| 10 | Treatment of Donor-derived Carbapenem-resistant Klebsiella pneumoniae Infection after Renal Transplantation with Tigecycline and Extended-infusion Meropenem显示文摘Objective Donor-derived carbapenem-resistant Klebsiella pneumoniae(CRKP)infection has recently emerged as a critical early complication after renal transplantation.Although CRKP is usually sensitive to tigecycline,monotherapy with this drug is often less than effective.We investigated the efficacy of a combined regimen of tigecycline with high-dose,extended-infusion meropenem in the treatment of donor-derived CRKP infection after kidney transplantation.Methods From Jan.2016 to Dec.2017,a total of 12 CRKP isolates were detected from cultures of the organ preservation solution used for soaking the donor kidneys at our institute.Probable or possible donor-derived infection(DDI)was identified in 8 transplant recipients.Clinical data were retrospectively analyzed.Results Klebsiella pneumoniae carbapenemase-2(KPC-2)-producing CRKP was reported to be positive in organ preservation solution cultures at 3.5±0.9 days after transplantation,leading to surgical site(n=3),urinary tract(n=4),and/or bloodstream(n=2)infections in 8 recipients.The drug susceptibility tests showed that CRKP was sensitive to tigecycline,but resistant to meropenem.In 7 patients who received tigecycline combined with high-dose extended-infusion meropenem,DDIs were successfully cured.The length of hospital stay was 31(18–129)days,and the serum creatinine at discharge was 105.8±16.7µmol/L.The one remaining patient who received tigecycline combined with intravenous-drip meropenem died of septic shock.A median follow-up of 43 months(33–55)showed no recurrence of new CRKP infection in the 7 surviving recipients.Conclusion It was suggested that a prompt and appropriate combination therapy using tigecycline with high-dose extended-infusion meropenem is effective in treating donor-derived KPC-2-producing CRKP infection after renal transplantation. | Zhi-qiang WANG Zhi-liang GUO Hao FENG Cheng FU Guang-yuan ZHAO Ke MA Lan ZHU Gang CHEN | 2021 | Current Medical Science2021,41,4: | 8 |
| 11 | Combined effects of p53 and MDM2 polymorphisms on susceptibility and surgical prognosis in hepatitis B virus-related hepatocellular carcinoma显示文摘The p53 signaling pathway works as a potent barrier to tumor progression.Two single nucleotide polymor-phisms(SNPs)in the gene loci of p53 pathway,p53 codon 72 Arg72Pro and MDM2 SNP309(T>G),have been shown to cause perturbation of p53 function,but the effect of the two SNPs on the risk of hepatocellular carcinoma(HCC)remains inconsistent.This study in-vestigated the influence of combined p53 Arg72Pro and MDM2 SNP309 on the risk of developing HCC in pa-tients with chronic hepatitis B virus infection,and evaluated the significance of the two combined SNPs on patient prognosis.In total,350 HCC patients,230 non-HCC patients,and 96 healthy controls were geno-typed for the p53 Arg72Pro and MDM2 SNP309.The combined p53 Pro/Pro and MDM2 G/G genotype was significantly associated with HCC risk(P=0.047).Mul-tivariate analysis indicated that combined p53 Pro/Pro and MDM2 G/G genotype was an independent factor affecting recurrence and survival(P<0.05).Patients with combined p53 Pro/Pro and MDM2 G/G genotypes had a poorer prognosis than other genotypes,P<0.01 for both disease-free survival(DFS)and overall survival(OS).DFS and OS rates also differed significantly be-tween Barcelona Clinic Liver Cancer(BCLC)stage A patients with combined p53 Pro/Pro and MDM2 G/G and other genotypes(P<0.05).Thus,the combined p53 Pro/Pro and MDM2 G/G genotype is associated with increased risk of developing HCC and is an independ-ent adverse prognostic indicator in early stage HCC. | Yun Yang Tian Xia Ning Li Jin Zhang Yuan Yang Wenming Cong Qiang Deng Ke Lan Weiping Zhou | 2013 | Protein & Cell2013,4,1: | 8 |

| 13 | A Small-Scale Medication of Leflunomide as a Treatment of COVID-19 in an Open-Label Blank-Controlled Clinical Trial显示文摘We recently reported that inhibitors against human dihydroorotate dehydrogenase(DHODH)have broad-spectrum antiviral activities including their inhibitory efficacies on SARS-CoV-2 replication in infected cells.However,there are limited data from clinical studies to prove the application of DHODH inhibitors in Coronavirus disease 2019(COVID-19)patients.In the present study,we evaluated Leflunomide,an approved DHODH inhibitor widely used as a modest immune regulator to treat autoimmune diseases,in treating COVID-19 disease with a small-scale of patients.Cases of 10 laboratory-confirmed COVID-19 patients of moderate type with obvious opacity in the lung were included.Five of the patients were treated with Leflunomide,and another five were treated as blank controls without a placebo.All the patients accepted standard supportive treatment for COVID-19.The patients given Leflunomide had a shorter viral shedding time(median of5 days)than the controls(median of 11 days,P=0.046).The patients given Leflunomide also showed a significant reduction in C-reactive protein levels,indicating that immunopathological inflammation was well controlled.No obvious adverse effects were observed in Leflunomide-treated patients,and they all discharged from the hospital faster than controls.This preliminary study on a small-scale compassionate use of Leflunomide provides clues for further understanding of Leflunomide as a potential antiviral drug against COVID-19. | Ke Hu Mengmei Wang Yang Zhao Yunting Zhang Tao Wang Zhishui Zheng Xiaochen Li Shaolin Zeng Dong Zhao Honglin Li Ke Xu Ke Lan | 2020 | Virologica Sinica2020,35,6: | 7 |
