1Effect of restoration technique on resistance to fracture of endodontically treated anterior teeth with flared root canals显示文摘This study was designed to compare the impact of post and core systems on resistance to fracture of endodontically treated anterior teeth with flared root canals and to assess their fracture pattern. Sixty central incisors were cut horizontally 2 mm coronal to the cementoenamel junction(CEJ). After root canal therapy, teeth were assigned into 6 groups(n = 10 each) based on a post system and used as follows: Group C, non-flared root received size #1 glass fiber posts(Control); Group AP, flared root restored with anatomical post; Group RC, flared root restored with size #1 fiber post and cemented with thick layer of resin cement; Group CR, flared root restored with size #1 and reinforced with composite resin; Group CM, cast post-core; Group CP, CAD/CAM polymer-infiltrated ceramic post and core.Following post cementation, core build-up and crown insertion, the specimens were thermo-cycled up to 10,000 cycles(5 C/55 C; 30 seconds dwell time, 6 seconds transition time) and then statically loaded at 1 mm/minute crosshead speed using a universal testing machine. One-way ANOVA and Tukey HSD post hoc test(α= 0.05) were used for data analysis. Group C recorded significantly higher resistance to fracture values [(826.9±39.1) N] followed by group CP [(793.8±55.6) N] while group RC yielded the lowest fracture resistance values [(586.7±51.4) N]. The resistance to fracture of wide root canals can be enhanced by using one-piece CAM/CAM post and core as an alternative to the use of either glass fiber post, relined with composite resin increasing the thickness of luting cement or the use of cast post and core system. 2Leucine-rich repeat-containing G protein-coupled receptor 5 marks different cancer stem cell compartments in human Caco-2 and LoVo colon cancer lines显示文摘BACKGROUND Colon cancer cell lines are widely used for research and for the screening of drugs that specifically target the stem cell compartment of colon cancers.It was reported that colon cancer carcinoma specimens contain a subset of leucine-rich repeatcontaining G protein-coupled receptor 5(LGR5)-expressing stem cells,these socalled“tumour-initiating”cells,reminiscent in their properties of the normal intestinal stem cells(ISCs),may explain the apparent heterogeneity of colon cancer cell lines.Also,colon cancer is initiated by aberrant Wnt signaling in ISCs known to express high levels of LGR5.Furthermore,in vivo reports demonstrate the clonal expansion of intestinal adenomas from a single LGR5-expressing cell.AIM To investigate whether colon cancer cell lines contain cancer stem cells and to characterize these putative cancer stem cells.METHODS A portable fluorescent reporter construct based on a conserved fragment of the LGR5 promoter was used to isolate the cell compartments expressing different levels of LGR5 in two widely used colon cancer cell lines(Caco-2 and LoVo).These cells were then characterized according to their proliferation capacity,gene expression signatures of ISC markers,and their tumorigenic properties in vivo and in vitro.RESULTS The data revealed that the LGR5 reporter can be used to identify and isolate a classical intestinal crypt stem cell-like population from the Caco-2,but not from the LoVo,cell lines,in which the cancer stem cell population is more akin to B lymphoma Moloney murine leukemia virus insertion region 1 homolog(+4 crypt)stem cells.This sub-population within Caco-2 cells exhibits an intestinal cancer stem cell gene expression signature and can both self-renew and generate differentiated LGR5 negative progeny.Our data also show that cells expressing high levels of LGR5/enhanced yellow fluorescent protein(EYFP)from this cell line exhibit tumorigenic-like properties in vivo and in vitro.In contrast,cell compartments of LoVo that are expressing high levels of LGR5/EYFP did not show these stem cell-like properties.Thus,cells that exhibit high levels of LGR5/EYFP expression represent the cancer stem cell compartment of Caco-2 colon cancer cells,but not LoVo cells.CONCLUSION Our findings highlight the presence of a spectrum of different ISC-like compartments in different colon cancer cell lines.Their existence is an important consideration for their screening applications and should be taken into account when interpreting drug screening data.We have generated a portable LGR5-reporter that serves as a valuable tool for the identification and isolation of different colon cancer stem cell populations in colon cancer lines.Samah Abdulaali Alharbi Dmitry A Ovchinnikov Ernst Wolvetang 2021World Journal of Gastroenterology2021,27,15:2

5Medical treatment of inflammatory punctual stenosis monitored by anterior segment optical coherence tomography显示文摘AIM:To evaluate the role of medical treatment in the management of inflammatory punctum stenosis guided by spectral domain anterior segment optical coherence tomography(OCT).METHODS:This prospective study included 60 patients complaining of epiphora due to inflammatory punctual stenosis.They were divided into two groups:Group A(30 patients)treated with a combination of preservative free hydrocortisone sodium phosphate 3.35 mg/m L eye drops,and preservative free artificial tears based on sodium hyaluronate,polyethylene and propylene Glycol;Group B(30 patients)treated with the same preservative free artificial tears only.Thirty normal subjects were included for comparison of pre-treatment anatomical parameters.Before starting treatment,all patients underwent anterior segment assessment including slit lamp examination,measurement of intraocular pressure(IOP),fluorescein dye disappearance test,and measurement of the outer punctum diameter(OPD)by Spectral domain anterior segment OCT.All assessments were repeated at each follow up done at 1 and 3 mo later,together with subjective evaluation of patient’s satisfaction of the treatment outcome by simple rating questionnaire.RESULTS:Punctual diameter increased significantly with treatment in both groups(P<0.0001);although the widening was more in Group A as compared to group B(16.2%vs 8%of the original punctual size,mean difference of 28.933μm,P=0.0076).Subjective satisfaction with treatment outcome was also better in group A(70%vs 40%,Chi-square P=0.0397).CONCLUSION:A combination of preservative free steroid eye drops and artificial tears causes significant widening of inflammed stenotic punctae and improvement of the associated epiphora.Hanan Youssef Elalfy Mahmoud A Elsamkary Ahmed M Shafek Elridy Tarek M Saad Saad Mohamed Rashad Samah M Fawzy 2020International Journal of Ophthalmology(English edition)2020,13,7:1
