| 1 | Prevalence of clonorchiasis in patients with gastrointestinal disease: A Korean nationwide multicenter survey显示文摘AIM: To investigate prevalence of Clonorchis sinensis in patients with gastrointestinal symptoms, and the relation of the infection to hepatobiliary diseases in 26 hospitals in Korea.METHODS: Consecutive patients who had been admitted to the Division of Gastroenterology with gastrointestinal symptoms were enrolled from March to April 2005. Of those who had been diagnosed with clonorchiasis, epidemiology and correlation between infection and hepatobiliary diseases were surveyed by questionnaire.RESULTS: Of 3080 patients with gastrointestinal diseases, 396 (12.9%) had clonorchiasis and 1140 patients (37.2%) had a history of eating raw freshwater fish. Of those with a history of raw freshwater fish ingestion, 238 (20.9%) patients had clonorchiasis. Cholangiocarcinoma was more prevalent in C. sinensis-infected patients than non-infected patients [34/396 (8.6%) vs 145/2684 (5.4%), P = 0.015]. Cholangiocarcinoma and clonorchiasis showed statistically significant positive cross-relation (P = 0.008). Choledocholithiasis, cholecystolithiasis, cholangitis, hepatocellular carcinoma, and biliary pancreatitis did not correlate with clonorchiasis.CONCLUSION: Infection rate of clonorchiasis was still high in patients with gastrointestinal diseases in Korea, and has not decreased very much during the last two decades. | 2 | Comparison of postoperative optical quality according to the degree of decentering of V4c implantable collamer lens显示文摘AIM:To evaluate the clinical outcomes of V4 c implantable collamer lens(Hole ICL) implantation with regard to the optical quality assessed according to different degrees of decentering.METHODS:This included 49 eyes that received conventional ICL and 94 eyes that received Hole ICL.The eyes that received Hole ICL were divided into three groups according to the degree of decentering:group 1,central hole within 1 hole diameter(HD) from the pupil center;group 2,central hole within 1 HD to 2 HD;and group 3,central hole within 2 HD to 3 HD.Visual acuity(VA),intraocular pressure(IOP),and spherical equivalent(SE) values were assessed at 1wk,1 and 3mo after surgery.The ocular modulation transfer function,Strehl ratio,objective scattering index,and higher order aberrations(HOAs) were measured for 4-mm pupils at 3mo after surgery.RESULTS:There were no significant differences in VA,IOP,and SE among the conventional and Hole ICL groups.With regard to HOAs,values for coma and spherical aberrations showed no differences.The total HOA and trefoil values were significantly higher in group 2 than in group 1(P=0.02,0.03,respectively).There were no significant differences among groups with regard to other optical quality parameter at 3mo after surgery.CONCLUSION:Our results suggest that Hole ICL implantation provides satisfactory visual quality that is equivalent to that provided by conventional ICL,regardless of the presence of central hole and degree of decentering. | Min Ji Park Hye Min Jeon Kyoung Heon Lee Sang Youp Han | 2017 | International Journal of Ophthalmology(English edition)2017，10，4： | 7 |
| 3 | Assessment of scoring systems for acute-on-chronic liver failure at predicting short-term mortality in patients with alcoholic hepatitis显示文摘AIM To assess the performance of proposed scores specific for acute-on-chronic liver failure in predicting shortterm mortality among patients with alcoholic hepatitis.METHODS We retrospectively collected data from 264 patients with clinically diagnosed alcoholic hepatitis from January to December 2013 at 21 academic hospitals in Korea. The performance for predicting short-term mortality was calculated for Chronic Liver FailureSequential Organ Failure Assessment(CLIF-SOFA), CLIF Consortium Organ Failure score(CLIF-C OFs), Maddrey'sdiscriminant function(DF), age, bilirubin, international normalized ratio and creatinine score(ABIC), Glasgow Alcoholic Hepatitis