2Prevalence of Autism Spectrum Disorder in China:A Nationwide Multi-center Population-based Study Among Children Aged 6 to 12 Years显示文摘This study aimed to obtain the first national estimate of the prevalence of autism spectrum disorder(ASD) in Chinese children.We targeted the population of 6 to 12-year-old children for this prevalence study by multistage convenient cluster sampling.The Modified Chinese Autism Spectrum Rating Scale was used for the screening process.Of the target population of 142,086 children,88.5%(n=125,806) participated in the study.A total of 363 children were confirmed as having ASD.The observed ASD prevalence rate was 0.29%(95% CI:0.26%-0.32%) for the overall population.After adjustment for response rates,the estimated number of ASD cases was867 in the target population sample,thereby achieving an estimated prevalence of 0.70%(95% CI:0.64%-0.74%).The prevalence was significantly higher in boys than in girls(0.95%;95% CI:0.87%-1.02% versus 0.30%;95%CI:0.26%-0.34%;P <0.001).Of the 363 confirmed ASD cases,43.3% were newly diagnosed,and most of those(90.4%) were attending regular schools,and 68.8% of the children with ASD had at least one neuropsychiatric comorbidity.Our findings provide reliable data on the estimated ASD prevalence and comorbidities in Chinese children.Hao Zhou Xiu Xu Weili Yan Xiaobing Zou Lijie Wu Xuerong Luo Tingyu Li Yi Huang Hongyan Guan Xiang Chen Meng Mao Kun Xia Lan Zhang Erzhen Li Xiaoling Ge Lili Zhang Chunpei Li Xudong Zhang Yuanfeng Zhou Ding Ding Andy Shih Eric Fombonne Yi Zheng Jisheng Han Zhongsheng Sun Yong-hui Jiang Yi Wang LATENT-NHC Study Team 2020Neuroscience Bulletin2020,36,9:158
3Prevalence of fatty liver disease and the economy in China: A systematic review显示文摘AIM: To investigate the relationship between the economy and the adult prevalence of fatty liver disease(FLD) in China's Mainland. METHODS: Literature searches on the Pub Med and Chinese National Knowledge Infrastructure databases were performed to identify eligible studies published before July 2014. Records were limited to crosssectional surveys or baseline surveys of longitudinal studies that reported the adult prevalence of FLD and recruited subjects from the general population or community. The gross domestic product(GDP) per capita was chosen to assess the economic status. Multiple linear regression and Loess regression were chosen to fit the data and calculate the 95%CIs. Fitting and overfitting of the models were considered in choosing the appropriate models. RESULTS: There were 27 population-based surveys from 26 articles included in this study. The pooled mean prevalence of FLD in China was 16.73%(95%CI: 13.92%-19.53%). The prevalence of FLD was correlated with the GDP per capita and survey years in the country(adjusted R2 = 0.8736, P GDP per capita = 0.00426, P years = 0.0000394), as well as in coastal areas(R2 = 0.9196, P GDP per capita = 0.00241, P years = 0.00281). Furthermore, males [19.28%(95%CI: 15.68%-22.88%)] presented a higher prevalence than females [14.1%(95%CI: 11.42%-16.61%), P = 0.0071], especially in coastal areas [21.82(95%CI: 17.94%-25.71%) vs 17.01%(95%CI: 14.30%-19.89%), P = 0.0157]. Finally, the prevalence was predicted to reach 20.21% in 2020, increasing at a rate of 0.594% per year. 4Global epidemiology of hepatitis C virus infection: an up-date of the distribution and circulation of hepatitis C virus genotypes显示文摘AIM To review Hepatitis C virus(HCV) prevalence and genotypes distribution worldwide.METHODS We conducted a systematic study which represents one of the most comprehensive effort to quantify global HCV epidemiology,using the best available published data between 2000 and 2015 from 138 countries(about 90% of the global population),grouped in 20 geographical areas(with the exclusion of Oceania),as defined by the Global Burden of Diseases project(GBD). Countries for which we were unable to obtain HCV genotype prevalence data were excluded from calculations of regional proportions,although their populations were included in the total population size of each region when generating regional genotype prevalence estimates.RESULTS Total global HCV prevalence is estimated at 2.5%(177.5 million of HCV infected adults),ranging from 2.9% in Africa and 1.3% in Americas,with a global viraemic rate of 67%(118.9 million of HCV RNA positive cases),varying from 64.4% in Asia to 74.8% in Australasia. HCV genotype 1 is the most prevalent worldwide(49.1%),followed by genotype 3(17.9%),4(16.8%) and 2(11.0%). Genotypes 5 and 6 are responsible for the remaining < 5%. While genotypes 1 and 3 are common worldwide,the largest proportion of genotypes 4 and 5 is in lower-income countries. 