1Imbalance between expression of matrix metalloproteinase-9 and tissue inhibitor of metalloproteinase-1 in invasiveness and metastasis of human gastric carcinoma显示文摘AIM: The expressive balance between matrix metalloproteinase-9 (MMP-9) and its tissue inhibitor of metalloproteinase-1 (TIMP-1) plays a critical role in maintaining the degradation and synthesis of extracellular matrix. Loss of such balance is associated with invasion and metastasis of tumors. This study aimed to determine the expression of MMP-9 and TIMP-1 in gastric carcinoma, and the association of the expressive imbalance between MMP9 and TIMP-1 with the invasion and metastasis and prognosis of gastric carcinoma.METHODS: We used immunohistochemistry to determine the expressions of MMP-9, TTMP-1 and proliferating cell nuclear antigen Ki-67 in the gastric specimens taken from 256 patients with primary gastric carcinoma. The patients were followed-up for up to 96 months.RESULTS: No association between the expression of MMP9 and TIMP-1 and patients' sex and age, tumor size and location of gastric carcinoma was observed. The incidence of the positive expression of MMP-9 in cases with tumors invasion to muscularis propria and visceral peritoneum (70.13% and 69.09%, respectively) was significantly higher than that in cases with tumor invasion only to lamina propria or submucosa (42.50 %, P=0.0162). The positive correlation between MMP-9 expression and the depth of tumor invasion was observed (Pearson correlation coefficient=0.2129,P=0.016). Along with the increase of the metastatic station of lymph nodes, the incidence of the MMP-9 expression was increased by degrees; a positive correlation between them was observed (Pearson correlation coefficient=0.2910,P=0.0001). There was also a significant correlation between MMP-9 expression and the TNM stage in gastric carcinoma (Pearson correlation coefficient=0.3027, P<0.0001). The incidence of MMP-9 expression in stage Ⅱ and Ⅲ/Ⅳ (75.00%and 76.15%, respectively) was significantly higher than those in stage Ⅰ (46.15 %, P<0.0001). A negative correlation between TIMP-1 immunoreactivity and the depth of invasion,status of lymph node metastasis and TNM stage was observed (Pearson correlation coefficient =-0.1688, -0.3556and -0.3004, P=0.023, <0.0001 and <0.0001, respectively).Four types of co-expression of MMP-9 and TIMP-1 were observed; i.e. MMP-9 positive but T IMP-1 negative (n=115),both positive (n=52), both negative (n=62) and MMP-9negative but TIMP-1 positive (n=27). The frequency of serosal invasiveness was significant higher in patients with MMP-9 but without TIMP-1 expression than those with other types of the co-expression (P=0.0303). The incidence of lymph node metastasis was highest in patients with MMP-9but without TIMP-1 expression, and lowest in those with TIMP-1 but without MMP-9 expression (P<0.0001). The survival rate in patients with MMP-9 but without TIMP-1expression was lower than that in those with TIMP-1 but without MMP-9 expression (P=0.0014).CONCLUSION: Our results in gastric carcinoma demonstrated a significant positive association of MMP-9 over-expression with proliferation of tumor cells, the depth of invasiveness,lymph node metastasis and TNM stage, suggesting MMP-9can serve as a molecular marker of tumor invasion and metastasis. We also demonstrate a significant negative relationship of TIMP-1 expression with the depth of invasiveness and lymph node metastasis, which provide a new idea in the tumor biological and genetic treatment.The interaction between MMP-9 and TIMP-1 in the processes of tumor invasion and metastasis is that MMP-9 mainly promotes tumor invasion and metastasis and TIMP-1 inhibits functions of MMP-9. The imbalance between MMP-9 and TIMP-1 expression may suggest the occurrence of tumor invasion and metastasis, predict poor prognosis. 4The effect of aneurismal-wall mechanical properties on patient-specific hemodynamic simulations:two clinical case reports显示文摘Hemodynamic factors such as the wall shearstress play an important role in the pathogenesis and treatmentof cerebral aneurysms. In present study, we apply computationalfluid-structure interaction analyses on cerebralaneurysms with two different constitutive relations for aneur-ismalwall in order to investigate the effect of the aneur-ismal wall mechanical properties on the simulation results.We carry out these analyses by using two patient-specificmodels of cerebral aneurysms of different sizes located indifferent branches of the circle of Willis. The models areconstructed from 3D rotational angiography image data andblood flow dynamics is studied under physiologically representativewaveform of inflow. From the patient models analyzedin this investigation, we find that the deformations ofcerebral aneurysms are very small. But due to the nonlinearcharacter of the Navier-Stokes equations, these smalldeformations could have significant influences on the flowcharacteristics. In addition, we find that the aneurismal-wallmechanical properties have great effects on the deformationdistribution of the aneurysm, which also affects the wall shearstress distribution and flow patterns. 6Hemodynamic performance of coil embolization and stentassisted coil embolization treatments:a numerical comparative study based on subject-specific models of cerebral aneurysms显示文摘Hemodynamic characteristics such as blood velocity,blood pressure,flow impingement,wall shear stress and oscillatory shear index are considered to play important roles in the initiation,growth,rupture and recurrence of the cerebral aneurysms.Endovascular therapy is widely implemented to treat the cerebral aneurysms by releasing coils into the aneurysm sac for limiting the blood flow to the sac and stent-assisted coil embolization is adopted to occlude the wide-necked or complex aneurysms.Some researchers believe that stents are not only a mechanical device but may act as a biological system and contribute to vessel wall healing.Hemodynamics simulation helps people understand the effect of hemodynamic characteristics on the recurrence of the coiled aneurysm and it also benefits the interventional planning of neurosurgeons.This study constructed the numerical model for a subject-specific ICA aneurysm treated with stent-assisted coil embolization,which combined the coiled model of the aneurysm with a porous stent placement,and simulated the pulsatile blood flow in these aneurysm models.When a stent was placed across the aneurysm orifice in the coiled aneurysm,the high wall shear stress around the distal aneurysm root was reduced more than that of the coiled aneurysm without a stent.The simulated results point to the conclusion that the stent not only protects the parent artery from occlusion due to extension of coils or thrombosis,but may also reduce the recurrence risk of the stent-assisted coiled aneurysm.WANG ShengZhang ZHANG YiSen LU Gang YANG XinJian ZHANG XiaoLong DING GuangHong 2011Science China(Physics,Mechanics & Astronomy)2011,54,11:2

