| 2 | Predictors of recurrent bile duct stone after clearance by endoscopic retrograde cholangiopancreatography:A case-control study显示文摘Background:Recurrent common bile duct(CBD)stone is a long-term sequalae among patients who un-derwent endoscopic retrograde cholangiopancreatography(ERCP)with CBD stone extraction.Data regard-ing risk factors for recurrent CBD stone are scarce.We aimed to identify predictors of recurrent CBD stone.Methods:We performed a retrospective case-controlled study from January 2010 to December 2019.In-clusion criteria included patients who had recurrent CBD stone at least 6 months after the index ERCP,in which complete stone extraction was performed and normal cholangiogram was obtained.Overall,457 patients were included.Forty-two patients(9.2%)had recurrent CBD stone,and 415 patients(90.8%)did not have recurrent CBD stone.Results:In univariate analysis,male sex[odds ratio(OR)=0.49,P=0.033]was a protective factor,while endoscopic stone extraction by basket vs.balloon(OR=2.55,P=0.005),older age(OR=1.03,P=0.003),number of CBD stones(OR=1.99,P=0.037),size of CBD stone(OR=4.06,P=0.003)and mechanical lithotripsy(OR=9.22,P=0.004)were risk factors for recurrent CBD stone.In multivariate logistic regression analysis,mechanical lithotripsy[OR=9.73,95%confidence interval(CI):1.69-55.89,P=0.010],basket clearance vs.combined basket and balloon(OR=18.25,95%CI:1.05-318.35,P=0.046)and older age(OR=1.02,95%CI:1.00-1.05,P=0.023)were risk factors,and male sex(OR=0.39,95%CI:0.19-0.81,P=0.012)was a protective factor. | Wisam Sbeit Anas Kadah Matta Simaan Amir Shahin Tawfik Khoury | 2022 | Hepatobiliary & Pancreatic Diseases International2022,21,1: | 5 |
| 3 | Predicting common bile duct stones by non-invasive parameters显示文摘Background:Common bile duct(CBD)stone affect about 10%of patients with symptomatic cholelithiasis.The American Society for Gastrointestinal Endoscopy(ASGE)published a strategy in 2010 for managing patients with suspected choledocholithiasis.This study aimed to assess the performance of different clinical parameters in predicting CBD stones.Methods:A total of 344 patients suspected to suffer from CBD stone and referred to endoscopic ultrasound(EUS)were included.Parameters were collected and their prediction power for CBD stones was assessed.Results:One hundred and sixty-seven patients without CBD stone according to EUS(group A)were compared to 177 patients with CBD stones(group B).Several predictive factors for CBD stone were identified on univariate analysis.In multivariate regression analysis,CBD width by US(OR=1.224,95%CI:1.073–1.359;P=0.0026),age(OR=1.023,95%CI:1.011–1.035;P=0.0002)and gamma glutamyl transferase(GGT)level(OR=1.001,95%CI:1.000–1.002;P=0.0018)were significantly correlated with CBD stone,with receiver operator characteristics(ROC)of 0.7259.We generated a diagnostic equation[age(yr)×0.1+CBD width(mm)by US×1+GGT(U/L)×0.005]to predict CBD stone with ROC of 0.7287.Conclusions:We suggest this score as a very strong predictor for CBD stones,and to reduce the strength of total bilirubin and transaminases as predictors. | Anas Kadah Tawfik Khoury Mahmud Mahamid Nimer Assy Wisam Sbeit | 2020 | Hepatobiliary & Pancreatic Diseases International2020,19,3: | 4 |
| 4 | History of cupping(Hijama):a narrative review of literature显示文摘Cupping(Hijama in Arabic) is an ancient, holistic method for the treatment of a variety of diseases. Though the exact origin of cupping therapy is a matter of controversy, its use has been documented in early Egyptian and Chinese medical practices. Diverse human civilizations have contributed to the historical development and continuation of cupping therapy. This narrative review describes the history of cupping, historical definitions, cupping instruments and uses of cupping therapy. Electronic searches of relevant databases(Pub Med, Google Scholar and Ovid SP) were conducted using keywords and Boolean operators. Manual searches and references of published articles and books were also conducted. A number of articles(N = 625) were retained for extensive review, and finally 83 articles were included in this paper. The historical descriptions of cupping therapy were found in ancient human civilizations of the Eastern and Western world. There were inconsistent data concerning the origin of cupping, definitions, instruments, procedures, definite advancements and research in Hijama over centuries. Cupping therapy fell out of favor in 17 th and mid-18 th centuries but recovered popularity in modern medicine. Currently, cupping therapy is used for health promotion, prophylaxis and treatment of a variety of diseases around the world. Cupping therapy with a good safety profile has a checkered history and is a well-recognized traditional method for managing medical conditions. | 6 | Barrett's esophagus:Prevalence and risk factors in patients with chronic GERD in Upper Egypt显示文摘AIM:To determine the prevalence and possible risk factors of Barrett's