5Clinical epidemiology and disease burden of nonalcoholic fatty liver disease显示文摘Nonalcoholic fatty liver disease(NAFLD) is defined as the presence of hepatic fat accumulation after the exclusion of other causes of hepatic steatosis, including other causes of liver disease, excessive alcohol consumption, and other conditions that may lead to hepatic steatosis. NAFLD encompasses a broad clinical spectrum ranging from nonalcoholic fatty liver to nonalcoholic steatohepatitis(NASH), advanced fibrosis, cirrhosis, and finally hepatocellular carcinoma(HCC). NAFLD is the most common liver disease in the world and NASH may soon become the most common indication for liver transplantation. Ongoing persistence of obesity with increasing rate of diabetes will increase the prevalence of NAFLD, and as this population ages, many will develop cirrhosis and end-stage liver disease. There has been a general increase in the prevalence of NAFLD, with Asia leading the rise, yet the United States is following closely behind with a rising prevalence from 15% in 2005 to 25% within 5 years. NAFLD is commonly associated with metabolic comorbidities, including obesity, type Ⅱ diabetes, dyslipidemia, and metabolic syndrome. Our understanding of the pathophysiology of NAFLD is constantly evolving. Based on NAFLD subtypes, it has the potential to progress into advanced fibrosis, end-stage liver disease and HCC. The increasing prevalence of NAFLD with advanced fibrosis, is concerning because patients appear toexperience higher liver-related and non-liver-related mortality than the general population. 6Epidemiology, demographic characteristics and prognostic predictors of ulcerative colitis显示文摘Ulcerative colitis(UC)is a chronic disease characterized by diffuse inflammation of the mucosa of the colon and rectum.The hallmark clinical symptom of UC is bloody diarrhea.The clinical course is marked by exacerbations and remissions,which may occur spontaneously or in response to treatment changes or intercurrent illnesses.UC is most commonly diagnosed in late adolescence or early adulthood,but it can occur at any age.The incidence of UC has increased worldwide over recent decades,especially in developing nations.In contrast,during this period,therapeutic advances have improved the life expectancy of patients,and there has been a decrease in the mortality rate over time.It is important to emphasize that there is considerable variability in the phenotypic presentation of UC.Within this context,certain clinical and demographic characteristics are useful in identifying patients who tend to have more severe evolution of the disease and a poor prognosis.In this group of patients,better clinical surveillance and more intensive therapy may change the natural course of the disease.The aim of this article was to review the epidemiology and demographic characteristics of UC and the factors that may be associated with its clinical prognosis.Bruno César da Silva Andre Castro Lyra Raquel Rocha Genoile Oliveira Santana 2014World Journal of Gastroenterology2014,20,28:48
7Prevalence of and risk factors for non-alcoholic fatty liver disease in a Chinese population: An 8-year follow-up study显示文摘AIM: To investigate the prevalence of and risk factors for non-alcoholic fatty liver disease(NAFLD) in a Chinese population.METHODS: A total of 1948 adults from China was followed for 8 years. A cross-sectional study was performed to investigate the prevalence of NAFLD at baseline, and then the participants were followed for 8 years to investigate risk factors for the development of NAFLD.RESULTS: A total of 1948 participants were enrolled at baseline, of whom 691 were diagnosed with NAFLD. During the 8-year follow-up, 337 baseline NAFLD-free participants developed NAFLD. They had a greaterincrease in body mass index(BMI), serum uric acid, fasting plasma glucose, very low-density lipoprotein cholesterol and a considerable decrease in high-density lipoprotein cholesterol. 