2中医药辨证治疗急性加重期慢性阻塞性肺疾病随机对照试验疗效评价指标的系统评价显示文摘目的:在系统评价中医药辨证治疗急性加重期慢性阻塞性肺疾病(AECOPD)随机对照试验临床疗效基础上探讨其疗效评价指标的选择和使用。方法:计算机检索CBM、CNKI、VIP、万方数据库、Cochrane图书馆、PubMed和Embase数据库,收集公开发表中医药辨证治疗AECOPD随机对照试验的临床研究,由两位研究者按照纳入标准独立筛选文献、提取资料后,应用RevMan5.1软件进行Meta分析。结果:共纳入73篇相关文献,Meta分析结果显示中医辨证治疗能提高AECOPD患者的临床疗效和中医证候疗效,改善临床症状、肺功能,提高氧分压,降低二氧化碳分压,降低白细胞计数和CRP水平;疗效评价指标使用较多的有临床疗效、证候疗效、肺功能、血气分析、血常规、C反应蛋白、临床症状体征、中医证候积分等,其余如生活质量、影像学评价、住院时间和6 min步行距离等则相对较少。结论:中医药辨证治疗AECOPD具有较好的临床疗效,但由于纳入研究的质量不高,仍需开展大样本、多中心、方法科学的高质量临床试验;临床疗效评价指标的选用仍不规范,疗效指标的选择应根据临床研究的目的不同而有所侧重,另外还应体现中医药的临床特点。张海龙 王明航 李风雷 王海峰 余学庆 李建生 2013中医学报2013,28,6:53

5Rho/ROCK pathway and neural regeneration: a potential therapeutic target for central nervous system and optic nerve damage显示文摘· Rho-associated kinase (ROCK) is a serine/threonine kinase and one of the major downstream effectors of the small GTPase RhoA. The Rho/ROCK pathway is closely related to the pathogenesis of several central nervous system (CNS) disorders, and involved in many aspects of neuronal functions including neurite outgrowth and retraction. In the adult CNS, the damaged neuron regeneration is very difficult due to the presence of myelin-associated axon growth inhibitors such as Nogo, myelin-associated glycoprotein (MAG) and oligodendrocyte-myelin glycoprotein (Omgp), etc. The effects of these axon growth inhibitors are reversed by blocking the Rho/ROCK pathway in vitro, and the inhibition of Rho/ROCK pathway can promote axon regeneration and functional recovery in the injured CNS in vivo. In addition, the therapeutic effects of the Rho/ROCK inhibitors have also been demonstrated in some animal models and the Rho/ROCK pathway becomes an attractive target for the development of drugs for treating CNS disorders. 6Experimental study on the mechanical properties of rocks at high temperature显示文摘The mechanical properties of marble, limestone, and sandstone as well as the stress-strain curve, the varying characteristics of the peak strength, the peak strain and elastic modulus were studied by using the MTS810 Rock Mechanics Servo-controlled Testing System under the action of temperatures rang- ing from room temperature to 800℃ . Results show that (1) the peak strength and elastic modulus of marble fluctuate at the temperature from normal to 400 ℃ ; and they decrease gradually over 400℃ . (2) With the rise of the temperature, the peak strength and elastic modulus of limestone show downward trend from normal temperature to 200 ℃ ; have little change from 200 ℃ to 600 ℃; and decrease sharply over 600 ℃ . (3) The peak strength of sandstone shows a downward trend while a little change for elastic modulus at normal temperature to 200 ℃ ; and from 200 ℃ to 600 ℃ , the peak strength of sandstone in- creases while a little change for elastic modulus; the peak strength and elastic modulus decrease rap- idly at the temperature over 600 ℃ . (4) The peak strain of limestone shows little change at normal tem- perature to 600 ℃ , however, the peak strain increases rapidly over 600 ℃; and for marble and sandstone, the peak strain decreases with the rise of the temperature from normal temperature to 200℃ , the peak strain increases rapidly over 200℃. 7Development and distribution of geohazards triggered by the 5.12 Wenchuan Earthquake in China显示文摘As the Wenchuan Earthquake was of high magnitude and shallow seismic focus, it caused great dam- age and serious geohazards. By the field investigation and the interpretation of remote-sensing infor- mation after the earthquake and by using means of GIS technology, the distribution of geohazards triggered by the earthquake are analyzed and the conclusions are as follows: (1) The earthquake geo- hazards showed the feature of zonal distribution along the earthquake fault zone and linear distribution along the rivers; (2) the distribution of earthquake geohazards had a marked hanging wall effect, for the development density of geohazards in the hanging wall of earthquake fault zone was obviously higher than that in the foot wall and the width of strong development zone in the hanging wall was about 10 km; (3) the topographical slope was a main factor which controlled the development of earthquake geo- hazards and a vast majority of hazards were distributed on the slopes of 20° to 50°; (4) the earthquake geohazards had a corresponding relationship with the elevation and micro-landform, for most hazards happened in the river valleys and canyon sections below the elevation of 1500 to 2000 m, particularly in the upper segment of canyon sections (namely, the turning point from the dale to the canyon). Thin ridge, isolated or full-face space mountains were most sensitive to the seismic wave, and had a striking amplifying effect. 