1Procedure for prolapse and hemorrhoids vs traditional surgery for outlet obstructive constipation显示文摘AIM: To compare the clinical efficacies of two surgical procedures for hemorrhoid rectal prolapse with outlet obstruction-induced constipation.METHODS: One hundred eight inpatients who underwent surgery for outlet obstructive constipation caused by internal rectal prolapse and circumferential hemorrhoids at the First Affiliated Hospital of Xinjiang Medical University from June 2012 to June 2013 were prospectively included in the study.The patients with rectal prolapse hemorrhoids with outlet obstructioninduced constipation were randomly divided into two groups to undergo either a procedure for prolapse and hemorrhoids(PPH)(n = 54) or conventional surgery(n = 54; control group).Short-term(operative time,postoperative hospital stay,postoperative urinary retention,postoperative perianal edema,and postoperative pain) and long-term(postoperative anal stenosis,postoperative sensory anal incontinence,postoperative recurrence,and postoperative difficulty in defecation) clinical effects were compared between the two groups.The short- and long-term efficacies of the two procedures were determined.RESULTS: In terms of short-term clinical effects,operative time and postoperative hospital stay were significantly shorter in the PPH group than in the control group(24.36 ± 5.16 min vs 44.27 ± 6.57 min,2.1 ± 1.4 d vs 3.6 ± 2.3 d,both P < 0.01).The incidence of postoperative urinary retention was higher in the PPH group than in the control group,but the difference was not statistically significant(48.15% vs 37.04%).Theincidence of perianal edema was significantly lower in the PPH group(11.11% vs 42.60%,P < 0.05).The visual analogue scale scores at 24 h after surgery,first defecation,and one week after surgery were significantly lower in the PPH group(2.9 ± 0.9 vs 8.3 ± 1.1,2.0 ± 0.5 vs 6.5 ± 0.8,and 1.7 ± 0.5 vs 5.0 ± 0.7,respectively,all P < 0.01).With regard to long-term clinical effects,the incidence of anal stenosis was lower in the PPH group than in the control group,but the difference was not significant(1.85% vs 5.56%).The incidence of sensory anal incontinence was significantly lower in the PPH group(3.70% vs 12.96%,P < 0.05).The incidences of recurrent internal rectal prolapse and difficulty in defecation were lower in the PPH group than in the control group,but the differences were not significant(11.11% vs 16.67% and 12.96% vs 24.07%,respectively).CONCLUSION: PPH is superior to the traditional surgery in the management of outlet obstructive constipation caused by internal rectal prolapse with circumferential hemorrhoids.Ming Lu Bo Yang Yang Liu Qing Liu Hao Wen 2015World Journal of Gastroenterology2015,21,26:57
2Milligan-Morgan hemorrhoidectomy with anal cushion suspension and partial internal sphincter resection for circumferential mixed hemorrhoids显示文摘AIM:To identify a more effective treatment protocol for circumferential mixed hemorrhoids.METHODS:A total of 192 patients with circumferential mixed hemorrhoids were randomized into the treatment group,where they underwent Milligan-Morgan hemorrhoidectomy with anal cushion suspension and partial internal sphincter resection,or the control group,where traditional external dissection and internal ligation were performed.Postoperative recovery and complications were monitored.RESULTS:The time to wound healing was 12.96 ± 2.25 d in the treatment group shorter than 19.58 ± 2.71 d in the control group.Slight pain rate was 58.3% in the treatment group higher than 22.9% in the control group;moderate pain rate was 33.3% in the treatment group lower than 56.3% in the control group severe pain rate was 8.4% in the treatment group lower than 20.8% in the control group.No edema rate was 70.8% in the treatment group higher than 43.8% in the control group;mild local edema rate was 26% in the treatment group lower than 39.6% in the control group obvious local edema was 3.03% in the treatment group lower than 16.7% in the control group.No stenosis rate was 85.4% in the treatment group higher than 63.5% in the control group;moderate stenosis rate was 14.6% in the treatment group Lower than 27.1% in the control group severe anal stenosis rate was 0% in the treatment group lower than 9.4% in the control group.CONCLUSION:Milligan-Morgan hemorrhoidectomy with anal cushion suspension and partial internal sphincter resection is the optimal treatment for circumferential mixed hemorrhoids and can be widely applied in clinical settings.Ming Lu Guang-Ying Shi Guo-Qiang Wang Yan Wu Yang Liu Hao Wen 2013World Journal of Gastroenterology2013,19,30:44
