2Feasibility and safety of autologous bone marrow mononuclear cell transplantation in patients with advanced chronic liver disease显示文摘AIM: To evaluate the safety and feasibility of bone marrow cell (BMC) transplantation in patients with chronic liver disease on the waiting list for liver transplantation. METHODS: Ten patients (eight males) with chronic liver disease were enrolled to receive infusion of autologous bone marrow-derived cells. Seven patients were classified as Child-Pugh B and three as Child-Pugh C. Baseline assessment included complete clinical and laboratory evaluation and abdominal MRI. Approximately 50 ml of bone marrow aspirate was prepared by centrifugation in a ficoll-hypaque gradient. At least of 100 millions of mononuclear-enriched BMCs were infused into the hepatic artery using the routine technique for arterial chemoembolization for liver tumors. Patients were followed up for adverse events up to 4 mo. RESULTS: The median age of the patients was 52 years (range 24-70 years). All patients were discharged 48 h after BMC infusion. Two patients complained ofmild pain at the bone marrow needle puncture site. No other complications or specific side effects related to the procedure were observed. Bilirubin levels were lower at 1 (2.19 ± 0.9) and 4 mo (2.10 ± 1.0) after cell transplantation that baseline levels (2.78 ± 1.2). Albumin levels 4 mo after BMC infusion (3.73 ± 0.5) were higher than baseline levels (3.47 ± 0.5). International normalized ratio (INR) decreased from 1.48 (SD = 0.23) to 1.43 (SD = 0.23) one month after cell transplantation. CONCLUSION: BMC infusion into hepatic artery of patients with advanced chronic liver disease is safe and feasible. In addition, a decrease in mean serum bilirubin and INR levels and an increase in albumin levels are observed. 3Metabolic surgery and intestinal gene expression:Digestive tract and diabetes evolution considerations显示文摘AIM: To investigate the effects of bariatric surgery on metabolic parameters, incretin hormone secretion, and duodenal and ileal mucosal gene expression.METHODS: Nine patients with type 2 diabetes mellitus(T2DM), chronic serum hyperglycemia for more than2 years, and a body mass index(BMI) of 30-35 kg/m2 underwent metabolic surgery sleeve gastrectomy with transit bipartition between May 2011 and December2011. Blood samples were collected pre and 3, 6 and12 mo postsurgery. Duodenal and ileal mucosa samples were collected pre- and 3 mo postsurgery. Pre- and postoperative blood samples were collected in the fasting state before ingestion of a standard meal(520kcal) and again 30, 60, 90, and 120 min after the mealto determine hemoglobin A1c(HbA1c) levels and the lipid profile, which consisted of triglyceride and total cholesterol levels. Intestinal gene expression of p53 and transforming growth factor(TGF)-β was analyzed using quantitative reverse-transcription PCR. Gastric inhibitory polypeptide(GIP) and glucagon-like peptide-1(GLP-1)were quantified using the enzyme-linked immunoassay method and analyzed pre- and postoperatively.Student's t test or repeated measurements analysis of variance with Bonferroni corrections were performed as appropriate.RESULTS: BMI values decreased by 15.7% within the initial 3 mo after surgery(31.29 ± 0.73 vs 26.398± 0.68, P < 0.05) and then stabilized at 22% at 6mo postoperative, resulting in similar values 12 mo postoperatively(20-25 kg/m2). All of the patients experienced improved T2 DM, with 7 patients(78%)achieving complete remission(HbA1c < 6.5%), and 2patients(22%) achieving improved diabetes(HbA1c< 7.0% with or without the use of oral hypoglycemic agents). At 3 mo postoperatively, fasting plasma glucose had also decreased(59%)(269.55 ±18.24 mg/dL vs 100.77 ± 3.13 mg/dL, P < 0.05)with no further significant changes at 6 or 12 mo postoperatively. In the first month postoperatively,there was a complete withdrawal of hypoglycemic medications in all patients, who were taking at least2 hypoglycemic drugs preoperatively. 20Biological and chemical attributes of soils under forest species in Northeast Brazil显示文摘Timber forests contribute to the sustainable development of the biomes in tropical regions.The aim of this study was to evaluate the biological and chemical properties of the soil as a consequence of the cover with native and non-native species in the Acarau basin,a transition area from the coast to the Brazilian semi-arid region.Areas planted with four native species(Anadenanthera colubrina,Astronium fraxinifolium,Handroanthus impetiginosus,Colubrina glandulosa)and three exotic species(Acacia mangium,Casuarina equisetifolia,Eucalyptus urophylla)plus a non-forested agricultural area were evaluated for organic carbon contents,and microbial and chemical soil properties.The levels of soil organic carbon were highest in A.colubrina and C.equisetifolia plantations.Low basal soil respiration was observed but the microbial biomass was particularly low in the non-forested area.In the C.equisetifolia,E.urophylla,and H.impetiginosus plantations,elevated soil metabolic quotients were found.The A.colubrina and H.impetiginosus plantations had the highest levels of easily extracted-glomalin related soil protein.Tree species affect concentrations of essential nutrients and the biological quality of the soil in different ways.They can also improve the biological and chemical properties of the soil in the coastal plains of tropical regions.Olmar Baller Weber Maria Catia Barroso da Silva Cristiane Figueira da Silva Joao Alencar de Sousa Carlos Alberto Kenji Taniguch Deborah dos Santos Garruti Ricardo Espindola Romero 2020Journal of Forestry Research2020,31,5:1
