1Targeting the PI3K/Akt signaling pathway in gastric carcinoma: A reality for personalized medicine?显示文摘Frequent activation of phosphatidylinositol-3 kinases(PI3K)/Akt/m TOR signaling pathway in gastric cancer(GC) is gaining immense popularity with identification of mutations and/or amplifications of PIK3 CA gene or loss of function of PTEN,a tumor suppressor protein,to name a few; both playing a crucial role in regulating this pathway. These aberrations result in dysregulation of this pathway eventually leading to gastric oncogenesis,hence,there is a need for targeted therapy for more effective anticancer treatment. Several inhibitors are currently in either preclinical or clinical stages for treatment of solid tumors like GC. With so many inhibitors under development,further studies on predictive biomarkers are needed to measure the specificity of any therapeutic intervention. Herein,we review the common dysregulation of PI3K/Akt/m TOR pathway in GC and the various types of single or dual pathway inhibitors under development that might have a superior role in GC treatment. 2Update on the management of gastrointestinal varices显示文摘Cirrhosis of liver is a major problem in the western world. Portal hypertension is a complication of cirrhosis and can lead to a myriad of pathology of which include the development of porto-systemic collaterals. Gastrointestinal varices are dilated submucosal veins, which often develop at sites near the formation of gastroesophageal collateral circulation. The incidence of varices is on the rise due to alcohol and obesity. The most significant complication of portal hypertension is life-threatening bleeding from gastrointestinal varices, which is associated with substantial morbidity and mortality. In addition, this can cause a significant burden on the health care facility. Gastrointestinal varices can happen in esophagus, stomach or ectopic varices. There has been considerable progress made in the understanding of the natural history, pathophysiology and etiology of portal hypertension. Despite the development of endoscopic and medical treatments, early mortality due to variceal bleeding remains high due to significant illness of the patient. Recurrent variceal bleed is common and in some cases, there is refractory variceal bleed. This article aims to provide a comprehensive review of the management of gastrointestinal varices with an emphasis on endoscopic interventions, strategies to handle refractory variceal bleed and newer endoscopic treatment modalities. 3Hypoxia-inducible factor 2α is a negative regulator of osteoblastogenesis and bone mass accrual显示文摘Osteoblasts,which are the bone-forming cells,operate in a hypoxic environment.The transcription factors hypoxia-inducible factor-1α(HIF1)and HIF2 are key mediators of the cellular response to hypoxia.Both are expressed in osteoblasts.HIF1 is known to be a positive regulator of bone formation.Conversely,the role of HIF2 in the control osteoblast biology is still poorly understood.In this study,we used mouse genetics to demonstrate that HIF2 is an inhibitor of osteoblastogenesis and bone mass accrual.Moreover,we provided evidence that HIF2 impairs osteoblast differentiation at least in part,by upregulating the transcription factor Sox9.Our findings constitute a paradigm shift,as activation of the hypoxia-signaling pathway has traditionally been associated with increased bone formation through HIF1.Inhibiting HIF2 could thus represent a therapeutic approach for the treatment of the low bone mass observed in chronic diseases,osteoporosis,or aging.Christophe Merceron Kavitha Ranganathan Elizabeth Wang Zachary Tata Shreya Makkapati Mohd Parvez Khan Laura Mangiavini Angela Qing Yao Laura Castellini Benjamin Levi Amato J.Giaccia Ernestina Schipani 2019Bone Research2019,7,1:7
4Parenteral nanosuspensions:a brief review from solubility enhancement to more novel and specific applications显示文摘Advancements in in silico techniques of lead molecule selection have resulted in the failure of around 70% of new chemical entities(NCEs). Some of these molecules are getting rejected at final developmental stage resulting in wastage of money and resources. Unfavourable physicochemical properties affect ADME profile of any efficacious and potent molecule, which may ultimately lead to killing of NCE at final stage. Numerous techniques are being explored including nanocrystals for solubility enhancement purposes. Nanocrystals are the most successful and the ones which had a shorter gap between invention and subsequent commercialization of the first marketed product. Several nanocrystal-based products are commercially available and there is a paradigm shift in using approach from simply being solubility enhancement technique to more novel and specific applications. Some other aspects in relation to parenteral nanosuspensions are concentrations of surfactant to be used, scalability and in vivo fate. At present, there exists a wide gap due to poor understanding of these critical factors,which we have tried to address in this review. 