3Estimation of internal nutrient release in large shallow Lake Taihu, China显示文摘Based on field investigation of wave, sediment suspension and the changes in nutrient concentration of the water column in Lake Taihu, China, we proposed two release models to quantify nutrient release under static and dynamic conditions, respectively. Under static conditions, nutrient release from sediments to the overlying water mainly depends on chemical diffusion induced by concentration gradient, in which the nutrient release is controlled by the temperature, dissolved oxygen concentration in the sediment-water interface, oxidation-reduction potential and the concentration difference between porewater and overlying water. Under dynamic condition (or disturbed condition),both dissolved and particulate nutrients in sediments are released into the water column because of wind-induced sediment suspension. The amount of nutrient release under dynamic conditions is larger than that under the static condition. The release of dissolved nutrients, however, does not increase because the wind induced turbulence made oxidation of metallic elements such as Fe (ferric iron), Mn which are capable of precipitating soluble reactive phosphate (SRP). Under dynamic conditions, therefore, the release of total phosphorus (TP) increases dramatically but the release of SRP is close to those under static conditions. In sediments of Lake Taihu, high Fe content leads to a high ratio of Fe to P contents in sediments (Fe:P ratio). Under dynamic conditions, therefore, nutrient release is controlled by the intensity of disturbance, sediment consolidation and nutrient content in sediments.As for dissolved nutrients, especially SRP, the release is also controlled by the intensity of dynamic re-oxidation, Fe content in sediments and nutrient concentration gradient between porewater and overlying water. Based on these two release modes, the release flux in Lake Taihu has been estimated. In the static condition (i.e. laboratory experimental condition), total release of NH4+-N for whole lake is ca. 10,000 ton/a, and PO43--P is ca. 900 ton/a. In the dynamic condition, nutrient release following sediment suspension was estimated according to three different intensities of wind forcing which were defined as 'calm' (wind speed is less than 2 m/s), 'gentle' (wind speed is greater than 2 m/s and less than 6 m/s) and 'gust' (wind speed is greater than 6 m/s). The release rate in the condition of 'calm' was estimated in terms of the nutrient release in the laboratory experimental static condition; whereas the release rate in conditions of 'gentle' and 'gust' was estimated in terms of measurement during sediment resuspension conducted in flume experiments. With the observation of wind velocity and frequency in 2001, each type of wind forcing took the frequency of 12%, 82% and 6% for 'calm', 'gentle' and 'gust', respectively. 4阿尔金淡水泉早古生代泥质高压麻粒岩及其P-T演化轨迹显示文摘南阿尔金构造带淡水泉一带出露的含石榴石蓝晶石黑云母片麻岩是一套典型的泥质高压麻粒岩,其峰期特征矿物组合为石榴子石+蓝晶石+钾长石+金红石+石英。根据矿物内部一致性热力学数据和Thermocalc 3.23程序计算,确定其峰期变质温压条件为T>850℃和P>11kbar。结合岩相学研究和P-T视剖面图计算,可识别出该岩石经历了4个阶段的变质演化,构成了一个早期快速等温降压,后期近等压降温的顺时针型的退变质P-T演化轨迹,为与陆壳俯冲碰撞有关的高压变质事件的产物。该岩石锆石阴极发光图像显示其内部具有明显的核-边结构,核部为残留的原岩碎屑锆石,边部则表现为面状或扇状生长的变质锆石的特征。微区原位LA-ICP-Ms微量元素分析表明,核部测点的重稀土含量较高,对应Th/U接近于0.4,具有岩浆锆石的特征;边部测点的重稀土相对亏损,重稀土配分曲线平坦,对应Th/U比值均小于0.1,显示与石榴子石平衡共生的变质锆石特征。LA-ICP-MS微区定年获得其变质年龄为486±5Ma,该年龄值与阿尔金江尕勒萨依和英格利萨依两地超高压变质岩石的变质年龄相近,进一步证明沿阿尔金构造带南缘断续存在一条早古生代的高压-超高压变质岩带。另外,本次研究在获得该泥质高压麻粒岩峰期变质时代的同时,还获得该岩石原岩的形成时代上限值约为719Ma,从而限定阿尔金构造带南缘阿尔金群的形成时代可能不属古元古代,而应属新元古代。曹玉亭 刘良 王超 陈丹玲 张安达 2009岩石学报2009,25,9:35