8Nonalcoholic fatty liver disease as a multi-systemic disease显示文摘Nonalcoholic fatty liver disease(NAFLD) is the most common cause of chronic liver disease. NAFLD includes a wide spectrum of liver conditions ranging from simple steatosis to nonalcoholic steatohepatitis and advanced hepatic fibrosis. NAFLD has been recognized as a hepatic manifestation of metabolic syndrome linked with insulin resistance. NAFLD should be considered not only a liver specific disease but also an early mediator of systemic diseases. Therefore, NAFLD is usually associated with cardiovascular disease, chronic kidney disease, type 2 diabetes, obesity, and dyslipidemia. NAFLD is highly prevalent in the general population and is associated with increased cardiovascular morbidity and mortality. The underlying mechanisms and pathogenesis of NAFLD with regard to other medical disorders are not yet fully understood. 10Microcalorimetric investigation of the effect of berberine alkaloids from Coptis chinensis Franch on Staphylococcus aureus growth显示文摘The inhibitory effects of three berberine alkaloids (BAs) from rhizome of Coptis chinensis Franch, a traditional Chinese medicinal (TCM) herb, on Staphylococcus aureus growth were investigated by mi- crocalorimetry. The power–time curves of S. aureus with and without BAs were acquired; meanwhile the extent and duration of inhibitory effects on the metabolism were evaluated by studying the growth rate constant (k), half inhibitory ratio (IC50), maximum heat-output power (Pmax), peak time of maximum heat-output power (tp) and total heat production (Qt). The value of k of S. aureus in the presence of the three BAs decreased with the increasing concentrations of BAs. Moreover, Pmax was reduced and the value of tp increased with increasing concentrations of the three drugs. The inhibitory activity varied with different drugs. The values of IC50 of the three BAs are respectively, 101.4 μg/mL for berberine, 241.0 μg/mL for palmatine and 792.3 μg/mL for jateorrhizine. The sequence of antimicrobial activity of the three BAs is: berberine > palmatine > jateorrhizine. 12Temperature effect and thermal impact in lithium-ion batteries: A review显示文摘Lithium-ion batteries, with high energy density(up to 705 Wh/L) and power density(up to 10,000 W/L), exhibit high capacity and great working performance. As rechargeable batteries, lithium-ion batteries serve as power sources in various application systems. Temperature, as a critical factor, significantly impacts on the performance of lithium-ion batteries and also limits the application of lithium-ion batteries. Moreover, different temperature conditions result in different adverse effects. Accurate measurement of temperature inside lithiumion batteries and understanding the temperature effects are important for the proper battery management. 13Honey:its medicinal property and antibacterial activity显示文摘Indeed,medicinal importance of honey has been documented in the world's oldest medical literatures,and since the ancient times,it has been known to possess antimicrobial property as well as wound-healing activity.The healing property of honey is due to the fact that it offers antibacterial activity,maintains a moist wound condition,and its high viscosity helps to provide a protective barrier to prevent infection.Its immunomodulatory property is relevant to wound repair too.The antimicrobial activity in most honeys is due to the enzymatic production of hydrogen peroxide.However,another kind of honey,called non-peroxide honey(viz.,manuka honey),displays significant antibacterial effects even when the hydrogen peroxide activity is blocked.Its mechanism may be related to the low pH level of honey and its high sugar content(high osmolality) that is enough to hinder the growth of microbes.The medical grade honeys have potent in vitro bactericidal activity against antibiotic-resistant bacteria causing several life-threatening infections to humans. 