2经腹股沟及腹股沟下显微精索静脉结扎术:精索静脉曲张的最佳手术治疗方式的荟萃分析显示文摘以往精索静脉曲张各种治疗方法疗效的荟萃分析存在不一致的结果。因此,我们进行了多种干预措施的荟萃分析,评估10种精索静脉曲张栓塞/硬化治疗方法的有效性和安全性,包括腹腔镜、腹膜后开放、经腹股沟开放和腹股沟下开放静脉结扎,腹股沟下显微和经腹股沟显微精索静脉结扎术,经皮静脉栓塞,TauberJl~行硬化,硬化逆行和期待治疗(无处理)。我们系统回顾了从1966年至2013年8月的35个随机对照试验和观察性研究。经腹股沟、腹股沟下显微手术、腹股沟开放、腹腔镜精索静脉结扎与期待治疗相比显著提高夫妻怀孕率(优势比[OR]分别为:3.48,2.68,2.92和2.9)。与腹膜后开放手术相比,经腹股沟显微手术显著提高患者的精子密度(平均差【MD]:10.60,95%CI:1.92—19.60)并改善精子运动(MD:9.09,95%CI:4.88—13.30)。经腹股沟下和腹股沟显微手术在复发率方面的表现优于开放腹膜后手术(OR分别为0.05,O.06)。Tauber顺行硬化和腹股沟下显微精索静脉结扎术的鞘膜积液形成的风险最低。与腹膜后开放精索静脉曲张术相比,腹股沟显微精索静脉结扎术的总的并发症率最低(OR=O.07,95%CI:0.02。0.19)。其次是腹股沟下显微(OR:O.09,95%CI:0.02.0.19)。经腹股沟显微和腹股沟下显微精索静脉曲张结扎术对提高患者怀孕率有良好的效果,能显著增加精子数量,并有着较低的并发症率。本文研究结果仍需要经过良好设计的随机对照临床研究进一步证实。Jun Wang Shu-Jie Xia Zhi-Hong Liu Le Tao Ji-Fu Ge Chen-Min Xu Jian-Xin Qiu 2015Asian Journal of Andrology2015,17,1:16

8Oxaliplatin and 5-fluorouracil hepatic infusion with lipiodolized chemoembolization in large hepatocellular carcinoma显示文摘AIM: To investigate transarterial chemoembolization(TACE) with hepatic infusion of oxaliplatin and 5-fluorouracil and Lipiodol chemoembolization in large hepatocellular carcinoma(HCC).METHODS: In this retrospective study, 132 patients with unresectable HCCs larger than 10 cm were treated with hepatic infusion of oxaliplatin and 5-fluorouracil followed by Lipiodol chemoembolization. The primary endpoint was overall survival(OS). Sixteen-week disease-control rate, time to progression(TTP), and major complications were also studied. Univariate and multivariate analyses were performed to identify prognostic factors affecting OS and TTP.RESULTS: A total of 319 procedures were performed in the 132 patients. Eleven(8.3%) patients received radical resection following TACE treatment(median time to initial TACE 4.3 ± 2.3 mo). The median OS and TTP were 10.3 and 3.0 mo respectively, with a 50.0% 16-wk disease-control rate. Major complications were encountered in 6.0%(8/132) of patients following TACE and included serious jaundice in 1.5%(2/132) patients, aleukia in 1.5%(2/132), and hepatic failure in 3.0%(4/132). One patient died within one month due to serious hepatic failure and severe sepsis after receiving the second TACE. 9Correlation of Deactivation of Ni-Mo-W/Al2O3 during Ultra-Low-Sulfur Diesel Production with Surface Carbon Species显示文摘The deactivation of a Ni-Mo-W/Al_2O_3 catalyst during ultra-low-sulfur diesel production was investigated in a pilot plant. The reasons of catalyst deactivation were analyzed by the methods of elemental analysis, BET and TG-MS. The results showed that the catalyst deactivation rate was notable at the beginning of run, and then gradually reached a steady state after 448 h. In the initial period the catalyst deactivation may mainly be caused by the formation of the carbon deposits. The carbon deposits blocked the catalyst pores and the accessibility of active center decreased. The TG-MS analysis identified three types of carbon species deposited on the catalysts, viz.: the low temperature carbon deposit with high H/C atomic ratio, the medium temperature carbon deposit, and the high temperature carbon with low H/C atomic ratio. 10Force chains based mesoscale simulation on the dynamic response of Al-PTFE granular composites显示文摘Force chains based mesoscale simulation is conducted to investigate the response behavior of aluminumpolytetrafluoroethylene(Al-PTFE)granular composites under a low-velocity impact.A two-dimensional model followed the randomly normal distribution of real Al particles size is developed.The dynamic compressive process of Al-PTFE composites with varied Al mass fraction is simulated and validated against the experiments.The results indicate that,force chains behavior governed by the number and the size of agglomerated Al particles,significantly affects the impact response of the material.The failure mode of the material evolves from shear failure of matrix to debonding failure of particles with increasing density.A high crack area of the material is critical mechanism to arouse the initiation reaction.The damage maintained by force chains during large plastic strain builds up more local stresses concentration to enhance a possible reaction performance.In addition,simulation is performed with identical mass fraction but various Al size distribution to explore the effects of size centralization and dispersion on the mechanical properties of materials.It is found that smaller sized Al particle of composites are more preferred than its bulky material in ultimate strength.Increasing dispersed degree is facilitated to create stable force chains in samples with comparable particle number.The simulation studies provide further insights into the plastic deformation,failure mechanism,and possible energy release capacity for Al-PTFE composites,which is helpful for further design and application of reactive materials.Le Tang Chao Ge Huan-guo Guo Qing-bo Yu Hai-fu Wang 2021Defence Technology(防务技术)2021,17,1:5