5Pathophysiology, diagnosis, and treatment of discogenic low back pain显示文摘Discogenic low back pain is a serious medical and social problem, and accounts for 26%-42% of the patients with chronic low back pain. Recent studies found that the pathologic features of discs obtained from the patients with discogenic low back pain were the formation of the zones of vascularized granulation tissue, with extensive innervation in fissures extending from the outer part of the annulus into the nucleus pulposus. Studies suggested that the degeneration of the painful disc might originate from the injury and subsequent repair of annulus fibrosus. Growth factors such as basic fibroblast growth factor, transforming growth factor β1, and connective tissue growth factor, macrophages and mast cells might play a key role in the repair of the injured annulus fibrosus and subsequent disc degeneration. Although there exist controversies about the role of discography as a diagnostic test, provocation discography still is the only available means by which to identify a painful disc. A recent study has classified discogenic low back pain into two types that were annular disruption-induced low back pain and internal endplate disruption-induced low back pain, which have been fully supported by clinical and theoretical bases. Current treatment options for discogenic back pain range from medicinal anti-inflammation strategy to invasive procedures including spine fusion and recently spinal arthroplasty. However, these treatments are limited to relieving symptoms, with no attempt to restore the disc's structure. 6Global warming weakening the inherent stability of glaciers and permafrost显示文摘The Cryosphere has been undergoing a worldwide retreat, as seen in the decrease in the areal extent and volume of glaciers and in the areal extent of permafrost. This paper presents a systematic examination of the inherent stability changes of glaciers and permafrost caused by warming. Various study results suggest that over the past 30 years, the internal temperature of glaciers and permafrost exhibits an overall accelerating warming trend. The warming rate peaked in the mid-2000s and slowed slightly for several years afterward. In recent years, however, the warming rate has seemed to pick up again. The warming of glaciers and permafrost has exerted great impact on their stability, displayed as intensified melting,increased glacier surging, enlargement of supraglacial lakes, and increased permafrost degradation.Even without a future temperature increase, some glaciers will continue to shrink, and the number of surging glaciers will increase. The transition from low-temperature to high-temperature permafrost is a noticeable warning sign of a comprehensive degradation of permafrost. 7Abnormal splenic artery diameter/hepatic artery diameter ratio in cirrhosis-induced portal hypertension显示文摘AIM:To determine an optimal cutoff value for abnormal splenic artery diameter/proper hepatic artery diameter(S/P) ratio in cirrhosis-induced portal hypertension.METHODS:Patients with cirrhosis and portal hypertension(n = 770) and healthy volunteers(n = 31) underwent volumetric computed tomography threedimensional vascular reconstruction to measure the internal diameters of the splenic artery and proper hepatic artery to calculate the S/P ratio.The cutoff value for abnormal S/P ratio was determined using receiver operating characteristic curve analysis,and the prevalence of abnormal S/P ratio and associations between abnormal S/P ratio and major complications of portal hypertension were studied using logistic regression.RESULTS:The receiver operating characteristic analysis showed that the cutoff points for abnormal splenic artery internal diameter and S/P ratio were > 5.19 mm and > 1.40,respectively.The sensitivity,specificity,positive predictive value,and negative predictive value were 74.2%,45.2%,97.1%,and 6.6%,respectively.The prevalence of an abnormal S/P ratio in the patients with cirrhosis and portal hypertension was 83.4%.Patients with a higher S/P ratio had a lower risk of developing ascites [odds ratio(OR) = 0.708,95%CI:0.508-0.986,P = 0.041] and a higher risk of developing esophageal and gastric varices(OR = 1.483,95%CI:1.010-2.175,P = 0.044) and forming collateral circulation(OR = 1.518,95%CI:1.033-2.230,P = 0.034).After splenectomy,the portal venous pressure and maximum and mean portal venous flow velocities were reduced,while the flow rate and maximum and minimum flow velocities of the hepatic artery were increased(P < 0.05).CONCLUSION:The prevalence of an abnormal S/P ratio is high in patients with cirrhosis and portal hypertension,and it can be used as an important marker of splanchnic hemodynamic disturbances.Dao-Bing Zeng Chuan-Zhou Dai Shi-Chun Lu Ning He Wei Wang Hong-Jun Li 2013World Journal of Gastroenterology2013,19,8:28