6A Context Sensitive Multilevel Thresholding Using Swarm Based Algorithms显示文摘In this paper, a comprehensive energy function is used to formulate the three most popular objective functions:Kapur's, Otsu and Tsalli's functions for performing effective multilevel color image thresholding. These new energy based objective criterions are further combined with the proficient search capability of swarm based algorithms to improve the efficiency and robustness. The proposed multilevel thresholding approach accurately determines the optimal threshold values by using generated energy curve, and acutely distinguishes different objects within the multi-channel complex images. 7Disentangling multiple scattering with deep learning:application to strain mapping from electron diffraction patterns显示文摘A fast,robust pipeline for strain mapping of crystalline materials is important for many technological applications.Scanning electron nanodiffraction allows us to calculate strain maps with high accuracy and spatial resolutions,but this technique is limited when the electron beam undergoes multiple scattering.Deep-learning methods have the potential to invert these complex signals,but require a large number of training examples.We implement a Fourier space,complex-valued deep-neural network,FCU-Net,to invert highly nonlinear electron diffraction patterns into the corresponding quantitative structure factor images.FCU-Net was trained using over 200,000 unique simulated dynamical diffraction patterns from different combinations of crystal structures,orientations,thicknesses,and microscope parameters,which are augmented with experimental artifacts.We evaluated FCU-Net against simulated and experimental datasets,where it substantially outperforms conventional analysis methods.Our code,models,and training library are open-source and may be adapted to different diffraction measurement problems.Joydeep Munshi Alexander Rakowski Benjamin H.Savitzky Steven E.Zeltmann Jim Ciston Matthew Henderson Shreyas Cholia Andrew M.Minor Maria K.Y.Chan Colin Ophus 2022npj Computational Materials2022,,1:2
8Do medical students studying in the United Kingdom have an adequate factual knowledge of basic life support?显示文摘BACKGROUND: Healthcare professionals have a duty to maintain basic life support(BLS) skills. This study aims to evaluate medical students' factual knowledge of BLS and the training they receive.METHODS: A cross-sectional, closed-response questionnaire was distributed to the fi rst-and fourth-year students studying at institutions in the United Kingdom. The paper questionnaire sought to quantify respondent's previous BLS training, factual knowledge of the BLS algorithm using five multiple choice questions(MCQs), and valuate their desire for further BLS training. Students received 1 point for each correctly identifi ed answer to the 5 MCQ's.RESULTS: A total of 3,732 complete responses were received from 21 medical schools. Eighty percent(n=2,999) of students completed a BLS course as part of their undergraduate medical studies. There was a signifi cant difference(P<0.001) in the percentage of the fourth-year students selecting the correct answer in all the MCQ's compared to the fi rst-year students except in identifyingthe correct depth of compressions required during CPR(P=0.095). Overall 10.3%(95% CI 9.9% to 10.7%) of respondents correctly identified the answer to 5 MCQ's on BLS: 9% of the first-year students(n=194) and 12% of the fourth-year students(n=190). On an institutional level the proportion of students answering all MCQ's correctly ranged from 2% to 54% at different universities. Eighty-one percent of students(n=3,031) wished for more BLS training in their curriculum.CONCLUSION: Factual knowledge of BLS is poor among medical students in the UK. 9In vitro study of enhanced photodynamic cancer cell killing effect by nanometer-thick gold nanosheets显示文摘Photodynamic therapy(PDT)by near-infrared(NIR)irradiation is a promising technique for treating various cancers.Here,we reported the development of free-standing wafer-scale Au nanosheets(NSs)that exhibited an impressive PDT effect.The Au NSs were synthesized by ionic layer epitaxy at the air-water interface with a uniform thickness in the range from 2 to 8.5 nm.These Au NSs were found very effective in generating singlet oxygen under NIR irradiation.In vitro cellular study showed that the Au NSs had very low cytotoxicity and high PDT efficiency due to their uniform 2D morphology.Au NSs could kill cancer cells after 5 min NIR irradiation with little heat generation.This performance is comparable to using 10 times mass loading of Au nanoparticles(NPs).This work suggests that two-dimensional(2D)Au NSs could be a new type of biocompatible nanomaterial for PDT of cancer with an extraordinary photon conversion and cancer cell killing efficiency.Ziyi Zhang Dalong Ni Fei Wang Xin Yin Shreya Goel Lazarus NGerman Yizhan Wang Jun Li Weibo Cai Xudong Wang 2020Nano Research2020,13,12:2
