| 1 | Factors influencing subjective symptoms in dry eye disease显示文摘AIM: To retrospectively investigate the association between dry eye symptoms and clinical or in vivo confocal microscopy parameters in patients with dry eye disease(DED), and to compare these parameters between eyes with DED and normal subjects.METHODS: This retrospective, cross-sectional, controlled study comprised 25 consecutive patients with non-Sj?gren dry eye disease and age-and sex-matched 25 healthy subjects. Each patient underwent a complete examination of the ocular surface in the following order: tear osmolarity measurements, InflammaDry test, tear break-up time, corneal fluorescein staining, Schirmer I test, subjective symptoms questionnaire using the dry eye-related qualityof-life score(DEQS), and in vivo confocal microscopy analysis of the central cornea. Beck depression inventory(BDI) as depressive scale and history of medications and smoking were also evaluated. Stepwise multiple regression analysis was used to assess the factors affecting the DEQS.RESULTS: In univariate analysis, DEQS was associated with tear break-up time(ρ=-0.48, P=0.01), oral medications, such as hypotensive drug(ρ=0.56, P=0.004) and antidepressant(ρ=0.57, P=0.003), and BDI(ρ=0.61, P=0.001) in patients with DED. In multiple regression analysis, explanatory variables relevant to the DEQS were the anti-depressant medications(P=0.04, partial regression coefficient B=21.04) and BDI(P=0.02, B=0.76, adjusted R2=0.54) in these patients. CONCLUSION: Our study shows a significant association between depression and dry eye symptoms. | 2 | Association of primary biliary cirrhosis with idiopathic thrombocytopenic purpura显示文摘Although both primary biliary cirrhosis (PBC) and idiopathic thrombocytopenic purpura (ITP) are autoimmune diseases, the association of the 2 diseases is rare. Here, we report a case of ITP that developed during the follow-up of PBC in a 74-year- old man. The patient had been diagnosed with PBC 12 years previously, and had received treatment with ursodeoxycholic acid. The platelet count decreased from approximately 60 × 109/L to 8 × 109/L, and the association of decompensated liver cirrhosis (PBC) with ITP was diagnosed. Steroid and immune gamma globulin therapy were successful in increasing the platelet count. Interestingly, human leukocyte antigen genotyping detected the alleles DQB10601 and DRB10803, which are related to both PBC and ITP in Japanese patients. | 3 | Screening for hilar biliary invasion in ampullary cancer patients显示文摘BACKGROUND The treatment for ampullary cancer is pancreatoduodenectomy or local ampullectomy.However,effective methods for the preoperative investigation of hilar biliary invasion in ampullary cancer patients have not yet been identified.AIM To determine the necessity of and an appropriate method for investigating hilar biliary invasion of ampullary cancer.METHODS Among 43 ampullary cancer patients,34 underwent endoscopic treatment(n=9)or surgery(n=25).The use of imaging findings(thickening and enhancement of the bile duct wall on contrast-enhanced computed tomography,irregularity on endoscopic retrograde cholangiography,thickening of the entire bile duct wall on intraductal ultrasonography(IDUS),and partial thickening of the bile duct wall on IDUS)and biliary biopsy results for diagnosing hilar biliary invasion of ampullary cancer was compared.RESULTS Hilar invasion was not observed in every patient.Among the patients who did not undergo biliary stent insertion,the combination of partial thickening of the bile duct wall on IDUS and biliary biopsy results showed the highest accuracy(100%)for diagnosing hilar biliary invasion.However,each imaging method and biliary biopsy yielded some false-positive results.CONCLUSION Although some false-positive results were obtained with each method,the combination of partial thickening of the bile duct wall on IDUS and biliary biopsy results was useful for diagnosing hilar biliary invasion of ampullary cancer.However,hilar invasion of ampullary cancer is rare;therefore,the investigation of hilar biliary invasion of ampullary cancer might be unnecessary. | Tadayuki Takagi Mitsuru Sugimoto Rei Suzuki Naoki Konno Hiroyuki Asama Yuki Sato Hiroki Irie Jun Nakamura Mika Takasumi Minami Hashimoto Tsunetaka Kato Ryoichiro Kobashi Takumi Yanagita Yuko Hashimoto Shigeru Marubashi Takuto Hikichi Hiromasa Ohira | 2022 | World Journal of Gastrointestinal Endoscopy2022,14,9: | 2 |