10Temperature effect and thermal impact in lithium-ion batteries: A review显示文摘Lithium-ion batteries, with high energy density(up to 705 Wh/L) and power density(up to 10,000 W/L), exhibit high capacity and great working performance. As rechargeable batteries, lithium-ion batteries serve as power sources in various application systems. Temperature, as a critical factor, significantly impacts on the performance of lithium-ion batteries and also limits the application of lithium-ion batteries. Moreover, different temperature conditions result in different adverse effects. Accurate measurement of temperature inside lithiumion batteries and understanding the temperature effects are important for the proper battery management. 11Aggregation-induced emission: a coming-of-age ceremony at the age of eighteen显示文摘Aggregation-induced emission(AIE) has drawn great attention worldwide for its unique optical phenomenon and huge potential applications. Since coined by Ben Zhong Tang et al. in 2001, AIE has been deeply investigated and widely utilized in many important areas, such as organic light-emitting diode(OLED), sensor, and bio-imaging. Herein, we highlight some important progress of AIE in these eighteen years, including the exploration of internal mechanism and potential applications. Furthermore,some other interestingly emissive behaviors, originating but distinguishing from the AIE concept, are presented. 12Design concept of three-dimensional section controllable internal waverider hypersonic inlet显示文摘A new hypersonic inlet named three-dimensional section controllable internal waverider inlet is presented in this paper to achieve the goal of section shape geometric transition and complete capture of the upstream mass. On the basis of the association between hypersonic waverider airframe and streamtraced hypersonic inlet, the waverider concept is extended to yield results for the internal flows, namely internal waverider concept. It is proven theoretically that not osculating cones but osculating axisymmetric theory is appropriate for the design of section controllable internal waverider inlet. And two design methods out of the internal waverider concept are proposed subsequently to construct two inlets with specific section shape request, triangle to ellipse and rectangle to ellipse ones. The calculation results show that the inlets are capable of keeping their shock structures and the main flow characteristics exactly as their derived flowfield. Further, the inlets successfully capture all the upstream mass despite their complicated cross-section transitions. It is believed that the concept proposed ex- plores a new way of designing three-dimensional hypersonic inlets with special demand of section shape transition. 13The nutrient forms, cycling and exchange flux in the sediment and overlying water system in lakes from the middle and lower reaches of Yangtze River显示文摘This paper is a review of research works concerning the nutrient transportation,transformation and exchange between water, sediment and biota in the lakes from the middle and lower reaches of the Yangtze River conducted in the context of project entitled 'The Processes and Mechanism of Lake Eutrophication in Middle and Lower Reaches of Yangtze River'. All the lakes from this area are shallow lakes. According to the typical lake site research, the lakes from the middle and lower reaches of Yangtze River have a higher baseline of nutrition in the history. Normally the trophic status of these lakes can be categorized into medium-trophic or eutrophic. Human activities have been enhanced during the last decades, which speed up the lake eutrophic process. Lake eutrophication control needs to reduce not only the external nutrient inputs from watershed but also the internal loading from the sediments. Investigations revealed that the lake sediments in this area are considerablly high in nutrition in which at most about 30% of phosphorus exists in the form of bio-available in the sediment. The surface sediment will exert great effects on the nutrient exchange between water-sediment interface via adsorption and