1Two-stage vs single-stage management for concomitant gallstones and common bile duct stones显示文摘AIM:To evaluate the safety and effectiveness of twostage vs single-stage management for concomitant gallstones and common bile duct stones.METHODS:Four databases,including PubMed,Embase,the Cochrane Central Register of Controlled Trials and the Science Citation Index up to September 2011,were searched to identify all randomized controlled trials(RCTs).Data were extracted from the studies by two independent reviewers.The primary outcomes were stone clearance from the common bile duct,postoperative morbidity and mortality.The secondary outcomes were conversion to other procedures,number of procedures per patient,length of hospital stay,total operative time,hospitalization charges,patient acceptance and quality of life scores.RESULTS:Seven eligible RCTs [five trials(n = 621) comparing preoperative endoscopic retrograde cholangiopancreatography(ERCP)/endoscopic sphincterotomy(EST) + laparoscopic cholecystectomy(LC) with LC + laparoscopic common bile duct exploration(LCBDE);two trials(n = 166) comparing postoperative ERCP/EST + LC with LC + LCBDE],composed of 787 patients in total,were included in the final analysis.The metaanalysis detected no statistically significant difference between the two groups in stone clearance from the common bile duct [risk ratios(RR) =-0.10,95% confidence intervals(CI):-0.24 to 0.04,P = 0.17],postoperative morbidity(RR = 0.79,95% CI:0.58 to 1.10,P = 0.16),mortality(RR = 2.19,95% CI:0.33 to 14.67,P = 0.42),conversion to other procedures(RR = 1.21,95% CI:0.54 to 2.70,P = 0.39),length of hospital stay(MD = 0.99,95% CI:-1.59 to 3.57,P = 0.45),total operative time(MD = 12.14,95% CI:-1.83 to 26.10,P = 0.09).Two-stage(LC + ERCP/EST) management clearly required more procedures per patient than single-stage(LC + LCBDE) management.CONCLUSION:Single-stage management is equivalent to two-stage management but requires fewer procedures.However,patient's condition,operator's expertise and local resources should be taken into account in making treatment decisions.Jiong Lu Yao Cheng Xian-Ze Xiong Yi-Xin Lin Si-Jia Wu Nan-Sheng Cheng 2012World Journal of Gastroenterology2012,18,24:107
2Decreased osteogenesis of adult mesenchymal stem cel s by reactive oxygen species under cyclic stretch: a possible mechanism of age related osteoporosis显示文摘Age related defect of the osteogenic differentiation of mesenchymal stem cells(MSCs) plays a key role in osteoporosis. Mechanical loading is one of the most important physical stimuli for osteoblast differentiation.Here, we compared the osteogenic potential of MSCs from young and adult rats under three rounds of 2 h of cyclic stretch of 2.5% elongation at 1 Hz on 3 consecutive days. Cyclic stretch induced a significant osteogenic differentiation of MSCs from young rats, while a compromised osteogenesis in MSCs from the adult rats.Accordingly, there were much more reactive oxygen species(ROS) production in adult MSCs under cyclic stretch compared to young MSCs. Moreover, ROS scavenger N-acetylcysteine rescued the osteogenic differentiation of adult MSCs under cyclic stretch. Gene expression analysis revealed that superoxide dismutase 1(SOD1) was significantly downregulated in those MSCs from adult rats. 4Dysregulation of the miR-146a-Smad4 axis impairs osteogenesis of bone mesenchymal stem cells under inflammation显示文摘Osteoporosis is a common disease that affects patient quality of life, especially among the elderly population.Although inflammation contributes significantly to osteoporosis, the underlying mechanism is unclear. In this study, we found that tumor necrosis factor(TNF)-α, an inflammatory environment mimic, inhibits osteogenesis of bone mesenchymal stem cells(BMSCs), induces mi R-146 a and decreases Smad4. Moreover,overexpression of mi R-146 a inhibited the osteogenic ability of BMSCs, whereas blocking mi R-146 a partially rescued the osteogenesis deficiency under TNF-α treatment. Molecularly, mi R-146 a decreased Smad4 expression at the protein level by binding to an element located in the Smad4 30-untranslated region, and restoration of Smad4 reversed the inhibitory effects of mi R-146 a on osteogenesis. 