| 2 | China National Medical Products Administration approval summary:anlotinib for the treatment of advanced non-small cell lung cancer after two lines of chemotherapy显示文摘Background:On May 8,2018,the China National Medical Products Administration(NMPA)approved anlotinib,an orally administered anti-angiogenesis inhibitor,for the treatment of patients with advanced non-small cell lung can-cer(NSCLC)who have progressed after treatment with two or more lines of prior systemic chemotherapy.Main body of the abstract:China NMPA reviewed and inspected a regional double-blinded,placebo-controlled,Phase III trial comparing the overall survival(OS)of NSCLC patients between the anlotinib and placebo arms.A total of 437 patients were randomized(2:1)to receive either anlotinib(n=294)or placebo(n=143)once daily on a 2-week on and 1-week off schedule.Patients with epidermal growth factor receptor(EGFR)or activating anaplastic lymphoma kinase(ALK)genomic tumor aberrations should have disease progression on NMPA-approved therapy.Anlotinib is the first NMPA-approved drug for patients with advanced NSCLC who have progressed on at least two lines of prior systemic chemotherapies in China.The approval was based on a statistically and clinically significant improvement in median OS with anlotinib(9.46 months)compared with placebo[6.37 months;hazard ratio(HR])=0.70,95%confidence interval(CI)=0.55-0.89;two-sided log-rank P=0.002].The confirmed objective response rate(ORR)was 9.2%in the anlotinib arm and 0.7%in the placebo arm.The median duration of response(DoR)was 4.83 months,with a 95%CI of 3.31-6.97 months.The toxicity profile of anlotinib was consistent with that of known anti-angiogenesis inhibitors.Common adverse drug reactions(ADRs)in anlotinib-treated patients included hypertension(67.4%),hand-foot syndrome(43.9%),hemoptysis(14.0%),thyroid stimulating hormone(TSH)elevation(46.6%),and corrected QT interval(QTc)prolongation(26.2%).Short conclusion:Anlotinib demonstrated a clinically significant OS prolongation as a novel therapeutic option for advanced or metastatic NSCLC following at least two lines of chemotherapy. | Ming Zhou Xiaoyuan Chen Hong Zhang Lin Xia Xin Tong Limin Zou Ruimin Hao Jianhong Pan Xiao Zhao Dongmei Chen Yuanyuan Song Yueli Qi Ling Tang Zhifang Liu Rong Gao Yuankai Shi Zhimin Yang | 2019 | Cancer Communications2019,39,1: | 35 |
| 3 | EXPERIMENTAL STUDY OF FLOWFIELD STRUCTURE AROUND AN OGIVE-CYLINDER显示文摘The flowfield structure and their aerodynamic characteristics over an ogive cylinder were studied by means of flow visualization and surface pressure measurement in a water tunnel and a wind tunnel. The existence of multi asymmetric vortices over long slender bodies was experimentally confirmed at large angles of attack and in the subcritical Reynolds number range. The spatial 3 D characteristics of the multi vortices system were analyzed and a physical model was developed. The topological structure of different patterns in cross flow plane was studied and the mechanism governing the formation of asymmetric vortices and multi vortices was discussed from the viewpoint of stability of the topological structure. | 4 | Minimally invasive surgery for gastric cancer: A comparison between robotic, laparoscopic and open surgery显示文摘AIM To investigate the role of minimally invasive surgery for gastric cancer and determine surgical, clinical, and oncological outcomes.METHODS This is a propensity score-matched case-control study, comparing three treatment arms: robotic gastrectomy(RG), laparoscopic gastrectomy(LG), open gastrectomy(OG). Data collection started after sharing a specific study protocol. Data were recorded through a tailored and protected web-based system. Primary outcomes: harvested lymph nodes, estimated blood loss, hospital stay, complications rate. Among the secondary outcomes, there are: operative time, R0 resections, POD of mobilization, POD of starting liquid diet and soft solid diet. The analysis includes the evaluation of type and grade of postoperative complications. Detailed information of anastomotic leakages is also provided.RESULTS The present analysis was carried out of 1026 gastrectomies. To guarantee homogenous distribution of cases, patients in the RG, LG and OG groups were 1:1:2 matched using a propensity score analysis with a caliper = 0.2. The successful matching resulted in a total sample of 604 patients(RG = 151; LG = 151; OG = 302). The three groups showed no differences in all baseline patients characteristics, type of surgery(P = 0.42) and stage of the disease(P = 0.16). Intraoperative blood loss was significantly lower in the LG(95.93 ± 119.22) and RG(117.91 ± 68.11) groups compared to the