1APACHE system is better than Ranson system in the prediction of severity of acute pancreatitis显示文摘BACKGROUND: It has been suggested that addition of obesity score to the APACHE-Ⅱ system can lead to more accurate prediction of severity of acute pancreatitis. However there is scanty information on the usefulness of the combined APACHE-O scoring system in Asian patients. This study aimed to compare the accuracy of Ranson, APACHE-Ⅱ and APACHE-O systems in assessing severity of acute pancreatitis in a local Chinese population. METHODS: One hundred and one consecutive patients with acute pancreatitis were prospectively studied. Body mass index (BMI) was measured on admission. Ranson score, APACHE-Ⅱ and APACHE-O scores were recorded on admission and at 48 hours. By adopting the cut-off levels and definitions advocated in the Atlanta consensus for severe disease, the diagnostic accuracy of the three scoring systems was compared by the area under the curve (AUC) under the receiver operator characteristic curve. RESULTS: Of the 101 patients, 12 (11.9%) patients suffered from severe pancreatitis. Obesity was uncommon and only two patients (2.0%) had BMI >30. Eighty-two (81.2%) patients were normal weight (BMI≤25) whereas 17 (16.8%) were overweight ( BMI 25-30 ). Overweight or obesity (BMI >25) was not associated with severe pancreatitis (P= 0.40). The AUC for admission scores of Ranson, APACHE-Ⅱ, and APACHE-O systems was 0. 549, 0. 904 and 0. 904, respectively. The AUC for 48-hour scores of Ranson, APACHE-Ⅱ and APACHE-O systems was 0.808, 0.955 and 0.951, respectively. CONCLUSIONS: The APACHE-Ⅱ scoring system is more accurate than the Ranson scoring system of the prediction of severity in acute pancreatitis. 2羊蹄甲属3种园艺树种分子鉴定及亲缘关系的ISSR分析显示文摘利用ISSRPCR方法对香港羊蹄甲属(Bauhinia)中最常见3种园艺树种:红花羊蹄甲(B.purpurea)、宫粉羊蹄甲(B.variegate)和洋紫荆(B.blakeana)的遗传变异进行了研究,从74个ISSR引物中筛选出9个多态性引物用于正式扩增,共扩增出80条DNA带,平均每个引物扩增的DNA带的数目为8.88条,多态性条带数目为55条,占总条带数的68.8%。其中,有8个引物扩增出差异条带和特异条带,并分别能准确地鉴定羊蹄甲属3个园艺树种。本研究的结果表明,ISSR-PCR的DNA电泳谱带在检测香港羊蹄甲(B.blakeana)品种的真实性方面非常有用。同时,通过对3个种扩增条带的叠加性进行分析,证实了洋紫荆杂交起源的父母本为红花羊蹄甲和宫粉羊蹄甲。罗瑜萍 龚维 邱英雄 Yuk Sing Gibert Chan 2006园艺学报2006,33,2:30
3Metamorphism of the northern Liaoning Complex:Implications for the tectonic evolution of Neoarchean basement of the Eastern Block,North China Craton显示文摘As one of the areas where typical late Archean crust is exposed in the Eastern Block of the North China Craton,the northern Laioning Complex consists principally of tonalitic-trondhjemitic-granodioritic(TTG) gneisses,massive granitoids and supracrustal rocks.The supracrustal rocks,named the Qingyuan Group, consist of interbedded amphibolite,hornblende granulite,biotite granulite and B1F.Petrological evidence indicates that the amphibolites experienced the early prograde(M1),peak(M2) and post-peak(M3) metamorphism.The early prograde assemblage(M1) is preserved as mineral inclusions,represented by actinotite + hornblende + plagioclase + epidote + quartz + sphene,within garnet porphyroblasts.The peak assemblage(M2) is indicated by garnet + clinopyroxene + hornblende + plagioclase + quartz + ilmenite, which occur as major mineral phases in the rock.The post-peak assemblage(M3) is characterized by the garnet + quartz symplectite.The P—T pseudosections in the NCFMASHTO system constructed by using THERMOCALC define the P-T conditions of Ml,M2 and M3 at 490-550℃/<4.5 kbar,780-810℃/7.65- 8.40 kbar and 630-670℃/8.15-9.40 kbar,respectively.As a result,an anticlockwise P-T path involving isobaric cooling is inferred for the metamorphic evolution of the amphibolites.Such a P-T path suggests that the late Archean metamorphism of the northern Liaoning Complex was related to the intrusion and underplating of mantle-derived magmas.The underplating of voluminous mantle-derived magmas leading to metamorphism with an anticlockwise P-T path involving isobaric cooling may have occurred in continental magmatic arc regions,above hot spots driven by mantle plumes,or in continental rift environments. 