| 12 | Real-time ultrasound-guided versus landmark-guided subclavian vein catheterization in the intensive care unit: a prospective randomized study显示文摘Background:The subclavian vein(SCV)is an alternative to the internal jugular vein when it is difficult to locate,such as in patients with hypovolemia or obesity.Ultrasonography(USG)guidance for SVC cannulation has evolved,resulting in fewer complications and higher first-pass success rates.This study aimed to compare the effectiveness and safety of SCV cannulation with USG-and landmark-guided techniques.Methods:In this prospective randomized interventional controlled study,80 patients admitted to the intensive care unit between July 2022 and October 2022 were randomly assigned to the landmark method group(LM group)and USG group.In the LM group,SCV cannulation was performed using the traditional landmark technique,whereas in the USG group,it was performed using USG guidance.The primary objective of this study was to evaluate the ease of subclavian central venous cannulation in critically ill patients using the 2 techniques.The secondary objectives were to compare the success rate of cannulation between these 2 techniques,evaluate the number of attempts,assess cannulation failure,and assess mechanical complications.Results:The first-pass success rates were 70%and 92.5%in the LM and USG groups,respectively(P<0.001).The average numbers of attempts in the LM and USG groups were 1.275(±0.520)and 1.075(±0.266),respectively(P=0.034).The average procedure durations were 7.45(±1.10)and 8(±0.933)minutes in the LM and USG groups(P=0.018),respectively.The rates of complications in both groups were not statistically significant.Conclusion:The USG guidance for SCV cannulation has an advantage over landmark-guided methods in a critical care setting.The SCV is a good alternative to internal jugular vein cannulation.The average time to cannulation was longer in the USG group than in the LM group,which can decrease with the frequent use of USG and increasing operator experience.Clinical trials:This study was registered in the Clinical Trials Registry-India(CTRI Trial No.CTRI/2022/07/043694,dated May 7,2022). | Sujit J.Kshirsagar Sanyogita V.Naik Anandkumar H.Pande Pradnya M.Bhalerao Chandraprabhu Birnale Shivprasad Thorve | 2023 | Emergency and Critical Care Medicine2023,3,2: | 0 |
| 13 | A four parameter cubic equation of state with temperature dependent covolume parameter显示文摘A four-parameter, Ghoderao–Dalvi–Narayan 2 cubic equation of state(GDN2 CEOS), is presented which incorporates the following: 1. The experimental value of the critical compressibility factor has been used as a fixed input parameter for calculations;2. All the parameters(a, b, c, d) of CEOS are temperature dependent functions in the subcritical region and are temperature independent functions in the supercritical region and;3. A new α function is introduced with two compound specific parameters which are estimated by matching saturated vapor pressure at two fixed temperature points Tr= 0.5, 0.7. Our formalism enables us to cast three of the four parameters of the CEOS as a function of the remaining parameter. The proposed CEOS is used to predict properties of 334 pure compounds, including saturated vapor pressure and liquid density, compressed liquid density, heat capacities at the constant pressure and volume, enthalpy of vaporization, sound velocity. To calculate thermodynamic properties of a pure compound, the present CEOS require the critical temperature, the critical pressure, the Pitzer’s acentric factor, the critical compressibility factor, and two parameters of the alpha function. The saturated liquid density predictions for pure fluids are very accurate when compared with GDN1(Ghoderao–Dalvi–Narayan 1),MPR(Modified Peng–Robinson), and PT(Patel–Teja) equations of state. Unlike MPR EOS, the proposed temperature dependent covolume parameter b in the present work satisfies all the constraints mentioned in the literature to avoid thermodynamic inconsistencies at the extreme temperature and pressure. | 14 | Phloretin-induced suppression of oxidative and nitrosative stress attenuates doxorubicin-induced cardiotoxicity in rats显示文摘Objective:To compare the cardioprotective efficacy of equimolar doses(50 mM/kg,p.o.)of phloretin and genistein against doxorubicin-induced cardiotoxicity in rats.Methods:Cardiotoxicity was induced in rats by intraperitoneal injection of 6 mg/kg doxorubicin on alternative days till the cumulative dose reached 30 mg/kg.This study included four treatment groups of rats(n=6):the control group(0.5%carboxymethyl cellulose solution-treated),the doxorubicin-treated group(0.5%carboxymethyl cellulose solution along with doxorubicin),the genistein-treated group(50 mM/kg/day;p.o.along with doxorubicin)and phloretin-treated group(50 mM/kg/day;p.o.along with doxorubicin).On the 10th day of dosing,rats were anesthetized for recording ECG,mean arterial pressure,and left ventricular function.Oxidative stress,nitric oxide levels,and inflammatory cytokines were estimated in the cardiac tissue.Cardiac function parameters(creatine kinase MB,lactate dehydrogenase,aspartate aminotransferase,and alanine