| 3 | Treatment and outcomes of 1041 pediatric patients with neuroblastoma who received multidisciplinary care in China显示文摘Importance:Neuroblastoma is the most common extracranial malignant solid tumor in children.Multidisciplinary care is critical to improving the survival of pediatric patients with neuroblastoma.Objective:To systematically summarize the clinical characteristics of children with neuroblastoma and evaluate their prognosis with multidisciplinary care provided in a single center.Methods:This retrospective study analyzed the clinical data of 1041 patients with neuroblastoma who were diagnosed,treated,and followed-up in the Hematology-Oncology Center of Beijing Children’s Hospital from 2007 to 2019.Results:The median age at diagnosis was 34 months;80.8%of the patients were younger than 5 years of age.Notably,243 patients(23.3%)were classified as low-risk,249 patients(23.9%)were classified as intermediate-risk,and 549(52.7%)were classified as high-risk.Furthermore,956 patients underwent surgical resections;986(94.7%)patients received chemotherapy;and 176 patients with high-risk neuroblastoma received hematopoietic stem cell transplantation.The 5-year event-free survival(EFS)rate was 91.3%and 5-year overall survival(OS)rate was 97.5%in low-risk group;in the intermediate-risk group,these rates were 85.1%and 96.7%,respectively,while they were 37.7%and 48.9%in the high-risk group(P<0.001 for both).The 5-year EFS and OS rates were significantly higher in patients diagnosed between 2015 and 2019 than in patients diagnosed between 2007 and 2014(P<0.001).In total,278 patients(26.7%)exhibited tumor relapse or progression;the median interval until relapse or progression was 14 months.Of the 233 patients who died,83%died of relapse or progression of neuroblastoma and 4.3%died of therapy-related complications.Interpretation:The 5-year OS rate was low in high-risk patients,compared with low-and intermediate-risk patients.Multidisciplinary care is critical for improvement of survival in pediatric patients with neuroblastoma.Additional treatment strategies should be sought to improve the prognosis of patients with high-risk neuroblastoma. | Yan Su Hong Qin Chenghao Chen Shengcai Wang Shihan Zhang Dawei Zhang Mei Jin Yun Peng Lejian He Xiaoman Wang Suyun Qian Maoquan Qin Ming Ge Fuquan Zhang Qi Zeng Huanmin Wang Xiaoli Ma Xin Ni | 2020 | Pediatric Investigation2020,4,3: | 16 |
| 4 | Altered oral microbiota in chronic hepatitis B patients with different tongue coatings显示文摘AIM To elucidate tongue coating microbiota and metabolic differences in chronic hepatitis B(CHB) patients with yellow or white tongue coatings.METHODS Tongue coating samples were collected from 53 CHBpatients(28 CHB yellow tongue coating patients and 25 CHB white tongue coating patients) and 22 healthy controls.Microbial DNA was extracted from the tongue samples,and the bacterial 16 S ribosomal RNA gene V3 region was amplified from all samples and sequenced with the Ion Torrent PGM^(TM)sequencing platform according to the standard protocols.The metabolites in the tongue coatings were evaluated using a liquid chromatographymass spectrometry(LC-MS) platform.Statistical analyses were then performed.RESULTS The relative compositions of the tongue coating microbiotas and metabolites in the CHB patients were significantly different from those of the healthy controls,but the tongue coating microbiota abundances and diversity levels were not significantly different.Compared with the CHB white tongue coating patients,the CHB yellow tongue coating patients had higher hepatitis B viral DNA(HBV-DNA) titers(median 21210 vs 500,respectively,P = 0.03) and a significantly lower level of Bacteroidetes(20.14% vs 27.93%,respectively,P = 0.013) and higher level of Proteobacteria(25.99% vs 18.17%,respectively,P = 0.045) in the microbial compositions at the phylum level.The inferred metagenomic pathways enriched in the CHB yellow tongue coating patients were mainly those involved in amino acid metabolism,which was consistent with the metabolic disorder.The abundances of bacteria from Bacteroidales at the order level were higher in the CHB white tongue coating patients(19.2% vs 27.22%,respectively,P = 0.011),whereas Neisseriales were enriched in the yellow tongue coating patients(21.85% vs 13.83%,respectively,P = 0.029).At the family level,the abundance of Neisseriaceae in the yellow tongue patients was positively correlated with the HBV-DNA level but negatively correlated with the S-adenosyl-L-methionine level.CONCLUSION This research illustrates specific clinical features and bacterial structures in CHB patients with different tongue coatings,which facilitates understanding of the traditional tongue diagnosis. | Yu Zhao Yu-Feng Mao Yi-Shuang Tang Ming-Zhu Ni Qiao-Hong Liu Yan Wang Qin Feng Jing-Hua Peng Yi-Yang Hu | 2018 | World Journal of Gastroenterology2018,24,30: | 13 |
