| 1 | Managing malignant biliary obstruction in pancreas cancer: Choosing the appropriate strategy显示文摘Most patients with pancreatic cancer develop malignant biliary obstruction.Treatment of obstruction is generally indicated to relieve symptoms and improve morbidity and mortality.First-line therapy consists of endoscopic biliary stent placement.Recent data comparing plastic stents to self-expanding metallic stents(SEMS)has shown improved patency with SEMS.The decision of whether to treat obstruction and the means for doing so depends on the clinical scenario.For patients with resectable disease,preoperative biliary decompression is only indicated when surgery will be delayed or complications of jaundice exist.For patients with locally advanced disease,self-expanding metal stents are superior to plastic stents for long-term patency.For patients with advanced disease,the choice of metallic or plastic stent depends on life expectancy.When endoscopic stent placement fails,percutaneous or surgical treatments are appropriate.Endoscopic therapy or surgical approach can be used to treat concomitant duodenal and biliary obstruction. | Brian R Boulay Mayur Parepally | 2014 | World Journal of Gastroenterology2014，20，28： | 18 |

| 3 | One fifth of hospitalizations for peptic ulcer-related bleeding are potentially preventable显示文摘AIM:To calculate the proportion of potentially preventable hospitalizations due to peptic ulcer disease(PUD),erosive gastritis(EG)or duodenitis(ED).METHODS:Retrospective cohort study using ICD-10codes to identify all patients with upper gastrointestinal hemorrhage secondary to endoscopically proven PUD,EG or ED during the period from March 2007 to October 2010 in three major metropolitan hospitals in Melbourne,Australia.Patients were divided into'high risk'(those who would benefit from gastroprotection)and'not high risk'groups as defined by established guidelines.Mean Rockall score,transfusion requirement,length of stay,rebleeding rates,need for surgery and in-hospital mortality was compared between'high risk'and'not high risk'groups.Within the'high risk'group,those on gastroprotection and those with no gastroprotection were also compared.RESULTS:Five hundred and seven patients were included for analysis of which 174 were classified as high risk.Median values of complete Rockall Score(5vs 4,P=0.002)and length of stay(5 d vs 4 d,P=0.04)were higher in the high risk group but in-hospital mortality was lower(0.6%vs 3.9%,P=0.03).130out of the 174 patients in the high risk group were not taking recommended gastroprotective therapy prior to hospitalization.Past history of PUD(OR=3.7,P=0.006)and clopidogrel use(OR=3.2,P=0.007)significantly predicted prescription of gastroprotective therapy.Using proton pump inhibitor protection rates of 50%-85%from published studies,an estimation of13%to 22%of the total number of the hospitalizations due to PUD or EG/ED related bleeding may have been preventable.CONCLUSION:Up to one fifth of all hospitalizations for bleeding secondary to PUD or EG/ED are potentially preventable. | Ray Boyapati Sim Ye Ong Bei Ye Anuk Kruavit Nora Lee Rhys Vaughan Sanjay Nurkar Peter Gibson Mayur Garg | 2014 | World Journal of Gastroenterology2014，20，30： | 8 |
| 4 | Low-velocity simultaneous bilateral femoral neck fracture following long-term antiepileptic therapy:A case report显示文摘BACKGROUND Simultaneous bilateral femoral neck fractures are relatively rare injuries.They are usually associated with underlying metabolic bone disorders or systemic diseases.Long-term use of narcotics and bisphosphonates can also result in similar fracture patterns;however,association of this fracture type with longterm use of antiepileptic drugs is not very common.Only one such case has been reported in the literature.This article describes the second.CASE REPORT We report a case of simultaneous displaced bilateral femoral neck fractures in a 50-year-old epileptic patient,who had taken phenytoin for the past 3 years.The fractures were a result of low-velocity injury following a fall from the bed.The fractures were managed with a bilateral hemi-replacement arthroplasty.Oral bisphosphonates were given to improve the bone quality in the post-operative period.The patient had a good post-operative outcome,that was sustained throughout the entire follow-up period of 1 year.CONCLUSION Antiepileptic drugs should be supplemented with bisphosphonates and vitamin D to improve bone quality and prevent fractures in epileptic patients. | Mohammed Sadiq Vikrant Kulkarni Syed Azher Hussain Mohammed Ismail Mayur Nayak | 2019 | World Journal of Orthopedics2019，10，10： | 5 |

| 6 | Differential fate of metabolism of a disperse dye by microorganisms Galactomyces geotrichum and Brevibacillus laterosporus and their consortium GG-BL显示文摘The present work aims to evaluate Brown 3 REL degrading potential of developed microbial consortium GG-BL using two microbial cultures, Galactomyces geotrichum MTCC 1360 (GG) and Brevibacillus laterosporus MTCC 2298 (BL). Microbial consortium GG-BL showed 100% decolorization of a dye Brown 3 REL, while individually G. geotrichum MTCC 1360 and B. laterosporus MTCC 2298 showed 26% and 86% decolorization under aerobic condition (shaking) respectively. Measurements of biochemical oxygen demand (BOD) (76%) and chemical oxygen demand (COD) (68%) were done after decolorization by consortium GG-BL. No induction in activities of oxidoreductive enzymes found in G. geotrichum while B. laterosporus showed induction of veratryl alcohol oxidase, Nicotineamide adenine dinucleotide-dichlorophenol indophenol (NADH-DCIP) reductase and riboflavin reductase indicating their role in dye metabolism. Consortium GG-BL showed induction in the activities of laccase, veratryl alcohol oxidase, tyrosinase, NADH-DCIP reductase and riboflavin reductase. Two different sets of induced enzymes from G. geotrichum and B. laterosporus work together in consortium GG-BL resulting in faster degradation of dye. The degradation of Brown 3 REL was analyzed using high performance thin layer chromatography (HPTLC), high performance liquid chromatography (HPLC), Fourier transform infrared spectroscopy (FT-IR) and gas chromatography mass spectroscopy (GC-MS).
