1Mechanisms of hepatocellular carcinoma progression显示文摘Hepatocellular carcinoma(HCC) is the most common primary malignancy of the liver. It is the second leading cause of cancer-related deaths worldwide, with a very poor prognosis. In the United States, there has been only minimal improvement in the prognosis for HCC patients over the past 15 years. Details of the molecular mechanisms and other mechanisms of HCC progression remain unclear. Consequently, there is an urgent need for better understanding of these mechanisms. HCC is often diagnosed at advanced stages, and most patients will therefore need systemic therapy, with sorafenib being the most common at the present time. However, sorafenib therapy only minimally enhances patient survival. This review provides a summary of some of the known mechanisms that either cause HCC or contribute to its progression. Included in this review are the roles of viral hepatitis, non-viral hepatitis, chronic alcohol intake, genetic predisposition and congenital abnormalities, toxic exposures, and autoimmune diseases of the liver. Well-established molecular mechanisms of HCC progression such as epithelial-mesenchymal transition, tumor-stromal interactions and the tumor microenvironment, cancer stem cells, and senescence bypass are also discussed. Additionally, we discuss the roles of circulating tumor cells,immunomodulation, and neural regulation as potential new mechanisms of HCC progression. 2Virulence factors of Enterococcus strains isolated from patients with inflammatory bowel disease显示文摘AIM: To determine the features of Enterococcus that contribute to the development and maintenance of the inflammatory process in patients with inflammatory bowel disease (IBD). METHODS: Multiplex polymerase chain reaction (PCR) was applied to assess the presence of genes that encode virulence factors [surface aggregating protein (asa1), gelatinase (gelE), cytolysin (cylA), extracellular surface protein (esp) and hyaluronidase (hyl)] in the genomic DNA of 28 strains of Enterococcus isolated from the intestinal tissues of children with IBD (n =16) and of children without IBD (controls; n = 12). Additionally, strains with confirmed presence of the gelE gene were tested by PCR for the presence of quorum sensing genes (fsrA, fsrB, fsrC) that control the gelatinase production. Gelatinase activity was tested on agar plates containing 1.6% gelatin. We also analysed the ability of Enterococcus strains to release and decompose hydrogen peroxide (using Analytical Merckoquant peroxide test strips) and tested their ability to adhere to Caco-2 human gut epithelium cells and form biofilms in vitro. RESULTS: A comparison of the genomes of Enterococcus strains isolated from the inflamed mucosa of patients with IBD with those of the control group showed statistically significant differences in the frequency of theasa1 gene and thegelE gene. Furthermore, the cumulative occurrence of different virulence genes in the genome of a single strain ofEnterococcus isolated from the IBD patient group is greater than in a strain from the control group, although no significant difference was found. Statistically significant differences in the decomposition of hydrogen peroxide and adherence to the Caco-2 epithelial cell line between the strains from the patient group and control group were demonstrated. The results also showed that profuse biofilm production was more frequent amongEnterococcus strains isolated from children with IBD than in control strains. 3Spatial pattern of grassland aboveground biomass and its environmental controls in the Eurasian steppe显示文摘Vegetation biomass is an important component of terrestrial ecosystem carbon stocks. Grasslands are one of the most widespread biomes worldwideplaying an important role in global carbon cycling. Thereforestudying spatial patterns of biomass and their correlations to environment in grasslands is fundamental to quantifying terrestrial carbon budgets. The Eurasian steppean important part of global grasslandsis the largest and relatively well preserved grassland in the world. In this studywe analyzed the spatial pattern of aboveground biomass(AGB)and correlations of AGB to its environment in the Eurasian steppe by meta-analysis. AGB data used in this study were derived from the harvesting method and were obtained from three data sources(literatureglobal NPP database at the Oak Ridge National Laboratory Distributed Active Archive Center(ORNL)some data provided by other researchers). Our results demonstrated that:(1) as for the Eurasian steppe overallthe spatial variation in AGB exhibited significant horizontal and vertical zonality. In detailAGB showed an inverted parabola curve with the latitude and with the elevationwhile a parabola curve with the longitude. In additionthe spatial pattern of AGB had marked horizontal zonality in the Black Sea-Kazakhstan steppe subregion and the Mongolian Plateau steppe subregionwhile horizontal and vertical zonality in the Tibetan Plateau alpine steppe subregion.