| 1 | Comprehensive management of full-thickness luminal defects: The next frontier of gastrointestinal endoscopy显示文摘Full thickness gastrointestinal defects such as perforations, leaks, and fistulae are a relatively common result of many of the endoscopic and surgical procedures performed in modern health care. As the number of these procedures increases, so too will the number of resultant defects. Historically, these were all treated by open surgical means with the associated morbidity and mortality. With the recent advent of advanced endoscopic techniques, these defects can be treated definitively while avoiding an open surgical procedure. Here we explore the various techniques and tools that are currently available for the treatment of gastrointestinal defects including through the scope clips, endoscopic suturing devices, over the scope clips, sealants, endoluminal stents, endoscopic suction devices, and fistula plugs. As fistulae represent the most recalcitrant of defects, we focus this editorial on a multimodal approach of treatment. This includes optimization of nutrition, treatment of infection, ablation of tracts, removal of foreign bodies, and treatment of distal obstructions. We believe that by addressing all of these factors at the time of attempted closure, the patient is optimized and has the best chance at longterm closure. | 2 | Preservation solutions used during abdominal transplantation:Current status and outcomes显示文摘Organ preservation remains an important contributing factor to graft and patient outcomes. During donor organ procurement and transportation, cellular injury is mitigated through the use of preservation solutions in conjunction with hypothermia. Various preservation solutions and protocols exist with widespread variability among transplant centers. In this review of abdominal organ preservation solutions, evolution of transplantation and graft preservation are discussed followed by classification of preservation solutions according to the composition of electrolytes, impermeants, buffers, antioxidants, and energy precursors. Lastly, pertinent clinical studies in the setting of hepatic, renal, pancreas, and intestinal transplantation are reviewed for patient and graft survival as well as financial considerations. In liver transplants there may be some benefit with the use of histidine-tryptophan-ketoglutarate(HTK) over University of Wisconsin solution in terms of biliary complications and potential cost savings. Renal grafts may experience increased initial graft dysfunction with the use of Euro-Collins thereby dissuading its use in support of HTK which can lead to substantial cost savings. University of Wisconsin solution and Celsior are favored in pancreas transplants given the concern for pancreatitis and graft thrombosis associated with HTK. No difference was observed with preservation solutions with respect to graft and patient survival in liver, renal, and pancreas transplants. Studies involving intestinal transplants are sparse but University of Wisconsin solution infused intraluminally in combination with an intra-vascular washout is a reasonable option until further evidence can be generated. | 3 | Lubiprostone used with polyethylene glycol in diabetic patients enhances colonoscopy preparation quality显示文摘AIM: To assess the additive effect of lubiprostone on the quality of colon preparation in diabetics given single-dosed polyethylene glycol electrolyte(PEG)for colonoscopy.METHODS:This was an investigator-initiated,singlecenter,single-blinded prospective trial comparing the efficacy of L+PEG to PEG alone on colon preparation quality in diabetics undergoing screening colonoscopy.The study was approved by our institution’s IRB.The PEG was given as a single-dose to address patientcompliance concerns voiced by our IRB with splitdosing.All patients received only clear liquids the day prior to colonoscopy.Experimental group(Grp L)received PEG+1 dose L 2 h prior to and 2 h after PEG completion.Control group(Grp C)received only PEG the evening prior to the colonoscopy.Patients were randomly assigned to one of the 2 groups.The endoscopist was blinded to which colon prep was given and all colonoscopies were complete.Upon colonoscopy completion,the endoscopist rated the colon prep-quality by a validated 5-point Likert scale(1-excellent to 5-inadequate).RESULTS:Sixty patients were enrolled in the study;30 Grp L and 30 Grp C.Overall,patients were excluded due to study non-completion in 12(41%)Grp L and 5(17%) Grp C,P=0.04.Average colon preparation score Grp L =2.47 and Grp C=3.00,P=0.09.Although this was not statistically significant,there was a trend towards improved colon prep in Grp L.Statistical significance may have been achieved if completion rates had been similar between both study groups.CONCLUSION:Use of 2-L capsules with PEG resulted in a trend towards improved colon prep over PEG alone in diabetic patients when given as a single-dose regimen. | Erika Grigg Moonkyung C Schubert Joshua Hall Fadi Rahhal Dimple Raina Subbaramiah Sridhar Sherman M Chamberlain | 2010 | World Journal of Gastrointestinal Endoscopy2010,2,7: | 5 |

| 5 | Computer applications for selecting operating parameters of stationary grain crop thresher显示文摘In this study,various operating parameters influencing performance of stationary grain crop threshers were established.These parameters were deduced from the established analytical models describing the underlying principles for the crop characteristics and machine variables as factors influencing the overall machine performance of a stationary multi-crop thresher.A computer programme written in Visual Basic was used to select optimum operating performance of the threshing process in a stationary tooth–peg grain crop thresher.An IITA-popularized stationary multi-crop thresher was used to test the practical feasibility of the computer based output of the threshing process.A split–split–unit statistical design was used for data collection.The data collected were analyzed using the GENSTAT 5 statistical package with its computer programme.The results showed that graphs of data from measured thresher performance indices against the predicted data for all the established models indicated high correlation between the models and the measured data at p≤5%significance level.The minimum energy requirements for detachment of sorghum and rice were observed at the threshing cylinder speed of 500 r/min(10.5 m/s)and 615 r/min(13.0 m/s),respectively.The combination of the threshing cylinder speed of 500 r/min(10.5 m/s)and 615 r/min(13.0 m/s)at crop moisture content of 12.8%and 16.2%indicated optimum threshing conditions for sorghum and rice,respectively. | Joshua Olanrewaju Olaoye Kayode C Oni Mary M Olaoye | 2010 | International Journal of Agricultural and Biological Engineering2010,3,3: | 3 |
