| 1 | Update on ischemia-reperfusion injury in kidney transplantation: Pathogenesis and treatment显示文摘Ischemia/reperfusion injury is an unavoidable relevant consequence after kidney transplantation and influences short term as well as long-term graft outcome. Clinically ischemia/reperfusion injury is associated with delayed graft function, graft rejection, chronic rejection and chronic graft dysfunction. Ischemia/reperfusion affects many regulatory systems at the cellular level as well as in the renal tissue that result in a distinct inflammatory reaction of the kidney graft. Underlying factors of ischemia reperfusion include energy metabolism, cellular changes of the mitochondria and cellular membranes, initiation of different forms of cell death-like apoptosis and necrosis together with a recently discovered mixed form termed necroptosis. Chemokines and cytokines together with other factors promote the inflammatory response leading to activation of the innate immune system as well as the adaptive immune system. If the inflammatory reaction continues within the graft tissue, a progressive interstitial fibrosis develops that impacts long-term graft outcome. | 4 | Hepatocellular carcinoma in non-cirrhotic liver: A comprehensive review显示文摘Hepatocellular carcinoma(HCC) is the most common type of primary liver cancer, which in turns accounts for the sixth most common cancer worldwide.Despite being the 6 th most common cancer it is the second leading cause of cancer related deaths. HCC typically arises in the background of cirrhosis, however,about 20% of cases can develop in a non-cirrhotic liver. This particular subgroup of HCC generally presents at an advanced stage as surveillance is not performed in a non-cirrhotic liver. HCC in non-cirrhotic patients is clinically silent in its early stages because of lack of symptoms and surveillance imaging; and higher hepatic reserve in this population. Interestingly, F3 fibrosis in non-alcoholic fatty liver disease, hepatitis B virus and hepatitis C virus infections are associated with high risk of developing HCC. Even though considerable progress has been made in the management of this entity, there is a dire need for implementation of surveillance strategies in the patient population at risk, to decrease the disease burden at presentation and improve the prognosis of these patients. | 5 | Coupling strategies for silicon photonics integrated chips[Invited]显示文摘Over the last 20 years, silicon photonics has revolutionized the field of integrated optics, providing a novel and powerful platform to build mass-producible optical circuits. One of the most attractive aspects of silicon photonics is its ability to provide extremely small optical components, whose typical dimensions are an order of magnitude smaller than those of optical fiber devices. This dimension difference makes the design of fiberto-chip interfaces challenging and, over the years, has stimulated considerable technical and research efforts in the field. Fiber-to-silicon photonic chip interfaces can be broadly divided into two principle categories:in-plane and out-of-plane couplers. Devices falling into the first category typically offer relatively high coupling efficiency, broad coupling bandwidth(in wavelength), and low polarization dependence but require relatively complex fabrication and assembly procedures that are not directly compatible with wafer-scale testing.Conversely, out-of-plane coupling devices offer lower efficiency, narrower bandwidth, and are usually polarization dependent. However, they are often more compatible with high-volume fabrication and packaging processes and allow for on-wafer access to any part of the optical circuit. In this paper, we review the current state-of-the-art of optical couplers for photonic integrated circuits, aiming to give to the reader a comprehensive and broad view of the field, identifying advantages and disadvantages of each solution. | 6 | Research advances in the detection of miRNA显示文摘MicroRNAs (miRNAs) are a family of endogenous, small (approximately 22 nucleotides in length), noncoding, functional RNAs. With the development of molecular biology, the research of miRNA biological function has attracted significant interest, as abnormal miRNA expression is identified to contribute to serious human diseases such as cancers. Traditional methods for miRNA detection do not meet current demands. In particular, nanomaterial-based methods, nucleic acid amplification-based methods such as rolling circle amplification (RCA), loop-mediated isothermal amplification (LAMP), strand-displacement amplification (SDA) and some enzyme-free amplifications have been employed widely for the highly sensitive detection of miRNA. MiRNA functional research and clinical diagnostics have been accelerated by these new techniques. With the progress of epidemiological research, CKD has brought about huge economic and psychological burdens. There is a considerable risk of cardiovascular events or death than progression to end-stage renal disease for patients. Particular attentions should be paid to the new goals of reducing cardiovascular events and all-cause mortality. It is important to analyze the etiology and pathogenesis according to patients' ages, regions, primary disease as well as different stages of disease, and choose the appropriate therapeutic strategies accordingly. In clinical practice, due to the uncertainty of therapeutic effects ofmodern medicine based on the risk factors, it is necessary to use Chinese medicine(CM) to delay the disease progression and reduce comorbidities. | 10 | Model predictive control and improved low-pass filtering strategies based on wind power fluctuation mitigation显示文摘The rapid development of renewable energy sources such as wind power has brought great challenges to the power grid. Wind power penetration can be improved by using hybrid energy storage(ES) to