(2) Of the examined environmental variablesthe spatial variation of AGB was related to mean annual precipitation(MAP)mean annual temperature(MAT)mean annual solar radiation(MAR)soil Gravel contentsoil p H and soil organic content(SOC) at the depth of 0–30 cm. NeverthelessMAP dominated spatial patterns of AGB in the Eurasian steppe and its three subregions.(3) A Gaussian function was found between AGB and MAP in the Eurasian steppe overallwhich was primarily determined by unique patterns of grasslands and environment in the Tibetan Plateau. AGB was significantly positively related to MAP in the Black Sea-Kazakhstan steppe subregion(elevation < 3000 m)the Mongolian Plateau steppe subregion(elevation < 3000 m) and the surface(elevation ≥ 4800 m) of the Tibetan Plateau. Neverthelessthe spatial variation in AGB exhibited a Gaussian function curve with the increasing MAP in the east and southeast margins(elevation < 4800 m) of the Tibetan Plateau. 5Fatty acid changes help to better understand regression of nonalcoholic fatty liver disease显示文摘AIM: To investigate whether liver steatosis reduction due to a six-month dietary intervention results in significant changes in the concentrations of fatty acids.METHODS: A group of 35 Caucasian individuals diagnosed with different levels of steatosis were prospectively enrolled in the present study.Analysis of the fatty acid profiles was performed according to changes in liver steatosis(liver steatosis reduction by one or two degrees) after a six-month dietary intervention.The diet helped reduce body mass in obese and overweight patients,and stabilize both glycemia and dyslipidemia.Fatty acids were extracted according to the Folch method and analyzed by gas chromatography.RESULTS: This study showed significant changes in fatty acid profiles in patients who had reduced liver steatosis by one as well as two degrees.A reduction in liver steatosis by one degree caused a significant increase in the level of the n-3 family: eicosapentaenoic acid(P < 0.055),docosapentaenoic acid-C 22:5(P < 0.05) and docosahexaenoic acid(P < 0.05).A reduction in liver steatosis by two degrees caused a significant decrease in serum palmitoleic acid-C 16:1(P < 0.05).CONCLUSION: Liver steatosis reduction is associated with changes in fatty acid profiles,and these changes may reflect an alteration in fatty acid synthesis and metabolism.These findings may help better understand regression of nonalcoholic fatty liver disease.Dominika Maciejewska Arleta Drozd Piotr Ossowski Karina Ryterska Dominika Jamio-Milc Marcin Banaszczak Joanna Raszeja-Wyszomirska Magorzata Kaczorowska Anna Sabinicz Ewa Stachowska 2015World Journal of Gastroenterology2015,21,1:4

8Comparison of the prognostic value of frailty assessment tools in patients aged≥65 years hospitalized in a cardiac care unit with acute coronary syndrome显示文摘BACKGROUND Frailty is associated with adverse events in elderly patients with acute coronary syndrome(ACS).Our aim was to compare the prognostic value of four frailty scales in patients aged≥65 years hospitalized with ACS in a cardiac care unit(CCU).METHODS Patients aged≥65 years with ACS were included.Frailty was assessed using the Fried frailty scale(reference standard),the Edmonton Frail Scale(EFS),the FRAIL scale,and the Clinical frailty scale(CFS).The primary end point was all-cause mortality and the secondary end point was unscheduled rehospitalization.RESULTS One hundred and seventy four patients aged≥65 years with ACS were recruited.The median follow-up was 637.5 days.Frailty was identified in 41.4%,40.2%,39.1%and 36.3%patients by the Fried frailty scale,EFS,FRAIL scale and CFS,re-spectively.The agreement coefficients were 0.88,0.86,and 0.79 for the FRAIL scale,EFS and CFS,respectively.In the Cox regres-sion model,frailty was associated with all-cause mortality regardless of the scale used(univariate:hazard ratio[HR]95%CI=10.5,2.4-46.8 Fried frailty scale;12.0,2.7-53.4 FRAIL scale;7.1,2.0-25.2 EFS;8.3,2.4-29.6 CFS.Multivariate:HR=5.1,1.1-23.8 Fried frailty scale;5.7,1.2-26.8 FRAIL scale;3.7,1.0-14.0 EFS;4.2,1.1-15.9 CFS).The FRAIL scale had the highest HR.In the uni-variate analysis,frailty was associated with unscheduled rehospitalization(HR=3.2,1.7-6.0 Fried frailty scale;3.4,1.8-6.3 FRAIL scale;3.5,1.8-6.6 EFS;3.1,1.7-5.8 CFS).In the multivariate analysis,only the EFS independently predicted unscheduled rehospit-alization(HR=2.2,1.1-4.63).CONCLUSIONS Frailty assessed by the Fried frailty scale,FRAIL scale,EFS and CFS is associated with all-cause mortality and unscheduled rehospitalization in elderly patients hospitalized in a CCU with ACS.The adjusted HR of the FRAIL scale for all-cause mortality was the highest among the scales compared,whereas the EFS was an independent predictor of unscheduled re-hospitalization.These data should be taken into consideration when choosing a frailty assessment tool.Wojciech Nowak Ilona Kowalik Małgorzata Kuzin Agnieszka Krauze Anna Mierzyńska Ewa Sadowy Kamil Marcinkiewicz Janina Stępińska 2022Journal of Geriatric Cardiology2022,19,5:2