esophagus(BE) in patients with chronic gastroesophageal reflux disease(GERD) in El Minya and Assuit, Upper Egypt.METHODS:One thousand consecutive patients with chronic GERD symptoms were included in the study over 2 years.They were subjected to history taking including a questionnaire for GERD symptoms, clinical examination and upper digestive tract endoscopy.Endoscopic signs suggestive of columnar-lined esophagus(CLE) were defined as mucosal tongues or an upward shift of the squamocolumnar junction.BE was diagnosed by pathological examination when specialized intestinal metaplasia was detected histologically in suspected CLE.pH was monitored in 40 patients.RESULTS:BE was present in 7.3% of patients with chronic GERD symptoms, with a mean age of 48.3 ± 8.2 years, which was signif icantly higher than patients with GERD without BE(37.4 ± 13.6 years).Adenocarcinomawas detected in eight cases(0.8%), six of them in BE patients.There was no signiflcant difference between patients with BE and GERD regarding sex, smoking, alcohol consumption or symptoms of GERD.Patients with BE had significantly longer esophageal acid exposure time in the supine position, measured by pH monitoring.CONCLUSION:The prevalence of BE in patients with GERD who were referred for endoscopy was 7.3%.BE seems to be associated with older age and more in patients with nocturnal gastroesophageal reflux. | Yasser M Fouad Madiha M Makhlouf Heba M Tawfik Hussein El Amin Wael Abdel Ghany Hisham R El-khayat | 2009 | World Journal of Gastroenterology2009,15,28: | 4 |
| 7 | Rotator cuff disorders:How to write a surgically relevant magnetic resonance imaging report?显示文摘Evaluation of rotator cuff is a common indication for magnetic resonance imaging(MRI) scanning of the shoulder. Conventional MRI is the most commonly used technique, while magnetic resonance(MR) arthrogra-phy is reserved for certain cases. Rotator cuff disorders are thought to be caused by a combination of internal and external mechanisms. A well-structured MRI report should comment on the relevant anatomic structures including the acromial type and orientation, the pres-ence of os acromiale, acromio-clavicular degenerative spurs and fluid in the subacromial subdeltoid bursa. In addition, specific injuries of the rotator cuff tendons and the condition of the long head of biceps should be accurately reported. | 8 | Azathioprine-induced fever in autoimmune hepatitis显示文摘Underdiagnosis of drug-induced fever leads to extensive investigation and prolongation of hospitalization, and may lead to multiple unnecessary invasive procedures and a wrong diagnosis. Azathioprine is a widely used immunosuppressive drug. We report a case of a 53-year-old female patient diagnosed with autoimmune hepatitis treated with azathioprine, who presented to the emergency room with a 6-wk history of fever and chills without other associated symptoms. Since the patient's fever was of unknown origin, she was hospitalized. All treatment was stopped and an extensive workup to explore the source of fever and chills was performed. Results of chest X-ray, viral, urine, and blood cultures, autoimmune serology, transthoracic and transesophageal echocardiography, and abdominal ultrasound revealed no source of infection. A rechallenge test of azathioprine was performed and the fever and chills returned within a few hours. Azathioprine was established as the definite cause following rechallenge. Fever as an adverse drug reaction is often unrecognized. Azathioprine has been reported to cause druginduced fever in patients with inflammatory bowel disease, rheumatoid arthritis, and sarcoidosis. To the bestof our knowledge there have been no previous reports documenting azathioprine-induced fever in patients with autoimmune hepatitis. The occurrence of fever following the readministration of azathioprine suggests the diagnosis of drug-induced fever, particularly after the exclusion of other causes. | 9 | Prevalence of intestinal parasitic infections and its relationship with socio-demographics and hygienic habits among male primary schoolchildren in Al-Ahsa,Saudi Arabia显示文摘Objective:To investigate the prevalence of different intestinal parasitic infections among male primary schoolchildren in Al-Ahsa,Saudi Arabia,and to explore the possible relative socioeconomic, environmental and behavioural sanitary correlates.Methods:A cross-sectional descriptive study was performed.A total of 1 289 male students aged from 7 to 12 years were selected randomly from 20 urban and 12 rural primary schools by multistage sampling method. | 10 | Inflammatory bowel disease patient profiles are related to specific information needs: A nationwide survey显示文摘BACKGROUND Inflammatory bowel diseases(IBD) is a heterogenous, lifelong disease, with an unpredictable and potentially progressive course, that may impose negative psychosocial impact on patients.While informed patients with chronic illness have improved adherence and outcomes, previous research