| 6 | Development of gradient microstructure in the lattice structure of AlSi10Mg alloy fabricated by selective laser melting显示文摘To identify the microstructural features of the lattice structures of Al alloys built via the selective laser melting(SLM)process,AlSil OMg alloy with a body-centered cubic(BCC)-type lattice structure was prepared.Characteristic microstructures comprising melt pools with several columnarα-Al phases with<001>orientations along the elongation direction and surrounded by eutectic Si particles were observed at all portions of the built lattice structure.In the node portions of the lattice structure,a gradient microstructure(continuous change in microstructure)was observed.The columnarα-Al phases were observed near the top surface of the node portion,whereas they became coarser and more equiaxed near the bottom surface,resulting in softening localized near the bottom surface.In the strut portions of the lattice structure,the columnarα-Al phases were elongated along the inclined direction of struts.This trend was more prevalent near the bottom surface.Theα-Al phases became coarser and more equiaxed near the bottom surface as well.The aforementioned results were the basis of a discussion of the development of the gradient microstructure in lattice-structured Al alloys during the SLM process in terms of thermal conductivities at the boundaries between the manufactured(locally melted and rapidly solidified)portions and adjacent(unmelted)alloy powder. | Mulin Liu Naoki Takata Asuka Suzuki Makoto Kobashi | 2020 | Journal of Materials Science & Technology2020,36,1: | 2 |
| 7 | Hyperdry human amniotic membrane application as a wound dressing for a full-thickness skin excision after a third-degree burn injury显示文摘Background:Severe burn injuries create large skin defects that render the host susceptible to bacterial infections.Burn wound infection often causes systemic sepsis and severe septicemia,resulting in an increase in the mortality of patients with severe burn injuries.Therefore,appropriate wound care is important to prevent infection and improve patient outcomes.However,it is difficult to heal a third-degree burn injury.The aim of this studywas to investigate whether hyperdry human amniotic membrane(HD-AM)could promote early granulation tissue formation after full-thickness skin excision in third-degree burn injury sites in mice.Methods:After the development of HD-AM and creation of a third-degree burn injury model,the HD-AM was either placed or not placed on the wound area in the HD-AM group or HD-AM group,respectively.The groups were prepared for evaluation on postoperative days 1,4 and 7.Azan staining was used for granulation tissue evaluation,and estimation of CD163,transforming growth factor beta-1(TGF-β1),vascular endothelial growth factor(VEGF),CD31,alpha-smooth muscle actin(α-SMA)and Iba1 expression was performed by immunohistochemical staining.Quantitative reverse-transcription polymerase chain reaction(PCR)was used to investigate gene expression of growth factors,cell migration chemokines and angiogenic and inflammatory markers.Results:The HD-AM group showed significant early and qualitatively good growth of granulation tissue on the full-thickness skin excision site.HD-AM promoted early-phase inflammatory cell infiltration,fibroblast migration and angiogenesis in the granulation tissue.Additionally,the early infiltration of cells of the immune system was observed.Conclusions:HD-AM may be useful as a new wound dressing material for full-thickness skin excision sites after third-degree burn injuries,and may be a new therapeutic technique for improving the survival rate of patients with severe burn injuries. | Jiro Oba Motonori Okabe Toshiko Yoshida Chika Soko Moustafa Fathy Koji Amano Daisuke Kobashi Masahiro Wakasugi Hiroshi Okudera | 2020 | Burns & Trauma2020,8,1: | 2 |
