1Methods of computed tomography screening and management of lung cancer in Tianjin: design of a population-based cohort study显示文摘Objective: European lung cancer screening studies using computed tomography(CT) have shown that a management protocol based on measuring lung nodule volume and volume doubling time(VDT) is more specific for early lung cancer detection than a diameter-based protocol. However, whether this also applies to a Chinese population is unclear. The aim of this study is to compare the diagnostic performance of a volume-based protocol with a diameter-based protocol for lung cancer detection and optimize the nodule management criteria for a Chinese population.Methods: This study has a population-based, prospective cohort design and includes 4000 participants from the Hexi district of Tianjin, China. Participants will undergo low-dose chest CT at baseline and after 1 year. Initially, detected lung nodules will be evaluated for diameter and managed according to a routine diameter-based protocol(Clinical Practice Guideline in Oncology for Lung Cancer Screening, Version 2.2018). Subsequently, lung nodules will be evaluated for volume and management will be simulated according to a volume-based protocol and VDT(a European lung nodule management protocol). Participants will be followed up for 4 years to evaluate lung cancer incidence and mortality. 2Clinical characteristics and work-up of small to intermediate-sized pulmonary nodules in a Chinese dedicated cancer hospital显示文摘Objectives:To evaluate the characteristics and work-up of small to intermediate-sized pulmonary nodules in a Chinese dedicated cancer hospital.Methods:Patients with pulmonary nodules 4–25 mm in diameter detected via computed tomography(CT)in 2013 were consecutively included.The analysis was restricted to patients with a histological nodule diagnosis or a 2-year follow-up period without nodule growth confirming benign disease.Patient information was collected from hospital records.Results:Among the 314 nodules examined in 299 patients,212(67.5%)nodules in 206(68.9%)patients were malignant.Compared to benign nodules,malignant nodules were larger(18.0 mm vs.12.5 mm,P<0.001),more often partly solid(16.0%vs.4.7%,P<0.001)and more often spiculated(72.2%vs.41.2%,P<0.001),with higher density in contrast-enhanced CT(67.0 HU vs.57.5 HU,P=0.015).Final diagnosis was based on surgery in 232 out of 314(73.9%)nodules,166 of which were identified as malignant[30(18.1%)stage III or IV]and 66 as benign.In 36 nodules(11.5%),diagnosis was confirmed by biopsy and the remainder verified based on stability of nodule size at follow-up imaging(n=46,14.6%).Among 65 nodules subjected to gene(EGFR)mutation analyses,28(43.1%)cases(EGFR19 n=13;EGFR21 n=15)were identified as EGFR mutant and 37(56.9%)as EGFR wild-type.Prior to surgery,the majority of patients[n=194(83.6%)]received a contrast-enhanced CT scan for staging of both malignant[n=140(84.3%)]and benign[n=54(81.8%)]nodules.Usage of positron emission tomography(PET)-CT was relatively uncommon[n=38(16.4%)].Conclusions:CT-derived nodule assessment assists in diagnosis of small to intermediate-sized malignant pulmonary nodules.Currently,contrast-enhanced CT is commonly used as the sole diagnostic confirmation technique for pre-surgical staging,often resulting in surgery for late-stage disease and unnecessary surgery in cases of benign nodules.Xiaonan Cui Daiwei Han Marjolein AHeuvelmans Yihui Du Yingru Zhao Lei Zhang Harry JMGroen Geertruida Hde Bock Monique DDorrius Matthijs Oudkerk Rozemarijn Vliegenthart Zhaoxiang Ye 2020Cancer Biology & Medicine2020,17,1:5