4Changes in Soil Organic Carbon Dynamics in a Native C4 Plant-Dominated Tidal Marsh Following Spartina alterniflora Invasion显示文摘Invasion of an exotic C_4 plant Spartina alterniflora has been shown to increase soil organic carbon(SOC) concentrations in native C_3 plant-dominated coastal wetlands of China. However, little is known about the effects of S. alterniflora invasion on SOC concentrations and fractions in tidal marshes dominated by native C_4 plants. In this study, a field experiment was conducted in a tidal marsh dominated by the native C_4 plant Cyperus malaccensis in the Minjiang River estuary, China. Concentrations of SOC and liable SOC fractions, dissolved organic carbon(DOC), microbial biomass carbon(MBC), and easily oxidizable carbon(EOC),were measured in the top 50-cm soils of the C. malaccensis community, as well as those of three S. alterniflora communities with an invasion duration of 0–4 years(SA-4), 4–8 years(SA-8), and 8–12 years(SA-12), respectively. Results showed that both SOC stocks in the 50-cm soils and mean SOC concentrations in the surface soils(0–10 cm) of the C. malaccensis community increased with the duration of S. alterniflora invasion, whereas SOC concentrations in the 10–50-cm soils decreased slightly during the initial period of S. alterniflora invasion, before increasing again. The pattern of changes in labile SOC fractions(DOC, MBC, and EOC) with invasion duration was generally similar to that of SOC, while the ratios of labile SOC fractions to total SOC(DOC:SOC, MBC:SOC, and EOC:SOC) decreased significantly with the duration of S. alterniflora invasion. 5Tough and tunable adhesion of hydrogels:experiments and models显示文摘As polymer networks infiltrated with water, hydrogels are major constituents of animal and plant bodies and have diverse engineering applications. While natural hydrogels can robustly adhere to other biological materials, such as bonding of tendons and cartilage on bones and adhesive plaques of mussels, it is challenging to achieve such tough adhesions between synthetic hydrogels and engineering materials. Recent experiments show that chemically anchoring long-chain polymer networks of tough synthetic hydrogels on solid surfaces create adhesions tougher than their natural counterparts, but the underlying mechanism has not been well understood. It is also challenging to tune systematically the adhesion of hydrogels on solids. Here, we provide a quantitative understanding of the mechanism for tough adhesions of hydrogels on solid materials via a combination of experiments, theory, and numerical simulations.Using a coupled cohesive-zone and Mullins-effect model validated by experiments, we reveal the interplays of intrinsic work of adhesion, interfacial strength, and energy dissipation in bulk hydrogels in order to achieve tough adhesions. 6Projections of ocean climate for northwestern Pacific Ocean显示文摘The long-term adjustment processes of atmosphere and ocean in response to gradually increased atmospheric CO2 concentration havebeen analyzed in 70 and 140 a integrations with NCAR fully-coupled climate system model (CSM). In these experiments the CO2concentration has been increased to double and quadruples the initial concentration, respectively. After 70 a, at the time of CO2doubling, the model predicts surface air temperature rises by 1.2 and 1.5 K for the globe and the northwestern Pacific Ocean,respectively. The behavior of the quadrupling run is similar: each global and regional mean surface air temperatures increase by 2.8 and 3.0K at the time of CO2 quadrupling. 7Simulation of Suspended Sediment in the Yellow and East China Seas显示文摘Described is an initial attempt to simulate the suspended sediment dynamics relating to tidal and wave forcing during summertime in June 1980 and August 1981 for the Yellow and East China Seas continental shelf. The cohesive/non-cohesive sediment resuspension and movement generated by the interaction between current and wave are modeled by use of ECOMSED and WAM Cycle 4. Model results are compared with observations in US-China Marine Sedimentation Dynamics Program performed for 1980-81 at off the Changjiang estuary. The main features of simulations show that suspended sediment concentrations during the summer decreased markedly offshore as observed during the simulation periods. As for some discrepancies for the mouth of the estuary with high river discharges, i.e., the Changjiang River, the model did not properly reproduce the over-mixing situation in the summer; thus distinct vertical concentration variation in this local region is not agreeable with observation. However, general dispersal patterns of suspended sediment movement seem to be agreeably reproduced for the nearshore shallow region. 