1Achieving the best bowel preparation for colonoscopy显示文摘Bowel preparation is a core issue in colonoscopy,as it is closely related to the quality of the procedure.Patients often find that bowel preparation is the most unpleasant part of the examination.It is widely accepted that the quality of cleansing must be excellent to facilitate detecting neoplastic lesions.In spite of its importance and potential implications,until recently,bowel preparation has not been the subject of much study.The most commonly used agents are high-volume polyethylene glycol(PEG)electrolyte solution and sodium phosphate.There has been some confusion,even in published meta-analyses,regarding which of the two agents provides better cleansing.It is clear now that both PEG and sodium phosphate are effectivewhen administered with proper timing.Consequently,the timing of administration is recognized as one of the central factors to the quality of cleansing.The bowel preparation agent should be administered,at least in part,a few hours in advance of the colonoscopy.Several low volume agents are available,and either new or modified schedules with PEG that usually improve tolerance.Certain adjuvants can also be used to reduce the volume of PEG,or to improve the efficacy of other agents.Other factors apart from the choice of agent can improve the quality of bowel cleansing.For instance,the effect of diet before colonoscopy has not been completely clarified,but an exclusively liquid diet is probably not required,and a low-fiber diet may be preferable because it improves patient satisfaction and the quality of the procedure.Some patients,such as diabetics and persons with heart or kidney disease,require modified procedures and certain precautions.Bowel preparation for pediatric patients is also reviewed here.In such cases,PEG remains the most commonly used agent.As detecting neoplasia is not the main objective with these patients,less intensive preparation may suffice.Special considerations must be made for patients with inflammatory bowel disease,including safety and diagnostic issues,so that the most adequate agent is chosen.Identifying neoplasia is one of the main objectives of colonoscopy with these patients,and the target lesions are often almost invisible with white light endoscopy.Therefore excellent quality preparation is required to find these lesions and to apply advanced methods such as chromoendoscopy.Bowel preparation for patients with lower gastrointestinal bleeding represents a challenge,and the strategies available are also reviewed here.Adolfo Parra-Blanco Alex Ruiz Manuel Alvarez-Lobos Ana Amorós Juan Cristóbal Gana Patricio Ibáez Akiko Ono Takahiro Fujii 2014World Journal of Gastroenterology2014,20,47:37

3Schistosoma mansoni proteins attenuate gastrointestinal motility disturbances during experimental colitis in mice显示文摘AIM:To investigate the therapeutic effect of Schistosoma mansoni(S.mansoni) soluble worm proteins on gastrointestinal motility disturbances during experimental colitis in mice. METHODS:Colitis was induced by intrarectal injection of trinitrobenzene sulphate(TNBS) and 6 h later,mice were treated ip with S.mansoni proteins.Experiments were performed 5 d after TNBS injection.Inflammationwas quantified using validated inflammation parameters. Gastric emptying and geometric center were measured to assess in vivo gastrointestinal motility.Peristaltic activity of distal colonic segments was studied in vitro using a modified Trendelenburg set-up.Cytokine profiles of T-lymphocytes isolated from the colon were determined by real time reverse transcriptase-polymerase chain reaction. RESULTS:Intracolonic injection of TNBS caused severe colitis.Treatment with S.mansoni proteins significantly ameliorated colonic inflammation after 5 d.TNBS did not affect gastric emptying but significantly decreased the geometric center and impaired colonic peristaltic activity 5 d after the induction of colitis.Treatment with S.mansoni proteins ameliorated these in vivo and in vitro motility disturbances.In addition,TNBS injection caused a downregulation of effector T cell cytokines after 5 d,whereas a S.mansoni protein effect was no longer observed at this time point. 4Outcome in obscure gastrointestinal bleeding after capsule endoscopy显示文摘AIM: To investigate the clinical impact of capsule endoscopy(CE) after an obscure gastrointestinal bleeding(OGIB) episode, focusing on diagnostic work-up, followup and predictive factors of rebleeding. METHODS: Patients who were referred to Hospital del Mar(Barcelona, Spain) between 2007 and 2009 for OGIB who underwent a CE were retrospectively analyzed. Demographic data, current treatment with non-steroid antiinflammtory drugs or anticoagulant drugs, hemoglobin levels, transfusion requirements, previous diagnostic tests for the bleeding episode, as well as CE findings(significant or non-significant), work-up and patient out-comes were analyzed from electronic charts. Variables were compared by χ 2 analysis and Student t test. Risk factors of rebleeding were assessed by Log-rank test, Kaplan-Meier curves and Cox regression model. RESULTS: There were 105 patients [45.7% women, median age of 72 years old(interquartile range 56-79)] and a median follow-up of 326 d(interquartile range 123-641) included in this study. The overall diagnostic yield of CE was 58.1%(55.2% and 63.2%, for patients with occult OGIB and overt OGIB, respectively). In 73 patients(69.5%), OGIB was resolved. Multivariate analysis showed that hemoglobin levels lower than 8 g/dL at diagnosis [hazard ratios(HR) = 2.7, 95%CI: 1.9-6.3], patients aged 70 years and above(HR = 2.1, 95%CI: 1.2-6.1) and significant findings in CE(HR = 2.4, 95%CI: 1.1-5.8) were independent predictors of rebleeding. 