| 1 | Effect of omeprazole and domperidone on adult asthmatics with gastroesophageal reflux显示文摘AIM: To study the effect of combined omeprazole (Ome) and domperidone (Dom) therapy on asthma symptoms and pulmonary function in asthmatics with gastroesoph- ageal reflux. METHODS: We selected 198 asthmatics with gastroesophageal reflux diagnosed by 24-h esophageal pH monitoring to receive Ome 20 mg twice daily and Dom 10 mg three times daily or placebo for 16 wk (1:1 double-blind randomization). Spirometry was done at baseline and af- ter 16 wk of treatment. The primary outcome measures were: mean daily daytime and nighttime asthma symp- tom scores. Mean daily reflux symptom scores, albuterol use as rescue medication (number of puffs), daytime and nighttime peak expiratory flow rate (PEFR), postbroncho- dilator forced expiratory volume in 1 second (FEV1) and postbronchodilator forced vital capacity (FVC) were secondary outcome measures. RESULTS: Comparison of mean change from baseline between antireflux therapy and placebo groups revealed significant reduction in daytime asthma symptom score (17.4% vs 8.9%), nighttime asthma symptom score (19.6% vs 5.4%), reflux symptom score (8.7% vs 1.6%) and rescue medication use (23.2% vs 3.1%) after antire- flux therapy compared to mean change in placebo group (P < 0.001). | 4 | Circulatory and hepatic failure at admission predicts mortality of severe scrub typhus patients: A prospective cohort study显示文摘Objective:To study the spectrum and outcome of severe scrub typhus in adult patients and to predict the hospital mortality by organ failure on admission.Methods:This was a prospective observational cohort study conducted between July 2017 and October 2020 at the medical emergency centre of PGIMER,Chandigarh,India.One hundred and twenty-six patients aged≥13 years were diagnosed with scrub typhus.Severe disease was defined as the presence of organ failure based on the Sequential Organ Failure Assessment(SOFA)score on admission.Results:About two-thirds of the patients were from geographic regions outside the endemic sub-Himalayan belt.Fever(99.21%)and dyspnea(79.36%)were the most frequent complaints.Respiratory failure(81.75%)was the most common organ failure,followed by hepatic(52.38%),coagulative(47.62%),circulatory(33.33%),renal(21.43%),and cerebral dysfunction(13.49%).The median(Q1,Q3)SOFA score was 8(6,9),and 48.41%of the patients had a quick-SOFA score≥2.Organ supports with invasive ventilation(40.48%),vasopressors(36.51%),and renal replacement therapy(7.14%)were frequently required.The in-hospital mortality was 11.90% and was independently predicted by circulatory and hepatic failures on multivariate logistic regression(OR 11.12,95%CI 1.73-71.31 and OR 8.49,95%CI 1.18-61.41,respectively).Conclusions:Most patients had pulmonary dysfunction;circulatory or hepatic failure on admission strongly predicts death. | Ashok Kumar Pannu Atul Saroch Saurabh Chandrabhan Sharda Manoj Kumar Debnath Manisha Biswal Navneet Sharma | 2021 | Asian Pacific Journal of Tropical Medicine2021，14，5： | 2 |

| 12 | Impact of vericiguat on heart failure with reduced ejection fraction:a review显示文摘Introduction:Heart failure is a major public health issue with a prevalence of about 26 million people worldwide.Reduced nitric oxide availability,lower soluble guanylate cyclase(sGC)activity,and decreased cyclic guanosine monophosphate(cGMP)production are the causes of HF's development.Vericiguat prescribed under the brand name Verquvo was approved by U.S.Food and Drug Administration(FDA)in January 2021.It is a novel agent and the first sGC stimulator which helps to treat patients suffering from heart failure with reduced ejection fraction(HFrEF).Objective:The mechanism of action(cGMP pathway)of vericiguat,its clinical trials,its use in the treatment of heart failure,and its possible future aspects in therapeutic recommendations are all covered in this review.It will also raise awareness amongst healthcare professionals about the pharmacokinetic and pharmacodynamic parameters,dosing,administration,and drug-related problems of this new drug.Methods:Various databases for drug review were used in this review like PubMed,Medline,Google scholar,Drug bank,U.s.FDA,Medscape,and European society of cardiology guidelines.A total of 58 articles were screened out of which 39 articles were included in this review.Results:This review discusses vericiguat's mechanism of action(cGMP pathway),clinical studies,application in the treatment of heart failure,and potential future considerations in therapeutic recommendations.It will also educate healthcare professionals about the new drug's pharmacokinetics and pharmacodynamics,dose,administration,and drug-related problems.Conclusion:After hospitalization for HFrEF,the 5-year survival rate is just 25%,and disease morbidity and death are stil significant.As adjunctive therapy for individuals with heart failure and a low ejection fraction,vericiguat has a moderate level of effectiveness.Vericiguat's efficacy as an adjunct therapy to different drugs used to cure HF has to be further investigated.Vericiguat's safety and dosage in patients who have severe renal or hepatic illness need to be studied further. | Manisha Vohra Mohammad Amir Ian Osoro Amit Sharma Ranjeet Kumar | 2023 | Global Health Journal2023，7，3： | 1 |
| 13 | Audiovestibular functioning of post-menopausal females with osteoporosis and osteopenia显示文摘Introduction:Osteoporosis and osteopenia are progressive disorders characterized by decreased bone mass,especially in postmenopausal women.These can be associated with body pain,fractures,hearing loss and balance disorders.The present study aims to evaluate audio-vestibular function in postmenopausal patients with osteopenia or osteoporosis.Methods:The study included 48 postmenopausal women(new subjects)diagnosed with osteoporosis(n=23)or osteopenia(n=25)in the age range of 50e66 years,as well as 28 normal women as controls.Audiological testing included pure tone audiometry(conventional and extended high-frequency audiometry),speech audiometry,impedance audiometry and otoacoustic emissions,including both transient evoked otoacoustic emissions(TEOAEs)and distortion product otoacoustic emissions(DPOAEs).All subjects also underwent vestibular evoked myogenic potentials testing(both ocular and cervical VEMPs).Results:In the present study,hearing was worse at all frequencies in the osteoporosis group in comparison with the osteopenia and control groups,with worse speech recognition and discrimination scores and OAEs.Vestibular function was affected in 95.65%of women with osteoporosis and 76%of those with osteopenia.Conclusion:Osteoporosis and osteopenia are risk factors for vestibular dysfunction and hearing deficits in postmenopausal women.Thus,hearing and vestibular function should be monitored by audiological and vestibular testing periodically in these individuals. | Manisha K.Juneja Sanjay Munjal Anuradha Sharma Ashok K.Gupta Sanjay Bhadada | 2021 | Journal of Otology2021，16，1： | 1 |
