| 1 | Multidisciplinary therapy for treatment of patients with peritoneal carcinomatosis from gastric cancer显示文摘There is no standard treatment for peritoneal carcinomatosis (PC) from gastric cancer.A novel multidisciplinary treatment combining bidirectional chemotherapy [neoadjuvant intraperitoneal-systemic chemotherapy protocol (NIPS)],peritonectomy,hyperthermic intraperitoneal chemoperfusion (HIPEC) and early postoperative intraperitoneal chemotherapy has been developed.In this article,we assess the indications,safety and eff icacy of this treatment,review the relevant studies and introduce our experiences.The aims of NIPS are stage reduction,the eradication of peritoneal free cancer cells,and an increased incidence of complete cytoreduction (CC-0) for PC.A complete response after NIPS was ob-tained in 15 (50%) out of 30 patients with PC.Thus,a signif icantly high incidence of CC-0 can be obtained in patients with a peritoneal cancer index (PCI) ≤ 6.Using a multivariate analysis to examine the survival benef it,CC-0 and NIPS are identified as significant indicators of a good outcome.However,the high morbidity and mortality rates associated with peritonectomy and perioperative chemotherapy make stringent patient selection important.The best indications for multidisciplinary therapy are localized PC (PCI≤6) from resectable gastric cancer that can be completely removed during a peritonectomy.NIPS and complete cytoreduction are essential treatment modalities for improving the survival of patients with PC from gastric cancer. | Yutaka Yonemura Ayman Elnemr Yoshio Endou Mitsumasa Hirano Akiyoshi Mizumoto Nobuyuki Takao Masumi Ichinose Masahiro Miura | 2010 | World Journal of Gastrointestinal Oncology2010，2，2： | 27 |

| 4 | Influence of proton pump inhibitor treatment on Helicobacter pylori stool antigen test显示文摘AIM: To investigate the effects of proton pump inhibitor (PPI) treatment on stool antigen test using the TestMate pylori enzyme immunoassay. METHODS: This study assessed 28 patients [16 men and 12 women; mean age (63.1 ± 5.9) years; range, 25-84 years] who underwent stool antigen test and urea breath test (UBT) before and after PPI administration. RESULTS: Using the UBT as the standard, the sensitivity, specif icity and agreement of the stool antigen test in all 28 patients were 95.2%, 71.4%, and 89.3%, respectively, before PPI administration, and 88.9%, 90.9%, and 89.3%, respectively, after PPI treatment. Mean UBT values were 23.98% ± 5.33% before and 16.19% ± 4.75% after PPI treatment and, in 15 patients treated for ≥ 4 wk, were signif icantly lower after than before 4 wk of PPI treatment (12.58% ± 4.49% vs 24.53% ± 8.53%, P = 0.048). | 5 | Ribavirin contributes to eradicate hepatitis C virus through polarization of T helper 1/2 cell balance into T helper 1 dominance显示文摘The mechanism of action of ribavirin(RBV) as an immunomodulatory and antiviral agent and its clinical significance in the future treatment of patients with hepatitis C virus(HCV) infection are reviewed.RBV up-regulates type 1 and/or 2 cytokines to modulate the T helper(Th) 1/2 cell balance to Th1 dominance.Examination of co-stimulatory signaling indicated that RBV down-modulates inducible co-stimulator on Th cells,which contributes to differentiating na?ve Th cells into Th2 cells while reducing their interleukin-10 production.The effects on T-regulatory(Treg) cells were also investigated,and RBV inhibited the differentiation of na?ve Th cells into adaptive Treg cells by downmodulating forkhead box-P3.These findings indicate that RBV mainly down-regulates the activity of Th2 cells,resulting in the maintenance of Th1 activity that contributes to abrogating HCV-infected hepatocytes.Although an interferon-free treatment regimen exhibits almost the same efficacy without serious complications,regimens with RBV will be still be used because of their ability to facilitate the cellular immune response,which may contribute to reducing the development of hepatocellular carcinogenesis in patients infected with HCV. | Katsuhisa Nakatsuka Masanori Atsukawa Masumi Shimizu Hidemi Takahashi Chiaki Kawamoto | 2015 | World Journal of Hepatology2015，7，25： | 4 |
| 6 | Experimental Study on a Single Layer Two-Way Grid Shell with Tension Members显示文摘Single layer two-way grid shell with in-plane and out-of-plane tension members is a new type of single-layer latticed shell roofs. Compared with traditional single-layer latticed shells,this new type has a unique mesh form and excellent rigidity. In order to further understand the buckling behaviors of single layer two-way grid cylindrical shell roof with tension members,the buckling experiments have been undertaken to investigate the effect of tension members,in either out-of-plane or in-plane placement. A single layer two-way grid cylindrical shell roof with out-of-plane tension members has been tested under symmetric and asymmetric loading. The tension member placement,the introducing initial axial force to tension members and the load patterns are considered to investigate the buckling behavior. Experimental results indicate that four long out-ofplane tension members work well under symmetrical loading,but only two long out-of-plane tension members work under asymmetrical loading.
