| 1 | Anterior resection for rectal carcinoma-risk factors for anastomotic leaks and strictures显示文摘AIM:To determine the incidence and factors responsible for anastomotic leaks and stricture following anterior resection(AR)and its subsequent management.METHODS:Retrospective analysis of data from 108 patients with rectal carcinoma who underwent AR or low anterior resection(LAR)to identify the various preoperative,operative,and post operative factors that might have influence on anastomotic leaks and strictures.RESULTS:There were 68 males and 40 females with an average of 47 years(range 21-75 years).The median distance of the tumor from the anal verge was 8 cm(range 3-15 cm).Sixty(55.6%)patients underwent handsewn anastomosis and 48(44.4%)were stapled.The median operating time was 3.5 h(range2.0-7.5 h).Sixteen(14.6%)patients had an anastomotic leak.Among these,11 patients required reexploration and five were managed expectantly.The anastomotic leak rate was similar in patients with and without diverting stoma(8/60,13.4%with stoma and 8/48;16.7%without stoma).In 15(13.9%)patients,resection margins were positive for malignancy.Ninteen(17.6%)patients developed anastomotic strictures at a median duration of 8 mo(range 3-20 mo).Among these,15 patients were successfully managed with per-anal dilatation.On multivariate analysis,advance age(>60 years)was the only risk factor for anastomotic leak(P=0.004).On the other hand,anastomotic leak(P=0.00),mucin positive tumor(P =0.021),and lower rectal growth(P=0.011)were found as risk factors for the development of an anastomotic stricture.CONCLUSION:Advance age is a risk factor for an anastomotic leak.An anastomotic leak,a mucin-secreting tumor,and lower rectal growth predispose patients to develop anastomotic strictures. | Ashok Kumar Ram Daga Paari Vijayaragavan Anand Prakash Rajneesh Kumar Singh Anu Behari Vinay K Kapoor Rajan Saxena | 2011 | World Journal of Gastroenterology2011，17，11： | 28 |

| 3 | Uniqueness,reciprocity theorem,and plane waves in thermoelastic diffusion with a fractional order derivative显示文摘In this work, a theory of thermoelasticity with diffusion is taken into consideration by using the methodology of fractional calculus. The governing equations for particle motion in a homogeneous anisotropic fractional order generalized thermoelastic diffusive medium are presented. Uniqueness and reciprocity theorems are proved. The plane wave propagation in the homogeneous transversely isotropic thermoelastic diffusive medium with fractional order derivative is studied. For the two-dimensional problem, there exist a quasi-longitudinal wave, a quasi-transverse wave, a quasi-mass diffusion wave, and a quasi-thermal wave. From the obtained results, the different characteristics of waves, like phase velocity, attenuation coefficient, specific loss, and penetration depth, are computed numerically and presented graphically. | 5 | Change in model for end-stage liver disease score at two weeks,as an indicator of mortality or liver transplantation at 60 days in acute-on-chronic liver failure显示文摘Background:Acute-on-chronic liver failure(ACLF)is characterised by a sudden deterioration of underlying chronic liver disease,resulting in increased rates of mortality and liver transplantation.Early prognostication can benefit optimal allocation of resources.Methods:ACLF was defined as per the disease criteria of the Asian Pacific Association for the Study of the Liver.Inpatient discharge summaries from between January 2001 and April 2013 were reviewed.The primary outcome was mortality or liver transplantation within 60 days from onset of ACLF.Absolute‘model for end-stage liver disease’(MELD)score and change in MELD at Weeks 1,2 and 4 were reviewed in order to identify the earliest point for prediction of mortality or liver transplantation.Results:Clinical data were collected on 53 subjects who fulfilled the inclusion and exclusion criteria.At 60 days from presentation,20 patients(37.7%)died and 4(7.5%)underwent liver transplantation.Increased MELD of-2 after 2 weeks was 75.0%sensitive and 75.9%specific for predicting mortality or liver transplantation.If the MELD score did not increase at 2 weeks,predictive chance of survival was 93.8%over the next 60 days.MELD change at 1 week showed poor sensitivity and specificity.Change at 4 weeks was too late for intervention.Conclusion:Change in MELD score at 2 weeks provides an early opportunity for prognostication in ACLF.A MELD score that does not deteriorate by Week 2 would predict 93.8%chance of survival for the next 60 days.This finding warrants further validation in larger cohort studies. | Rajneesh Kumar Thinesh Lee Krishnamoorthy Hiang Keat Tan Hock Foong Lui Wan Cheng Chow | 2015 | Gastroenterology Report2015，3，2： | 2 |