6Single-incision laparoscopic appendectomy vs conventional laparoscopic appendectomy:Systematic review and meta-analysis显示文摘AIM: To assess the differences in clinical benefits and disadvantages of single-incision laparoscopic appendectomy(SILA) and conventional laparoscopic appendectomy(CLA).METHODS: The Cochrane Library,MEDLINE,Embase,Science Citation Index Expanded,and Chinese Biomedical Literature Database were electronically searched up through January 2013 to identify randomized controlled trails(RCTs) comparing SILA with CLA.Data was extracted from eligible studies to evaluate the pooled outcome effects for the total of 1068 patients.The meta-analysis was performed using Review Manager 5.2.0.For dichotomous data and continuous data,the risk ratio(RR) and the mean difference(MD) were calculated,respectively,with 95%CI for both.For continuous outcomes with different measurement scales in different RCTs,the standardized mean difference(SMD) was calculated with 95%CI.Sensitivity and subgroup analyses were performed when necessary.RESULTS: Six RCTs were identified that compared SILA(n = 535) with CLA(n = 533).Five RCTs had a high risk of bias and one RCT had a low risk of bias.SILA was associated with longer operative time(MD = 5.68,95%CI: 3.91-7.46,P < 0.00001),higher conversion rate(RR = 5.14,95%CI: 1.25-21.10,P = 0.03) and better cosmetic satisfaction score(MD = 0.52,95%CI: 0.30-0.73,P < 0.00001) compared with CLA.No significant differences were found for total complications(RR = 1.15,95%CI: 0.76-1.75,P = 0.51),drain insertion(RR = 0.72,95%CI: 0.41-1.25,P = 0.24),or length of hospital stay(SMD = 0.04,95%CI:-0.08-0.16,P = 0.57).Because there was not enough data among the analyzed RCTs,postoperative pain was not calculated.CONCLUSION: The benefit of SILA is cosmetic satisfaction,while the disadvantages of SILA are longer operative time and higher conversion rate.Yu-Long Cai Xian-Ze Xiong Si-Jia Wu Yao Cheng Jiong Lu Jie Zhang Yi-Xin Lin Nan-Sheng Cheng 2013World Journal of Gastroenterology2013,19,31:8

8Admissibility analysis and control synthesis for switched linear singular systems显示文摘The problem of admissibility analysis and control synthesis of discrete-time switched linear singular (SLS) systems for arbitrary switching laws is solved.By using the switched Lyapunov function approach,some new sufficient conditions under which the SLS system is admissible for arbitrary switching laws are derived in terms of linear matrix inequalities(LMIs).Based on the admissibility results,control synthesis is then to design switched state feedback and static output feedback controllers,guaranteeing that the resulting closed-loop system is admissible.The presented results can be viewed as the extensions of previous works on switched Lyapunov function approach from the regular switched systems to singular switched cases.Examples are provided to demonstrate the reduced conservatism and effectiveness of the proposed conditions.Lin Jinxing Fei Shumin Shen Jiong 2009Journal of Systems Engineering and Electronics2009,20,5:4

10Robust stability and stabilization for uncertain discrete-time switched singular systems with time-varying delays显示文摘The problems of robust stability and stabilization via memoryless state feedback for a class of discrete-time switched singular systems with time-varying delays and linear fractional uncertainties are investigated.By constructing a novel switched Lyapunov-Krasovskii functional,a delay-dependent criterion for the unforced system to be regular,causal and uniformly asymptotically stable is established in terms of linear matrix inequalities(LMIs).An explicit expression for the desired memoryless state feedback stabilization controller is also given.The merits of the proposed criteria lie in their less conservativeness and relative simplicity,which are achieved by considering additionally useful terms(ignored in previous methods) when estimating the upper bound of the forward difference of the Lyapunov-Krasovskii functional and by avoiding utilizing any model augmentation transformation.Some numerical examples are provided to illustrate the validity of the proposed methods.Jinxing Lin Shumin Fei Jiong Shen 2010Journal of Systems Engineering and Electronics2010,21,4:2