| 5 | Clinical features of chronic hepatitis B patients with YMDD mutation after lamivudine therapy显示文摘Objective: To study the clinical features of chronic hepatitis B (CHB) patients with tyrosine-methionine-aspartateaspartate (YMDD) mutation after lamivudine therapy. Methods: This investigation was a retrospective study of 63 CHB patients with YMDD mutation during lamivudine therapy. Clinical data, including period and types of YMDD mutation; hepatitis B virus (HBV) DNA levels and alanine aminotransferase (ALT) levels before and after YMDD mutation were measured. YMDD mutation in the HBV DNA polymerase gene was determined using polymerase chain reaction (PCR) and direct sequencing. HBV DNA quantification was determined using real-time PCR. Relevant serum markers of HBV were measured. The follow-up period was 12 months after YMDD mutation. Results: YMDD mutation occurred 7~44 months (median, 21.5 months) after the start of lamivudine therapy. The majority of the cases (42/63, 66.6%) had YMDD mutants detected between 12 and 24 months. Four types of YMDD mutation were observed in this study, rtL180M/M204V mutation was the predominant type (26/63, 41.3%). A proportion of patients (16/63, 25.4%; 12/63, 19.1%) had higher HBV DNA levels and ALT levels (after mutation vs before mutation),respectively. Conclusion: The majority of patients with YMDD mutants had similar or lower HBV DNA levels and ALT levels compared with baseline values. This subset of patients might have benefited from the continued lamivudine therapy. | 6 | Tree-based backoff protocol for fast RFID tag identification显示文摘This paper proposes a tree-based backoff (TBB) protocol that reduces the number of iterations implemented in the procedure of tag collision arbitration in radio frequency identification (RFID) systems. This is achieved by employing the following mechanisms: one is send the request command iteratively to all tags in the interrogation zone until a single tag is identified. The other is backward to the parent node instead of root node to obtain the request parameters and send the request command again until all tags are identified. | 7 | Diffusion kurtosis imaging: correlation analysis of quantitative model parameters with molecular features in advanced lung adenocarcinoma显示文摘Background:Due to development of magnetic resonance-based functional imaging, it is easier to detect micro-structural alterations of tumor tissues. The aim of this study was to conduct a preliminary evaluation of the correlation of non-Gaussian diffusion kurtosis imaging (DKI) parameters with expression of molecular markers (epidermal growth factor receptor [ EGFR];anaplastic lymphoma kinase [ ALK];Ki-67 protein) in patients with advanced lung adenocarcinoma, using routine diffusion-weighted imaging as the reference standard. Methods::Data from patients with primary lung adenocarcinoma diagnosed at Cancer Hospital, Chinese Academy of Medical Sciences (CHCAMS) from 2016 to 2019 were collected for retrospective analysis. The pathologic and magnetic resonance imaging data of 96 patients who met the inclusion criteria were included in this study. Specifically, the Kapp and Dapp parameters measured from the DKI model;apparent diffusion coefficient (ADC) value from the diffusion-weighted imaging model;and the EGFR, ALK, and Ki-67 biomarkers detected by immunohistochemistry and/or molecular biology techniques after biopsy or surgery were evaluated. The relations between quantitative parameters (ADC, Kapp, Dapp) and pathologic outcomes ( EGFR, ALK, and Ki-67 expression) were analyzed by Spearman correlation test. Results:Of the 96 lung adenocarcinoma lesions (from 96 patients), the number of EGFR- and ALK-positive and high Ki-67 expressing lesions were 53, 12, and 83, respectively. The Kapp values were significantly higher among patients with EGFR-positive mutations (0.81 ± 0.12 vs. 0.66 ± 0.10, t = 6.41, P < 0.001), ALK rearrangement-negative (0.76 ± 0.12 vs. 0.60 ± 0.15, t = 4.09, P < 0.001), and high Ki-67 proliferative index (PI) (0.76 ± 0.12 vs. 0.58 ± 0.13, t = 4.88, P < 