1Anterior resection for rectal carcinoma-risk factors for anastomotic leaks and strictures显示文摘AIM:To determine the incidence and factors responsible for anastomotic leaks and stricture following anterior resection(AR)and its subsequent management.METHODS:Retrospective analysis of data from 108 patients with rectal carcinoma who underwent AR or low anterior resection(LAR)to identify the various preoperative,operative,and post operative factors that might have influence on anastomotic leaks and strictures.RESULTS:There were 68 males and 40 females with an average of 47 years(range 21-75 years).The median distance of the tumor from the anal verge was 8 cm(range 3-15 cm).Sixty(55.6%)patients underwent handsewn anastomosis and 48(44.4%)were stapled.The median operating time was 3.5 h(range2.0-7.5 h).Sixteen(14.6%)patients had an anastomotic leak.Among these,11 patients required reexploration and five were managed expectantly.The anastomotic leak rate was similar in patients with and without diverting stoma(8/60,13.4%with stoma and 8/48;16.7%without stoma).In 15(13.9%)patients,resection margins were positive for malignancy.Ninteen(17.6%)patients developed anastomotic strictures at a median duration of 8 mo(range 3-20 mo).Among these,15 patients were successfully managed with per-anal dilatation.On multivariate analysis,advance age(>60 years)was the only risk factor for anastomotic leak(P=0.004).On the other hand,anastomotic leak(P=0.00),mucin positive tumor(P =0.021),and lower rectal growth(P=0.011)were found as risk factors for the development of an anastomotic stricture.CONCLUSION:Advance age is a risk factor for an anastomotic leak.An anastomotic leak,a mucin-secreting tumor,and lower rectal growth predispose patients to develop anastomotic strictures.Ashok Kumar Ram Daga Paari Vijayaragavan Anand Prakash Rajneesh Kumar Singh Anu Behari Vinay K Kapoor Rajan Saxena 2011World Journal of Gastroenterology2011,17,11:28

5Laparoscopic management of hydatid cysts using long ribbon gauze:An initial experience of 37 consecutive cases显示文摘Objective:The laparoscopic approach is becoming the standard of care for many surgical disorders.However,in the case of hydatid cysts,laparoscopic management is challenging due to the risk of spillage of hydatid fluid,which can cause an anaphylactic reaction and recurrence.Here,we report our initial experience with laparoscopic partial pericystectomy of hydatid cysts using long ribbon gauze to decrease intra-operative spillage.Method:This was a retrospective study(between January 2010 and December 2021)in the Department of Surgical Gastroenterology,Sanjay Gandhi Postgraduate Institute of Medical Science,a tertiary care referral center in northern India.Here,we have included 37 consecutive patients with hydatid cysts of the liver and spleen.Diagnosis was made by laboratory and imaging findings(abdominal sonography or contrast enhanced CT scans).All patients were managed with laparoscopic partial pericystectomy.Intraoperatively,a betadine-soaked long ribbon gauze,high-pressure suction canula,and an endo-bag were used in all patients.The collected data included patient demography,location,size,and number of cysts,WHO type,operative time,blood loss,postoperative complications,hospital stay and follow-up.Result:In our series,the mean age was 38.4±13.6 years,15(40.5%)were men and 22(59.5%)were women.The right lobe of the liver was the most commonly affected site(21,56.8%).The mean operative time was 80.0±32.0 min,and intraoperative blood loss was 23.6±11.5 mL.Bile leak was present in 6(16.2%)patients.There was no mortality.The hospital stay was 5(3,9)days,and no recurrence was observed at a median follow-up of 36 months.Conclusion:Laparoscopic partial pericystectomy using this technique is safe in the management of hydatid cysts.Simply,proper packing and safe removal of soaked gauzes can minimize the incidence of postoperative complications and recurrence.Ashok Kumar II Nalini Kanta Ghosh Anu Behari Ashish Singh Rahul Rai Somanath Malage Rajneesh Kumar Singh 2023Laparoscopic, Endoscopic and Robotic Surgery2023,6,3:0
6Analysis of thermal injuries using classical Fourier and DPL models for multi-layer of skin under different boundary conditions显示文摘In this paper,the temperature distribution in the multi-layer of the skin is studied when the skin surface is subjected to most generalized boundary condition.Our skin model consists of three layers known as the epidermis,dermis,and subcutaneous layers.All layers of skin are assumed to be connected with point of interface condition and taking the barrier in between each of the two layers by symmetric flux condition and analyzing each layer separately.The classical Fourier and non-Fourier(DPL)models are extended to analyze the behavior of heat transfer in the multi-layer of the skin.The Laplace transform technique is used to derive analytical solutions for the multi-layer of skin models.The effects of the variability of different parameters such as relaxation time,layer thickness,and different types of boundary conditions on the behavior of temperature distribution in the multi-layer of skin are analyzed and discussed in detail.All the effects are shown graphically.It has been observed that during temperature distribution in the multi-layer of skin,the measurement of skin damage is less on the DPL model(rq>Tt)in comparison to the classical Fourier model.Rajneesh Kumar Chaudhary Dinesh Kumar Kabindra Nath Rai Jitendra Singh 2021International Journal of Biomathematics2021,14,6:0
7Propagation of plane waves in thermoelastic cubic crystal material with two relaxation times显示文摘A problem concerned with the reflection and refraction of thermoelastic plane waves at an imperfect interface between two generalized thermally conducting cubic crystal solid half-spaces of different elastic and thermal properties with two relaxation times has been investigated. The generalized thermoelastic theory with two relaxation times developed by Green and Lindsay has been used to study the problem. The expressions for the reflection and refraction coefficients which are the ratios of the amplitudes of reflected and refracted waves to the amplitude of incident waves are obtained for an imperfect boundary and deduced for normal stiffness, transverse stiffness, thermal contact conductance, slip and welded boundaries. Amplitude ratios of different reflected and refracted waves for different boundaries with angle of emergence have been compared graphically for different incident waves.