14Clinical effects and complications of TIPS for portal hypertension due to cirrhosis:A single center显示文摘AIM:To determine the clinical effects and complications of transjugular intrahepatic portosystemic shunt(TIPS)for portal hypertension due to cirrhosis.METHODS:Two hundred and eighty patients with portal hypertension due to cirrhosis who underwent TIPS were retrospectively evaluated.Portal trunk pressure was measured before and after surgery.The changes in hemodynamics and the condition of the stent were assessed by ultrasound and the esophageal and fundic veins observed endoscopically.RESULTS:The success rate of TIPS was 99.3%.The portal trunk pressure was 26.8±3.6 cmH2O after surgery and 46.5±3.4 cmH2O before surgery(P<0.01).The velocity of blood flow in the portal vein increased.The internal diameters of the portal and splenic veins were reduced.The short-term hemostasis rate was100%.Esophageal varices disappeared completely in68%of patients and were obviously reduced in 32%.Varices of the stomach fundus disappeared completely in 80%and were obviously reduced in 20%of patients.Ascites disappeared in 62%,were markedly reduced in 24%,but were still apparent in 14%of patients.The total effective rate of ascites reduction was 86%.Hydrothorax completely disappeared in 100%of patients.The incidence of post-operative stent stenosis was 24%at 12 mo and 34%at 24 mo.The incidence of post-operative hepatic encephalopathy was 12%at3 mo,17%at 6 mo and 19%at 12 mo.The incidence of post-operative recurrent hemorrhage was 9%at 12mo,19%at 24 mo and 35%at 36 mo.The cumulative survival rate was 86%at 12 mo,81%at 24 mo,75%at 36 mo,57%at 48 mo and 45%at 60 mo.CONCLUSION:TIPS can effectively lower portal hypertension due to cirrhosis.It is significantly effective for hemorrhage of the digestive tract due to rupture of esophageal and fundic veins and for ascites and hydrothorax caused by portal hypertension.Jian-Ping Qin Ming-De Jiang Wen Tang Xiao-Ling Wu Xin Yao Wei-Zheng Zeng Hui Xu Qian-Wen He Ming Gu 2013World Journal of Gastroenterology2013,19,44:25
15Investigation of changes in properties of water under the action of a magnetic field显示文摘The properties of water and their changes under the action of a magnetic field were gathered by the spectrum techniques of infrared, Raman, visible, ultraviolet and X-ray lights, which may give an insight into molecular and atomic structures of water. It was found that some properties of water were changed, and a lot of new and strange phenomena were discovered after magnetization. Magnetized water really has magnetism, which has been verified by a peak shift of X-ray diffraction of magnetized water + Fe3O4 hybrid relative to that of pure water + Fe3O4 hybrid, that is a saturation and memory effect. The properties of infrared and ultraviolet absorp-tions, Raman scattering and X-ray diffraction of magnetized water were greatly changed relative to those of pure water; their strengths of peaks were all increased, the frequencies of some peaks did also shift, and some new peaks, for example, at 5198, 8050 and 9340 cm?1, occurred at 25℃ after water was magnetized. In the meanwhile, the magnetized effects of water are related to the magnetized time, the intensity of an externally applied magnetic field, and the temperature of water, but they are not a linear relationship. The study also showed a lot of new and unusual properties of magnetized water, for example, the six peaks in 3000-3800 cm?1 in infrared absorption, the exponential increase of ultraviolet absorption of wave with the decreasing wavelength of light of 200-300 nm, the frequency-shifts of peaks, a strange irreversible effect in the increasing and decreasing processes, as well as a stronger peak of absorption occurring at 50℃, 70℃ and 80℃, the existence of many models of motion from 85℃ to 95℃ in 8000-10000 cm?1, and so on. These results show that the molecular structure of water is very complicated, which needs further study. Furthermore, the macroscopic feature of mechanics, for instance, surface tension force of magnetized water, was also measured. Experiments dis-covered that the size in contact angles of magnetized water on the surface of hy-drophobic materials decreases, thus the surface tension force of magnetized water decreases relative to that of pure water. It is seen from the above results that the clustering structure of hydrogen-bonded chains and polarization effects of water molecules are enhanced after magnetization. 