0.001). The Dapp values were significantly lower among patients with high Ki-67 PI (3.19 ± 0.69 μm 2/ms vs. 4.20 ± 0.83 μm 2/ms, t = 4.80, P < 0.001) and EGFR-positive mutations (3.11 ± 0.73 μm 2/ms vs. 3.59 ± 0.77 μm ^2/ms, t = 3.12, P = 0.002). The differences in mean Dapp (3.73 ± 1.26 μm^ 2/ms vs. 3.26 ± 0.68 μm 2/ms, t = 1.96, P = 0.053) or ADC values ([1.34 ± 0.81] × 10^ -3 mm ^2/s vs. [1.33 ± 0.41] × 10 ^-3 mm ^2/s, t = 0.07, P = 0.941) between the groups with or without ALK rearrangements were not statistically significant. The ADC values were significantly lower among patients with EGFR-positive mutation ([1.19 ± 0.37] × 10 ^-3 mm^ 2/s vs. [1.50 ± 0.53] × 10 ^-3 mm ^2/s, t = 3.38, P = 0.001) and high Ki-67 PI ([1.28 ± 0.39] × 10 -3 mm 2/s vs. [1.67 ± 0.77] × 10^ -3 mm^ 2/s, t = 2.88, P = 0.005). Kapp was strongly positively correlated with EGFR mutations ( r = 0.844, P = 0.008), strongly positively correlated with Ki-67 PI ( r = 0.882, P = 0.001), and strongly negatively correlated with ALK rearrangements ( r = -0.772, P = 0.001). Dapp was moderately correlated with EGFR mutations ( r = -0.650, P = 0.024) or Ki-67 PI ( r = -0.734, P = 0.012). ADC was moderately correlated with Ki-67 PI ( r = -0.679, P = 0.033). Conclusions:The Kapp value of DKI parameters was strongly correlated with different expression of EGFR, ALK, and Ki-67 in advanced lung adenocarcinoma. We report here that by identifying the Raman characteristics of isomers of monochlorobiphenyls, these compounds can be recognized, even at trace levels, by using the surface-enhance Raman scattering method with silver nanorods as a substrate. When dissolved in acetone, 2-, 3-, and 4-chlorobiphenyls were detected at a concentration of 10^(-8) mol/L, at which their characteristic Raman peaks were visible. | 12 | Correlation between BRAF^(V600E) mutation and clinicopathological features in pediatric papillary thyroid carcinoma显示文摘In adults, the presence of the BRAF^(V600E) mutation in papillary thyroid cancer(PTC) has been demonstrated to be strongly associated with aggressive cancer-cell characteristics and poor patient prognosis. In contrast, the frequency of this mutation in pediatric PTC has undergone limited study, and the few available estimates range from 0 to 63%. Furthermore, the role of the BRAF^(V600E) mutation in pediatric PTC is controversial; thus, the present study aimed to investigate the prevalence and role of the BRAF^(V600E) mutation in48 pediatric patients with PTC, aged 3–13 years. Of these patients, 41 were diagnosed with classic PTC, five were found to have a follicular variant of PTC, and two to exhibit a diffuse sclerosing PTC variant. The BRAF^(V600E) mutation was identified to be present in 35.4% of the 48 analyzed patients, and in 41.5% of the patients diagnosed with classical PTC. Furthermore, the presence of the BRAF^(V600E) mutation was found to be associated with a patient age at diagnosis of less than ten years(P=0.011), the performance of a thyroidectomy(P=0.03), exhibited tumor multifocality(P=0.02) and/or extra-thyroidal invasion(P=0.003), and both a low MACIS(Metastases, Age, Completeness of resection, Invasion, Size)(P=0.036) and AMES(Age, Metastasis, Extent of tumor,Size)(P=0.001)score. | 14 | Simultaneous monitoring of PCB profiles in the urban air of Dalian,China with active and passive samplings显示文摘The concentration of polychlorinated biphenyls (PCBs) in the urban air of Dalian, China was monitored from November 2009 to October 2010 with active high-volume sampler and semipermeable membrane device (SPMD) passive sampler. The concentration of PCBs (particle + gas) ( ∑ PCBs) ranged from 18.6 to 91.0 pg/m 3 , with an average of 50.9 pg/m 3 , and the most abundant dioxin-like PCB (DL-PCBs) was PCB118. The WHO-TEQ values of DL-PCBs were 3.6-22.1 fg/m3 , with an average of 8.5 fg/m 3 , and PCB126 was the maximum contributor to ∑ TEQ. There was a much larger amount of PCBs in the gas phase than in the particulate phase. The dominant PCB components were lower and middle molecular weight PCBs. With increasing chlorination level, the concentration of the PCB congeners in the air decreased. The gas-particulate partitioning of PCBs was different for the four seasons. The gas- particulate partitioning coefficients (logK p ) vs. subcooled liquid vapor pressures (logP L 0 ) of PCBs had reasonable correlations for different sampling sites and seasons. The absorption mechanism contributed more to the gas-particulate partitioning process