1血管性认知损害诊断的进一步规范化:血管性认知损害分类共识研究组指南显示文摘引言血管性认知损害(vascular cognitive impairment,VCI)诊断共识的缺乏(体现为多种不同评估方案的使用),妨碍了对其理解和治疗的推进.多个国家的大量临床医生和研究人员参与了2个阶段血管性认知损害分类共识研究(Vascular Impairment of Cognition Classification Consensus Study,VICCCS),旨在就VCI的诊断原则(VICCCS-1)和诊断方案(VICCCS-2)达成一致意见.本文提供了VICCCS-2的相关内容.方法使用VICCCS-1达成的原则和已发表的诊断指南作为在线德尔菲(Delphi)调查的参考基点,以期对VCI的临床诊断达成共识.结果共进行了6轮调查,每轮有65~79名专家参与,他们就VICCCS修订的轻度和重度VCI的诊断指南达成共识,并肯定了美国国立神经疾病与卒中研究所-加拿大卒中网(National Institute of Neurological Disorders and Stroke–Canadian Stroke Network,NINDS-CSN)发布的神经心理学评估方案和对影像学检查的推荐意见.讨论VICCCS-2建议规范化应用NINDS-CSN推荐的神经心理学和影像学评估方案诊断VCI,以促进研究协作.Olivia A. 2Strategies to modulate the intestinal microbiota and their effects on nutrient utilization, performance, and health of poultry显示文摘Poultry is widely produced and consumed meat globally.Its demand is expected to continue increasing to meet the animal protein requirement for ever-increasing human population.Thus,the challenge that poultry scientists and industry face are to produce sufficient amount of poultry meat in the most efficient way.In the past,using antibiotics to promote the growth of poultry and manage gut microbiota was a norm.However,due to concerns over potential fatalistic impacts on food animals and indirectly to humans,their use as feed additives are banned or regulated in several jurisdictions.In this changed context,several alternative strategies have been proposed with some success that mimics the functions of antibiotics as growth promoters and modulate gut microbiota for their beneficial roles.These include the use of probiotics,prebiotics,organic acids,and exogenous enzyme,among others.Gut microbiota and their metabolic products improve nutrient digestion,absorption,metabolism,and overall health and growth performance of poultry.This paper reviews the available information on the effect of feed additives used to modulate intestinal microbiota of poultry and their effects on overall health and growth performance.Understanding these functions and interactions will help to develop new dietary and managerial strategies that will ultimately lead to enhanced feed utilization and improved growth performance of poultry.This review will help future researchers and industry to identify alternative feed ingredients having properties like prebiotics,probiotics,organic acids,and exogenous enzymes.Sudhir Yadav Rajesh Jha 2019Journal of Animal Science and Biotechnology2019,10,3:37
3Role of Helicobacter pylori infection in pathogenesis of atherosclerosis显示文摘Though a century old hypothesis, infection as a cause for atherosclerosis is still a debatable issue. Epidemiological and clinical studies had shown a possible association but inhomogeneity in the study population and study methods along with potential confounders have yielded conflicting results. Infection triggers a chronic inflammatory state which along with other mechanisms such as dyslipidemia, hyper-homocysteinemia, hypercoagulability, impaired glucose metabolism and endothelial dysfunction, contribute in pathogenesis of atherosclerosis. Studies have shown a positive relations between Cytotoxic associated gene-A positive strains of Helicobacter pylori and vascular diseases such as coronary artery disease and stroke. Infection mediated genetic modulation is a new emerging theory in this regard. 5Transjugular intrahepatic portosystemic shunt in cirrhosis: An exhaustive critical update显示文摘More than five decades after it was originally conceptualized as rescue therapy for patients with intractable variceal bleeding,the transjugular intrahepatic portosystemic shunt(TIPS)procedure continues to remain a focus of intense clinical and biomedical research.By the impressive reduction in portal pressure achieved by this intervention,coupled with its minimally invasive nature,TIPS has gained increasing acceptance in the treatment of complications of portal hypertension.The early years of TIPS were plagued by poor long-term patency of the stents and increased