| 1 | Experimental small bowel preservation using Polysol: A new alternative to University of Wisconsin solution, Celsior and histidine-tryptophan-ketoglutarate solution?显示文摘AIM: To evaluate the potential of Polysol, a newly developed preservation solution, in cold storage of small bowel grafts, compared with the current standards, University of Wisconsin solution (UW), Celsior and histidine-tryptophan-ketoglutarate solution (HTK). METHODS: Male Wistar rats were used as donors. Small bowels were retrieved, flushed and then stored in the respective 4 solutions for 18 h at 4℃. Functional integrity of the grafts was evaluated by isolated reperfusion with oxygenated Krebs-Henseleit buffer at 37℃ for 30 min in all 4 groups. RESULTS: Polysol preservation exhibited the highest tissue ATP concentration and the lowest release of LDH. Malondialdehyde, an index for tissue lipid peroxidation, was also the lowest in Polysol. Tissue oxygen consumption was significantly higher in Polysol than in the others. Of interest, UW-storage promoted 10-fold higher apoptosis than in the others. Moreover, electron microscopy revealed that the mucosal villi/micro-villi formation and the cell organelles, including mitochondria, were both significantly better preserved in Polysol, while deleterious alterations were apparent in the others, most notably in UW. Although Celsior and HTK exhibited the better trend of results than UW in some parameters, but could not reach the over-all superiority to UW.CONCLUSION: Cold storage using Polysol resulted in significantly better integrity and function of small bowel grafts than UW. | 2 | Role of interventional radiology in the management of acute gastrointestinal bleeding显示文摘Acute gastrointestinal bleeding(GIB) can lead to significant morbidity and mortality without appropriate treatment. There are numerous causes of acute GIB including but not limited to infection, vascular anomalies, inflammatory diseases, trauma, and malignancy. The diagnostic and therapeutic approach of GIB depends on its location, severity, and etiology. The role of interventional radiology becomes vital in patients whose GIB remains resistant to medical and endoscopic treatment. Radiology offers diagnostic imaging studies and endovascular therapeutic interventions that can be performed promptly and effectively with successful outcomes. Computed tomography angiography and nuclear scintigraphy can localize the source of bleeding and provide essential information for the interventional radiologist to guide therapeutic management with endovascular angiography and transcatheter embolization. | 3 | Parameters of a severe disease course in ulcerative colitis显示文摘AIM:To detect high risk patients with a progressive disease course of ulcerative colitis(UC) requiring immunosuppressive therapy(IT).METHODS:A retrospective,multicenter analysis of 262 UC patients from eight German tertiary inflammatory bowel disease centres was performed.Patients were divided into two groups depending on the patients need to initiate immunosuppressive therapy in the disease course.A comparison between the two groups was made with regard to demographics,clinical and laboratory parameters obtained within three months after UC diagnosis and the response to first medical therapy.Using this data,a prognostic model was established to predict the individual patients probability of requiring an immunosuppressive therapy.RESULTS:In 104(39.7%) out of 262 patients,UC therapy required an immunosuppressive treatment.Patients in this group were significantly younger at time of diagnosis(HR = 0.981 ± 0.014 per year,P = 0.009),and required significantly more often a hospitalisation(HR = 2.5 ± 1.0,P < 0.001) and a systemic corticosteroid therapy at disease onset(HR = 2.4 ± 0.8,P < 0.001),respectively.Response to steroid treatment was significantly different between the two groups of patients(HR = 5.2 ± 3.9 to 50.8 ± 35.6 compared to no steroids,P = 0.016 to P < 0.001).Furthermore,in the IT group an extended disease(HR = 3.5 ± 2.4 to 6.1 ± 4.0 compared to proctitis,P = 0.007 to P = 0.001),anemia(HR = 2.2 ± 0.8,P < 0.001),thrombocytosis(HR = 1.9 ± 1.8,P = 0.009),elevated C-reactive protein(CRP)(HR = 2.1 ± 0.9,P < 0.001),and extraintestinal manifestations in the course of disease(HR = 2.6 ± 1.1,P = 0.004) were observed.Six simple clinical items were used to establish a prognostic model to predict the individual risk requiring an IT.This probability ranges from less than 2% up to 100% after 5 years.Using this,the necessity of an immunosuppressive therapy can be predicted in 60% of patients.Our model can determine the need for an immunosuppressive drug therapy or if a 'watch and wait' approach is reasonable already early in the treatment course of UC.CONCLUSION:Using six simple clinical parameters,we can estimate the patients individual risk of developing a progressive disease course. | Andreas Stallmach Luisa Nickel Thomas Lehmann Bernd Bokemeyer Martin Bürger Dietrich Hüppe Wolfgang Kruis Susanna Nikolaus Jan C Preiss Andreas Sturm Niels Teich Carsten Schmidt | 2014 | World Journal of Gastroenterology2014，20，35： | 2 |
| 4 | Comparison of cytokine and phosphoprotein profiles in idiopathic and Crohn’s disease-related perianal fistula显示文摘BACKGROUND Perianal fistulae are either primary(idiopathic)or secondary[commonly associated with Crohn’s disease,(CD)].It is assumed,although not proven,that the pathophysiology differs.AIM To systematically compare the clinical phenotypes,cytokine and phosphoprotein profiles of idiopathic and CD-related perianal fistulae.METHODS Sixty-one patients undergoing surgery for perianal fistula were prospectively recruited(48 idiopathic,13 CD)into a cohort study.Clinical data,including the Perineal Disease Activity Index(PDAI)and EQ-5D-5L were collected.Biopsies of the fistula tract,granulation tissue,internal opening mucosa and rectal mucosa were obtained at surgery.Concentrations of 30 cytokines and 39 phosphoproteins were measured in each biopsy using a magnetic bead multiplexing instrument and a chemiluminescent antibody array respectively.Over 12000 clinical and 23500 laboratory measurements were made.RESULTS The PDAI was significantly higher(indicating more active disease)in the CD group with a mean difference of 2.40(95%CI:0.52-4.28,P=0.01).Complex pathoanatomy was more prevalent in the CD group,namely more multiple fistulae,supralevator extensions,collections and rectal thickening.The IL-12p70 concentration at the internal opening specimen site was significantly higher(median difference 19.7 pg/mL,99%CI:0.2-40.4,P=0.008)and the IL-1RA/IL-1βratio was significantly lower in the CD group at the internal opening specimen site(median difference 15.0,99%CI=0.4-50.5,P=0.008).However in the remaining 27 cytokines and all 39 of the phosphoproteins across the four biopsy sites,no significant differences were found between the groups.CONCLUSION CD-related perianal fistulae are more clinically severe and anatomically complex than idiopathic perianal fistulae.However,overall there are no major differences in cytokine and phosphoprotein profiles. | James B Haddow Omar Musbahi Thomas T MacDonald Charles H Knowles | 2019 | World Journal of Gastrointestinal Pathophysiology2019，10，4： | 2 |