release of nutrient. The nutrient release from the sediment in these shallow lakes is mainly in two ways, i.e. in the undisturbed condition the nutrient is released through diffusion created by the nutrient gradient from sediment to overlying water; whereas in disturbed condition, the nutrient release is determined by the hydrodynamic forcing intensity and the sediment resuspension. Metallic elements such as the iron, manganese and aluminium and the aerobic-anaerobic ambience will affect the release of nutrients. The disturbed release will increase the total nutrients in the water column significantly in the short period. At the beginning of sediment resuspension, the dissolved nutrient concentration will increase. This increase will be damped if the ferric oxide and aluminium are rich in sediment because of the adsorption and flocculation. This means that the lakes have capability of eliminating the nutrient loadings. Investigations for the lakes from middle and down stream of Yangtze River have suggested that most lakes have the self-cleaning capability. Dredging the control of the internal loading, therefore, is only applicable to the small lakes or undisturbed bays which normally are situated nearby the city or town and rich in organic materials in the sediment. In addition, the strong reduction condition and weak aeration of these lakes and bays make these small lakes and bays release much more bio-available nutrient and without much self-eliminating capability. Moreover, eutrophication induced algal bloom in these lakes will change www. scichina.com www. springerlink.comthe pH of water, which further induces the increase in the nutrient release. In turn, the increase in nutrient release promotes the growth of phytoplankton and results in severe algal bloom. For the heavily polluted water, research suggests that the biomass of bacteria and alkaline phosphatase activity will be higher corresponding to the higher concentration of nutrients, which accelerates the nutrient recycling between water, sediment and biota. Quick recycling of nutrient, in turn, promotes the production and biomass growth of microorganism and leads to more severe eutrophication. 14Some characteristics and evolution of the internal soliton in the northern South China Sea显示文摘Based on the observational data of an internal soliton in the northern South China Sea (SCS) on June 14, 1998, the possible source of the internal soliton is analyzed, and some of its characteristic parameters such as phase speed, wave length, etc. are computed. Based on the analyses of the vertical modes of the internal wave, the characteristics of the wave-induced current during the propagation of the internal soliton are studied. 15腰椎后路椎间融合术后椎间融合的X线片及三维CT评价显示文摘目的 探讨X线片和三维CT(3D-CT)在判定单节段后路腰椎椎间融合术后椎间融合情况的临床价值.方法 选取接受后路腰椎椎间融合术并随访1年以上的患者43例,男19例,女24例;年龄16~74岁,平均47.2岁.峡部裂性滑脱21例,退变性滑脱15例,腰椎间盘突出并不稳症3例,腰椎间盘突出症复发3例,极外侧椎间盘突出症1例.单纯骨粒植骨26例,骨粒加椎间融合器17例.单节段内固定24例,双节段内固定19例.均为单节段椎间融合,其中L3,4,6例,L4,5,17例,L5 S1 20例.应用改良Brantigan评分判定腰椎正侧位X线片及3D-CT扫描所示椎间融合程度;动力位X线片判断椎间稳定性.结果 随访12~85个月,平均18个月.末次随访时X线片及3D-CT所示椎间融合率分别为64%(28/43例)和40%(17/43例).3D-CT的Brantigan评分小于正侧位X线片(P<0.05).根据诊断、固定节段及融合方式不同分组,各组3D-CT评分均低于正侧位X线片(P<0.05).动力位X线片示椎间不稳定者仅3例(7%).结论 腰椎3D-CT能更准确地评价椎间融合情况.拆除内固定前进行3D-CT检查非常必要.拆除内固定前摄动力位X线片对判定椎体间融合的临床意义不大.郑燕平 刘新宇 贾龙 王延国 黎君彦 2009中华骨科杂志2009,29,12:22

Based on an analysis of documented internal-tide and internal-wave deposits, five facies types can be recognized: ( ⅰ ) bidirectional cross-laminated sandstone facies, ( ⅱ ) unidirectional cross-bedded and cross-laminated sandstone facies, ( ⅲ ) rhythmic alternating sandstone facies and mudstone, ( Ⅳ ) flaser, wavy and lenticular bedded foraminiferal limestone facies, ( Ⅴ ) sandy oolitic limestone facies with flaser, wavy and lenticular bedding and epsilon cross bedding, which formed in submarine channel, unchannelized continental slope and plateau environments respectively. 