mitigate wind power fluctuation. We studied the strategy of smoothing wind power fluctuation and the strategy of hybrid ES power distribution. Firstly, an effective control strategy can be extracted by comparing constant-time low-pass filtering(CLF), variable-time low-pass filtering(VLF), wavelet packet decomposition(WPD), empirical mode decomposition(EMD) and model predictive control algorithms with fluctuation rate constraints of the identical grid-connected wind power. Moreover, the mean frequency of ES as the cutoff frequency can be acquired by the Hilbert Huang transform(HHT), and the time constant of filtering algorithm can be obtained. Then, an improved low-pass filtering algorithm(ILFA) is proposed to achieve the power allocation between lithium battery(LB) and supercapacitor(SC), which can overcome the over-charge and over-discharge of ES in the traditional low-pass filtering algorithm(TLFA). In addition, the optimized LB and SC power are further obtained based on the SC priority control strategy combined with the fuzzy control(FC) method. | 11 | Cotton bollworm resistance to Bt transgenic cotton: A case analysis显示文摘Cotton bollworm (Helicoverpa armigera) is one of the most serious insect pests of cotton. Transgenic cotton expressing Cry toxins derived from a soil bacterium, Bacillus thuringiensis (Bt), has been produced to target this pest. Bt cotton has been widely planted around the world, and this has resulted in efficient control of bollworm populations with reduced use of synthetic insecticides. However, evolution of resistance by this pest threatens the continued success of Bt cotton. To date, no field populations of bollworm have evolved significant levels of resistance; however, several laboratory-selected Cry-resistant strains of H. armigera have been obtained, which suggests that bollworm has the capacity to evolve resistance to Bt. The development of resistance to Bt is of great concern, and there is a vast body of research in this area aimed at ensuring the continued success of Bt cotton. | 13 | A combined application of micro-vortex generator and boundary layer suction in a high-load compressor cascade显示文摘In the current study, the effects of a combined application between micro-vortex generator and boundary layer suction on the flow characteristics of a high-load compressor cascade are investigated. The micro-vortex generator with a special configuration and the longitudinal suction slot are adopted. The calculated results show that a reverse flow region, which is considered the main reason for occurring stall at 7.9° incidence, grows and collapses rapidly near the leading edge and leads to two critical points occurring on the end-wall with the increasing incidence in the baseline. As the micro-vortex generator is introduced in the baseline cascade, the corner separation is switched to a trailing edge separation by the thrust from the induced vortex. Meanwhile, the occurrence of failure is delayed due to the mixed low energy fluid and main flow. The synergistic effects between the micro-vortex generator and the boundary layer suction on the performance of the cascade are superior to the baseline at all the incidence conditions before the occurrence of failure, and the sudden deterioration of the cascade occurs at 10.3° incidence. The optimal results show that the farther upstream suction position, the lower total pressure loss of the cascade with vortex generator at the near stall condition. These risks often interact with each other and cannot be dealt with in isolation. As a means to identify, assess, and manage multiple risks, the concept of ‘‘risk governance'' has been developed, which promises to provide integrative and comprehensive tools to deal with the many manifestations of risks.In this article, risk governance concept has been specifically applied to complex risk situations in urban areas. The concept of risk governance pertains to the many ways in which multiple actors, individuals and institutions, public and private, deal with risks. It includes formal institutions and regimes and informal arrangements. | 15 | Reasonable choice of surgical procedures for patients with portal hypertension显示文摘OBJECTIVE: To assess individualized therapeutic protocol for patients with portal hypertension on thebasis of accumulated knowledge about the mechanism of portal hypertension.DATA SOURCES: Patients data on shunt and other surgical procedures from Ruijin Hospital, Shanghai,China and the published papers.RESULTS: The direction of blood flow of the collateral vessels in the gastro-splenic region is animportant factor in deciding surgical strategy because there is a close relationship between surgical riskand the classification of liver function. Clinically it is confirmed that each patient needs an individualizedsurgical procedure and that prophylactic operation is suitable for patients with splenomegaly, splenismassociated with serious esophageal varices and hemorrhagic tendency under endoscopy but acceptableliver function. The shunt diameter (SD) (SD=0.67×PVD) is determined in our patients according toindividualized hemodynamics. The rehemorrhagic rate after shunt being higher than that in others may berelated to lesioned gastric mucosa caused by portal hypertension or bleeding and temporary melena. Thisfinding is good for prevention of hepatic encephalopathy. The life quality and labor ability of patients willbe improved because of hepatopetal flow in the portal vein. With strict indications for reoperation,selective operation is performed as soon as possible when hemorrhage is controlled conservatively andliver function improved. Once the patient with cirrhosis associated with portal hypertension is scheduledfor liver transplantation, treatment of hemorrhage should aim to keep the patient in good condition and toavoid the protocol that may be disadvantageous to liver transplantation in