| 1 | 2001-2005年中国4省份精神疾病的患病率、治疗情况和相关残疾:一项流行病学调查显示文摘背景在中国和其他中等收入国家,神经精神状况是影响男性和女性健康的最重要的原因,但由于缺乏有关各种精神疾病患病率、治疗情况和相关残疾的高质量的和具有国家特异性的资料,这些国家提高精神卫生服务的工作受到了限制。因此,我们在中国4个省份开展了一系列流行病学研究,以此来估算相关数据。方法采用多阶段分层随机抽样的方法在中国的4个省抽取了96个城市和267个农村作为初级抽样点;1.13亿个年龄≥18岁的成年人的抽样框架涵盖了中国12%的成年人。在抽样点使用简单随机抽样法选取63004例受试者,采用一般健康问卷(GHQ)的扩展版对这些个体进行筛查,并由精神科医生使用中文版的诊断与统计手册(DSM).Iv轴I障碍定式临床检查(SCID)问卷对其中的16577例受试者进行调查。结果所有精神疾病近1个月的校正患病率为17.5%(95%CI16.6%~18.5%)。其中,心境障碍患病率为6.1%(95%CI5.7%~6.6%),焦虑障碍为5.6%(95%cI5.0%~6.3%),物质滥用障碍为5.9%(95%CI5-3%~6.5%),精神病性障碍为1.0%(95%C10.8%~1.1%)。女性的心境障碍和焦虑障碍的患病率比男性高,年龄〉140岁个体的患病率高于年龄〈40岁的个体。男性酒精使用障碍的患病率是女性的48倍。农村居民比城市居民更可能患有抑郁障碍和酒精依赖。在被诊断患有某种精神疾病的人群中,24%存在由所患精神疾病引起的中度或重度残疾,8%曾向专业人员寻求过帮助,5%曾就诊于精神科。结论本研究结果与全球疾病负担分析中使用的中国精神疾病的患病率、残疾和治疗率估计值有相当大的差距,这需要引起中、低收入国家注意,在提高精神卫生服务前需针对国家具体情况进行详细的现状分析。 | Jingxuan Zhang Qichang Shi Zhiqiang Song Zhijie Ding Shutao Pang Xianyun Li Yali Zhang Zhiqing Wang 张亚利(译) 安静(审校) 费立鹏(审校) | 2009 | 世界临床医学2009，，11： | 5 |