Score(GAHS), model for end-stage liver disease(MELD), and MELD-Na.RESULTS Of 264 patients, 32(12%) patients died within 28 d. The area under receiver operating characteristic curve of CLIF-SOFA, CLIF-C OFs, DF, ABIC, GAHS, MELD, and MELD-Na was 0.86(0.81-0.90), 0.89(0.84-0.92), 0.79(0.74-0.84), 0.78(0.72-0.83), 0.81(0.76-0.86), 0.83(0.78-0.88), and 0.83(0.78-0.88), respectively, for 28-d mortality. The performance of CLIF-SOFA had no statistically significant differences for 28-d mortality. The performance of CLIF-C OFs was superior to that of DF, ABIC, and GAHS, while comparable to that of MELD and MELD-Na in predicting 28-d mortality. | 12 | Effect of music on colonoscopy performance: A propensity scorematched analysis显示文摘BACKGROUND Music has been used to reduce stress and improve task performance during medical therapy.AIM To assess the effects of music on colonoscopy performance outcomes.METHODS We retrospectively reviewed patients who underwent colonoscopy performed by four endoscopists with popular music.Colonoscopy performance outcomes,such as insertion time,adenoma detection rate(ADR),and polyp detection rate(PDR),were compared between the music and non-music groups.To reduce selection bias,propensity score matching was used.RESULTS After one-to-one propensity score matching,169 colonoscopies were selected from each group.No significant differences in insertion time(4.97 vs 5.17 min,P=0.795)and ADR(39.1%vs 46.2%,P=0.226)were found between the two groups.Subgroup analysis showed that the insertion time(3.6 vs 3.8 min,P=0.852)and ADR(51.1%vs 44.7%,P=0.488)did not significantly differ between the two groups in experts.However,in trainees,PDR(46.9%vs 66.7%,P=0.016)and ADR(25.9%vs 47.6%,P=0.006)were significantly lower in the music than in the nonmusic group.CONCLUSION The current study found that listening to music during colonoscopy did not affect procedure performance.Moreover,it suggested that music may distract trainees from appropriately detecting adenomas and polyps. | Eun Jeong Choi Sam Ryong Jee Sang Heon Lee Jun Sik Yoon Seung Jung Yu Jong Hyun Lee Han Byul Lee Sang Wook Yi Myeong Pyo Kim Byung Cheol Chung Hong Sub Lee | 2023 | World Journal of Gastrointestinal Endoscopy2023，15，5： | 0 |
| 13 | Autoimmune hepatitis in a patient with immunoglobulin A nephropathy:A case report显示文摘BACKGROUND Immunoglobulin A nephropathy(IgAN)is the most commonly encountered glomerular disease in Asian countries.It has a broad clinical presentation,and it is frequently associated with other conditions.Chronic liver disease is well recognized as the leading cause of secondary IgAN.However,cases of IgAN associated with autoimmune hepatitis(AIH)have seldom been reported.CASE SUMMARY A 63-year-old Korean woman was admitted to Pusan National University Hospital for an evaluation of abdominal pain and elevated liver enzymes.Two weeks prior,she had presented to our hospital with proteinuria of approximately 1350 mg/d and hematuria and was diagnosed with IgAN.Autoimmune profiles were highly positive for antinuclear antibodies,and symptoms related to portal hypertension including ascites and peripheral edema were present.A diagnosis of AIH was made according to the simplified scoring system of the International Autoimmune Hepatitis Group.Despite immunosuppression with prednisolone and azathioprine,rapid deterioration of liver function led to end-stage liver disease.After a living-donor liver transplantation,liver function gradually improved,and she had maintained stable liver and kidney function at the six months follow-up.CONCLUSION Cases of secondary IgAN with chronic liver disease have been frequently reported in the literature but are rarely associated with AIH.We encountered an IgAN patient with concurrent progressive liver failure due to AIH. | You Hyun Jeon Da Woon Kim So Jeong Lee Young Joo Park Hyo Jin Kim Miyeun Han Il Young Kim Dong Won Lee Sang Heon Song Soo Bong Lee Eun Young Seong | 2020 | World Journal of Clinical Cases2020，8，17： | 0 |
