8Management of borderline and locally advanced pancreatic cancer:Where do we stand?显示文摘Many patients with pancreas cancer present with locally advanced pancreatic cancer(LAPC).The principle tools used for diagnosis and staging of LAPC include endoscopic ultrasound,axial imaging with computed tomography and magnetic resonance imaging,and diagnostic laparoscopy.The definition of resectability has historically been vague,as there is considerable debate and controversy as to the definition of LAPC.For the patient with LAPC,there is some level of involvement of the surrounding vascular structures,which include the superior mesenteric artery,celiac axis,hepatic artery,superior mesenteric vein,or portal vein.When feasible,most surgeons would recommend possible surgical resection for patients with borderline LAPC,with the goal of an R0 resection.For initially unresectable LAPC,neoadjuvant should be strongly considered.Specifically,these patients should be offered neoadjuvant therapy,and the tumor should be assessed for possible response and eventual resection.The efficacy of neoadjuvant therapy with this approach as a bridge to potential curative resection is broad,ranging from 3%-79%.The different modalities of neoadjuvant therapy include sin-gle or multi-agent chemotherapy combined with radiation,chemotherapy alone,and chemotherapy followed by chemotherapy with radiation.This review focuses on patients with LAPC and addresses recent advances and controversies in the field.Jin He Andrew J Page Matthew Weiss Christopher L Wolfgang Joseph M Herman Timothy M Pawlik 2014World Journal of Gastroenterology2014,20,9:11

10Stem Cell Ophthalmology Treatment Study (SCOTS): bone marrow-derived stem cells in the treatment of Leber's hereditary optic neuropathy显示文摘The Stem Cell Ophthalmology Treatment Study(SCOTS) is currently the largest-scale stem cell ophthalmology trial registered at Clinical Trials.gov(identifier: NCT01920867). SCOTS utilizes autologous bone marrow-derived stem cells(BMSCs) to treat optic nerve and retinal diseases. Treatment approaches include a combination of retrobulbar, subtenon, intravitreal, intra-optic nerve, subretinal, and intravenous injection of autologous BMSCs according to the nature of the disease, the degree of visual loss, and any risk factors related to the treatments. Patients with Leber's hereditary optic neuropathy had visual acuity gains on the Early Treatment Diabetic Retinopathy Study(ETDRS) of up to 35 letters and Snellen acuity improvements from hand motion to 20/200 and from counting fingers to 20/100. Visual field improvements were noted. Macular and optic nerve head nerve fiber layer typically thickened. No serious complications were seen. The increases in visual acuity obtained in our study were encouraging and suggest that the use of autologous BMSCs as provided in SCOTS for ophthalmologic mitochondrial diseases including Leber's hereditary optic neuropathy may be a viable treatment option.Jeffrey N. 11Modalities of Microfinance Delivery in Asia and Latin America: Lessons for China显示文摘Microfinance, the provision of small size loans and other financial services to low income households, is often seen as the key innovation of the last 25 years in terms of means of reaching out to the poor and vulnerable. There is extensive experience in microfinance provision in both Asia and Latin America, but as yet relatively little use of the approach in China. This paper assesses different approaches to microfinance delivery using a threefold distinction, the credit union approach, the non-government organization approach and the banking approach, to generalize across recent Asian and Latin American experience and discuss the role of microfinance in poverty reduction in a theoretical framework. 16Response to endoscopic therapy for biliary anastomotic strictures in deceased versus living donor liver transplantation显示文摘BACKGROUND:Endoscopic therapy has been successful in the management of biliary complications after both deceased donor liver transplantation(DDLT) and living donor liver transplantation(LDLT).LDLT is thought to be associated with higher rates of biliary complications,but there are few studies comparing the success of endoscopic management of anastomotic strictures between the two groups.This study aims to compare our experience in the endoscopic management of anastomotic strictures in DDLT versus LDLT.METHODS:This is a retrospective database review of all liver transplant patients undergoing endoscopic retrograde cholangiopancreatography(ERCP) after liver transplantation.The frequency of anastomotic stricture and the time to develop and to resolve anastomotic stricture were compared between DDLT and LDLT.The response of anastomotic stricture to endoscopic therapy was also analyzed.RESULTS:A total of 362 patients underwent liver transplantation between 2003 and 2011,with 125 requiring ERCP to manage biliary complications.Thirty-three(9.9%) cases of DDLT and 8(27.6%) of LDLT(P=0.01) were found to have anastomotic stricture.When comparing DDLT and LDLT,there was no difference in the mean time to the development of anastomotic strictures(98±17 vs 172±65 days,P=0.11),likelihood of response to ERCP [22(66.7%) vs 6(75.0%),P=0.69],mean time to the resolution of anastomotic strictures(268±77 vs 125±37 days,P=0.34),and the number of ERCPs required to achieve resolution(3.9±0.4 vs 4.7±0.9,P=0.38).CONCLUSIONS:Endoscopic therapy is effective in the majority of biliary complications relating to liver transplantation.Anastomotic strictures occur more frequently in LDLT compared with DDLT,with equivalent endoscopic treatment response and outcomes for both groups.Calvin HY Chan Fergal Donnellan Michael F Byrne Alan Coss Mazhar Haque Holly Wiesenger Charles H Scudamore Urs P Steinbrecher Alan A Weiss Eric M Yoshida 2013Hepatobiliary & Pancreatic Diseases International2013,12,5:7
