| 1 | Preliminary multiproxy surface air temperature field reconstruction for China over the past millennium显示文摘We present the first millennial-length gridded field reconstruction of annual temperature for China, and analyze the reconstruction for spatiotemporal changes and associated uncertainties, based on a network of 415 well-distributed and accurately dated climatic proxy series.The new reconstruction method is a modified form of the point-by-point regression (PPR) approach.The main difference is the incorporation of the 'composite plus scale' (CPS) and 'Regularized errors-in-variables' (EIV) algorithms to allow for the assimilation of various types of the proxy data.Furthermore, the search radius is restricted to a grid size; this restriction helps effectively exclude proxy data possibly correlated with temperature but belonging to a different climate region.The results indicate that: 1) the past temperature record in China is spatially heterogenic, with variable correlations between cells in time; 2) the late 20th century warming in China probably exceeds mean temperature levels at any period of the past 1000 years, but the temperature anomalies of some grids in eastern China during the Medieval climate anomaly period are warmer than during the modern warming; 3) the climatic variability in the eastern and western regions of China was not synchronous during much of the last millennium, probably due to the influence of the Tibetan Plateau.Our temperature reconstruction may serve as a reference to test simulation results over the past millennium, and help to finely analyze the spatial characteristics and the driving mechanism of the past temperature variability.However, the lower reconstruction skill scores for some grid points underline that the present set of available proxy data series is not yet sufficient to accurately reconstruct the heterogeneous climate of China in all regions, and that there is the need for more highly resolved temperature proxies, particularly in the Tibetan Plateau. | SHI Feng YANG Bao Lucien Von GUNTEN | 2012 | Science China Earth Sciences2012,55,12: | 18 |
| 2 | Ampullary carcinoma:Effect of preoperative biliary drainage on surgical outcome显示文摘AIM: To evaluate the influence of preoperative biliary drainage on morbidity and mortality after surgical resection for ampullary carcinoma. METHODS: We analyzed retrospectively data for 82 patients who underwent potentially curative surgery for ampullary carcinoma between September 1993 and July 2007 at the Singapore General Hospital, a tertiary referral hospital. Diagnosis of ampullary carcinoma was confirmed histologically. Thirty-five patients underwent preoperative biliary drainage (PBD group), and 47 were not drained (non-PBD group). The mode of biliary drainage was endoscopic retrograde cholangiopancreatography (n = 33) or percutaneous biliary drainage (n = 2). The following parameters were analyzed: wound infection, intra-abdominal abscess, intra-abdominal or gastrointestinal bleeding, septicemia, biliary or pancreatic leakage, pancreatitis, gastroparesis, and re-operation rate. Mortality was assessed at 30 d (hospital mortality) and also longterm. The statistical endpoint of this study was patient survival after surgery. RESULTS: The groups were well matched for demographic criteria, clinical presentation and operative characteristics, except for lower hemoglobin in the non-PBD group (10.9 ± 1.6 vs 11.8 ± 1.6 in the PBD group).Of the parameters assessing postoperative morbidity, incidence of wound infection was signif icantly less in the PBD than the non-PBD group [1 (2.9%) vs 12 (25.5%)]. However, the rest of the parameters did not differ signif icantly between the groups, i.e. sepsis [10 (28.6%) vs 14 (29.8%)], intra-abdominal bleeding [1 (2.9%) vs 5 (10.6%)], intra-abdominal abscess [1 (2.9%) vs 8 (17%)], gastrointestinal bleeding [3 (8.6%) vs 5 (10.6%)], pancreatic leakage [2 (5.7%) vs 3 (6.4%)], biliary leakage [2 (5.7%) vs 3 (6.4%)], pancreatitis [2 (5.7%) vs 2 (4.3%)], gastroparesis [6 (17.1%) vs 10 (21.3%)], need for blood transfusion [10 (28.6%) vs 17 (36.2%)] and re-operation rate [1 (2.9%) vs 5 (10.6%)]. There was no early