| 1 | Prognostic value of cortisol and thyroid function tests in poisoned patients admitted to toxicology ICU显示文摘BACKGROUND: Prognostic value of cortisol and thyroid function tests(TFTs) has previously been evaluated in medical ICUs. We aimed to evaluate prognostic efficacy of cortisol and TFTs in critically ill poisoned patients admitted to toxicology intensive care unit(ICU).METHODS: In a prospective study of consecutively enrolled subjects admitted to the toxicology ICU, lab analyses included TFTs(total T3 and T4 as well as TSH) and cortisol levels drawn between 8 am–10 am during period of the first 24 hours post-ingestion/exposure. Simplified Acute Physiology Score Ⅱ(SAPS Ⅱ) and Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) were recorded. All scores were compared to detect the best prognostic factor. Type of poisoning was also included.RESULTS: In 200 patients evaluated, 129 were male and mean age was 31 years. In general, SAPS Ⅱ, T4, and cortisol could prognosticate death. After regression analysis, only cortisol had such efficacy(P=0.04; OR=1.06; 95%CI=1.05–1.08; cut-off=42 μg/d L; sensitivity=70%; specificity=82%). Between aluminium phosphide(ALP)-and non ALP-poisoned patients, level of consciousness, mean arterial pressure, and cortisol level could prognosticate death in ALP poisoning(all Ps<0.001 in both uni and multivariate analyses). Median(interquartile range; IQR) GCS was 7(6, 10) and 15(8, 15) in non-ALP and ALP-poisoned patients(P<0.003). SAPS Ⅱ and APACHE Ⅱ could not prognosticate death at all.CONCLUSION: Cortisol best prognosticated outcomes for subjects admitted to the toxicology ICU. | 3 | Effects of Zataria oxymel on obesity, insulin resistance and lipid profile:A randomized, controlled, triple-blind trial显示文摘Background: Obesity is a major public health problem and its occurrence is markedly increasing in developed and developing countries. However, few studies have investigated the use of natural products to treat obesity. The effects of taking a combination of oxymel and Zataria multiflora Boiss.(ZM), herein referred to as Zataria oxymel(ZO), on obesity, lipid profile and insulin resistance have not yet been studied.Objective: This study evaluates the effects of oxymel and ZO on obesity, lipid profile and insulin resistance.Design, setting, participants and interventions: In this randomized, controlled, triple-blind trial, overweight patients were randomly divided into three groups and received doses of study compounds twice per day for twelve weeks. Group A received 0.75 g ZM in 10 mL oxymel in each treatment;group B received 1.5 g ZM in 10 m L of oxymel in each treatment and group C(control) only received 10 mL of oxymel in each treatment.Main outcome measures: Anthropometric parameters, including body mass index(BMI), waist circumference and hip circumference, were measured at the time of registration. Blood tests were carried out at the beginning and once again at end of the study. Blood parameters included fasting blood sugar(FBS), insulin levels, serum lipid profile(total cholesterol, triglyceride, low-density lipoprotein cholesterol and highdensity lipoprotein cholesterol) and liver enzymes(aspartate transaminase and alanine transaminase).Serum creatinine was also measured at the beginning of the project and in monthly intervals for three months. The homeostasis model assessment index was calculated as fasting insulin(μIU/mL)×FBS(mg/dL)/405.Results: The results showed that patients receiving ZO experienced significant reduction in waist circumference in groups A, B and C, respectively(P < 0.001) but no significant change in BMI. Group A also experienced reduction in hip circumference(P = 0.01). Groups B and C had reduction in the homeostatic model assessment of insulin resistance(P = 0.05 and P = 0.01, respectively), with no significant reduction in FBS.No effect on lipid profile, liver enzymes or serum creatinine was observed in the three groups.Conclusion: In this study, treatment with ZO and oxymel reduced insulin resistance, and waist and hip circumferences in overweight patients. | 5 | Outbreak of methanol-induced optic neuropathy in early COVID-19 era;effectiveness of erythropoietin and methylprednisolone therapy显示文摘BACKGROUND Methanol is a highly toxic,non-potable alcohol.Outbreaks of methanol toxicity occur due to its fraudulent addition to alcoholic beverages as a cheaper substitute for ethanol.Recently,alongside the coronavirus disease 2019(COVID-19)pandemic,rumors circulated on social media that consuming alcohol can prevent or cure the virus,leading to a COVID-19 and methanol-induced optic neuropathy(MON)syndemic.AIM To investigate the impact of erythropoietin(EPO)on the outcomes of patients diagnosed with MON.METHODS In this prospective study,105 patients presenting with acute bilateral visual loss secondary to methanol intoxication were enrolled from March to May 2020 at Farabi Eye Hospital.A comprehensive ocular examination was conducted for all participants.Recombinant human EPO and methylprednisolone were administered intravenously to all patients for three consecutive days.RESULTS The mean age of the participants was 39.9 years(±12.6).Ninety-four patients were male and eleven were female.The mean pre-treatment best corrected visual acuity(BCVA)improved from 2.0±0.86 to 1.39±0.69 logarithm of the minimum angle of resolution post-treatment(P<0.001),with significant improvement observed in all age categories and genders(P<0.001).Visual acuity improvement was also significant regardless of whether the patient presented before or after 72 h(P<0.001),and the post-treatment BCVA remained significant at all monthly follow-up visits(P<0.001).CONCLUSION EPO and methylprednisolone therapy have been shown to be effective in improving visual outcomes in patients with MON when administrated within the first month of exposure.Public awareness efforts are necessary to prevent further outbreaks of methanol toxicity in the current COVID-19 era. | Seyed Ali Tabatabaei Mohammad Amini Ali A Haydar Mohammad Soleimani Kasra Cheraqpour Mansoor Shahriari Hossein Hassanian-Moghaddam Nasim Zamani Mohammad Reza AkbariDepartment of Ophthalmology Eye Research Center Farabi Eye | 2023 | World Journal of Clinical Cases2023,11,15: | 0 |
