Successful minimal approach transcatheter aortic valve replacement in an allograft heart recipient 19 years post transplantation for severe aortic stenosis: A case report显示文摘BACKGROUND Aortic stenosis is one of the rare valvular complications in a transplanted heart.Over the past 8 years, transcatheter approach for aortic valve replacement(TAVR)has been slowly evolving to be the preferred approach in these patient population when compared to the surgical approach. We report a second case in the United States with successful transfemoral minimal approach with minimal sedation for TAVR in a heart transplant recipient 19 years post transplantation for severe symptomatic calcified aortic stenosis.CASE SUMMARY We present a case of 73-year-old male who has undergone successful minimal approach transcatheter aortic valve replacement in an allograft heart.Patient had received orthotopic heart transplantation 19 years ago for non-ischemic cardiomyopathy.Follow up transthoracic echocardiograms as per routine protocol did not show any aortic valve disease until 15 years post transplantation.Aortic valve was noted to be mildly sclerotic at that time and gradually progressed to severe symptomatic aortic stenosis over the next 4 years.Patient had complaints of worsening shortness of breath that limited his functional capacity.Overall his post heart transplantation period has been mostly uneventful except for allograft non occlusive vasculopathy and aortic stenosis. His Society of Thoracic Surgery risk score was 12.205%and he was considered to be a high-risk surgical candidate by surgeon.Decision was made to undergo transcatheter aortic valve replacement.CONCLUSION With the improved survival of these patients,we think it is time to look intopathophysiology of valvular disease in transplant heart recipients. Some other unanswered questions include, underlying donor and recipient risk factors for valvular diseases in heart transplant recipients.Sravani Avula Sudhir Mungee Marco A Barzallo 2019World Journal of Cardiology2019,11,8:0
Antipoisoning catalysts for the selective oxygen reduction reaction at the interface between metal nanoparticles and the electrolyte显示文摘One of the primary challenges in relation to phosphoric acid fuel cells is catalyst poisoning by phosphate anions that occurs at the interface between metal nanoparticles and the electrolyte. The strong adsorption of phosphate anions on the catalyst surface limits the active sites for the oxygen reduction reaction(ORR),significantly deteriorating fuel cell performance. Here, antipoisoning catalysts consisting of Pt-based nanoparticles encapsulated in an ultrathin carbon shell that can be used as a molecular sieve layer are rationally designed. The pore structure of the carbon shells is systematically regulated at the atomic level by high-temperature gas treatment, allowing O_(2) molecules to selectively react on the active sites of the metal nanoparticles through the molecular sieves.Besides, the carbon shell,as a protective layer, effectively prevents metal dissolution from the catalyst during a long-term operation.Consequently, the defect-controlled carbon shell leads to outstanding ORR activity and durability of the hybrid catalyst even in phosphoric acid electrolytes.Sourabh S.Chougule A.Anto Jeffery Sreya Roy Chowdhury Jiho Min Yunjin Kim Keonwoo Ko Bathinapatla Sravani Namgee Jung 2023Carbon Energy2023,5,7:0
Phytochemical Analysis,Estimation of Quercetin, and in Vitro Anti-Diabetic Potential of Stevia Leaves Samples Procured from Two Geographical Origins显示文摘The current study used RP-HPLC to compare phytochemicals and estimate the bioactive constituents found in Stevia rebaudiana Bert.(SRB)leaves collected from two different geographical sources.SRB leaves were collected from Bangalore,Karnataka,India, and Reduit,Mauritius.Extracts were prepared using ethanol and aqueous solvents.Proximate analysis was used to evaluate moisture content, ash values, crude fibers, and extractive values.Following that, preliminary phytochemical screening was done on both ethanol leaves extracts, and subsequently total flavonoid content was determined. In addition,TLC chromatograms and RP-HPLC studies were performed on both plant extracts to determine the presence of flavonoid components in both leaves extracts, followed by in vitro anti-diabetic activity was performed with alpha amylase and alpha glucosidase enzymes against Acarbose as standard. Results revealed that both the extracts from two different geographical sources varied significantly with the yield, content of chemicals, and presence of quercetin(flavonoid)content when estimated through the RP-HPLC standardized