incidence of hepatic encephalopathy.Moreover,the diversion of portal flow after placement of TIPS often resulted in derangement of hepatic functions,which was occasionally severe.While the incidence of shunt dysfunction has markedly reduced with the advent of covered stents,hepatic encephalopathy and instances of early liver failure continue to remain a significant issue after TIPS.It has emerged over the years that careful selection of patients and diligent post-procedural care is of paramount importance to optimize the outcome after TIPS.The past twenty years have seen multiple studies redefining the role of TIPS in the management of variceal bleeding and refractory ascites while exploring its application in other complications of cirrhosis like hepatic hydrothorax,portal hypertensive gastropathy,ectopic varices,hepatorenal and hepatopulmonary syndromes,non-tumoral portal vein thrombosis and chylous ascites.It has also been utilized to good effect before extrahepatic abdominal surgery to reduce perioperative morbidity and mortality.The current article aims to review the updated literature on the status of TIPS in the management of patients with liver cirrhosis.Sasidharan Rajesh Tom George Cyriac Abby Philips Rizwan Ahamed Sandeep Kumbar Narain Mohan Meera Mohanan Philip Augustine 2020World Journal of Gastroenterology2020,26,37:26

7Xanthogranulomatous cholecystitis: What every radiologist should know显示文摘Xanthogranulomatous cholecystitis(XGC) is an uncommon variant of chronic cholecystitis characterized by xanthogranulomatous inflammation of the gallbladder. Intramural accumulation of lipid-laden macrophages and acute and chronic inflammatory cells is the hallmark of the disease. The xanthogranulomatous inflammation of the gallbladder can be very severe and can spill over to the neighbouring structures like liver, bowel and stomach resulting in dense adhesions, perforation, abscess formation, fistulous communication with adjacent bowel. Striking gallbladder wall thickening and dense local adhesions can be easily mistaken for carcinoma of the gallbladder, both intraoperatively as well as on preoperative imaging. Besides, cases of concomitant gallbladder carcinoma complicating XGC have also been reported in literature. So, we have done a review of the imaging features of XGC in order to better understand the entity as well as to increase the diagnostic yield of the disease summarizing the characteristic imaging findings and associations of XGC. Among other findings, presence of intramural hypodense nodules is considered diagnostic of this entity. However, in some cases, an imaging diagnosis of XGC is virtually impossible. Fine needle aspiration cytology might be handy in such patients. 8Strategy to differentiate autoimmune pancreatitis from pancreas cancer显示文摘Autoimmune pancreatitis (AIP) is a newly described entity of pancreatitis in which the pathogenesis appears to involve autoimmune mechanisms. Based on histological and immunohistochemical examinations of various organs of AIP patients, AIP appears to be a pancreatic lesion reflecting a systemic 'IgG4-related sclerosing disease'. Clinically, AIP patients and patients with pancreatic cancer share many features, such as preponderance of elderly males, frequent initial symptom of painless jaundice, development of new-onset diabetes mellitus, and elevated levels of serum tumor markers. It is of uppermost importance not to misdiagnose AIP as pancreatic cancer. Since there is currently no diagnostic serological marker for AIP, and approach to the pancreas for histological examination is generally difficult, AIP is diagnosed using a combination of clinical, serological, morphological, and histopathological features. Findings suggesting AIP rather than pancreatic cancer include:fluctuating obstructive jaundice; elevated serum IgG4 levels; diffuse enlargement of the pancreas; delayed en- hancement of the enlarged pancreas and presence of a capsule-like rim on dynamic computed tomography; low apparent diffusion coefficient values on diffusion-weighted magnetic resonance image; irregular narrowing of the main pancreatic duct on endoscopic retrograde cholangiopancreatography; less upstream dilatation of the main pancreatic duct on magnetic resonance cholangiopancreatography, presence of other organ involvement such as bilateral salivary gland swelling, retroperitoneal fibrosis and hilar or intrahepatic sclerosing cholangitis; negative work-up for malignancy including endoscopic ultrasound-guided fine needle aspiration; and steroid responsiveness. 