| 1 | 评述“经自然腔道标本取出术(NOSES)治疗结直肠癌的国际共识”显示文摘Natural-orifice specimen-extraction surgery(NOSES)for colorectal disease is not a new surgical procedure,although its lack of penetration into common colorectal surgical practice may make it a relatively new procedure.Stewart et al.[1]were among the first to report the extraction of a colectomy specimen through the vagina in 1991 and,shortly thereafter,Franklin et al.[2]published the first report of partial colectomy with naturalorifice specimen extraction(NOSE)via the anus.Since then,there have been several publications on the extraction of both malignant and benign colonic diseases from the caecum to the distal rectum through natural-orifice extraction sites.Unlike procedures such as laparoscopic cholecystectomy,which was introduced in 1985 and rapidly diffused into the surgical community over a 2-to 3-year period,the dispersion of NOSES has not been equivocal[3].As there are several reasons why some new surgical innovations may be taken up more quickly than others,the adoption curves of new procedures can take many forms.However,the tipping point that describes the onset of the peak rate of diffusion of the new technology usually occurs after the first 10%–20%of users have adopted it[3].From a global perspective,the adoptions curve for NOSES has not obtained a peak rate of diffusion and appears to have arrested in the developmental and explorative phase of innovation[3]. | Joseph W.Nunoo-Mensah Mariam Rizk | 2020 | Gastroenterology Report2020，8，6： | 5 |
| 2 | Impact of pediatric traumatic brain injury on hippocampal neurogenesis显示文摘Traumatic brain injury(TBI)is a major cause of mortality and morbidity in the pediatric population.With advances in medical care,the mortality rate of pediatric TBI has declined.However,more children and adolescents are living with TBI-related cognitive and emotional impairments,which negatively affects the quality of their life.Adult hippocampal neurogenesis plays an important role in cognition and mood regulation.Alterations in adult hippocampal neurogenesis are associated with a variety of neurological and neurodegenerative diseases,including TBI.Promoting endogenous hippocampal neurogenesis after TBI merits significant attention.However,TBI affects the function of neural stem/progenitor cells in the dentate gyrus of hippocampus,which results in aberrant migration and impaired dendrite development of adult-born neurons.Therefore,a better understanding of adult hippocampal neurogenesis after TBI can facilitate a more successful neuro-restoration of damage in immature brains.Secondary injuries,such as neuroinflammation and oxidative stress,exert a significant impact on hippocampal neurogenesis.Currently,a variety of therapeutic approaches have been proposed for ameliorating secondary TBI injuries.In this review,we discuss the uniqueness of pediatric TBI,adult hippocampal neurogenesis after pediatric TBI,and current efforts that promote neuroprotection to the developing brains,which can be leveraged to facilitate neuroregeneration. | Mariam Rizk Justin Vu Zhi Zhang | 2021 | Neural Regeneration Research2021，16，5： | 4 |

| 5 | Assessment of lnc RNA GAS5, lnc RNA HEIH, lnc RNA BISPR and its m RNA BST2 as serum innovative non-invasive biomarkers: Recent insights into Egyptian patients with hepatitis C virus type 4显示文摘BACKGROUND Hepatitis C virus(HCV)infection and its consequent complications are undeniably a public health burden worldwide,particularly in Egypt.Emerging evidence suggests that many lncRNAs have relevant roles in viral infections and antiviral responses.AIM To investigate the expression profiles of circulating lncRNAGAS5,lncRNAHEIH,lncRNABISPR and mRNABST2 in naïve,treated and relapsed HCV Egyptian patients,to elucidate relation to HCV infection and their efficacy as innovative biomarkers for the diagnosis and prognosis of HCV GT4.METHODS One hundred and thirty HCV-infected Egyptian patients and 20 healthy controls were included in this study.Serum lncRNAs and mRNABST2 were measured using quantitative real-time polymerase chain reaction(qRT-PCR).RESULTS Our results indicated that serum lncRNAGAS5 and LncRNABISPR were upregulated,whereas mRNA BST2 and LncRNA HEIH were downregulated in naïve patients.In contrast,HCV patients treated with sofosbuvir and simeprevir;with sofosbuvir and daclatasvir;or with sofosbuvir,daclatasvir and ribavirin exhibited lower levels of lncRNAGAS5 and lncRNABISPR with higher mRNABST2 compared to naïve patients.Notably,patients relapsed from sofosbuvir and simeprevir showed higher levels of these lncRNAs with lower mRNABST2 compared to treated patients.LncRNAGAS5 and lncRNABISPR were positively correlated with viral load and ALT at P<0.001,whereas mRNABST2 was negatively correlated with viral load at P<0.001 and ALT at P<0.05.Interestingly,a significant positive correlation between lncRNA HEIH and AFP was observed at P<0.001.CONCLUSION Differential expression of these RNAs suggests their involvement in HCV pathogenesis or antiviral response and highlights their promising roles in diagnosis and prognosis of HCV. | Nourhan M El Samaloty Marwa I Shabayek Ramy S Ghait Shohda A El-Maraghy Sherine M Rizk Maha M El-Sawalhi | 2020 | World Journal of Gastroenterology2020，26，2： | 2 |