17Current status of laparoscopic and robotic ventral mesh rectopexy for external and internal rectal prolapse显示文摘External and internal rectal prolapse with their affiliated rectocele and enterocele, are associated with debilitating symptoms such as obstructed defecation, pelvic pain and faecal incontinence. Since perineal procedures are associated with a higher recurrence rate, an abdominal approach is commonly preferred. Despite the description of greater than three hundred different procedures, thus far no clear superiority of one surgical technique has been demonstrated. Ventral mesh rectopexy(VMR) is a relatively new and promising technique to correct rectal prolapse. In contrast to the abdominal procedures of past decades, VMR avoids posterolateral rectal mobilisation and thereby minimizes the risk of postoperative constipation. Because of a perceived acceptable recurrence rate, good functional results and low mesh-related morbidity in the short to medium term, VMR has been popularized in the past decade. Laparoscopic or robotic-assisted VMR is now being progressively performed internationally and several articles and guidelines propose the procedure as the treatment of choice for rectal prolapse. 19Surgical treatment for both-column acetabular fractures using pre-operative virtual simulation and three-dimensional printing techniques显示文摘Background:Surgical treatment of both-column acetabular fractures is challenging because of the complex acetabular fracture patterns and the curved surface of the acetabulum.Seldom study has compared the application of three-dimensional(3D)printing technology and traditional methods of contouring plates intra-operatively for the surgical treatment of both-column acetabular fractures.We presented the use of both 3D printing technology and a virtual simulation in pre-operative planning for both-column acetabular fractures.We hypothesized that 3D printing technology will assist orthopedic surgeons in shortening the surgical time and improving the clinical outcomes.Methods:Forty patients with both-column acetabular fractures were recruited in the randomized prospective case-control study from September 2013 to September 2017 for this prospective study(No.ChiCTR1900028230).We allocated the patients to two groups using block randomization(3D printing group,n=20;conventional method group,n=20).For the 3D printing group,1:1 scaled pelvic models were created using 3D printing,and the plates were pre-contoured according to the pelvic models.The plates for the conventional method group were contoured during the operation without 3D printed pelvic models.The operation time,instrumentation time,time of intra-operative fluoroscopy,blood loss,number of times the approach was performed,blood transfusion,post-operative fracture reduction quality,hip joint function,and complications were recorded and compared between the two groups.Results:The operation and instrumentation times in the 3D printing group were significantly shorter(130.8±29.2 min,t=-7.5,P<0.001 and 32.1±9.5 min,t=-6.5,P<0.001,respectively)than those in the conventional method group.The amount of blood loss and blood transfusion in the 3D printing group were significantly lower(500[400,800]mL,Mann-Whitney U=74.5,P<0.001 and 0[0,400]mL,Mann-Whitney U=59.5,P<0.001,respectively)than those in the conventional method group.The number of the approach performed in the 3D printing group was significantly smaller than that in the conventional method group(pararectus+Kocher-Langenbeck[K-L]approach rate:35%vs.85%;x2=10.4,P<0.05).The time of intra-operative fluoroscopy in the 3D printing group was significantly shorter than that in the conventional method group(4.2±1.8 vs.7.7±2.6 s;t=-5.0,P<0.001).The post-operative fracture reduction quality in the 3D printing group was significantly better than that in the conventional method group(good reduction rate:80%vs.30%;x2=10.1,P<0.05).The hip joint function(based on the Harris score 1 year after the operation)in the 3D printing group was significantly better than that in the conventional method group(excellent/good rate:75%vs.30%;χ^2=8.1,P<0.05).The complication was similar in both groups(5.0%vs.25%;χ^2=3.1,P=0.182).Conclusions:The use of a pre-operative virtual simulation and 3D printing technology is a more effective method for treating bothcolumn acetabular fractures.This method can shorten the operation and instrumentation times,reduce blood loss,blood transfusion and the time of intra-operative fluoroscopy,and improve the post-operative fracture reduction quality.Ji-Hui Huang Hui Liao Xin-Yu Tan Wei-Rong Xing Qi Zhou Yu-Shi Zheng Hong-Yu Cao Can-Jun Zeng 2020Chinese Medical Journal2020,,4:15