10Endoscopic ultrasound guided gallbladder drainage - is it ready for prime time?显示文摘Management of acute cholecystitis includes initial stabilization and antibiotics. However, the most definitive treatment is cholecystectomy. A small percentage of patients who are not suitable for surgery due to the severity of cholecystitis or comorbidities will require a temporary measure as a bridge to surgery or permanent nonoperative management to decrease the mortality and morbidity. Most of these patients who require conservative management were managed with percutaneous transhepatic cholecystostomy or trans-papillary drainage of gallbladder drainage with cystic duct stenting through endoscopic retrograde cholangiopancreaticography(ERCP). Although, these conservative measures are effective, they can cause significant discomfort to the patients especially if used as a long-term measure. In view of this, there is a need for further minimally invasive procedures, which is safe, effective and comfortable to patients. Endoscopic ultrasound(EUS) guided gallbladder drainage is a novel method of gallbladder drainage first described in 2007^([1]). Over the last decade, EUS guided gallbladder drainage has evolved as an effective alternative to percutaneous cholecystostomy and trans-papillary gallbladder drainage. 12Taste masking of ciprofloxacin by ion-exchange resin and sustain release at gastric-intestinal through interpenetrating polymer network显示文摘The aim of the study was to taste mask ciprofloxacin(CP)by using ion-exchange resins(IERs)followed by sustain release of CP by forming interpenetrating polymer network(IPN).IERs based on the copolymerization of acrylic acid with different cross linking agents were synthesised.Drug-resin complexes(DRCs)with three different ratios of drug to IERs(1:1,1:2,1:4)were prepared&evaluated for taste masking by following in vivo and in vitro methods.Human volunteers graded ADC 1:4,acrylic acid-divinyl benzene(ADC-3)resin as tasteless.Characterization studies such as FTIR,SEM,DSC,P-XRD differentiated ADC 1:4,from physical mixture(PM 1:4)and confirmed the formation of complex.In vitro drug release of ADC 1:4 showed complete release of CP within 60 min at simulated gastric fluid(SGF)i.e.pH 1.2.IPN beads were prepared with ADC 1:4 by using sodium alginate(AL)and sodium alginate-chitosan(AL-CS)for sustain release of CP at SGF pH and followed by simulated intestinal fluid(SIF i.e.pH 7.4).FTIR spectra confirmed the formation of IPN beads.The release of CP was sustain at SGF pH(<20%)whereas in SIF media it was more(>75%).The kinetic model of IPN beads showed the release of CP was non-Fickian diffusion type.A.Michael Rajesh Shreya A.Bhatt Harshad Brahmbhatt Pritpal Singh Anand Kiritkumar Mangaldas Popat 2015Asian Journal of Pharmaceutical Sciences2015,10,4:2
13Amniotic fluid: Source of trophic factors for the developingintestine显示文摘The gastrointestinal tract(GIT) is a complex system, which changes in response to requirements of the body. GIT represents a barrier to the external environment. To achieve this, epithelial cells must renew rapidly. This renewal of epithelial cells starts in the fetal life under the influence of many GIT peptides by swallowing amniotic fluid(AF). Development and maturation of GIT is a very complex cascade that begins long before birth and continues during infancy and childhood by breastfeeding. Many factors like genetic preprogramming, local and systemic endocrine secretions and many trophic factors(TF) from swallowed AF contribute and modulate the development and growth of the GIT. GIT morphogenesis, differentiation and functional development depend on the activity of various TF in the AF. 14In Vivo Tumor-Targeted Dual-Modality PET/Optical Imaging with a Yolk/Shell-Structured Silica Nanosystem显示文摘Silica nanoparticles have been one of the most promising nanosystems for biomedical applications due to their facile surface chemistry and non-toxic nature. However, it is still challenging to effectively deliver them into tumor sites and noninvasively visualize their in vivo biodistribution with excellent sensitivity and accuracy for effective cancer diagnosis.