| 6 | Utility of endoscopic ultrasound in pancreatitis:A review显示文摘The close proximity of the endoscopic ultrasound probe to the pancreas results in superior spatial resolution compared to CT scan and MRI. In addition, endoscopic ultrasound (EUS) is a minimally invasive procedure that does not share the relatively high complication rate of ERCP. Due to these advantages, EUS has evolved into an important technique to assess pancreatobiliary disease. This review will discuss the role of EUS in patients with pancreatitis. The indications can be divided into acute pancreatitis and chronic pancreatitis. In acute pancreatitis, EUS is used to determine the etiology; in suspected chronic pancreatitis it is helpful to establish the diagnosis. | 7 | Interleukin-28 and hepatitis C virus genotype-4:Treatment-induced clearance and liver fibrosis显示文摘AIM:To investigate the association between interleukin-28B(IL28B) genotype and response to treatment and hepatic fibrosis in patients with hepatitis C virus(HCV) genotype 4.METHODS:Two hundred and one HCV-genotype 4 patients were included.All patients were treated with Peginterferon alph2a/Ribavirin for 48 wk.End of treatment response(ETR) was defined as loss of detectable serum HCV RNA at the end of treatment.Sustained viral response(SVR) was defined as loss of detectable serum HCV RNA at the end of 24 wk follow up.Genotyping of IL28B rs12979860 was performed using the TaqMan assay.We used logistic regression to estimate the adjusted odds ratio(aOR) and 95%CI.RESULTS:The study included 201 HCV-genotype 4 patients.The majority of patients were men(89.6%),with a median age of 47 years,inter-quartile range(40-51).Approximately 62.5% of patients had ETR,and 49.6% had SVR.Individuals who achieved SVR were more likely to be younger(χ 2 = 4.91,P = 0.027),and less likely to have fibrosis(χ 2 = 15.54,P < 0.0001),or inflammation(χ 2 = 7.58,P = 0.006).The genotype distribution of rs12979860 was 36.2%,49.0% and 14.8% for genotypes CC,CT,and TT,respectively.In these participants,rs12979860 genotype distribution did not differ by gender(P = 0.466),pretreatment viral load(P = 0.600),inflammation(P = 0.435),or fibrosis(P = 0.291).The frequencies of IL28B rs12979860 genotypes were TT(14.8%),CT(49.0%),and CC(36.2%).Compared to rs12979860 genotype TT,aORs(95%CI) for ETR and SVR were:CC genotype,[17.55(5.34-57.69) and 5.92(2.09-16.76),respectively];CT genotype,[5.15(1.80-14.78) and 2.48(0.94-6.52),respectively].In the current study,the patients who did not achieve ETR or SVR had a lower prevalence of rs12979860 CC(17.4% and 23.3%,respectively) than individuals who had ETR or SVR(47.9% and 47.2%,respectively).Individuals with rs12979860 CC genotype had approximately 6 times the odds of SVR compared to individuals with TT genotype(aOR = 5.92;95%CI:2.09-16.76).Similarly,patients with CT genotype had SVR more often than patients with TT genotype(aOR = 2.48;95%CI:0.94-6.52).Carrying at least one copy of the C allele(genotypes CT and CC) had almost 8 times the probability of ETR compared to those with genotype rs12979860 TT(aOR = 7.87;95%CI:2.84-21.82),and approximately 3 times the odds of SVR compared to those with genotype rs12979860 TT(aOR = 3.46;95%CI:1.37-8.74).In addition,data were consistent with a significant gene-dose relationship(aOR = 4.05/allele;95%CI:2.27-7.22).The association between rs12979860 genotype and SVR was similar among those who achieved and those who did not achieve SVR.CONCLUSION:In HCV-genotype 4 patients,rs12979860 is a sensitive predictor of viral clearance,independent of viral load,age,gender or fibrosis,with no similar relation to severity of fibrosis. | Moutaz Derbala Nasser Rizk Fatima Shebl Saad Alkaabi Nazeeh Eldweik Anil John Manik Sharma Rafie Yaqoob Muneera Almohanadi Mohammed Butt Khaled Alejji | 2012 | World Journal of Gastroenterology2012，18，47： | 2 |

| 9 | Factors influencing the yield of mesenteric angiography in lower gastrointestinal bleed显示文摘AIM: To assess if certain triaging rules could be established to optimize the yield of mesenteric angiography. METHODS: Medical records of 101 patients were retrospectively reviewed and parameters relating to age, gender, pulse rate, blood pressure, serum hemoglobin, intensive care unit(ICU) admission, and the number of packed red blood cells(PRBC) transfused in the 12 and 24 h prior to the angiography were tabulated in two groups with positive and negative angiography results. RESULTS: We found no correlation between gender, pulse rate, blood pressure or serum hemoglobin and positivity of the mesenteric angiogram. But patients with positive angiogram were found to be on average 7 years older(73.2 years vs 65.9 years old)(P = 0.02). Angiogram was positive in 39.3 %(11/28) of patientsadmitted in ICU vs 23.2%(17/73) who were admitted elsewhere in the hospital(P = 0.03). In the 12 and 24 h prior to angiography, patients with a positive angiogram received a mean of 2.7 ± 2.3 and 3.3 ± 2.6 units of PRBC s respectively, while patients with a negative angiogram had a mean of 1.6 ± 1.9(P = 0.02) and 2.1 ± 2.6 units(P = 0.04) received respectively in the same period.