than adsorption. Correlation analysis of meteorological parameters with the concentration of PCBs was conducted using SPSS packages. The ambient temperature and atmospheric pressure were important factors influencing the concentration of PCBs in the air. The distribution pattern of the congeners of PCBs and the dominant contributors to DL-PCBs and TEQ in active samples and SPMDs passive samples were similar. | 16 | Distribution,cycling and impact of water in the Earth's interior显示文摘The Earth's deep interior is a hidden water reservoir on a par with the hydrosphere that is crucial for keeping the Earth as a habitable planet. In particular, nominally anhydrous minerals(NAMs) in the silicate Earth host a significant amount of water by accommodating H point defects in their crystal lattices. Water distribution in the silicate Earth is highly heterogeneous, and the mantle transition zone may contain more water than the upper and lower mantles. Plate subduction transports surface water to various depths, with a series of hydrous minerals and NAMs serving as water carriers. Dehydration of the subducting slab produces liquid phases such as aqueous solutions and hydrous melts as a metasomatic agent of the mantle.Partial melting of the metasomatic mantle domains sparks off arc volcanism, which, along with the volcanism at mid-ocean ridges and hotspots, returns water to the surface and completes the deep water cycle. There appears to have been a steady balance between hydration and dehydration of the mantle at least since the Phanerozoic. Earth's water probably originates from a primordial portion that survived the Moon-forming giant impact, with later delivery by asteroids and comets. Water could play a critical role in initiating plate tectonics. In the modern Earth, the storage and cycling of water profoundly modulates a variety of properties and processes of the Earth's interior, with impacts on surface environments. Notable examples include the hydrolytic weakening effect on mantle convection and plate motion, influences on phase transitions(on the solidus of mantle peridotite in particular) and dehydration embrittlement triggering intermediate-to deep-focus earthquakes. | 17 | Risk factors for gastric cancer:a large-scale,population-based case-control study显示文摘Background:Early detection of gastric cancer(GC)has been the topic of major efforts in China.This study aimed to explore the risk factors associated with GC and to provide evidence for the selection of a high-risk population of GC.Methods:Based on the cancer screening cohort of the National Cancer Screening Program in Urban China,GC patients diagnosed by endoscopy and pathological examinations constituted the case group,and controls were 1:3 matched by sex and age(±5 years)individually.The variables were selected by univariable analysis of factors such as body mass index(BMI),dietary habits,lifestyle,stomach disease history,and family history of GC;and multivariable logistic regression was used to analyze the influencing factors of GC and to calculate the odds ratio(OR)of related factors and its 95%confidence interval(CI).Results:A total of 215 GC cases and 645 matched healthy controls were included in the final analysis,with a median age of 61 years for the case and control groups.Overall analysis showed that high educational level(above primary school)(OR=0.362,95%CI=0.219–0.599,P<0.001),overweight/obesity(BMI≥24 kg/m2;OR=0.489,95%CI=0.329–0.726,P<0.001),cigarette smoking(OR=3.069,95%CI=1.700–5.540,P<0.001),alcohol consumption(OR=1.661,95%CI=1.028–2.683,P=0.038),history of stomach disease(OR=6.917,95%CI=4.594–10.416,P<0.001),and family history of GC in first-degree relatives(OR=4.291,95%CI=1.661–11.084,P=0.003)were significantly correlated with the occurrence of GC.Subgroup analyses by age and gender indicated that GC risk was still increased in the presence of a history of stomach disease.A history of chronic gastritis,gastric ulcer,or gastric polyposis was positively associated with GC,with adjusted ORs of 4.155(95%CI=2.711–6.368),1.839(95%CI=1.028–3.288),and 2.752(95%CI=1.197–6.326).Conclusions:Subjects who smoke,drink,with history of stomach disease and family history of GC in first-degree relatives are the high-risk populations for GC.Therefore,attention should be paid to these subjects for GC screening. | Rui Zhang He Li Ni Li Ju-Fang Shi Jiang Li Hong-Da Chen Yi-Wen Yu Chao Qin Jian-Song Ren Wan-Qing Chen Jie He | 2021 | Chinese Medical Journal2021,,16: | 7 |