| 14 | Experimental demonstration of ultra-large-scale terahertz all-dielectric metamaterials显示文摘All-dielectric metamaterials have emerged as a promising platform for low-loss and highly efficient terahertz devices. However, existing fabrication methods have difficulty in achieving a good balance between precision and cost. Here, inspired by the nano-template-assisted self-assembly method, we develop a micro-templateassisted self-assembly(MTAS) method to prepare large-scale, high-precision, and flexible ceramic microsphere all-dielectric metamaterials with an area exceeding 900 cm × 900 cm. Free from organic solvents, vacuum, and complex equipment, the MTAS method ensures low-cost and environmentally friendly fabrication. The ceramic microsphere resonators can be readily assembled into nearly arbitrary arrangements and complex aggregates, such as dimers, trimers, quadrumers, and chains. These malignancies often occur in immunosuppressed individuals, making KSHV infection-associated diseases an increasing global health concern with persistence of the AIDS epidemic. KSHV exhibits biphasic life cycles between latent and lytic infection and extensive transcriptional and posttranscriptional regulation of gene expression. As a member of the herpesvirus family, KSHV has evolved many strategies to evade the host immune response, which help the virus establish a successful lifelong infection. | 19 | Association between physiological responses after exercise at low altitude and acute mountain sickness upon ascent is sex-dependent显示文摘Background:Acute mountain sickness(AMS)is the mildest form of acute altitude illnesses,and consists of nonspecific symptoms when unacclimatized persons ascend to elevation of≥2500 m.Risk factors of AMS include:the altitude,individual susceptibility,ascending rate and degree of pre-acclimatization.In the current study,we examined whether physiological response at low altitude could predict the development of AMS.Methods:A total of 111 healthy adult healthy volunteers participated in this trial;and 99(67 men and 32 women)completed the entire study protocol.Subjects were asked to complete a 9-min exercise program using a mechanically braked bicycle ergometer at low altitude(500 m).Heart rate,blood pressure(BP)and pulse oxygen saturation(SpO2)were recorded prior to and during the last minute of exercise.The ascent from 500 m to 4100 m was completed in 2 days.AMS was defined as≥3 points in a 4-item Lake Louise Score,with at least one point from headache wat 6–8 h after the ascent.Results:Among the 99 assessable subjects,47(23 men and 24 women)developed AMS at 4100 m.In comparison to the subjects without AMS,those who developed AMS had lower proportion of men(48.9%vs.84.6%,P<0.001),height(168.4±5.9 cm vs.171.3±6.1 cm,P=0.019),weight(62.0±10.0 kg vs.66.7±8.6 kg,P=0.014)and proportion of smokers(23.4%vs.51.9%,P=0.004).Multivariate regression analysis revealed the following independent risks for AMS:female sex(odds ratio(OR)=6.32,P<0.001),SpO2 change upon exercise at low altitude(OR=0.63,P=0.002)and systolic BP change after the ascent(OR=0.96,P=0.029).Women had larger reduction in SpO2 after the ascent,higher AMS percentage and absolute AMS score.Larger reduction of SpO2 after exercise was associated with both AMS incidence(P=0.001)and AMS score(P<0.001)in men but not in women.Conclusions:Larger SpO2 reduction after exercise at low altitude was an independent risk for AMS upon ascent.Such an association was more robust in men than in women.Trial registration:Chinese Clinical Trial Registration,ChiCTR1900025728.Registered 6 September 2019. | Yang Shen Yuan-Qi Yang Chuan Liu Jie Yang Ji-Hang Zhang Jun Jin Hu Tan Fang-Zheng-Yuan Yuan Jing-Bin Ke Chun-Yan He Lai-Ping Zhang Chen Zhang Jie Yu Lan Huang | 2021 | Military Medical Research2021,8,2: | 3 |
| 20 | Massive Preparation of Coumarone-indene Resin-based Hyper-crosslinked Polymers for Gas Adsorption显示文摘Hyper-crosslinked polymers(HCPs) are promising materials for gas capture and storage because of their low cost and easy preparation. In this work, we report the massive preparation of coumarone-indene resin-based hyper-crosslinked polymers via one-step Friedel-Crafts alkylation. Low-cost coumarone-indene resin serves as the new building block and chloroform is employed as both solvent and external crosslinker. A maximum surface area of 966 m^2·g^(-1) is achieved, which is comparable with that of previously-reported coal tar-based porous organic polymers. Most importantly, a large number of heteroatoms including inherent oxygen atoms and introduced chlorine atoms in obtianed HCPs further enhance the interaction between specific sorbate molecule and adsorbent. Therefore, optimal structural and chemical property endow the new coumarone-indene resin-based HCPs with decent gas storage capacity(14.60 wt% at273 K and 0.1 MPa for CO_2; 1.18 wt% at 77.3 K and 0.1 MPa for H_2).
