| 1 | How important is donor age in liver transplantation?显示文摘The age of liver donors has been increasing in the past several years because of a donor shortage. In the United States, 33% of donors are age 50 years or older, as are more than 50% in some European countries. The impact of donor age on liver transplantation(LT) has been analyzed in several studies with contradictory conclusions. Nevertheless, recent analyses of the largest databases demonstrate that having an older donor is a risk factor for graft failure. Donor age is included as a risk factor in the more relevant graft survival scores, such as the Donor Risk Index, donor age and Model for End-stage Liver Disease, Survival Outcomes Following Liver Transplantation, and the Balance of Risk. The use of old donors is related to an increased rate of biliary complications and hepatitis C virus-related graft failure. Although liver function does not seem to be significantly affected by age, the incidence of several liver diseases increases with age, and the capacity of the liver to manage or overcome liver diseases or external injuries decreases. | 2 | Biometric and ICL-related risk factors associated to sub-optimal vaults in eyes implanted with implantable collamer lenses显示文摘Background:To identify biometric and implantable collamer lens(ICL)-related risk factors associated with suboptimal postoperative vault in eyes implanted with phakic ICL.Methods:This study reports a retrospective case series of the first operated eye in 360 patients implanted with myopic spherical or toric ICL.Preoperatively,white-to-white(WTW),central keratometry(Kc)and central corneal thickness(CCT)were measured using the Pentacam.Anterior-segment optical coherence tomography(AS-OCT,Visante)was applied preoperatively for measuring the horizontal anterior-chamber angle-to-angle distance(ATA),internal anterior chamber depth(ACD),crystalline lens rise(CLR),anterior-chamber angle(ACA)and postoperatively the vault.Eyes were divided into three vault groups:low(LVG:≤250μm),optimal(OVG:>250 and<1000μm)and high(HVG:≥1000μm).Multinomial logistic regression(MLR)was used to find the sub-optimal vault predictors.Results:MLR showed that CLR,ICL size minus the ATA(ICL size-ATA),age,ICL spherical equivalent(ICLSE)and ICL size as contributing factors for sub-optimal vaults(pseudo-R2=0.40).Increased CLR(OR:1.01,CI:1.00-1.01)and less myopic ICLSE(OR:1.22,CI:1.07-1.40)were risk factors for low vaults.Larger ICL size-ATA(OR:41.29,CI:10.57-161.22)and the 13.7mm ICL(OR:7.08,CI:3.16-15.89)were risk factors for high vaults,whereas less myopic ICLSE(OR:0.85,CI:0.76-0.95)and older age(OR:0.92,CI:0.88-0.98)were protective factors.Conclusion:High CLR and low ICLSE were the major risk factors in eyes presenting low vaults.In the opposite direction,ICL size-ATA was the major contributor for high vaults.This relationship was more critical in higher myopic ICLSE,younger eyes and when 13.7mm ICL were used.The findings show that factors influencing the vault have differentiated weight of influence depending on the type of vault(low,optimal or high). | Santiago Cerpa Manito Angel Sánchez Trancón Oscar Torrado Sierra António Manuel Baptista Pedro Miguel Serra | 2021 | Eye and Vision2021,8,1: | 5 |
| 3 | Emerging concepts in liver graft preservation显示文摘The urgent need to expand the donor pool in order to attend to the growing demand for liver transplantation has obliged physicians to consider the use of suboptimal liver grafts and also to redefine the preservation strategies. This review examines the different methods of liver graft preservation, focusing on the latest advances in both static cold storage and machine perfusion(MP). The new strategies for static cold storage are mainly designed to increase the fatty liver graft preservation via the supplementation of commercial organ preservation solutions with additives. In this paper we stress the importance of carrying out effective graft washout after static cold preservation, and present a detailed discussion of the future perspectives for dynamic graft preservation using MP at different temperatures(hypothermia at 4 ℃, normothermia a t 3 7 ℃ and subnormothermia at 20 ℃- 2 5 ℃). Finally, we highlight some emerging applications of regenerative medicine in liver graft preservation. | 5 | Liver bioengineering:Current status and future perspectives显示文摘The present review aims to illustrate the strategies that are being implemented to regenerate or bioengineer livers for clinical purposes.There are two general pathways to liver bioengineering and regeneration.The first consists of creating a supporting scaffold,either synthetically or by decellularization of human or animal organs,and seeding cells on the scaffold,where they will mature either in bioreactors or in vivo.This strategy seems to offer the quickest route to clinical