| 6 | Heparin resistance in severe thermal injury:a prospective cohort study显示文摘Background:Low molecular-weight heparin(LMWH)is routinely administered to burn patients for thromboprophylaxis.Some studies have reported heparin resistance,yet the mechanism(s)and prevalence have not been systematically studied.We hypothesized that nucleosomes,composed of histone structures with associated DNA released from injured tissue and activated immune cells in the form of neutrophil extracellular traps(NETs or NETosis),neutralize LMWH resulting in suboptimal anticoagulation,assessed by reduction in anti-factor Xa activity.Methods:Blood was sampled from>15%total body surface area(TBSA)burn patients receiving LMWH on days 5,10 and 14.Peak anti-factor Xa(AFXa)activity,anti-thrombin(ATIII)activity,cellfree DNA(cfDNA)levels and nucleosome levels were measured.Mixed effects regression was adjusted for multiple confounders,including injury severity and ATIII activity,and was used to test the association between nucleosomes and AFXa.Results:A total of 30 patients with severe burns were included.Mean TBSA 43%(SD 17).Twentythree(77%)patients were affected by heparin resistance(defined by AFXa activity<0.2 IU/mL).Mean peak AFXa activity across samples was 0.18 IU/mL(SD 0.11).Mean ATIII was 81.9%activity(SD 20.4).Samples taken at higher LWMH doses were found to have significantly increased AFXa activity,though the effect was not observed at all doses,at 8000 IU no samples were heparin resistant.Nucleosome levels were negatively correlated with AFXa(r=−0.29,p=0.050)consistent with the hypothesis.The final model,with peak AFXa as the response variable,was adjusted for nucleosome levels(p=0.0453),ATIII activity(p=0.0053),LMWH dose pre-sample(p=0.0049),drug given(enoxaparin or tinzaparin)(p=0.03),and other confounders including severity of injury,age,gender,time point of sample.Conclusions:Heparin resistance is a prevalent issue in severe burns.Nucleosome levels were increased post-burn,and showed an inverse association with AFXa consistent with the hypothesis that they may interfere with the anticoagulant effect of heparin in vivo and contribute to heparin resistance.Accurate monitoring of AFXa activity with appropriate therapy escalation plans are recommended with dose adjustment following severe burn injury. | Liam D Cato Benjamin Bailiff Joshua Price Christos Ermogeneous Jon Hazeldine William Lester Gillian Lowe Christopher Wearn Jonathan RB Bishop Janet M Lord Naiem Moiemen Paul Harrison | 2021 | Burns & Trauma2021,9,1: | 3 |
| 7 | Effect of bone loss in anterior shoulder instability显示文摘Anterior shoulder instability with bone loss can be a difficult problem to treat. It usually involves a component of either glenoid deficiency or a Hill-Sachs lesion. Recent data shows that soft tissue procedures alone are typically not adequate to provide stability to the shoulder. As such, numerous surgical procedures have been described to directly address these bony deficits. For glenoid defects, coracoid transfer and iliac crest bone block procedures are popular and effective. For humeral head defects, both remplissage and osteochondral allografts have decreased the rates ofrecurrent instability. | 10 | Evaluation and treatment of internal impingement of the shoulder in overhead athletes显示文摘One of the most common pathologic processes seen in overhead throwing athletes is posterior shoulderpain resulting from internal impingement. 'Internal impingement' is a term used to describe a constellation of symptoms which result from the greater tuberosity of the humerus and the articular surface of the rotator cuff abutting the posterosuperior glenoid when the shoulder is in an abducted and externally rotated position. The pathophysiology in symptomatic internal impingement is multifactorial,involving physiologic shoulder remodeling,posterior capsular contracture,and scapular dyskinesis. Throwers with internal impingement may complain of shoulder stiffness or the need for a prolonged warm-up,decline in performance,or posterior shoulder pain. On physical examination,patients will demonstrate limited internal rotation and posterior shoulder pain with a posterior impingement test. Common imaging findings include the classic 'Bennett lesion' on radiographs,as well as articular-sided partial rotator cuff tears and concomitant SLAP lesions. Mainstays of treatment include intense non-operative management focusing on rest and stretching protocols focusing on the posterior capsule. Operative management is variable depending on the exact pathology,but largely consists of rotator cuff debridement. | 11 | Evolving roles of magnifying endoscopy and endoscopic resection for neoplasia in inflammatory bowel diseases显示文摘Magnifying endoscopy is a useful technique to differentiate neoplasia from non-neoplastic lesions. Data regarding the clinical utility of magnifying endoscopy for neoplasia in patients with inflammatory bowel disease(IBD) has been emerging.While Kudo’s pit pattern types Ⅲ-Ⅴ are findings suggestive of neoplasia in non-IBD patients, these pit patterns are predictive of IBD-associated neoplasia as well.However, active chronic inflammatory processes, particularly regenerative changes, can mimic neoplastic pit patterns and may affect a meticulous evaluation of pit pattern diagnosis in patients with IBD. The clinical evidence regarding the utility of magnifying endoscopy with narrow band imaging or endocytoscopy has also been evolving in regard to the diagnosis of IBD-associated neoplasia. These advanced endoscopic techniques are promising for multiple reasons;not only for making an accurate diagnosis of neoplasia, but also in determining if endoscopic resection is appropriate for such lesions in patients with IBD.
