which was found suitable for agroforestry practices.The agroforestry setups in those wastelands can absorb 637 t carbon annually in long run and can provide direct economic benefits to the locals besides additional income for carbon emission reduction.This study concluded that Jharkhand State has plenty of high potential land for agroforestry,and adoption of agroforestry at the village level must be given high priority.This study could guide the nodal authorities to prepare appropriate strategies for scaling the tree cover in agroforestry systems in village-level landscape planning which needs policy attention and investment for achieving 9 out of the 17 SDGs.Firoz AHMAD Nazimur Rahman TALUKDAR Laxmi GOPARAJU Chandrashekhar BIRADAR Shiv Kumar DHYANI Javed RIZVI 2022Regional Sustainability2022,3,3:0