9Total hip replacement for arthritis following tuberculosis of hip显示文摘AIM: To present the results of total hip arthroplasty(THA) for post tubercular arthritis of the hip joint.METHODS: Sixty-five patients(45 male, 20 female) with previously treated tuberculosis of the hip joint underwent cementless THA for post tubercular arthritis. The average age at the time of THA was 48 years(range 29 to 65 years). Erythrocyte sedimentation rate, C reactive protein, chest X-ray and contrast enhanced magnetic resonance imaging were done preoperatively to confirm resolution of the disease and to rule out any residual disease. Intra-operative samples were taken for microbiological examination, polymerase chain reaction(PCR) and histological examination. Patients were started on anti-tubercular drugs one week before the operation and continued for 6 mo post operatively. The patients were followed up clinically using the Harris hip score as well as radiologically for any loosening of the implants, osteolysis and any recurrence of tuberculosis. Any complications especially the recurrence of the infection was also recorded.RESULTS: The mean interval from completion of antitubercular therapy for tuberculosis to surgery was 4.2 years(range, 2-6 years). Preoperatively, 17 patients had ankylosis whereas 48 patients had functional but painful range of motion. The mean surgical time was 97 min(range, 65-125) whereas the mean blood loss was 600 m L(range, 400-900 m L). The average follow up was 8.3 years(range 6-11 years). The average Harris Hip score improved from 27 preoperatively to 91 at the final follow up. Seventeen patients had acetabularprotrusion which was managed with impaction grafting and cementless acetabular cup. The bone graft had consolidated in all these 17 patients at the follow up. Two patients developed discharging sinuses at 9 and 11 mo postoperatively respectively. The discharge tested positive for tuberculosis on the PCR. Both these patients were put on antitubercular therapy for another year. Both of them recovered and had no evidence of any loosening or osteolysis on X-rays. 10华北地区不同规模畜禽养殖场粪便中抗生素抗性基因污染特征显示文摘为调查华北地区畜禽养殖环境抗生素抗性基因(ARGs)污染特征及影响因素,采集河北省和天津市不同规模的养猪场和养鸡场新鲜粪便样品,分析粪便中ARGs污染水平及有机质与ARGs含量的相关性。实时荧光定量PCR结果显示,不同种类畜禽养殖场粪便ARGs的相对丰度有显著区别,鸡粪中sul和erm基因的相对丰度高于猪粪,而编码核糖体保护蛋白的tet基因(4.24×10^-3~5.85×10^-1)在猪粪和鸡粪中的相对丰度无明显差异,均显著高于sul(1.07×10^-4~2.26×10^-1)、erm(6.36×10^-4~2.62×10^-1)以及编码外派泵蛋白和酶抑制剂的tet基因(1.24×10^-4~5.41×10^-2)。不同规模的养猪场粪便ARGs污染水平趋势为:中型>大型>小型,而不同规模养鸡场粪便中ARGs相对丰度无显著性差异(P=0.551);此外,正交偏最小二乘判别(OPLS-DA)与典型相关分析(CCA)结果显示,畜禽粪便中编码核糖体保护蛋白的tet基因(tetM、tetO和tetW)相对丰度与有机碳(OC)和有机氮(ON)含量高度相关(VIP>1),sul基因则与OC/ON明显相关。综上,粪便中有机质和生物可利用碳氮比是影响畜禽养殖业ARGs污染水平的重要因素。邹威 金彩霞 魏闪 Ramasamy Rajesh Kumar 周启星 2020农业环境科学学报2020,39,11:17

14Heart failure after myocardial infarction in the era of primary percutaneous coronary intervention:Mechanisms,incidence and identification of patients at risk显示文摘Myocardial infarction(MI) remains the most common cause of heart failure(HF) worldwide. For almost 50 years HF has been recognised as a determinant ofadverse prognosis after MI, but efforts to promote myocardial repair have failed to translate into clinical therapies. Primary percutaneous coronary intervention(PPCI) has driven improved early survival after MI, but its impact on the incidence of downstream HF is debated. The effects of PPCI are confounded by the changing epidemiology of MI and HF, with an ageing patient demographic, an increasing proportion of non-STelevation myocardial infarction, and the recognition of HF with preserved ejection fraction. Herein we review the mechanisms of HF after MI and discuss contemporary data on its incidence and outcomes. 