3Genesis of the Gold Deposit in the Indus-Yarlung Tsangpo Suture Zone,Southern Tibet:Evidence from Geological and Geochemical Data显示文摘The Nianzha gold deposit,located in the central section of the Indus-Yarlung Tsangpo suture(IYS) zone in southern Tibet,is a large gold deposit(Au reserves of 25 tons with average grade of 3.08 g/t) controlled by a E-W striking fault that developed during the main stage of Indo-Asian collision(~65-41 Ma).The main orebody is 1760 m long and 5.15 m thick,and occurs in a fracture zone bordered by Cretaceous diorite in the hanging wall to the north and the Renbu tectonic melange in the footwall to the south.High-grade mineralization occurs in a fracture zone between diorite and ultramafic rock in the Renbu tectonic melange.The wall-rock alteration is characterized by silicification in the fracture zone,serpentinization and the formation of talc and magnesite in the ultramafic unit,and chloritization and the formation of epidote and calcite in diorite.Quartz veins associated with Au mineralization can be divided into three stages.Fluid inclusion data indicate that the deposit formed from H_2O-NaCl-organic gas fluids that homogenize at temperatures of 203℃-347℃ and have salinities of 0.35wt%-17.17wt%NaCl equivalent.The quartz veins yield δ^(18)O_(fluid) values of 0.15‰-10.45‰,low δD_(V-SMOW)values(-173‰ to-96‰),and the δ^(13)C values of-17.6‰ to-4.7‰,indicating the ore-forming fluids were a mix of metamorphic and sedimentary orogenic fluids with the addition of some meteoric and mantle-derived fluids.The pyrite within the diorite has δ^(34)S_(V-CDT) values of-2.9‰-1.9‰(average-1.1‰),^(206)Pb/^(204)Pb values of 18.47-18.64,^(207)Pb/^(204)Pb values of 15.64-15.74,and ^(208)Pb/^(204)Pb values of 38.71-39.27,all of which are indicative of the derivation of S and other ore-forming elements from deep in the mantle.The presence of the Nianzha,Bangbu,and Mayum gold deposits within the IYS zone indicates that this area is highly prospective for large orogenic gold deposits.We identified three types of mineralization within the IYS,namely Bangbu-type accretionary,Mayum-type microcontinent,and Nianzha-type ophiolite-associated orogenic Au deposits.The three types formed at different depths in an accretionary orogenic tectonic setting.The Bangbu type was formed at the deepest level and the Nianzha type at the shallowest.ZHANG Xiong DENG Xueguo YANG Zhusen HOU Zengqian ZHENG Yuanchuan LIU Yingchao ZHAO Xiaoyan XU Bo PEI Yingru ZHOU Jinsheng ZHAO Miao YUAN Jianfei 2017Acta Geologica Sinica(English Edition)2017,91,3:4

11Maximum power and corresponding efficiency of an irreversible blue heat engine for harnessing waste heat and salinity gradient energy显示文摘In this study,a novel irreversible cyclic model of a capacitive mixing blue heat engine mainly consisting of super capacitors,charging and discharging circuits,a heat source,as well as two water sources with given salt concentrations is established for harvesting salinity gradient energy and waste heat.Additionally,the effects of the charging voltage and ratio of the minimum to maximum surface electric charge density on the thermodynamic efficiency and power output of the cycle are discussed.The maximum power output of the cycle is calculated.The optimized ranges of efficiency and power output as well as the temperatures of two isothermal processes are determined.It is established that during the isoelectric quantity process,there is not only an increase in thermal voltage owing to the temperature difference,but also an increase in concentration voltage owing to the salinity gradient.Consequently,the blue heat engine can obtain higher energy conversion efficiency than a conventional heat engine.When the temperature ratio of the heat source to the heat sink is 1.233,the maximum efficiency can reach approximately36%.The results obtained can promote the application of capacitive mixing technology in real life,reducing the consumption of fossil fuels.LIN Jian XIE Shan JIANG ChenXing SUN YiFei CHEN JinCan ZHAO YingRu 2022Science China(Technological Sciences)2022,65,3:1

13Effect of Detail Nursing Intervention on Headache and Negative Emotion in Patients with Subarachnoid Hemorrhage显示文摘objective: to explore the effect of detail nursing intervention on headache and negative emotion in patients with subarachnoid hemorrhage. Methods: the clinical data of 100 cases of subarachnoid hemorrhage from January 2015 to January 2017 of neurosurgery in our hospital were randomly divided into the control group and the experimental group, Detail nursing intervention in experimental group, Comparison of two groups of clinical nursing effects. Results the duration of hospitalization and headache in the experimental group were shorter than those in the control group. Fewer headaches than control group (P<0.05) Less headache than control group (p<0.05).