the future.CONCLUSION: Surgical procedures for patients with portal hypertension should follow the principle ofindividualization. | 17 | Climate change and Chinese farmers: Perceptions and determinants of adaptive strategies显示文摘Farmers' perceptions, beliefs, adaptive strategies, and barriers regarding climate change are critical to promoting sustainable ecosystems and societal stability. This paper is based on an extensive survey of 1 500 farmers and their households in Henan Province in China during 2013–2014. Henan is the largest agricultural province in China with over 51 million farmers. The survey results showed that approximately 57% of the respondents perceived the direct impact of climate change during the past 10 years, with 70.3% believing that climate change posed a risk to their livelihood. Not surprisingly, most farmers reported that they have adopted new measures to mitigate the negative impacts of climate change. The main barriers hindering farmers' adopting adaptation measures were lack of funds and timely information. A multinomial logit model revealed that land ownership, knowledge of crop variety and the causes of climate change, as well as the belief of climate change, were all positively related to the likelihood of employing adaptive strategies. Moreover, the percentage of households engaging in agriculture activity, and years of engaging in farming were both negatively correlated with famer's likelihood of adopting adaptation strategies. More importantly, farmers with high incomes were less likely to adopt adaptive strategies and more willing to engage in other business activities. | 18 | Treating comorbid anxiety and depression: Psychosocial and pharmacological approaches显示文摘Comorbid anxiety with depression predicts poor outcomes with a higher percentage of treatment resistance than either disorder occurring alone. Overlap of anxiety and depression complicates diagnosis and renders treatment challenging. A vital step in treatment of such comorbidity is careful and comprehensive diagnostic assessment. We attempt to explain various psychosocial and pharmacological approaches for treatment of comorbid anxiety and depression. For the psychosocial component, we focus only on generalized anxiety disorder based on the following theoretical models:(1) 'the avoidance model';(2) 'the intolerance of uncertainty model';(3) 'the meta-cognitive model';(4) 'the emotion dysregulation model'; and(5) 'the acceptance based model'. For depression, the following theoretical models are explicated:(1) 'the cognitive model';(2) 'the behavioral activation model'; and(3) 'the interpersonal model'. Integration of these approaches is suggested. The treatment of comorbid anxiety and depression necessitates specific psychopharmacological adjustments as compared to treating either condition alone. Serotonin reuptake inhibitors are considered first-line treatment in uncomplicated depression comorbid with a spectrum of anxiety disorders. Short-acting benzodiazepines(BZDs) are an important 'bridging strategy' to address an acute anxiety component. In patients with comorbid substance abuse, avoidance of BZDs is recommended and we advise using an atypical antipsychotic in lieu of BZDs. For mixed anxiety and depression comorbid with bipolar disorder, we recommend augmentation of an antidepressant with either lamotrigine or an atypical agent. Combination and augmentation therapies in the treatment of comorbid conditions vis-à-vis monotherapy may be necessary for positive outcomes. Combination therapy with tricyclic antidepressants, gabapentin and selective serotonin/norepinephrine reuptake inhibitors(e.g., duloxetine) are specifically useful for comorbid chronic pain syndromes. Aripiprazole, quetiapine, risperidone and other novel atypical agents may be effective as augmentations. In this Review, we first introduce the structure,work principle and their relevant efficiency calculations of a PEC cell. We then give a summary of the state-of the-art research in the preparation strategies and growth mechanism for the metal oxide based nanoarrays, and some details about the performances of metal oxide based nanoarray photoanodes for PEC water splitting. | 20 | Extractive distillation: Advances in conceptual design, solvent selection,and separation strategies显示文摘Extractive distillation(ED) is one of the most promising approaches for the separation of the azeotropic or closeboiling mixtures in the chemical industry. The purpose of this paper is to provide a broad overview of the recent development of key aspects in the ED process involving conceptual design, solvent selection, and separation strategies. To obtain the minimum entrainer feed flow rate and reflux ratio for the ED process, the conceptual design of azeotropic mixture separation based on a topological analysis via thermodynamic feasibility insights involving residue curve maps, univolatility lines, and unidistribution curves is presented. The method is applicable to arbitrary multicomponent mixtures and allows direct screening of design alternatives. The determination of a suitable solvent is one of the key steps to ensure an effective and economical ED process. Candidate entrainers can be obtained from heuristics or literature studies while computer aided molecular design(CAMD) has superiority in efficiency and reliability. To achieve optimized extractive distillation systems, a brief review of evaluation method for both entrainer design and selection through CAMD is presented. Extractive distillation can be operated either in continuous extractive distillation(CED) or batch extractive distillation(BED), and both modes have been well-studied depending on the advantages in flexibility and low capital costs. To improve the energy efficiency, several configurations and technological alternatives can be used for both CED and BED depending on strategies and main azeotropic feeds.