11Carbon dioxide insufflation for endoscopic retrograde cholangiopancreatography:A meta-analysis and systematic review显示文摘AIM:To assess the safety and efficacy of carbon dioxide (CO2) insufflation during endoscopic retrograde cholangiopancreatography (ERCP). METHODS:The Cochrane Library, Medical Literature Analysis and Retrieval System Online, Excerpta Medica Database, Science Citation Index Expanded, Chinese Biomedical Literature Database, and references in relevant publications were searched up to December 2011 to identify randomized controlled trials (RCTs) comparing CO2 insufflation with air insufflation during ERCP. The trials were included in the review irrespec-tive of sample size, publication status, or language. Study selection and data extraction were performed by two independent authors. The meta-analysis was performed using Review Manager 5.1.6. A random-effects model was used to analyze various outcomes.Sensitivity and subgroup analyses were performed if necessary. R ESULTS:Seven double-blind RCTs involving a total of 818 patients were identified that compared CO2 insufflation (n = 404) with air insufflation (n = 401) during ERCP. There were a total of 13 post-random- ization dropouts in four RCTs. Six RCTs had a high risk of bias and one had a low risk of bias. None of the RCTs reported any severe gas-related adverse events in either group. A meta-analysis of 5 RCTs (n = 459) indicated that patients in the CO2 insufflation group had less post-ERCP abdominal pain and distension for at least 1 h compared with patients in the air insuf-flation group. There were no significant differences in mild cardiopulmonary complications [risk ratio (RR) = 0.43, 95% CI:0.07-2.66, P = 0.36], cardiopulmonary (e.g., blood CO2 level) changes [standardized mean difference (SMD) = -0.97, 95% CI: -2.58-0.63, P = 0.23], cost analysis (mean difference = 3.14, 95% CI:-14.57-20.85, P = 0.73), and total procedure time (SMD = -0.05, 95% CI:-0.26-0.17, P = 0.67) between the two groups. 14Kupffer cells contribute to concanavalin A-induced hepatic injury through a Th1 but not Th17 type response-dependent pathway in mice显示文摘BACKGROUND:Increasing evidence suggests that a close interaction of Kupffer cells with T cells plays a central role in concanavalin A-induced hepatic injury in mice,but the underlying mechanisms remain obscure.The present study aimed to determine the relative roles of Th1 and Th17 type responses in concanavalin A-induced hepatic injury in mice, and to investigate whether or not Kupffer cells contribute to hepatic injury via a Th1 or Th17 type response-dependent pathway. METHODS:Immune-mediated hepatic injury was induced in C57BL/6 mice by intravenous injection of concanavalin A. Kupffer cells were inactivated by pretreatment with gadolinium chloride 24 hours before the concanavalin A injection.The interferon-gamma(IFN-γ)and interleukin-17(IL-17)pathways were blocked by specific neutralizing antibodies.Hepatic injury was assessed using serum transferase activity and pathological analysis.Expression of inflammatory cytokines within the liver was detected by real-time polymerase chain reaction and immunohistochemistry. 15Mouse neural stem cells cultured in vitro and expressing an exogenous gene显示文摘Neural stem cells are the multipotential, self-renewing cells in central nerve system, and play an essential role in the development and differentiation of nerve system. Neural stem cells can be used to treat the nerve system diseases, especially, the transplantation of neural stem cells to rescue the degenerated neural cells has become a very promising therapeutic way. We successfully cultured neural stem cells isolated from the brains of embryonic mice in vitro and determined their distribution in the E17 mice brains. The neural stem cells were