mortality in either group. Median survival was 44 mo (95% CI: 34.2-53.8) in the PBD group and 41 mo (95% CI: 27.7-54.3; P = 0.86) in the non-PBD group. CONCLUSION: Biliary drainage before surgery for ampullary cancer significantly reduced postoperative wound infection. | 3 | Impact of osteoporosis in dental implants:A systematic review显示文摘AIM: To assess the failure and bone-to-implant contact rate of dental implants placed on osteoporotic subjects. METHODS: Extensive examination strategies were created to classify studies for this systematic review. MEDLINE(via Pub Med) and EMBASE database were examined for studies in English up to and including May 2014. The examination presented a combination of the MeS H words described as follow: 'osteoporosis' or 'osteopenia' or 'estrogen deficiency' AND 'implant' or 'dental implant' or 'osseointegration'. Assessment of clinical and/or histological peri-implant conditions in osteoporosis subjects treated with titanium dental implants. The examination included a combination of the MeS H terms described as follow: 'osteoporosis' or 'osteopenia' or 'estrogen deficiency' AND 'implant' or 'dental implant' or 'osseointegration'.RESULTS: Of 943 potentially eligible articles, 12 were included in the study. A total of 133 subjects with osteoporosis, 73 subjects diagnosed with osteopenia and 708 healthy subjects were assessed in this systematic review. In these subjects were installed 367, 205, 2981 dental implants in osteoporotic, osteopenic and healthy subjects, respectively. The failure rate of dental implant was 10.9% in osteoporotic subjects, 8.29% in osteopenic and 11.43% in healthy ones. Bone-to-implant contact obtained from retrieved implants ranged between 49.96% to 47.84%, for osteoporosis and non-osteoporotic subjects. CONCLUSION: Osteoporotic subjects presented higher rates of implant loss, however, there is a lower evidence to strengthen or refute the hypothesis that osteoporosis may have detrimental effects on bone healing. Consequently, final conclusions regarding the effect of osteoporosis in dental implant therapy cannot be made at this time. | 11 | Femtosecond laser-assisted deep anterior lamellar keratoplasty in phototherapeutic keratectomy versus the big-bubble technique in keratoconus显示文摘AIM:To compare the functional and anatomic results of femtosecond laser(FSL)-assisted deep anterior lamellar keratoplasty(DALK) associated with phototherapeutic keratectomy(PTK) and FSL-assisted DALK performed using the big-bubble technique in keratoconus.METHODS:During the first phase of the study,an electron microscopy histopathology pilot study was conducted that included four unsuitable donor corneas divided into two groups:in FSL group,FSL lamellar cuts were performed on two corneas and in FSL+PTK group,PTK was performed at the stromal beds of two corneas after FSL lamellar cuts were made.During the second phase of the study,a randomized clinical trial was conducted that included two treatment groups of patients with keratoconus:group 1(n=14 eyes) underwent FSL-assisted DALK associated with PTK and group 2(n=12 eyes) underwent FSL-assisted DALK associated with the bigbubble technique.The main outcome measures were the postoperative visual acuity(VA) and optical coherence tomography(OCT) measurements,confocal microscopic findings,and contrast sensitivity.RESULTS:In the pilot study,histopathology showed a more regular stromal bed in the FSL+PTK group.In the clinical trial,group 1 had significantly worse best spectaclecorrected VA and contrast sensitivity(P<0.05 for both comparisons).The residual stromal bed measured by OCT was significantly(P<0.05) thicker in group 1.Confocal microscopy detected opacities only at the donor-receptor interface in group 1.CONCLUSION:Patients with keratoconus treated with FSL-assisted DALK performed using the big-bubble technique fare better than treated with FSL-assisted DALK associated with PTK. | Jarbas Pereira de Macedo Lauro Augusto de Oliveira Flavio Hirai Luciene Barbosa de Sousa | 2018 | International Journal of Ophthalmology(English edition)2018,11,5: | 2 |