8Incarceration of a colonoscope in an inguinal hernia: Case report and literature review显示文摘Incarceration of an endoscope in an inguinal hernia may occur during the course of routine colonoscopy. The incarceration may occur on insertion or withdrawal and frequently the hernia is not suspected prior to the colonoscopy. Most commonly, a left sided inguinal hernia is involved, however right inguinal hernias may be implicated in subjects with altered anatomy post abdominal surgery. Incarceration of an endoscope in an inguinal hernia has been seldom reported in the literature which is likely to be related to under reporting. A range of techniques have been suggested by various authors over the last four decades to manage this unusual complication of colonoscopy. These techniques include utilizing fluoroscopy, manual external pressure and/or the fitting of a cap onto the tip of the colonoscope to facilitate colonoscopic navigation. The authors present a case report of incarceration of the colonoscope on withdrawal in an unsuspected left inguinal hernia with a review of the literature on the management of this colonoscopic complication. 9地球倾角驱动的末次冰期期间南亚地区变暖显示文摘当前对晚第四纪以来轨道尺度上地球气候变化的认识,主要包括高纬驱动的以~10万年周期为主的冰期-间冰期循环和夏季风影响的以~2.3万年周期为主的低纬水文变化.但是,由于低纬陆地没有较为可靠的温度记录,使得对低纬陆地的长期气候变化仍然缺乏全面的理解.本文通过获取云南腾冲青海的一根湖泊沉积岩芯,分析了沉积物中的支链甘油二烷基甘油四醚酯(brGDGTs)和叶蜡氢同位素,分别重建了~9万年以来南亚地区的年平均温度和季风降水的变化序列.结果表明,夏季风降水与之前的研究一致,表现出明显的2.3万年周期,但是年均温度记录揭示了8.8~7.1万年和4.5~2.2万年存在两个暖期,温度变幅2~3℃,与该记录中的冰期-间冰期变幅接近.通过对比TraCE21ka瞬态模拟结果和相关气候驱动因子,发现这两个暖期很可能受当地的年均太阳轨道辐射影响;而低纬地区的年均太阳辐射由以~4.1万年周期为主的地球倾角变化控制,并与高纬地区的年均太阳轨道辐射呈反向关系.此外,该记录中的两个暖期与古人类活动的年代有较强的相关性,表明末次冰期内温度变化可能对亚洲地区古人类的生存有重要的影响.赵成 Eelco J.Rohling Zhengyu Liu 杨小强 张恩楼 程军 Zhonghui Liu 安芷生 羊向东 冯小平 孙晓双 张灿 闫天龙 隆浩 晏宏 Zicheng Yuk 刘卫国 于世永 沈吉 2021Science Bulletin2021,66,11:5
10Wave-Tide-Surge Coupled Simulation for Typhoon Maemi显示文摘The main task of this study focuses on studying the effect of wave-current interaction on currents, storm surge and wind wave as well as effects of current induced wave refraction and current on waves by using numerical models which consider the bottom boundary layer and sea surface roughness parameter for shallow and smooth bed area around Korean Peninsula. The coupled system (unstructured-mesh SWAN wave and ADCIRC) run on the same unstructured mesh. This identical and homogeneous mesh allows the physics of wave-circulation interactions to be correctly resolved in both models. The unstructured mesh can be applied to a large domain allowing all energy from deep to shallow waters to be seamlessly followed. There is no nesting or overlapping of structured wave meshes, and no interpolation is required. In response to typhoon Maemi (2003), all model components were validated independently, and shown to provide a faithful representation of the system's response to this storm. The waves and storm surge were allowed to develop on the continental shelf and interact with the complex nearshore environment. The resulting modeling system can be used extensively for prediction of the typhoon surge. The result show that it is important to incorporate the wave-current interaction effect into coastal area in the wave-tide-surge coupled model. At the same time, it should consider effects of depth-induced wave breaking, wind field, currents and sea surface elevation in prediction of waves. Specially, we found that: (1) wave radiation stress enhanced the current and surge elevation otherwise wave enhanced nonlinear bottom boundary layer decreased that, (2) wind wave was significantly controlled by sea surface roughness thus we cautiously took the experimental expression. 11Risk factors and natural history of breast cancer in younger Chinese women显示文摘AIM: To investigate the age differences in the risk factors, clinicopathological characteristics and patterns of treatment of female breast cancer patients. METHODS: Seven thousand one hundred and fiftytwo women with primary breast cancer from the Hong Kong Breast Cancer Registry were recruited after receiving patients' consent, they were asked to complete standardized questionnaires which captured their sociodemographic characteristics and risk factors associated with breast cancer development. Among them, clinicopathological data and patterns of treatment were further collected from medical records of 5523 patients with invasive breast cancers. Patients were divided into two groups according to the age at diagnosis: younger(< 40 years old) vs older patients(≥ 40 years old) for subsequent analyses.RESULTS: Analysis on the sociodemographic characteristics and exposure to risk factors were performed on 7152 women with primary breast cancer and the results revealed that younger patients were more likely to have unhealthy lifestyles; these include a lack of exercise(85.4% vs 73.2%, P < 0.001), having high stress in life(46.1% vs 35.5%, P < 0.001), having dairy/meat-rich diets(20.2% vs 12.9%, P < 0.001),having alcohol drinking habit(7.7% vs 5.2%, P = 0.002). Younger patients were also more likely to have hormone-related risk factors including nulliparity(43.3% vs 17.8%, P < 0.001) and an early age at menarche(20.7% vs 13.2%, P < 0.001). Analyses on clinicopathological characteristics and patterns of treatment were performed on 5523 women diagnosed with invasive breast cancer. 