15Epidural anesthesia is effective for extracorporeal shock wave lithotripsy of pancreatic and biliary calculi显示文摘AIM: To evaluate the efficacy of thoracic epidural analgesia for extracorporeal shock wave lithotripsy (ESWL). METHODS: ESWL is an effective, non-invasive technique for the treatment of difficult pancreatic and large bile duct calculi. The procedure is often painful and requires large doses of analgesics. Many different anesthetic techniques have been used. Patients with either large bile duct calculi or pancreatic duct calculi which could not be extracted by routine endoscopic methods were selected. Thoracic epidural anesthesia (TEA) was routinely used in all the subjects unless contraindicated. Bupivacaine 0.25% with or without clonidine was used to block the segments D6 to D12. The dose was calculated depending on the age, height and weight of the patient. It was usually 1-2 mL per segment blocked.RESULTS: Ninety eight percent of the 1509 patients underwent ESWL under TEA. The subjects selected were within American Society of Anesthesiologists grade Ⅰ to Ⅲ. ESWL using EA permitted successful elimination of bile duct or pancreatic calculi with minimal morbidity. The procedure time was shorter in patients with TEA than in those who underwent ESWL under total intravenous anesthesia. 16The International Study Group of Pancreatic Surgery definition of delayed gastric emptying and the effects of various surgical modifications on the occurrence of delayed gastric emptying after pancreatoduodenectomy显示文摘BACKGROUND:A number of definitions have been used for delayed gastric emptying(DGE) after pancreatoduodenectomy and the reported rates varied widely.The International Study Group of Pancreatic Surgery(ISGPS) definition is the current standard but it is not used universally.In this comprehensive review,we aimed to determine the acceptance rate of ISGPS definition of DGE,the incidence of DGE after pancreatoduodenectomy and the effect of various technical modifications on its incidence.DATA SOURCE:We searched PubM ed for studies regarding DGE after pancreatoduodenectomy that were published from 1 January 1980 to 1 July 2015 and extracted data on DGE definition,DGE rates and comparison of DGE rates among different technical modifications from all of the relevant articles.RESULTS:Out of 435 search results,178 were selected for data extraction.The ISGPS definition was used in 80% of the studies published since 2010 and the average rates of DGE and clinically relevant DGE were 27.7%(range:0-100%;median:18.7%) and 14.3%(range:1.8%-58.2%;median:13.6%),respectively.Pylorus preservation or retrocolic reconstruction were not associated with increased DGE rates.Although pyloric dilatation,Braun's entero-enterostomy and Billroth Ⅱ reconstruction were associated with significantly lower DGE rates,pyloric ring resection appears to be most promising with favorable results in 7 out of 10 studies.CONCLUSIONS:ISGPS definition of DGE has been used in majority of studies published after 2010.Clinically relevant DGE rates remain high at 14.3% despite a number of proposed surgical modifications.Pyloric ring resection seems to offer the most promising solution to reduce the occurrence of DGE.Rajesh Panwar Sujoy Pal 2017Hepatobiliary & Pancreatic Diseases International2017,16,4:13