17The Arabidopsis Cysteine-Rich GASA5 Is a Redox-Active Metalloprotein that Suppresses Gibberellin Responses显示文摘Dear Editor, The plant-specific GIBBERELLIN (GA)-STIMULATED TRANSCRIPT 1 (GAST1)-like genes have been identified in numerous plant species and many of them are regulated by the phytohormone GA.All encode small proteins with a conserved cysteine-rich C-terminus,containing 12 cysteines in conserved positions.The fact that the number and positions of all 12 cysteines have not changed during the course of evolution strongly implies fundamental and important biochemical and biological functions.Although many of these genes were characterized,and different biological functions were suggested,almost nothing is known about their mode of action.We previously showed that the petunia GAST1-like GA INDUCED 2 (GIP2) and its Arabidopsis homolog GA STIMULATED 4 (GASA4) promote typical GA responses and have redox activity (Wigoda et al.,2006;Rubinovich and Weiss,2010).Mutated GASA4,in which four conserved cysteines were replaced by alanines,lost its redox activity and the ability to promote GA responses,suggesting that the two functions are linked (Rubinovich and Weiss,2010).Lior Rubinovich Sharon Ruthstein David Weiss 2014Molecular Plant2014,7,1:6
18Effect of welding parameters on the heat-affected zone of AISI409 ferritic stainless steel显示文摘One of the main problems during the welding of ferritic stainless steels is severe grain growth within the heat-affected zone(HAZ).In the present study,the microstructural characteristics of tungsten inert gas(TIG) welded AISI409 ferritic stainless steel were investigated by electron backscattered diffraction(EBSD),and the effects of welding parameters on the grain size,local misorientation,and low-angle grain boundaries were studied.A 3-D finite element model(FEM) was developed to predict the effects of welding parameters on the holding time of the HAZ above the critical temperature of grain growth.It is found that the base metal is not fully recrystallized.During the welding,complete recrystallization is followed by severe grain growth.A decrease in the number of low-angle grain boundaries is observed within the HAZ.FEM results show that the final state of residual strains is caused by competition between welding plastic strains and their release by recrystallization.Still,the decisive factor for grain growth is heat input.Eslam Ranjbarnodeh Stefanie Hanke Sabine Weiss Alfons Fischer 2012International Journal of Minerals,Metallurgy and Materials2012,19,10:6
19Dysregulation of iron and copper homeostasis in nonalcoholic fatty liver显示文摘Elevated iron stores as indicated by hyperferritinemiawith normal or mildly elevated transferrin saturation a n d m o s t l y m i l d h e p a t i c i r o n d e p o s i t i o n a r e a characteristic finding in subjects with non-alcoholic fatty liver disease(NAFLD). Excess iron is observed in approximately one third of NAFLD patients and is commonly referred to as the 'dysmetabolic iron overload syndrome'. Clinical evidence suggests that elevated body iron stores aggravate the clinical course of NAFLD with regard to liver-related and extrahepatic disease complications which relates to the fact that excess iron catalyses the formation of toxic hydroxylradicals subsequently resulting in cellular damage. Iron removal improves insulin sensitivity, delays the onset of type 2 diabetes mellitus, improves pathologic liver function tests and likewise ameliorates NAFLD histology. Several mechanisms contribute to pathologic iron accumulation in NAFLD. These include impaired iron export from hepatocytes and mesenchymal Kupffer cells as a consequence of imbalances in the concentrations of iron regulatory factors, such as hepcidin, cytokines, copper or other dietary factors. 20All-dielectric silicon metalens for two-dimensional particle manipulation in optical tweezers显示文摘Dynamic control of compact chip-scale contactless manipulation of particles for bioscience applications remains a challenging endeavor,which is restrained by the balance between trapping efficiency and scalable apparatus.Metasurfaces offer the implementation of feasible optical tweezers on a planar platform for shaping the exerted optical force by a microscale-integrated device.Here we design and experimentally demonstrate a highly efficient silicon-based metalens for two-dimensional optical trapping in the near-infrared.Our metalens concept is based on the Pancharatnam–Berry phase,which enables the device for polarization-sensitive particle manipulation.Our optical trapping setup is capable of adjusting the position of both the metasurface lens and the particle chamber freely in three directions,which offers great freedom for optical trap adjustment and alignment.Two-dimensional(2D)particle manipulation is done with a relatively low-numerical-aperture metalens(NA(ML)=0.6).We experimentally demonstrate both 2D polarization-sensitive drag and drop manipulation of polystyrene particles suspended in water and transfer of angular orbital momentum to these particles with a single tailored beam.Our work may open new possibilities for lab-on-a-chip optical trapping for bioscience applications and microscale to nanoscale optical tweezers.TEANCHAI CHANTAKIT CHRISTIAN SCHLICKRIEDE BASUDEB SAIN FABIAN MEYER THOMAS WEISS NATTAPORN CHATTHAM AND THOMAS ZENTGRAF 2020Photonics Research2020,8,9:6