translation,as demonstrated by the development of liver organoids from rodent livers which were repopulated with organ specific cells of animal and/or human origin.Liver bioengineering has potential for transplantation and for toxicity testing during preclinical drug development.The second possibility is to induce liver regeneration of dead or resected tissue by manipulating cell pathways.In fact,it is well known that the liver has peculiar regenerative potential which allows hepatocyte hyperplasia after amputation of liver volume.Infusion of autologous bone marrow cells,which aids in liver regeneration,into patients was shown to be safe and to improve their clinical condition,but the specific cells responsible for liver regeneration have not yet been determined and the underlying mechanisms remain largely unknown.A complete understanding of the cell pathways and dynamics and of the functioning of liver stem cell niche is necessary for the clinical translation of regenerative medicine strategies.As well,it will be crucial to elucidate the mechanisms through which cells interact with the extracellular matrix,and how this latter supports and drives cell fate. | Christopher Booth Tom Soker Pedro Baptista Christina L Ross Shay Soker Umar Farooq Robert J Stratta Giuseppe Orlando | 2012 | World Journal of Gastroenterology2012,18,47: | 2 |

| 12 | Exosomal glypican-1 is elevated in pancreatic cancer precursors and can signal genetic predisposition in the absence of endoscopic ultrasound abnormalities显示文摘BACKGROUND Individuals within specific risk groups for pancreatic ductal adenocarcinoma(PDAC)[mucinous cystic lesions(MCLs),hereditary risk(HR),and new-late onset diabetes mellitus(NLOD)]represent an opportunity for early cancer detection.Endoscopic ultrasound(EUS)is a premium image modality for PDAC screening and precursor lesion characterization.While no specific biomarker is currently clinically available for this purpose,glypican-1(GPC1)is overexpressed in the circulating exosomes(crExos)of patients with PDAC compared with healthy subjects or those harboring benign pancreatic diseases.AIM To evaluate the capacity of GPC1+crExos to identify individuals at higher risk within these specific groups,all characterized by EUS.METHODS This cross-sectional study with a prospective unicentric cohort included 88 subjects:40 patients with MCL,20 individuals with HR,and 20 patients with NLOD.A control group(CG)was submitted to EUS for other reasons than pancreatic pathology,with normal pancreas and absence of hereditary risk factors(n=8).The inclusion period was between October 2016 and January 2019,and the study was approved by the Ethics Committee of Centro Hospitalar Universitário de São João,Porto,Portugal.All patients provided written informed consent.EUS and blood tests for quantification of GPC1+crExos by flow cytometry and carbohydrate antigen 19-9(CA 19-9)levels by ELISA were performed in all subjects.EUS-guided tissue acquisition was done whenever necessary.For statistical analysis,SPSS®27.0(IBM Corp.,Armonk,NY,United States)version was used.All graphs were created using GraphPad Prism 7.00(GraphPad Software,San Diego,CA,United States).RESULTS Half of MCLs harbored worrisome features(WF)or high-risk stigmata(HRS).Pancreatic abnormalities were detected by EUS in 10.0%and 35.0%in HR and NLOD individuals,respectively,all considered non-malignant and“harmless.”Median levels of GPC1+crExos were statistically different:MCL[99.4%,interquartile range(IQR):94.9%-99.8%],HR(82.0%,IQR:28.9%-98.2%),NLOD(12.6%,IQR:5.2%-63.4%),and CG(16.2%,IQR:6.6%-20.1%)(P<0.0001).Median levels of CA 19-9 were within the normal range in all groups(standard clinical cut-off of 37 U/mL).Within HR,individuals with a positive history of cancer had higher median levels of GPC1+crExos(97.9%;IQR:61.7%-99.5%),compared to those without(59.7%;IQR:26.3%-96.4%),despite no statistical significance(P=0.21).Pancreatic cysts with WF/HRS were statistically associated with higher median levels of GPC1+crExos(99.6%;IQR:97.6%-99.8%)compared to those without(96.5%;IQR:81.3%-99.5%)(P=0.011),presenting an area under the receiver operating characteristic curve value of 0.723(sensitivity 75.0%and specificity 67.7%,using a cutoff of 98.5%;P=0.012).CONCLUSION GPC1+crExos may act as biomarker to support the diagnosis and stratification of PDAC precursor lesions,and in signaling individuals with genetic predisposition in the absence of EUS abnormalities. | Pedro Moutinho-Ribeiro Ines A Batista Sofia T Quintas Bárbara Adem Marco Silva Rui Morais Armando Peixoto Rosa Coelho Pedro Costa-Moreira Renato Medas Susana Lopes Filipe Vilas-Boas Manuela Baptista Diogo Dias-Silva Ana L Esteves Filipa Martins Joanne Lopes Helena Barroca Fátima Carneiro Guilherme Macedo Sonia A Melo | 2022 | World Journal of Gastroenterology2022,28,31: | 0 |