| 13 | Hydrogeochemical and multi-tracer investigations of arsenic-affected aquifers in semi-arid West Africa显示文摘The semi-arid Sahel regions ofWest Africa rely heavily on groundwater from shallow to moderately deep (<100 m b.g.l.) crystalline bedrock aquifers for drinking water production.Groundwater quality may be affected by high geogenic arsenic (As) concentrations (>10 μg/L) stemming from the oxidation of sulphide minerals (pyrite,arsenopyrite) in mineralised zones.These aquifers are still little investigated,especially concerning groundwater residence times and the influence of the annual monsoon season on groundwater chemistry.To gain insights on the temporal aspects of As contamination,we have used isotope tracers (noble gases,3H,stable water isotopes (2H,18O)) and performed hydrochemical analyses on groundwater abstracted from tube wells and dug wells in a small study area in southwestern Burkina Faso.Results revealed a great variability in groundwater properties (e.g.redox conditions,As concentrations,water level,residence time) over spatial scales of only a few hundred metres,characteristic of the highly heterogeneous fractured underground.Elevated As levels are found in oxic groundwater of circum-neutral pH and show little relation with any of the measured parameters.Arsenic concentrations are relatively stable over the course of the year,with little effect seen by the monsoon.Groundwater residence time does not seem to have an influence on As concentrations,as elevated As can be found both in groundwater with short (<50 a) and long (>10^3 a) residence times as indicated by 3He/4He ratios spanning three orders of magnitude.These results support the hypothesis that the proximity to mineralised zones is the most crucial factor controlling As concentrations in the observed redox/pH conditions.The existence of very old water portions with residence times >10^3 years already at depths of <50 m b.g.l.is a new finding for the shallow fractured bedrock aquifers of Burkina Faso,suggesting that overexploitation of these relatively low-yielding aquifers may be an issue in the future. | Anja Bretzler Lucien Stolze Julien Nikiema Franck Lalanne Elaheh Ghadiri Matthias S. | 14 | Serum miR-339-3p as a potential diagnostic marker for non-small cell lung cancer显示文摘Objective:MicroRNA(miRNA),a short noncoding RNA,is claimed to be a potential blood-based biomarker.We aimed to identify and evaluate miRNAs as diagnostic biomarkers for non-small cell lung cancer(NSCLC).Methods:Profiles of 745 miRNAs were screened in the serum of 8 patients with NSCLC and 8 age-and sex-matched controls using TaqMan low-density arrays(TLDAs)and validated in 25 patients with NSCLC and 30 with other lung diseases(OLs)as well as in 19 healthy persons(HPs).The diagnostic performance of the candidate miRNAs was assessed in 117 cases of NSCLC and 113 OLs using quantitative real-time polymerase chain reaction(qRT-PCR).Differences in miRNA expression between patients with NSCLC and controls were assessed using the Mann–Whitney U test.The area under receiver operating characteristic(ROC)curve(AUC)was obtained based on the logistic regression model.Results:Ten miRNAs were found to be differentialy expressed between patients with NSCLC and controls,including miR-769,miR-339-3p,miR-339-5p,miR-519a,miR-1238,miR-99a#,miR-134,miR-604,miR-539,and miR-342.The expression of miR-339-3p was significantly higher in patients with NSCLC than in those with OLs(P<0.001)and HPs(P=0.020).ROC analysis revealed an miR-339-3p expression AUC of 0.616[95%confidence interval(CI):0.561–0.702].The diagnostic prediction was increased(AUC=0.706,95%CI:0.649–0.779)in the model combining miR-339-3p expression and other known risk factors(i.e.,age,smoking status,and drinking status).Conclusions:MiR-339-3p was significantly upregulated in patients with NSCLC compared with participants without cancer,suggesting a diagnostic prediction value for high-risk individuals.Therefore,miR-339-3p expression could be a potential blood-based biomarker for NSCLC. | Keson Trakunram Pichitpon Chaniad Sarayut Lucien Geater Warangkana Keeratichananont Voravit Chittithavorn Sumonmal Uttayamakul Suhaimee Buya Pritsana Raungrut Paramee Thongsuksai | 2020 | Cancer Biology & Medicine2020,17,3: | 2 |