19Early precut sphincterotomy and the risk of endoscopic retrograde cholangiopancreatography related complications: An updated meta-analysis显示文摘AIM: To study the cannulation and complication rates of early pre-cut sphincterotomy vs persistent attempts at cannulation by standard approach.METHODS: Systematic search of PubMed, EMBASE, Web of Science, and the Cochrane Library for relevant studies published up to February 2013. The main outcome measurements were cannulation rates and postendoscopic retrograde cholangiopancreatography(ERCP) complications. A comprehensive systematic search of the Cochrane library, PubMed, Google scholar, Scopus, National Institutes of Health, meta-register of controlled trials and published proceedings from major Gastroenterology journals and meetings until February 2013 was conducted using keywords. All Prospective randomized controlled trials(RCT) studies whichmet our inclusion criteria were included in the analysis. Prospective non-randomized studies and retrospective studies were excluded from our meta-analysis. The main outcomes of interest were post-ERCP pancreatitis, overall complication rates including cholangitis, ERCPrelated bleeding, perforation and cannulation success rates. RESULTS: Seven RCTs with a total of 1039 patients were included in the meta-analysis based on selection criteria. The overall cannulation rate was 90% in the pre-cut sphincterotomy vs 86.3% in the persistent attempts group(OR = 1.98; 95%CI: 0.70-5.65). The risk of post-ERCP pancreatitis(PEP) was not different between the two groups(3.9% in the pre-cut sphincterotomy vs 6.1% in the persistent attempts group, OR = 0.58, 95%CI: 0.32-1.05). Similarly, there was no statistically significant difference between the groups for overall complication rate including PEP, cholangitis, bleeding, and perforation(6.2% vs 6.9%, OR = 0.85, 95%CI: 0.51-1.41). CONCLUSION: This meta-analysis suggests that precut sphincterotomy and persistent attempts at cannulation are comparable in terms of overall complication rates. 20Fermentation characteristics of resistant starch, arabinoxylan, andβ-glucan and their effects on the gut microbial ecology of pigs:A review显示文摘Dietary fibers(DF)contain an abundant amount of energy,although the mammalian genome does not encode most of the enzymes required to degrade them.However,a mutual dependence is developed between the host and symbiotic microbes,which has the potential to extract the energy present in these DF.Dietary fibers escape digestion in the foregut and are fermented in the hindgut,producing shortchain fatty acids(SCFA)that after the microbial ecology in the gastrointestinal tract(GIT)of pigs.Most of the carbohydrates are fermented in the proximal part,allowing protein fermentation in the distal part,resulting in colonic diseases.The structures of resistant starch(RS),arabinoxylan(AX),and β-glucan(βG)are complex;hence,makes their way into the hindgut where these are fermented and provide energy substrates for the colonic epithelial cells.Different microbes have different preferences of binding to different substrates.The RS,AX and βG act as a unique substrate for the microbes and modify the relative composition of the gut microbial community.The granule dimension and surface area of each substrate are different,which influences the penetration capacity of microbes.Arabinose and xylan are 2 different hemicelluloses,but arabinose is substituted on the xylan backbone and occurs in the form of AX.Fermentation of xylan produces butyrate primarily in the small intestine,whereas arabinose produces butyrate in the large intestine.Types of RS and forms of βG also exert beneficial effects by producing different metabolites and modulating the intestinal microbiota.Therefore,it is important to have information of different types of RS,AX and(3 G and their roles in microbial modulation to get the optimum benefits of fiber fermentation in the gut.This review provides relevant information on the similarities and differences that exist in the way RS,AX,and βG are fermented,and their positive and negative effects on SCFA production and gut microbial ecology of pigs.These insights will help nutritionists to develop dietary strategies that can modulate specific SCFA production and promote beneficial microbiota in the GIT of swine.Utsav P.Tiwari Amit K.Singh Rajesh Jha 2019Animal Nutrition2019,,3:11