16The distribution rule and seepage effect of the fractures in the ultra-low permeability sandstone reservoir in east Gansu Province,Ordos Basin显示文摘To study the impact of the fractures on development in the ultra-low permeability sandstone reservoir of the Yangchang Formation of the Upper Triassic in the Ordos Basin,data on outcrops,cores,slices,well logging and experiments are utilized to analyze the cause of the formation of the fractures,their distribution rules and the control factors and discuss the seepage flow effect of the fractures. In the studied area developed chiefly high-angle tectonic fractures and horizontal bedding fractures,inter-granular fractures and grain boundary fractures as well. Grain boundary fractures and intragranular fractures serve as vital channels linking intragranular pores and intergranular solution pores in the reservoir matrix,thus providing a good connectivity between the pores in the ultra-low perme-ability sandstone reservoir. The formation of fractures and their distribution are influenced by such external and internal factors as the palaeo-tectonic stress field,the reservoir lithological character,the thickness of the rock layer and the anisotropy of a layer. The present-day stress field influences the preservative state of fractures and their seepage flow effect. Under the tec-tonic effect of both the Yanshan and Himalayan periods,in this region four sets of fractures are distributed,respectively assuming the NE-SW,NW-SE,nearly E-W and nearly S-N orientations,but,due to the effect of the rock anisotropy of the rock formation,in some part of it two groups of nearly orthogonal fractures are chiefly distributed. Under the effect of the present-day stress field,the fractures that assume the NE-SW direction have a good connectivity,big apertures,a high permeability and a minimum starting pressure,all of which are main advantages of the seepage fractures in this region. 17Balloon-occluded transcatheter arterial chemoembolization for hepatocellular carcinoma显示文摘Transcatheter arterial chemoembolization(TACE) is widely accepted as a treatment for patients with hepatocellular carcinoma(HCC) in the intermediate stage according to the Barcelona Clinic Liver Cancer(BCLC) guidelines. Recently, balloon-occluded TACE(B-TACE) was developed in Japan. Despite the lack of a clear definition, B-TACE is generally defined as the infusion of emulsion of chemotherapeutic agents with lipiodol followed by gelatin particles under the occlusion of feeding arteries by a microballoon catheter, which leads to the dense lipiodol emulsion(LE) accumulation in HCC nodules. This phenomenon cannot be explained only by the prevention of proximal migration and leakage of embolization materials; it further involves causing local changes in the hemodynamics of the surrounding occlusion artery and targeted HCC nodules. Balloon-occluded arterial stump pressure plays an important role in the dense LE accumulation in targeted HCC nodules. Although randomized controlled trials comparing the therapeutic effect and the prognosis of B-TACE to those of the other TACE procedures, such as conventional-TACE and drug-eluting beads TACE, are still lacking, B-TACE is thought to be a promising treatment. 18Cell–cell contact with proinflammatory macrophages enhances the immunotherapeutic effect of mesenchymal stem cells in two abortion models显示文摘Mesenchymal stem cells(MSCs),which are pluripotent cells with immunomodulatory properties,have been considered good candidates for the therapy of several immune disorders,such as inflammatory bowel diseases,concanavalin A-induced liver injury,and graft-versus-host disease.The embryo is a natural allograft to the maternal immune system.A successful pregnancy depends on the timely extinction of the inflammatory response induced by embryo implantation,followed by the switch to a tolerant immune microenvironment in both the uterus and the system.Excessive infiltration of immune cells and serious inflammatory responses are triggers for embryo rejection,which results in miscarriage.Here,we demonstrated that adoptive transfer of MSCs could prevent fetal loss in a lipopolysaccharide(LPS)-induced abortion model and immune response-mediated spontaneous abortion