123 participants who had NAFLD at baseline lost NAFLD during the 8-year follow-up period. They had a greater decrease in BMI, fasting plasma glucose, triglycerides, total cholesterol, low-density lipoprotein cholesterol, alanine aminotransferase, aspartate aminotransferase, and γ-glutamyl transpeptidase.CONCLUSION: NAFLD is prevalent in Chinese population with a rapidly increasing tendency. 8Report on Cardiovascular Health and Diseases in China 2021:An Updated Summary显示文摘In 2019,cardiovascular disease(CVD)accounted for 46.74%and 44.26%of all deaths in rural and urban areas,respectively.Two out of every five deaths were due to CVD.It is estimated that about 330 million patients suffer from CVD in China.The number of patients suffering from stroke,coronary heart disease,heart failure,pulmonary heart disease,atrial fibrillation,rheumatic heart disease,congenital heart disease,lower extremity artery disease and hypertension are 13.00 million,11.39 million,8.90 million,5.00 million,4.87 million,2.50 million,2.00 million,45.30 million,and 245.00 million,respectively.Given that China is challenged by the dual pressures of population aging and steady rise in the prevalence of metabolic risk factors,the burden caused by CVD will continue to increase,which has set new requirements for CVD prevention and treatment and the allocation of medical resources in China.It is important to reduce the prevalence through primary prevention,increase the allocation of medical resources for CVD emergency and critical care,and provide rehabilitation services and secondary prevention to reduce the risk of recurrence,re-hospitalization and disability in CVD survivors.The number of people suffering from hypertension,dyslipidemia and diabetes in China has reached hundreds of millions.Since blood pressure,blood lipids,and blood glucose levels rise mostly insidiously,vascular disease or even serious events such as myocardial infarction and stroke often already occured at the time of detection in this population.Hence,more strategies and tasks should be taken to prevent risk factors such as hypertension,dyslipidemia,diabetes,obesity,and smoking,and more efforts should be made in the assessment of cardiovascular health status and the prevention,treatment,and research of early pathological changes. 2022Biomedical and Environmental Sciences2022,35,7:36
9Prevalence of and risk factors for gallstones in Uighur and Han Chinese显示文摘AIM:To perform a single-centre survey of the prevalence of and possible risk factors for gallstones in Uighur and Han Chinese.METHODS:Complete medical data for 9455 patients were collected from the medical centre of our hospital,and the overall prevalence of gallstones as well as the prevalence in different ethnic groups was studied.The risk factors for gallstones in different ethnic groups were identified in a univariate analysis,and variables with statistical significance were analysed by unconditional multiple logistic regression,to primarily explore the similarities and differences in gallstone risk factors between different ethnic groups.RESULTS:The prevalence of gallstones was significantly higher in the Uighur population than in the Hanpopulation(22.87%vs 11.64%,P<0.05).Further analysis of risk factors for gallstones based on the different ethnic areas revealed that age was a risk factor for gallstones in both groups;triglycerides,body-mass index(BMI)and high-density lipoprotein were risk factors for gallstones in the Han population,while total cholesterol(TC),gender and fatty liver were risk factors in the Uighur population.The Uighur patients were older than the Han patients,and had higher BMI,TC,low-density lipoprotein,female rate and fatty liver rate,while the incidence of hypertension was lower than that in the Han patients.CONCLUSION:The prevalence of and risk factors for gallstones differ between the Uighur and Han populations.Li Zhu Aikebaier Aili Cheng Zhang Aili Saiding Kelimu Abudureyimu Xinjiang Medical University 2014World Journal of Gastroenterology2014,20,40:39