OG(127.26 ± 79.50, P = 0.002). The mean number of retrieved lymph nodes was similar between the RG(27.78 ± 11.45), LG(24.58 ± 13.56) and OG(25.82 ± 12.07) approach. A benefit in favor of the minimally invasive approaches was found in the length of hospital stay(P < 0.0001). A similar complications rate was found(P = 0.13). The leakage rate was not different(P = 0.78) between groups.CONCLUSION Laparoscopic and robotic surgery can be safely performed and proposed as possible alternative to open surgery. | 8 | RhoA and RhoC -siRNA inhibit the proliferation and invasiveness activity of human gastric carcinoma by Rho/PI3K/Akt pathway显示文摘AIM: To evaluate the effects of adenovirus-mediated gene transfer of RhoA siRNA and RhoC siRNA on proliferation and invasion of SGC7901 cells by Rho/ PI3K/Akt pathway. METHODS: Plasmid of RhoA siRNA and RhoC siRNA were constructed and transfected into SGC7901 cells. siRNA and LY294002 (PI3K inhibitor) were designed as the control group. The mRNA and protein expressions of RhoA and RhoC were respectively detected with RT-PCR and western blotting. In order to fi nd out the changes of proliferation and invasion power of SGC7901 cell lines, we analyzed the data by MTT, Boyden chamber and evaluated apoptosis of cell with flow cytometry. We treated BALB /C nude mice with RhoA and RhoC-siRNA, and tumor control rate (%) in nude mice was calculated. RESULTS: RhoA and RhoC siRNA transfections specifically down-regulated the corresponding mRNA and protein levels in SGC7901 Cells.The experiment of permeated artificial basal membrane showed that the invasion power of SGC7901 cell lines are on the decline after treatment of Ad-RhoA and RhoC-siRNA (12.64 ± 3.27 vs 87.38 ± 17.38, P < 0.05). The values of 490 nm wavelength light absorption were different in the five groups. The number of alive cells in the group of RhoA and RhoC-siRNA was lower than others in the 6th d (0.71 ± 0.01 vs 3.82 ± 0,11 P < 0.05). The apoptosis rate of transfected RhoA and RhoC-siRNA group with FACS were 19.07% ± 1.78 and there were significant differences between treated and control groups (19.07 ± 1.78% vs 1.23 ± 0.11%, P < 0.01). The tumor transplantation experiment in BALB/C nude mice showed intratumoralinjection of RhoA or RhoC siRNA can inhibit tumor growth. CONCLUSION: RhoA and RhoC siRNA gene therapy mediated by adenovirus may be useful for inhibiting growth and invasion of SGC7901 through a PI3K/Akt pathway. | 10 | Research on diagnosis-related group grouping of inpatient medical expenditure in colorectal cancer patients based on a decision tree model显示文摘BACKGROUND In 2018,the diagnosis-related groups prospective payment system(DRGs-PPS)was introduced in a trial operation in Beijing according to the requirements of medical and health reform.The implementation of the system requires that more than 300 disease types pay through the DRGs-PPS for medical insurance.Colorectal cancer(CRC),as a common malignant tumor with high prevalence in recent years,was among the 300 disease types.AIM To investigate the composition and factors related to inpatient medical expenditure in CRC patients based on disease DRGs,and to provide a basis for the rational economic control of hospitalization expenses for the diagnosis and treatment of CRC.METHODS The basic material and cost data for 1026 CRC inpatients in a Grade-A tertiary hospital in Beijing during 2014-2018 were collected using the medical record system.A variance analysis of the composition of medical expenditure was carried out,and a multivariate linear regression model was used to select influencing factors with the greatest statistical significance.A decision tree model based on the exhaustiveχ^2 automatic interaction detector(E-CHAID)algorithm for DRG grouping was built by setting chosen factors as separation nodes,and the payment standard of each diagnostic group and upper limit cost were calculated.The correctness and rationality of the data were re-evaluated and verified by clinical practice.RESULTS The average hospital stay of the 1026 CRC patients investigated was 18.5 d,and the average hospitalization cost was 57872.4 RMB yuan.Factors including age,gender,length of hospital stay,diagnosis and treatment,as well as clinical operations had significant influence on inpatient expenditure(P<0.05).By adopting age,diagnosis,treatment,and surgery as the grouping nodes,a decision tree model based on the E-CHAID algorithm was established,and the CRC patients were divided into 12 DRG cost groups.Among these 12 groups,the number of patients aged≤67 years,and underwent surgery and chemotherapy or radiotherapy was largest;while patients aged>67 years,and underwent surgery and chemotherapy or radiotherapy had the highest medical cost.In addition,the standard cost and upper limit cost in the 12 groups were calculated and re-evaluated.CONCLUSION It is important to strengthen the control over the use of drugs and management of the hospitalization process,surgery,diagnosis and treatment to reduce the economic burden on patients.Tailored adjustments to medical payment standards should be made according to the characteristics and treatment of disease types to improve the comprehensiveness and practicability of the DRGs-PPS. | Suo-Wei Wu Qi Pan Tong Chen | 2020 | World Journal of Clinical Cases2020,8,12: | 12 |