11Effect of wave shaper on reactive materials jet formation and its penetration performance显示文摘Wave shaper effect on formation behavior and penetration performance of reactive liner shaped charge(RLSC)are investigated by experiments and simulations.The reactive materials liner with a density of2.3 g/cm^3 is fabricated by cold pressing at a pressure of 300 MPa and sintering at a temperature of 380℃.Experiments of the RLSC with and without wave shaper against steel plates are carried out at standoffs of0.5,1.0,and 1.5 CD(charge diameter),respectively.The experimental results show that the penetration depths and structural damage effects of steel plates decrease with increasing the standoff,while the penetration depths and the damage effects of RLSC without wave shaper are much greater than that with wave shaper at the same standoff.To understand the unusual experimental results,numerical simulations based on AUTODYN-2 D code are conducted to discuss the wave shaper effect,including the propagation behavior of detonation wave,the velocity and temperature distribution of reactive jet,and penetration depth of reactive jet.The simulations indicate that,compared with RLSC without wave shaper,there is a higher temperature produced inside reactive jet with wave shaper.This unusual temperature rise effects are likely to be an important mechanism to cause the initiation delay time of reactive jet to decline,which results in significantly decreasing its penetration performance.Huan-guo Guo Yuan-feng Zheng Le Tang Qing-bo Yu Chao Ge Hai-fu Wang 2019Defence Technology(防务技术)2019,15,4:4

15Probability hypothesis density filter with adaptive parameter estimation for tracking multiple maneuvering targets显示文摘The probability hypothesis density(PHD) filter has been recognized as a promising technique for tracking an unknown number of targets. The performance of the PHD filter, however, is sensitive to the available knowledge on model parameters such as the measurement noise variance and those associated with the changes in the maneuvering target trajectories. If these parameters are unknown in advance, the tracking performance may degrade greatly. To address this aspect, this paper proposes to incorporate the adaptive parameter estimation(APE) method in the PHD filter so that the model parameters, which may be static and/or time-varying, can be estimated jointly with target states. The resulting APE-PHD algorithm is implemented using the particle filter(PF), which leads to the PF-APE-PHD filter. 16miR-140-3p enhanced the osteo/odontogenic differentiation of DPSCs via inhibiting KMT5B under hypoxia condition显示文摘Human dental pulp stem cells(DPSCs)have emerged as an important source of stem cells in the tissue engineering,and hypoxia will change various innate characteristics of DPSCs and then affect dental tissue regeneration.Nevertheless,little is known about the complicated molecular mechanisms.In this study,we aimed to investigate the influence and mechanism of miR-140-3p on DPSCs under hypoxia condition.Hypoxia was induced in DPSCs by Cobalt chloride(CoCl_(2))treatment.The osteo/dentinogenic differentiation capacity of DPSCs was assessed by alkaline phosphatase(ALP)activity,Alizarin Red S staining and main osteo/dentinogenic markers.A luciferase reporter gene assay was performed to verify the downstream target gene of miR-140-3p.This research exhibited that miR-140-3p promoted osteo/dentinogenic differentiation of DPSCs under normoxia environment.Furthermore,miR-140-3p rescued the CoCl_(2)-induced decreased osteo/odontogenic differentiation potentials in DPSCs.Besides,we investigated that miR-140-3p directly targeted lysine methyltransferase 5B(KMT5B).Surprisingly,we found inhibition of KMT5B obviously enhanced osteo/dentinogenic differentiation of DPSCs both under normoxia and hypoxia conditions.In conclusion,our study revealed the role and mechanism of miR-140-3p for regulating osteo/dentinogenic differentiation of DPSCs under hypoxia,and discovered that miR-140-3p and KMT5B might be important targets for DPSC-mediated tooth or bone tissue regeneration.Han Zheng Ning Wang Le Li Lihua Ge Haichao Jia Zhipeng Fan 2021International Journal of Oral Science2021,13,4:2