transfected with adenoviral vector carrying GFP (green fluorescence protein) gene and then highly expressed the exogenous gene. 16Associations of Daytime Napping with Incident Cardiovascular Diseases and Hypertension in Chinese Adults:A Nationwide Cohort Study显示文摘Objective This study aimed to examine the associations of daytime napping with incident risks of cardiovascular diseases(CVDs)and hypertension(HTN).Methods Data for napping and CVD outcomes in 25 provinces were collected from baseline(2010)and three waves of follow-up(2012-2017)investigations of the China Family Panel Studies.Cox frailty models with random intercepts for the surveyed provinces were used to assess the longitudinal effects of daytime napping on CVD and HTN.Results Compared with non-nappers,30+min nappers had higher risks of CVD and HTN,while no significant associations were observed among<30 min nappers.Incident risks among 30-to<60-min nappers increased by 22%[hazard ratio(HR)1.22,95% confidence interval(CI)1.08-1.39]for CVD and 21%(1.21,1.04-1.41)for HTN,respectively,with corresponding HRs of CVD and HTN of 1.27(1.09-1.47)and 1.38(1.16-1.65)among≥60 min nappers.Nap-associated CVD risks varied by subgroups,with stronger associations in participants with lower body mass index(<24 kg/m^(2)),physically inactive persons,smokers,and participants with longer nighttime sleep(≥7 h/night).Significant effects of daytime napping were observed on rural and northern residents only,highlighting great regional variations in CVD risks associated with napping habits.Conclusions This cohort study revealed strong evidence that long daytime napping(≥30 min)is associated with an increased incidence of cardiovascular events.WANG Lu WANG Ke LIU Lin Jiong ZHANG Yuan Yuan SHU Hai Nan WANG Kai ZHOU Pei Xuan ZHANG Yun Quan 2022Biomedical and Environmental Sciences2022,35,1:2

18PolyvasculaR Evaluation for Cognitive Impairment and vaScular Events (PRECISE)-a population-based prospective cohort study: rationale, design and baseline participant characteristics显示文摘Background and purpose Cardiovascular diseases and dementia are two major diseases in the elderly.Atherosclerosis is associated with future vascular events and cognitive impairment.The PolyvasculaR Evaluation for Cognitive Impairment and vaScular Events(PRECISE)study is a population-based prospective cohort study with comprehensive evaluation of multiterritorial artery stenosis and plaque using advanced vascular imaging techniques and prospective collection of vascular events and cognitive assessments.Methods Between May 2017 and September 2019,the PRECISE study enrolled 3067 community-dwelling adults with ages between 50 and 75 years cluster sampled from six villages and four communities of Lishui city in China.Data are collected in face-to face interviews at baseline,2-year and 4-year follow-up visits.Brain MRI including high-resolution sequences for intracranial and carotidal arteries and CT angiography for thoracoabdominal arteries were performed at baseline and will be rescanned after 4 years.Cardiovascular/cerebrovascular events and cognitive assessment will be prospectively collected after the enrollment.Blood and urine samples were collected and biomarkers were tested at baseline.Results A total of 3067 subjects were enrolled,among which 53.5%were female with an average age of 61.2±6.7 years.Among them,2.8%,8.1%,43.1%and 21.6%had a history of stroke,coronary heart diseases,hypertension and diabetes mellitus,respectively.Conclusions The PRECISE study is a population-based prospective cohort study with comprehensive evaluation of atherosclerotic stenosis and plaque using advanced vascular imaging techniques.Data from this cohort provide us an opportunity to precisely evaluate polyvascular atherosclerosis and its association with future vascular events and cognitive impairment.Yuesong Pan Jing Jing Xueli Cai Yilong Wang Suying Wang Xia Meng Chunlai Zeng Jiong Shi Jiansong Ji Jinxi Lin Lingchun Lyu Zhe Zhang Lerong Mei Shiping Li Shan Li Wanlin Zhu Hao Li Tiemin Wei Yongjun Wang 2021Stroke & Vascular Neurology2021,6,1:1