11Prevalence and features of fatty liver detected by physical examination in Guangzhou显示文摘AIM:To investigate the prevalence of fatty liver discovered upon physical examination of Chinese patients and determine the associated clinical characteristics.METHODS:A total of 3433 consecutive patients who received physical examinations at the Huangpu Division of the First Affiliated Hospital at Sun Yat-sen University in Guangzhou,China from June 2010 to December2010 were retrospectively enrolled in the study.Results of biochemical tests,abdominal ultrasound,electrocardiography,and chest X-ray were collected.The diagnosis of fatty liver was made if a patient met any two of the three following ultrasonic criteria:(1)liver and kidney echo discrepancy and presence of an increased liver echogenicity(bright);(2)unclear intrahepatic duct structure;and(3)liver far field echo decay.RESULTS:The study population consisted of 2201males and 1232 females,with a mean age of 37.4±12.8 years.When all 3433 patients were considered,the overall prevalence of hyperlipidemia was 38.1%,of fatty liver was 26.0%,of increased alanine aminotransferase(ALT)and/or aspartate aminotransferase(AST)levels was 11.9%,of gallstone was 11.4%,of hyperglycemia was 7.3%,of hypertension was 7.1%,and of hyperuricemia was 6.2%.Of the 2605 patients who completed the abdominal ultrasonography exam,677(26.0%)were diagnosed with fatty liver and the prevalence was higher in males(32.5%vs females:15.3%,P<0.001).The overall prevalence of fatty liver increased with age,with the peak prevalence(39.5%)found in the 60 to 70-year-old age group.Among patients between the ages of 18 to 50-year-old,the prevalence of fatty liver was significantly higher in males(20.2%vs females:8.7%,P<0.001);the difference in prevalence between the two sexes in patients>50-year-old did not reach statistical significance.Only 430 of the patients diagnosed with fatty liver had complete information;among those,increased ALT and/or AST levels were detected in only 30%,with all disturbances being mild or moderate.In these 430 patients,the overall prevalence of hypertriglyceridemia was 31.4%,of mixed type hyperlipidemia was 20.9%,of hypercholesterolemia was 12.3%,of hyperglycemia was 17.6%,of hypertension was 16.0%,of hyperuricemia was 15.3%,and of gallstone was 14.4%.Again,the prevalences of hypertriglyceridemia and hyperuricemia were higher in males(hypertriglyceridemia,36.0%vs females:12.0%,P<0.05;hyperuricemia,17.3%vs females:7.2%,P<0.05);in contrast,however,the prevalences of mixed type hyperlipidemia and hypercholesterolemia was higher in females(mixed type hyperlipidemia,18.7% vs females:30.1%,P<0.05,hypercholesterolemia,9.5%vs females:24.1%,P<0.05).Finally,comparison of the fatty liver group to the non-fatty liver group showed that prevalences of hyperlipidemia,hyperglycemia,hypertension,and hyperuricemia were higher in the former(all P<0.01).CONCLUSION:A high prevalence of fatty liver is detected upon physical examination in Guangzhou,and the primary associated clinical findings are hyperlipidemia,hyperglycemia,hypertension,and hyperuricemia.Xian-Hua Liao Xu Cao Jie Liu Xiao-Hua Xie Yan-Hong Sun Bi-Hui Zhong 2013World Journal of Gastroenterology2013,19,32:30
12Gestational diabetes mellitus: Challenges for different ethnic groups显示文摘Ethnicity is defined as'belonging to a social groupthat has a common national or cultural tradition'.Membership of certain ethnic groups has long been associated with increased risk of gestational diabetes mellitus(GDM).Studies that examined ethnic differences amongst women with GDM were often conducted in western countries where women from various ethnic backgrounds were represented.The prevalence of GDM appears to be particularly high among women from South Asia and South East Asia,compared to Caucasian,African-American and Hispanic communities.For some,but not all ethnic groups,the body mass index is a risk factor for the development of GDM.Even within a particular ethnic group,those who were born in their native countries have a different risk profile for GDM compared to those born in western countries.In terms of treatment,medical nutrition therapy(MNT)plays a key role in the management of GDM and the prescription of MNT should be culturally sensitive.Limited studies have shown that women who live in an English-speaking country but predominantly speak a language other than