10Significant flow velocity reduction at the intracranial aneurysm neck after endovascular treatment leads to favourable angiographic outcome:a prospective study显示文摘Background With widely usage of flow diverter in intracranial aneurysm treatment,some previously used predictors may not be effective in evaluating the recurrence risk.We aimed to comprehensively re-evaluate the predictors of intracranial aneurysm outcome with various endovascular treatment methods and devices.Methods This is a prospective registered study.We analysed 6-month and 18-month follow-up angiographic data from the prospective study.Data on patient demographics,aneurysm morphology and type of treatment were recorded.Patient-specific haemodynamic simulations were performed.An unfavourable angiographic outcome was defined as recurrence of aneurysm in cases with coiling or stent-assisted coiling,patency of aneurysm in cases with flow diverters or retreatment during follow-up.Results In total,165 patients(177 intracranial aneurysms)with at least one angiographic follow-up data were analysed.For the short-term(6-month)results,after univariate analysis,the demographic,morphological and treatment-related factors did not achieve significantly statistical differences.The reduction ratio(RR)of velocity at aneurysm neck after embolisation was significantly lower in the unfavourable angiographic group than the favourable angiographic outcome group(p=0.002).After the Cox regression analysis,the RR of velocity at aneurysm neck was the only independent factor associated with favourable angiographic outcome(OR 0.028;p=0.001)and had an acceptable area under the curve(0.714)with a clear cut-off value(46.14%).Similarly,for the analysis of midterm(18-month)results,the RR of velocity at the aneurysm neck was the only independent significant factor for the unfavourable angiographic outcome(OR 0.050;p=0.017).The area under the curve was 0.754 and the cut-off value was 48.20%.Conclusions The haemodynamics showed an independent effect on angiographic follow-up results and may provide helpful suggestions for clinical practice in the future.Zhongbin Tian Jian Liu Jay Kumar Wenqiang Li Yisen Zhang Ying Zhang Kun Wang Shengzhang Wang Zeguang Ren Xinjian Yang 2021Stroke & Vascular Neurology2021,6,3:1

12Numerical Analyses of Idealized Total Cavopulmonary Connection Physiologies with Single and Bilateral Superior Vena Cava Assisted by an Axial Blood Pump显示文摘Our study evaluated the hemodynamic performance of an axial flow blood pump surgically implanted in idealized total cavopulmonary connection(TCPC)models.This blood pump was designed to augment pressure from the inferior vena cava(IVC)to the pulmonary circulation.Two Fontan procedures with single and bilateral superior vena cava(SVC)were compared to fit the mechanical supported TCPC physiologies.Computational fluid dynamics(CFD)analyses of two Pump-TCPC models were performed in the analyses.Pressure-flow characteristics,energy efficiency,fluid streamlines,hemolysis and thrombosis analyses were implemented.Numerical simulations indicate that the pump produces pressure generations of 1 mm to 24 mm Hg for rotational speeds ranging from 2000 RPM to 5000 RPM and flow rates of 2 LPM to 4 LPM.Two surgical models incorporated with the pump were found to be insignificant in pressure augmentation and energy boost.The risk assessment of blood trauma and thrombosis generation was evaluated representatively through blood damage index(BDI),particle resident time(PRT)and relative resistant time(RRT).The hemolysis and thrombosis analyses declare the advantage of the pump supported bilateral SVC surgical scheme in balancing flow distribution and reducing the risk of endothelial cell destruction and trauma generation.Xudong Liu Yunhan Cai Bing Jia Shengzhang Wang Guanghong Ding 2018Computer Modeling in Engineering & Sciences2018,,8:1

20An Experimental Study of Mixture Corrosion Effects of Carbonate Rocks in the Transitional Zone of Littoral Karst Areas显示文摘The mechanism for development of littoral karst differs from that of inland karst, and the mixture corrosion effects are one of the most important factors that control the development of littoral karst.