transaminase)were estimated in the serum samples.Results:Phloretin treatment inhibited doxorubicin-induced oxidative stress and also reduced nitric oxide levels in cardiac tissues of rats.Phloretin administration attenuated doxorubicin-induced alterations in hemodynamic parameters(heart rate,mean arterial blood pressure,and left ventricular function)and suppressed the expression of pro-inflammatory cytokines.The cardiac injury markers like creatine kinase MB,lactate dehydrogenase,aspartate aminotransferase,and alanine transaminase were reduced by both genistein and phloretin.All these effects of phloretin were more prominent than genistein.Conclusions:Phloretin offers cardioprotection that is comparable to genistein,a clinically validated cardioprotectant against doxorubicin-induced cardiotoxicity.Further studies are needed to confirm and establish the therapeutic utility of phloretin as a chemopreventive adjuvant to doxorubicin chemotherapy. | Shivani S.Wagh Kalpesh R.Patil Umesh B.Mahajan Pradnya D.Bagal Avinash R.Wadkar Basavraj Bommanhalli Prabhakar R.Patil Sameer N.Goyal Shreesh Ojha Chandragouda R.Patil | 2022 | Asian Pacific Journal of Tropical Biomedicine2022,12,3: | 0 |

| 16 | Prevalence of precancerous lesions and conditions in India: A systematic review and meta-analysis显示文摘BACKGROUND Smoking and chewing tobacco are associated with numerous oral mucosal lesions and conditions,often leading to cancer progression.AIM To investigate the prevalence of precancerous lesions and conditions among the Indian population.METHODS Systematic search was conducted for population or community-based observational epidemiological studies in PubMed,EMBASE,Web of Science,IndMED,Google Scholar,reports of the WHO South-East Asia Region,MOHFW India reports,Science Citation Index,WHO Index Medicus of the South-East Asian Region,Reference Citation Analysis(http://gffzzb1f9d057b5954674snwonnu6o9vnu669u.ffgz.tsg.suse.edu.cn/)and Open Grey from the earliest available up to 31st January 2022.The effect size was calculated for the prevalence of precancerous lesions and conditions.RESULTS One hundred sixty-two estimates from 130 studies yielded 52 high,71 moderate,and seven low-quality studies from 823845.Point estimate based on crosssectional studies for leukoplakia was 4.3%(95%CI:4.0-4.6),oral submucous fibrosis was 2.7%(95%CI:2.5-3.0),palatal lesions in reverse smokers and nicotine palatine were 5.8%(95%CI:4.4-7.2),and Erythroplakia was 1.2%(95%CI:0.7-1.7),and lichen planus was 1.1%(95%CI:0.9-1.2).Amongst hospital-based studies,the pooled prevalence for Leukoplakia was 6.7%(95%CI:6.0-7.3),oral submucous fibrosis was 4.5%(95%CI:4.2-4.9),lichen planus was 7.5%(95%CI:5.3-9.6),and erythroplakia was 2.5%(95%CI:0.4-4.5),and palatal lesions in reverse smokers and nicotine palatini were 11.5%(95%CI:8.0-15.0).CONCLUSION Precancerous lesions and conditions are prevailing problems among the Indian population.It is mainly due to tobacco use,the smokeless form of tobacco.The meta-analysis indicates that hospital-based studies have a higher effect size of 6.7%than community-based studies.Patients who have already developed this condition may be advised to reduce their exposure to the risk factor to prevent the condition from progressing further. | Abhishek Kumbhalwar Sahana Hegde Shetiya Pradnya Kakodkar Vini Mehta Ankita Mathur Priyanka Porwal | 2022 | World Journal of Methodology2022,12,4: | 0 |
| 17 | Continuous melt granulation to develop high drug loaded sustained release tablet of Metformin HCl显示文摘The present work explores the application of melt granulation technology to develop a high drug loaded sustained release matrix tablet of Metformin HCl using hydroxypropylcellulose(HPC) as a hydrophilic binder and stearic acid as an extrusion aid for producing cohesive granules. This novel approach allowed the use of a minimum number of excipients to reduce the tablet size, and to enhance compressibility of the drug. This also offered a cost effective method owing to the elimination of a ‘drying step’ prevalent in wet granulation method.Moreover, this research also focuses on resolving the processability issues associated with the use of HPC Nisso-H at high drug loading. The thermal lubricants were screened for this purpose and evaluated for their impact on extrudability, granule and tablet characteristics. Stearic acid was selected as the thermal lubricant, which not only contributed to the inhibition of burst release, but also improved the flow property of the granules.The developed matrix tablet(75% drug loading) resulted in 670 mg of weight for 500 mg dose strength and showed sustained drug release over 10 h. When compared, with conventional granulation techniques, it was observed that, under identical compression force, the tablet prepared by MG exhibited superior compactibility along with tablet hardness and optimal drug release profile. FTIR suggested nonexistence of chemical interaction between the drug and the other excipients while XRD and DSC analysis revealed the crystalline state of the drug.Furthermore, the results obtained from Raman spectroscopy proved the uniform distribution of the Metformin HCl and polymer in the final dosage form.