English,have lower rates of dietary understanding compared with their English speaking counterparts,and this may affect compliance to therapy.Insulin therapy also plays an important role and there appears to be variation as to the progression of women who progress to requiring insulin among different ethnicities.As for peri-natal outcomes,women from Pacific Islander countries have higher rates of macrosomia,while women from Chinese backgrounds had lower adverse pregnancy outcomes.From a maternal outcome point of view,pregnant women from Asia with GDM have a higher incidence of abnormal glucose tolerance test results post-partum and hence a higher risk of future development of type2 diabetes mellitus.On the other hand,women from Hispanic or African-American backgrounds with GDM are more likely to develop hypertension post-partum.This review highlights the fact that management needs to be individualised and the clinician should be mindful of the impact that differences in ethnicity may have on the clinical characteristics and pregnancy outcomes inwomen affected by GDM,particularly those living in Western countries.Understanding these differences is critical in the delivery of optimal antenatal care for women from diverse ethnic backgrounds.Lili Yuen Vincent W Wong 2015World Journal of Diabetes2015,6,8:29
13Treatment of chronic prostatitis in Chinese men显示文摘The aim of this study is to assess the status of treatment of chronic prostatitis(CP)in Chinese men.A population-based cross-sectional survey was performed,in which 15000 men aged between 15 and 60 years were randomly selected to receive a questionnaire designed to assess National Institutes of Health Chronic Prostatitis Symptoms Index(NIH-CPSI)status,therapeutic efficacy and 28 other items.A total of 12743 men(84.95%)completed the questionnaire,of whom 1071(8.4%)were identified as having prostatitis-like symptoms and 517(4.5%)were diagnosed with CP according to NIH-CPSI criteria and prostatitis-like symptomatology.Of the CP patients,372(65%)underwent long-term routine treatment 12 times per year.Additionally,217(72.8%)patients received antibiotic therapy and 215(79.3%)men showed therapeutic effects.The treatment cost USD 1151(8059 yuan)per person per year on average.Most CP patients received routine treatment,in most cases with antibiotics.Treatment was costly and most CP patients were not satisfied with its effectiveness.Antibacterial treatment might have been effective primarily in patients with bacterial disease.Chao-Zhao Liang Hong-Jun Li Zhi-Ping Wang Jun-Ping Xing Wei-Lie Hu Tao-Fu Zhang Wei-Wei Ge Zong-Yao Hao Xian-Sheng Zhang Jun Zhou Yu Li Zheng-Xing Zhou Zhi-Guo Tang 2009Asian Journal of Andrology2009,11,2:27
14Epidemiology of urolithiasis in Asia显示文摘In Asia,about 1%-19.1% of the population suffer fromurolithiasis.However,due to variations in socio-economic status and geographic locations,the prevalence and incidence have changed in different countries or regions over the years.The research for risk factors of urinary tract stones is of predominant importance.In this review,we find the prevalence of urolithiasis is 5%-19.1% in West Asia,Southeast Asia,South Asia,aswellas somedevelopedcountries(SouthKorea and Japan),whereas,it is only 1%e8%inmost part of East Asia and North Asia.The recurrence rate ranges from 21% to 53% after 3e5 years.Calciumoxalate(75%-90%)is themost frequent component of calculi,followed by uric acid(5%-20%),calciumphosphate(6%-13%),struvite(2%-15%),apatite(1%)and cystine(0.5%-1%).The incidence of urolithiasis reaches its peak in population agedover 30 years.Males aremore likely to suffer fromurinary calculi.Because of different dietary habits or genetic background,differences of prevalence among races or nationalities also exist.Geneticmutation of specific locus may contribute to the formation of different kinds of calculi.Dietary habits(westernized dietary habits and less fluid intake),as well as climatic factors(hot temperature andmany hours of exposure to sunshine)play a crucial role in the development of stones.Other diseases,especially metabolic syndrome,may also contribute to urinary tract stones.Yu Liu Yuntian Chen Banghua Liao Deyi Luo Kunjie Wang Hong Li Guohua Zeng 2018Asian Journal of Urology2018,5,4:24