| 13 | Current Status and Future Challenges of Weather Radar Polarimetry: Bridging the Gap between Radar Meteorology/Hydrology/Engineering and Numerical Weather Prediction显示文摘After decades of research and development, the WSR-88 D(NEXRAD) network in the United States was upgraded with dual-polarization capability, providing polarimetric radar data(PRD) that have the potential to improve weather observations,quantification, forecasting, and warnings. The weather radar networks in China and other countries are also being upgraded with dual-polarization capability. Now, with radar polarimetry technology having matured, and PRD available both nationally and globally, it is important to understand the current status and future challenges and opportunities. The potential impact of PRD has been limited by their oftentimes subjective and empirical use. More importantly, the community has not begun to regularly derive from PRD the state parameters, such as water mixing ratios and number concentrations, used in numerical weather prediction(NWP) models.In this review, we summarize the current status of weather radar polarimetry, discuss the issues and limitations of PRD usage, and explore potential approaches to more efficiently use PRD for quantitative precipitation estimation and forecasting based on statistical retrieval with physical constraints where prior information is used and observation error is included. This approach aligns the observation-based retrievals favored by the radar meteorology community with the model-based analysis of the NWP community. Diffuse Reflectance Infrared Fourier Transform Spectroscopy (DRIFTS) is used to characterize the adsorbed products in situ, combined with Ion Chromatographic (IC), X-ray Photoelectron Spectroscopy (XPS) and Scan Electron Micros- copy (SEM). Our results indicate that the reaction is not limited to the surface of the NaCl particles, but penetrated into the upper layers. Surface reactive sites determine the reaction. However, it is still crucial to understand the mechanism of single-atom catalysis at molecular level for expanding them to other more difficult catalytic reactions, such as ammonia synthesis from nitrogen. Electron spin resonance measurements reveal that these isolated valence electrons can be easily excited to generate free electrons under photo-illumination, thus inducing high efficient photo-induced ammonia synthesis under ambient conditions.The NH_3 producing rate of copper modified carbon nitride(Cu-CN) reached 186 μmol g^(-1) h^(-1) under visible light irradiation with the quantum efficiency achieved 1.01% at 420 nm monochromatic light. This finding surely offers a model to open up a new vista for the ammonia synthesis at gentle conditions. | 20 | Soil erosion rates and characteristics of typical alpine meadow using ~137Cs technique in Qinghai-Tibet Plateau显示文摘Alpine meadow is the predominant ecosystem in Qinghai-Tibet Plateau.The firm turf of alpine meadow formed by sub-surface anfractuous roots can be effective in conserving water and soil.Alpine meadow is a primary contributor to the Chinese Water Tower.For quantitative assess anti-erosion ability of alpine meadow,this paper selected three typical meadow slopes with>60% vegetation coverage to evaluate soil erosion rates using 137Cs.The results showed that(1)soil erosion intensity of typical alpine meadow was slight to light.Erosion rates were 464 t km–2 a–1 in Malong Village,415 t km–2 a–1 in Yeniugou Town and 875 t km–2 a–1 in Zhenqin Town respectively;(2)soil erosion rates were correlated negatively with vegetation coverage,and the relationship was clearer at the slope scale than plot scale;(3)the relationship between soil erosion and vegetation coverage showed that vegetation coverage was a predominant factor in retaining soil and water on slopes.With complete turf and high vegetation coverage, alpine meadow was of great significance for soil conservation and prevention of soil erosion. | SHAO QuanQin XIAO Tong LIU JiYuan QI YongQing | 2011 | Chinese Science Bulletin2011,56,16: | 8 |