method.Glycosides, flavonoids, proteins, steroids, saponins, terpenoids, and phenols were found in various concentrations during phytochemical screening.Among both zones, the ethanol leaves extract of SRB taken from Mauritius had a greater content of phytochemicals and a higher yield than other extracts due to the soil nature. The Mauritius sample had greater total flavonoid levels as well as more quercetin(0.92±0.011)than the other extracts.Following that, ethanol extract inhibited enzymes(alpha amylase, alpha glucosidases)more than aqueous extract, and this inhibition was dose dependent.Among them, the Mauritius ethanol sample showed higher anti-diabetic efficacy than the Indian sample, but this difference was not significant.Overall,SRB ethanol leaves extracts outperformed other leaves extracts in terms of yield, phytoconstituents, and total flavonoids.Overall, both SRB samples had high quercetin levels and possessed anti-diabetic potential, but they were greater in the Mauritius sample, demonstrating that plant traits are influenced by geographic location.Kuntal Das Syed Mohammed Basheeruddin Asdaq M.Saifulla Khan Sravani Singirikonda Abdulhakeem S.Alamri Walaa F.Alsanie Majid Alhomrani Sreeharsha Nagaraja Katharigatta N.Venugopala 2022Phyton-International Journal of Experimental Botany2022,91,10:0
Artificial intelligence powered glucose monitoring and controlling system:Pumping module显示文摘BACKGROUND Diabetes,a globally escalating health concern, necessitates innovative solutions for efficient detection and management.Blood glucose control is an essential aspect of managing diabetes and finding the most effective ways to control it.The latest findings suggest that a basal insulin administration rate and a single, highconcentration injection before a meal may not be sufficient to maintain healthy blood glucose levels. While the basal insulin rate treatment can stabilize blood glucose levels over the long term,it may not be enough to bring the levels below the post-meal limit after 60 min.The short-term impacts of meals can be greatly reduced by high-concentration injections, which can help stabilize blood glucose levels.Unfortunately, they cannot provide long-term stability to satisfy the postmeal or pre-meal restrictions.However,proportional-integral-derivative(PID)control with basal dose maintains the blood glucose levels within the range for a longer period.AIM To develop a closed-loop electronic system to pump required insulin into the patient's body automatically in synchronization with glucose sensor readings.METHODS The proposed system integrates a glucose sensor, decision unit, and pumping module to specifically address the pumping of insulin and enhance system effectiveness.Serving as the intelligence hub,the decision unit analyzes data from the glucose sensor to determine the optimal insulin dosage, guided by a pre-existing glucose and insulin level table. The artificial intelligence detection block processes this information, providing decision instructions to the pumping module.Equipped with communication antennas, the glucose sensor and micropump operate in a feedback loop, creating a closed-loop system that eliminates the need for manual intervention.RESULTS The incorporation of a PID controller to assess and regulate blood glucose and insulin levels in individuals with diabetes introduces a sophisticated and dynamic element to diabetes management. The simulation not only allows visualization of how the body responds to different inputs but also offers a valuable tool for predicting and testing the effects of various interventions over time. The PID controller's role in adjusting insulin dosage based on the discrepancy between desired setpoints and actual measurements showcases a proactive strategy for maintaining blood glucose levels within a healthy range. This dynamic feedback loop not only delays the onset of steady-state conditions but also effectively counteracts post-meal spikes in blood glucose.CONCLUSION The WiFi-controlled voltage controller and the PID controller simulation collectively underscore the ongoing efforts to enhance efficiency, safety, and personalized care within the realm of diabetes management. These technological advancements not only contribute to the optimization of insulin delivery systems but also have the potential to reshape our understanding of glucose and insulin dynamics, fostering a new era of precision medicine in the treatment of diabetes.Sravani Medanki Nikhil Dommati Hema Harshitha Bodapati Venkata Naga Sai Kowsik Katru Gollapalli Moses Abhishek Komaraju Nanda Sai Donepudi Dhanya Yalamanchili Jasti Sateesh Pratap Turimerla 2024World Journal of Experimental Medicine2024,14,1:0