15Prevalence of irritable bowel syndrome in Chinese college and university students assessed using Rome Ⅲ criteria显示文摘AIM:To estimate the prevalence of irritable bowel syndrome(IBS)in college and university students of North China and certain related factors for IBS.METHODS:A total of 2500 students from Shandong University in North China were asked in February-March 2009 to complete questionnaires,including the Rome Ⅲquestionnaire,hospital anxiety and depression scale,and IBS-quality of life questionnaire(IBS-QOL).RESULTS:Among the 2126 students with complete data,the prevalence of IBS was 7.85%according to the RomeⅢcriteria,with a female/male ratio of 1.78:1.Most students had the IBS-constipation subtype (36.5%),followed by IBS-diarrhea subtype(31.1%)and IBS-mixed subtype(23.9%).The students with IBS had a higher anxiety and depression score than those without IBS.Low exercise level and anxiety indicated a high risk for IBS.The mean score of IBS patients was 74.2± 4.242 on the IBS-QOL.CONCLUSION:The prevalence of IBS is 7.85%in Chinese college and university students according to the RomeⅢcriteria.Low exercise level and anxiety may be the risk factors for IBS.Yan-Yan Dong Xiu-Li Zuo Chang-Qing Li Yan-Bo Yu QiuJie Zhao Yan-Qing Li 2010World Journal of Gastroenterology2010,16,33:23
16Prevalence and risk factors of fatty liver disease in Chengdu,Southwest China显示文摘BACKGROUND:Fatty liver disease(FLD)is increasingly recognized as one of the most common chronic liver diseases in China.This study aimed to investigate the prevalence and risk factors of FLD in Chengdu,Southwest China,and to provide a relevant basis for the prevention and intervention of FLD. METHODS:Altogether 9094 subjects(4721 men and 4373 women)of over 18 years old who had received a medical checkup in the West China Hospital of Sichuan University between January and December 2007 were evaluated for FLD.FLD was diagnosed by ultrasonography.Body mass index(BMI),height,body weight,blood pressure,fasting plasma glucose(FPG),triglycerides(TG),total cholesterol (TCh),alanine aminotransferase(ALT),hepatitis B surface antigen(HBsAg),high-density lipoprotein cholesterol (HDL-C),and low-density lipoprotein cholesterol(LDL-C) were measured using routine laboratory methods. RESULTS:The overall prevalence of FLD was 12.5%,which was more than 3-fold higher in males than in females (18.9%vs.5.7%,χ2=359.624,P<0.001).The prevalence increased with age in females and males of less than 50 years.The prevalence of alcoholic,suspected alcoholic,and non-alcoholic FLD was 2.6%,3.6%,and 6.3%,respectively. Multiple logistic regression analyses showed that 10 factors (male sex,age,BMI,FPG,hypertension,TG,TCh,HDL-C, LDL-C,and ALT abnormalities)were closely related to FLD. In heavy drinkers,obesity increased the risk of FLD by 23.78-fold(95%CI,10.22-55.33),but heavy drinking was only associated with a 2-fold(95%CI,1.50-2.66)increased risk in obese subjects.CONCLUSIONS:The prevalence of FLD among a health- checkup population in Chengdu,Southwest China was lower than the published for other areas of China.FLD in Chengdu adults was found to be closely associated with sex, age,BMI,and other metabolic syndrome features.Li, Hong Wang, You-Juan Tan, Ke Zeng, Li Liu, Li Liu, Feng-Jun Zhou, Tao-You Chen, En-Qiang Tang, Hong 2009Hepatobiliary & Pancreatic Diseases International2009,8,4:22
17Prevalence and risk factors of gallbladder polypoid lesions in Chinese petrochemical employees显示文摘AIM:To investigate the prevalence and risk factors of polypoid lesions of the gallbladder (PLGs) in petrochemical employees in Ningbo, Zhejiang Province, China. METHODS:All active and retired employees aged 20-90 years (n = 11098) of a refinery and chemical plant in eastern China were requested to participate in a health survey. The participants were subjected to interview, physical examination, laboratory assessments and ultrasonography. All the participants were invitedto have a physical examination after a face-to-face interview. Fasting blood samples were obtained from the antecubital vein, and the samples were used for the analysis of biochemical values. Abdominal ultrasonography was conducted. RESULTS:A total of 10461 (7331 men and 3130 women) current and former petrochemical employees attended for