5Mapping aboveground biomass and its prediction uncertainty using LiDAR and field data, accounting for tree-level allometric and LiDAR model errors显示文摘Background: The increasing availability of remotely sensed data has recently challenged the traditional way of performing forest inventories, and induced an interest in model-based inference. Like traditional design-based inference, model-based inference allows for regional estimates of totals and means, but in addition for wall-to-wall mapping of forest characteristics. Recently Light Detection and Ranging(LiDAR)-based maps of forest attributes have been developed in many countries and been well received by users due to their accurate spatial representation of forest resources. However, the correspondence between such mapping and model-based inference is seldom appreciated. In this study we applied hierarchical model-based inference to produce aboveground biomass maps as well as maps of the corresponding prediction uncertainties with the same spatial resolution. Further, an estimator of mean biomass at regional level, and its uncertainty, was developed to demonstrate how mapping and regional level assessment can be combined within the framework of model-based inference.Results: Through a new version of hierarchical model-based estimation, allowing models to be nonlinear, we accounted for uncertainties in both the individual tree-level biomass models and the models linking plot level biomass predictions with LiDAR metrics. In a 5005 km2 large study area in south-central Sweden the predicted aboveground biomass at the level of 18 m×18 m map units was found to range between 9 and 447 Mg·ha^-1. The corresponding root mean square errors ranged between 10 and 162 Mg·ha^-1. For the entire study region, the mean aboveground biomass was 55 Mg·ha^-1 and the corresponding relative root mean square error 8%. At this level 75%of the mean square error was due to the uncertainty associated with tree-level models.Conclusions: Through the proposed method it is possible to link mapping and estimation within the framework of model-based inference. Uncertainties in both tree-level biomass models and models linking plot level biomass with LiDAR data are accounted for, both for the uncertainty maps and the overall estimates. 8Future technology-enabled care for diabetes and hyperglycemia in the hospital setting显示文摘Patients with diabetes are increasingly common in hospital settings where optimal glycemic control remains challenging.Inpatient technology-enabled support systems are being designed,adapted and evaluated to meet this challenge.Insulin pump use,increasingly common in outpatients,has been shown to be safe among select inpatients.Dedicated pump protocols and provider training are needed to optimize pump use in the hospital.Continuous glucose monitoring(CGM)has been shown to be comparable to usual care for blood glucose surveillance in intensive care unit(ICU)settings but data on cost effectiveness is lacking.CGM use in non-ICU settings remains investigational and patient use of home CGM in inpatient settings is not recommended due to safety concerns.Compared to unstructured insulin prescription,a continuum of effective electronic medical record-based support for insulin prescription exists from passive order sets to clinical decision support to fully automated electronic Glycemic Management Systems.Relative efficacy and cost among these systems remains unanswered.An array of novel platforms are being evaluated to engage patients in technology-enabled diabetes education in the hospital.These hold tremendous promise in affording universal access to hospitalized patients with diabetes to effective self-management education and its attendant short/long term clinical benefits.Alex Renato Montero Jeffrey S Dubin Paul Sack Michelle F Magee 2019World Journal of Diabetes2019,10,9:2

10Imaging pancreatobiliary ductal system with optical coherence tomography: A review显示文摘An accurate, noninvasive and cost-effective method of in situ tissue evaluation during endoscopy would be highly advantageous for the detection of dysplasia or early cancer and for identifying different disease stages. Optical coherence tomography(OCT) is a noninvasive, high-resolution(1-10 μm) emerging optical imaging method with potential for identifying microscopic subsurface features in the pancreatic and biliary ductal system. Tissue microstructure of pancreaticobiliary ductal system has been successfully imaged by inserting an OCT probe through a standard endoscope operative channel. High-resolution OCT images and the technique's endoscopic compatibility have allowed for the microstructural diagnostic of thepancreatobiliary diseases. In this review, we discussed currently available pancreaticobiliary ductal imaging systems to assess the pancreatobiliary tissue microstructure and to evaluate varieties of pancreaticobiliary disorders and diseases. 11Novel biomarkers for patient stratification in colorectal cancer:A review of definitions,emerging concepts,and data显示文摘Colorectal cancer(CRC) treatment has become more personalised,incorporating a combination of the individual patient risk assessment,gene testing,and chemotherapy with surgery for optimal care.The improvement of staging with high-resolution imaging has allowed more selective treatments,optimising survival outcomes.The next step is to identify biomarkers that can inform clinicians of expected prognosis and offer the most beneficial treatment,while reducing unnecessary morbidity for the patient.The search for biomarkers in CRC has been of significant interest,with questions remaining on their impact and applicability.The study of biomarkers can be broadly divided into metabolic,molecular,micro RNA,epithelial-to-mesenchymal-transition(EMT),and imaging classes.Although numerous molecules have claimed to impact prognosis and treatment,their clinical application has been limited.Furthermore,routine testing of prognostic markers with no demonstrable influence on response to treatment is a questionable practice,as it increases cost and can adversely affect expectations of treatment.In this review we focus on recent developments and emerging biomarkers with potential utility for clinical translation in CRC.We examine and critically appraise novel imaging and molecular-based approaches; evaluate the promising array of micro RNAs,analyze metabolic profiles,and highlight key findings for biomarker potential in the EMT pathway.Manish Chand Deborah S Keller Reza Mirnezami Marc Bullock Aneel Bhangu Brendan Moran Paris P Tekkis Gina Brown Alex Mirnezami Mariana Berho 2018World Journal of Gastrointestinal Oncology2018,10,7:2
