1Obesity in kidney disease:A heavyweight opponen显示文摘Obesity is an important worldwide challenge that must be faced in most developed and developing countries because of unhealthy nutritional habits.The consequences of obesity and being overweight are observed in different organs,but the kidney is one of the most affected.Excess adipose tissue causes hemodynamic alterations in the kidney that can result in renal disease.However,obesity is also commonly associated with other comorbidities such as chronic inflammation,hypertension and diabetes.This association of several aggravating factors is still a matter of concern in clinical and basic research because the pathophysiologic mechanisms surrounding chronic kidney disease development in obese patients remain unclear.This review will discuss the consequences of obesity in the context of renal injury.Raphael Jose Ferreira Felizardo Marina Burgos da Silva Cristhiane Favero Aguiar Niels Olsen Saraiva Camara 2014World Journal of Nephrology2014,3,3:7

9Retention of eucalyptus harvest residues reduces soil compaction caused by deep subsoiling显示文摘Eucalyptus harvesting,forwarding and soil tillage operations are among the main causes for compaction of forest soils,with potential impacts on productivity.This concern is especially important in areas with soils that are naturally compacted(fragipans and duripans).In these soils,tillage operations include the use of subsoilers that can reach depths of more than one meter and require heavy tractors that exert high pressure on the soil.One of the ways to try to minimize the effect of this compaction is by retaining harvest residues.The objective of this study was to evaluate the impacts of eucalyptus harvesting on soil physical attributes,as well as to determine the potential of different types of residue management to reduce compaction from the soil tillage operation.Two experiments were conducted in the same area with a Yellow Argisol.In the first experiment,compaction caused by mechanized harvesting with harvester+forwarder was evaluated.In the second experiment,different managements of harvest residues were examined as potential modifiers of soil compaction during tillage for new plantings.For this,three managements systems were tested:(1)retention of all harvest residues and litter from the previous rotation(HR+L),(2)retention of litter from the previous rotation(L),and(3)removal of harvest residues and litter from the previous rotation(WR).Before and after harvest,sampling was carried out in the planting rows and inter-rows,and after tillage,samples were collected in the traffic line of the subsoiler-tractor set.In both experiments,undisturbed soil samples were collected from the center of the 0-10,10-20,20-40,40-60,and 60-100 cm layers to determine soil density and total porosity.In each period and site of evaluation,mechanical resistance to penetration up to the 60-cm depth was also determined.The harvesting operation increased soil density at 0-10 and 60-100 cm depths only in the inter-rows.Retention of harvest residues and litter(HR+L)after harvesting avoided increases in soil density and penetration resistance caused by machine traffic during tillage.The results indicate the importance of retaining harvest residues on forest soils for achieving sustainable utilization and for conserving soil quality.Raphael Oliveira de Melo AymbiréAngeletti da Fonseca Nairam Félix de Barros Raphael Bragança Alves Fernandes Rafael da Silva Teixeira Iago Nery Melo Ricardo Previdente Martins 2022Journal of Forestry Research2022,33,2:0
10Extensive right coronary artery thrombosis in a patient with COVID-19:A case report显示文摘BACKGROUND Occurring in approximately 30%of hospitalized patients,cardiovascular complications that take place during the course of coronavirus disease 2019(COVID-19)have been shown to cause morbidity and mortality.This case is the first report of extensive right coronary artery(RCA)thrombosis that was evaluated by intracoronary imaging and intracoronary invasive physiology in a patient with COVID-19.CASE SUMMARY A 62-year-old woman presented with flu-like symptoms;ten days later,she presented with inferior ST-segment elevations,chest pain,dyspnea,nausea and vomiting.The patient was diagnosed with COVID-19 following a positive test result.Emergency angiography of the RCA and its branches indicated intraluminal filling defects,suggesting a thrombus.Intravascular ultrasound confirmed a subacute thrombus in the RCA,the right posterior descending branch and the right posterior ventricular(RPV)branch.There was also an acute thrombus in the RPV branch and atherosclerosis in the RCA.Dual antiplatelet/anticoagulation therapy was administered.After 7 d,angiography revealed complete disappearance of the thrombi.Optical coherence tomography confirmed this with the exception of a small thrombus in the RPV branch and atherosclerotic plaque in the RCA.The atherosclerotic RCA was measured using the resting fullcycle ratio,indicating no impairment to coronary physiology.The patient was discharged on the 11^(th) day of hospitalization and remained asymptomatic through the 6-mo follow-up.CONCLUSION This was the first report of RCA thrombosis in a patient with COVID-19.Dual antiplatelet/anticoagulation therapy was successful.Clarissa Campo Dall’Orto Rubens Pierry Ferreira Lopes Mariana Torres Cancela Ciria de Sales Padilha Gilvan Vilella Pinto Filho Marcos Raphael da Silva 2022World Journal of Clinical Cases2022,10,31:0
11Combination approaches in hepatocellular carcinoma:How systemic treatment can benefit candidates to locoregional modalities显示文摘The management of hepatocellular carcinoma(HCC)is challenging because most patients have underlying cirrhosis,and the treatment provides,historically,a limited impact on the natural history of patients with advanced-stage disease.Additionally,recurrence rates are high for those patients who receive local and locoregional modalities,such as surgical(resection and transplantation)or imageguided(ablation and intra-arterial)therapies.Translational research has led to new concepts that are reshaping the current clinical practice.Substantial advancements were achieved in the understanding of the hallmarks that drive hepatocarcinogenesis.This has primed a successful incorporation of novel agents with different targets,such as anti-angiogenic drugs,targeted-therapies,and immune-checkpoint inhibitors.Although clinical trials have proven efficacy of systemic agents in advanced stage disease,there is no conclusive evidence to support their use in combination with loco-regional therapy.While novel local modalities are being incorporated(e.g.,radioembolization,microwave ablation,and irreversible electroporation),emerging data indicate that locoregional treatments may induce tumor microenvironment changes,such as hyperexpression of growth factors,release of tumor antigens,infiltration of cytotoxic lymphocytes,and modulation of adaptative and innate immune response.Past trials that evaluated the use of antiangiogenic drugs in the adjuvant setting after ablation or chemoembolization fail to demonstrate a substantial improvement.Current efforts are directed to investigate the role of immunotherapy-based regimens in this context.The present review aims to describe the current landscape of systemic and locoregional treatments for HCC,present evidence to support combination approaches,and address future perspectives.Leonardo Gomes da Fonseca Raphael L C Araujo 2022World Journal of Gastroenterology2022,28,28:0