screening. The overall prevalence of postcholecystectomy, gallstones and PLGs was 0.9%, 5.2% and 7.4%, respectively. Compared with the increased prevalence of either gallstones or post-cholecystectomy in older persons, PLGs were more common in the middle-aged, peaking in those aged 40-59 years. Excluding the patients with gallstones, gallstones mixed with PLGs, or those who had undergone cholecystectomy, in the remaining 9828 participants, the prevalence of PLGs in men (8.9%) was significantly higher than that in women (5.5%, P < 0.001). The analyzed risk factors with increased OR for the development of PLGs were male gender (OR = 1.799, P < 0.001), age ≥ 30 years (OR = 2.699, P < 0.001) and hepatitis B surface antigen (HBsAg) positivity (OR = 1.374, P = 0.006). CONCLUSION:PLGs are not rare among Chinese petrochemical employees. 19Osteoporosis and fractures in liver disease: Relevance, pathogenesis and therapeutic implications显示文摘It is being increasingly recognized that patients with liver disease develop bone loss that can be severe enough to lead to atraumatic fractures and thus markedly diminish life quality and expectancy.The estimated prevalence for liver-related osteoporosis is between20-420/100000 of the general population,and fractures between 60-880/100000.It should be kept in mind that up to 40%of patients with chronic liver disease may experience a fracture.The pathogenic mediators include fibronectin,insulin like growth factor-Ⅰ,and various cytokines,but decreased vitamin D and/or treatment with corticosteroids contribute to worsening bone health.Despite the advances in bone biology that have shed some light on the pathogenesis of this bone loss,treatment options remain nonspecific and tightly linked to treatments of other forms of osteoporosis.Thus,treatment should include calcium and vitamin D supplementation in all patients with chronic liver disease.Therapy with bisphosphonates should be considered,especially in patients receiving corticosteroids.This review focuses on the prevalence of this entity as well as the evidence available with regard to the pathogenesis of bone loss in liver disease,the diagnostic steps required in all patients,and the therapeutic options available.Inaam A Nakchbandi 2014World Journal of Gastroenterology2014,20,28:20
20Meta-analyses on prevalence of selected Parkinson’s nonmotor symptoms before and after diagnosis显示文摘Background:Nonmotor symptoms are common among patients with Parkinson’s disease(PD)and some may precede disease diagnosis.Methods:We conducted a meta-analysis on the prevalence of selected nonmotor symptoms before and after PD diagnosis,using random-effect models.We searched PubMed(1965 through October/November 2012)for the following symptoms:hyposmia,constipation,rapid eye movement sleep behavior disorder,excessive daytime sleepiness,depression,and anxiety.Eligible studies were publications in English with original data on one or more of these symptoms.Results:The search generated 2,373 non-duplicated publications and 332 met the inclusion criteria,mostly(n=320)on symptoms after PD diagnosis.For all symptoms,the prevalence was substantially higher in PD cases than in controls,each affecting over a third of the patients.Hyposmia was the most prevalent(75.5%in cases vs.19.1%in controls),followed by constipation(50%vs.17.7%),anxiety(39.9%vs.19.1%),rapid eye movement sleep behavior disorder(37.0%vs.7.0%),depression(36.6%vs.14.9%),and excessive daytime sleepiness(33.9%vs.10.5%).We observed substantial heterogeneities across studies and meta-regression analyses suggested that several factors might have contributed to this.However,the prevalence estimates were fairly robust in several sensitivity analyses.Only 20 studies had data on any symptoms prior to PD diagnosis,but still the analyses revealed higher prevalence in future PD cases than in controls.Conclusion:These symptoms are common among PD patients both before and after diagnosis.Further studies are needed to understand the natural history of nonmotor symptoms in PD and their etiological and clinical implications.Honglei Chen Edward J Zhao Wen Zhang Yi Lu Rui Liu Xuemei Huang Anna J Ciesielski-Jones Michele A Justice Deborah S Cousins Shyamal Peddada 2015Translational Neurodegeneration2015,4,1:18