showed that the majority of IBD patients receive insufficient information regarding their disease.The large heterogeneity of IBD and the wide range of information topics makes a one-size fits all knowledge resource overwhelming and cumbersome.We hypothesized that different patient profiles may have different and specific information needs, the identification of which will allow building personalized computer-based information resources in the future.AIM To evaluate the scope of disease-related knowledge among IBD patients and determine whether different patient profiles drive unique information needs.METHODS We conducted a nationwide survey addressing hospital-based IBD clinics.A Total of 571 patients completed a 28-item questionnaire, rating the amount of information received at time of diagnosis and the importance of information, as perceived by participants, for a newly diagnosed patient, and for the participants themselves, at current time.We performed an exploratory factor analysis of the crude responses aiming to create a number of representative knowledge domains(factors), and analyzed the responses of a set of 15 real-life patient profiles generated by the study team.RESULTS Participants gave low ratings for the amount of information received at disease onset(averaging 0.9/5) and high ratings for importance, both for the newly diagnosed patients(mean 4.2/5) and for the participants themselves at current time(mean 3.5/5).Factor analysis grouped responses into six informationdomains.The responses of selected profiles, compared with the rest of the participants, yielded significant associations(defined as a difference in rating of >0.5 points with a P < 0.05).Patients with active disease showed a higher interest in work-disability, stress-coping, and therapy-complications.Patients newly diagnosed at age > 50, and patients with long-standing disease(> 10 years)showed less interest in work-disability.Patients in remission with mesalamine or no therapy showed less interest in all domains except for nutrition and long-term complications.CONCLUSION We demonstrate unmet patient information needs.Analysis of various patient profiles revealed associations with specific information topics, paving the way for building patient-tailored information resources. | Saleh Daher Tawfik Khoury Ariel Benson John R Walker Oded Hammerman Ron Kedem Timna Naftali Rami Eliakim Ofer Ben-Bassat Charles N Bernstein Eran Israeli | 2019 | World Journal of Gastroenterology2019,25,30: | 3 |

| 12 | Copper oxide nanoparticles-loaded zeolite and its characteristics and antibacterial activities显示文摘In the present work, a simple and green co-precipitation method was used to prepare copper oxide-zeolite nanocomposites(CuO-zeolite NCs). The weight ratio(1, 3, 5, 8 and 10 wt%) of CuO nanoparticles(NPs)loaded into zeolite was investigated to obtain the optimum CuO distribution for antibacterial activities.The prepared CuO-zeolite NCs were characterized by ultraviolet-visible(UV–vis) spectroscopy, Fourier transform infrared(FT-IR) spectroscopy, powder X-ray diffraction(XRD), and energy dispersive X-ray fluorescence spectrometry(EDXRF). The transmission electron microscopy(TEM) and field emission scanning electron microscopy(FE-SEM) revealed a uniform surface morphology of the CuO-zeolite NCs.The UV–vis spectrum of NCs showed absorption peaks between 230 and 280 nm for nano-CuO in the XRD patterns, and new peaks appeared between(36.56?–38.83?) related to the CuO. At weight ratio less than 10 wt%, the CuO nanoparticles loaded to the zeolite exhibited spherical shapes with average particle diameter of 6.5 nm measured by TEM and XRD. Antibacterial activities were tested against Gram-negative and Gram-positive bacteria. | 15 | Escherichia coli-host macrophage interactions in the pathogenesis of inflammatory bowel disease显示文摘Multiple studies have demonstrated alterations in the intestinal microbial community(termed the microbiome) in Crohn's disease(CD) and several lines of evidence suggest these changes may have a significant role in disease pathogenesis. In active and quiescent disease, both the faecal and mucosa-associated microbiome are discordant with matched controls with reduced biodiversity, changes in dominant organisms and increased temporal variation described. Mucosaassociated adherent, invasive Escherichia coli(E. coli)(AIEC), pro-inflammatory and resistant to killing by mucosal macrophages, appear to be particularly important. AIEC possess several virulence factors which may confer pathogenic potential in CD. Type-1 pili(FimH) allow adherence to intestinal cells via cell-surface carcinoembryonic antigen-related cell adhesion molecules and possession of long polar fimbrae promotes translocation across the intestinal mucosa via microfold(M)-cells of the follicle-associated epithelium. Resistance to stress genes(htrA, dsbA and hfq) and tolerance of an acidic pH may contribute to survival within the phagolysosomal environment.