model.The immunosuppressive MSCs alleviated excessive inflammation by inhibiting CD4+T cell proliferation and promoting the decidual macrophage switch to M2 in a tumor necrosis factor-stimulated gene-6(TSG-6)-dependent manner.Cell-tocell contact with proinflammatory macrophages increased the TSG-6 production by the MSCs,thereby enhancing the suppressive regulation of T cells and macrophages.Moreover,proinflammatory macrophages in contact with the MSCs upregulated the expression of CD200 on the stem cells and facilitated the reprogramming of macrophages towards an anti-inflammatory skew through the interaction of CD200 with CD200R on proinflammatory macrophages.Therefore,the results demonstrate that a TSG-6-mediated paracrine effect,reinforced by cell-to-cell contact between MSCs and proinflammatory macrophages,is involved in the mechanism of MSC-mediated abortion relief through the induction of immune tolerance.Our study also indicates the potential application of MSCs in clinical recurrent miscarriages.Yanhong Li Di Zhang Ling Xu Lin Dong Ji Zheng Yikong Lin Jiefang Huang Yanyun Zhang Yu Tao Xingxing Zang Dajin Li Meirong Du 2019Cellular & Molecular Immunology2019,16,12:23
19Efficacy of intramuscular matrine in the treatment of chronic hepatitis B显示文摘BACKGROUND: Hepatitis B virus (HBV) infection, a glo- bal public health problem, is the leading cause of cirrhosis and hepatocellular carcinoma (HCC) worldwide. There are more than 350 million HBV carriers in the world and up to one million die annually due to hepatitis B associated liv- er disease. So far no optimal treatment is available for pa- tients with chronic hepatitis B. In the paper we investigated the efficacy of intramuscular matrine in the treatment of chronic hepatitis B. METHODS: One hundred and twenty patients with chronic hepatitis B were randomly divided into matrine treatment group (n =60) and control group (n =60). The patients of the matrine group were given intramuscularly with matrine (an alkaloid extracted from a traditional Chinese herb Radix Sophorae Flavescentis by Guangzhou Ming Xing Pharmaceu cal Factory, Guangzhou, China) of 100 mg daily for 90 days in addition to conventional liver-protective drugs in- cluding glucurone, inosine, compound vitamin B and caryophyllin. The control group received conventional liv- er-protective drugs alone. Clinical manifestations and labo- ratory parameters including liver biochemistry and serum hepatitis B virus markers were monitored before and after treatment in the two groups. RESULTS: Significant differences were seen between the two groups in terms of improvement of clinical symptoms and signs, recovery of liver functions, and serum conver- sion from hepatitis Be antigen to HBe antibody and from positive to negative serum HBV DNA (P <0.05-0.01). The result of the matrine group was more marked than that of the control group. Serious side-effects were not observed except mild pain at the site of injection of matrine in a few patients. 20Investigation of the effect of berberines alkaloids in Coptis chinensis Franch on Bacillus shigae growthby microcalorimetry显示文摘The inhibitory effects of five berberines alkaloids (BAs) from rhizoma of Coptis chinensis Franch, a traditional Chinese medicinal (TCM) herb, on Bacillus shigae (B. shigae) growth were investigated by microcalorimetry. The power-time curves of B. shigae with and without BAs were acquired; meanwhile, the extent and duration of inhibitory effects on the metabolism were evaluated by growth rate constants (k1, k2), half inhibitory ratio (IC50), maximum heat output (Pmax), and peak time (tp). The values of k1 and k2 of B. shigae in the presence of the five BAs decreased with the increasing concentrations of BAs. Moreover, Pmax was reduced, and the value of tp increased with increasing concentrations of the five drugs. The inhibitory activity varied with different drugs. IC50 of the five BAs was respectively 75 μg/mL for berberine, 90 μg/mL for coptisine, 115 μg/mL for palmatine, 220 μg/mL for epiberberine, and 400 μg/mL for jatrorrhizine. The sequence of antimicrobial activity of the five BAs: berberine > coptisine > palmatine > epiberberine > jatrorrhizine. The functional groups methylenedioxy at C2 and C3 on phenyl ring improve antimicrobial activity more strongly than methoxyl at C2 and C3 on phenyl ring.