14Simulation of Storm Surge and Wave Due to Typhoon Isewan(5915)显示文摘An integrally coupled wave-tide-surge model was developed and then applied to the simulation of the wave-typhoon surge for the typhoon Isewan(typhoon Vera(5915)), which is the strongest typhoon that has struck Japan and caused incalculable damage. An integrally coupled tide-surge-wave model using identical and homogeneous meshes in an unstructured grid system was used to correctly resolve the physics of wave-circulation interaction in both models. All model components were validated independently. The storm surge and wave properties such as the surge height, the significant wave height, wave period and direction were reproduced reasonably under the meteorological forcing, which was reprocessed to be close to the observations. 16Cystic meconium peritonitis with jejunoileal atresia:Is it associated with unfavorable outcome?显示文摘AIM To compare the outcome between patients with jejunoileal atresia(JIA) associated with cystic meconium peritonitis(CMP) and patients with isolated JIA(JIA without CMP).METHODS A retrospective study was conducted for all neonates with JIA operated in our institute from January 2005 to January 2016.Demographics including the gestation age,sex,birth weight,age at operation,the presence of associated syndrome was recorded.Clinical outcome including the type of operation performed,operative time,the need for reoperation and mortality were studied.The demographics and the outcome between the 2 groups were compared.RESULTS During the study period,53 neonates had JIA underwent operation in our institute.Seventeen neonates(32%) were associated with CMP.There was no statistical difference on the demographics in the two groups.Patients with CMP had earlier operation than patients with isolated JIA(mean 1.4 d vs 3 d,P = 0.038).Primaryanastomosis was performed in 16 patients(94%) with CMP and 30 patients(83%) with isolated JIA(P = 0.269).Patients with CMP had longer operation(mean 190 min vs 154 min,P = 0.004).There were no statistical difference the need for reoperation(3 vs 6,P = 0.606) and mortality(2 vs 1,P = 0.269) between the two groups.CONCLUSION Primary intestinal anastomosis can be performed in94% of patients with JIA associated with CMP.Although patients with CMP had longer operative time,the mortality and reoperation rates were low and were comparable to patients with isolated JIA.Kin Wai Edwin Chan Kim Hung Lee Hei Yi Vicky Wong Siu Yan Bess Tsui Yuen Shan Wong Kit Yi Kristine Pang Jennifer Wai Cheung Mou Yuk Him Tam 2017World Journal of Clinical Pediatrics2017,6,1:3

19Colonoscopy in Hong Kong Chinese children显示文摘AIM:To investigate the safety and diagnostic yield of colonoscopy in Chinese children in whom the procedure is not often done.METHODS:We conducted a retrospective review of all colonoscopies in consecutive children who underwent their fi rst diagnostic colonoscopy from Jan 2003 to 2008.RESULTS:Seventy-nine children (48 boys,31 girls;mean age 9.2 ± 4.2 years) were identified and reviewed with a total of 82 colonoscopies performed.Successful caecal and ileal intubation rates were 97.6% and 75.6% respectively.Forty patients (50.6%) had a positive diagnosis made in colonoscopy and that included colonic polyps (23),Crohn's disease (12),ulcerative colitis (1),and miscellaneous causes (4).80% of polyps were in the rectosigmoid colon.All but one were juvenile hamartomatous polyps.The exceptionwas an adenomatous polyp.The mean ages for children with inflammatory bowel disease (IBD) and polyps were 11.3 and 4.3 years respectively.There was no procedure-related complication.CONCLUSION:Colonoscopy is a safe procedure in our Chinese children.The increasing diagnosis of IBD in recent decades may reflect a rising incidence of the disease in our children.Yuk Him Tam Kim Hung Lee Kin Wai Chan Jennifer Dart Yin Sihoe Sing Tak Cheung Jennifer Wai Cheung Mou 2010World Journal of Gastroenterology2010,16,9:3
