| 2 | A new algorithm for the diagnosis of celiac disease显示文摘Celiac disease(CD)affects at least 1%of the Western population but remains largely unrecognized.In our laboratory,we utilize a novel algorithm to diagnose pediatric CD that offers both high sensitivity and high specificity for diagnosis in an outpatient setting.The aim of the present study was to challenge this algorithm and to test its performance in children and adults suspected of having CD.Using a three-assay algorithm,screening with the most sensitive tissue transglutaminase(tTG)complexed with deamidated gliadin peptide neoepitope immunoglobulin A(IgA)1IgG assay and confirming with the two specific tTG IgA and tTG IgA1IgG assays,we examined the serological results from 112 children aged 0–17 years old and 60 adults in comparison to their respective biopsy results.The algorithm performance was calculated by statistical analysis.The use of the new algorithm enabled us to diagnose CD with 98%sensitivity,93%specificity and 95%accuracy in the pediatric group and 94%sensitivity,92%specificity and 93%accuracy in the total population studied.The false-negative cases in the adult group were attributed to previous adherence to a gluten-free diet,and the single false-negative result in a young child became a true positive after 6 months.We have also monitored three celiac patients before and after diagnosis and found that the algorithm may be suitable for disease monitoring.The newly proposed three-assay algorithm for celiac detection is very reliable in both children and adults.Due to the high performance of this assay,the further need for confirmatory intestinal biopsies will be reassessed. | Orit Rozenberg Aaron Lerner Avi Pacht Maya Grinberg Dina Reginashvili Clara Henig Mira Barak | 2011 | Cellular & Molecular Immunology2011,8,2: | 1 |
| 3 | Microbial transglutaminase should be considered as an environmental inducer of celiac disease显示文摘Due to the recent interest in food additives that can act as triggering factors in autoimmune diseases including celiac disease(CD),the present letter to the editor expands on the microbial transglutaminase(mTG).It is heavily consumed by a plethora of food processing industries as“glue of proteins”thus improving product’s stability,texture and shelf life.However,more and more information is accumulated lately,questioning its safety.Its cross-linked gliadin complexes are immunogenic in CD.The enzyme increases gliadin uptake,is transported in a trans-epithelial way and deposited below the enterocyte’s line,has antiphagocytic activity,enhances intestinal permeability and creates luminal resistant isopeptide bonds.No doubt that mTG is beneficial to food industries but a caveat to public health is highly recommended. | Aaron Lerner Torsten Matthias | 2019 | World Journal of Clinical Cases2019,7,22: | 1 |

| 7 | Nutritional deficiencies in the pediatric age group in a multicultural developed country,Israel显示文摘Nutrient deficiencies are prevalent worldwide.Diseases and morbid conditions have been described to result from nutritional deficiencies.It is essential to address nutrient deficiencies as these may lead to chronic longterm health problems such as rickets,iron deficiency anemia,goiter,obesity,coronary heart disease,type2 diabetes,stroke,cancer and osteoporosis.In the present review we surveyed the extent and severity of nutritional deficiencies in Israel through a selective and comprehensive Medline review of previous reports and studies performed during the last 40 years.Israel populations have multiple nutritional deficiencies,including iron,calcium,zinc,folic acid,and vitamins B12,C,D and E,spanning all age groups,several minorities,and specific regions.In Israel,some of the nutrients are mandatorily implemented and many of them are implemented voluntarily by local industries.We suggest ways to prevent and treat the nutritional deficiencies,as a step to promote food fortification in Israel. | Motti Haimi Aaron Lerner | 2014 | World Journal of Clinical Cases2014,2,5: | 0 |
| 8 | Presarcopenia and chronic cough due to chronic obstructive pulmonary disease successfully treated with nutraceutical supplements and physical exercise显示文摘Objective:To describe a patient with presarcopenia and chronic cough secondary to chronic obstructive pulmonary disease(COPD)successfully treated with supplements and physical exercise.Case report:A 75-year-old female patient with a positive past medical history of systemic hypertension,dyslipidemia,heart arrhythmia,and smoking during 20 years evolved with chronic cough due to a chronic obstructive pulmonary disease diagnosed ten years ago.She came to our private clinic due to low weight,low energy.Her weight was 44.8 kg,her height 1.57 m,body mass index of 18.18 kg/m^(2).Laboratory tests showed SDHEA 76.3,vitamin D of 15.6 ng/ml(nr:>30 ng/ml),C-reactive protein(CRP)of 55 mg/ml.Computed tomography showed bronchiectasis.She had an skeletal muscle mass index of 5.0 kg/m^(2)(nr:>5.5 kg/m^(2))by DXA.A diagnosis of presarcopenia was determined based on DXA evaluation with low muscle mass but normal gait speed handgrip strength.We suggested to the patient to increase physical exercise and prescribed a supplement formula.After five months,she returned asymptomatic,without cough,marked improvement of fatigue,increased energy levels,and weight increased to 50 kg,BMI of 20.28 kg/m^(2).Laboratory tests showed SDHEA to 140,vitamin D3 to 64.5 ng/ml,reduced CRP reduced to 5 mg/dl,and amlodipine was excluded due to better blood pressure control.Currently,two years later,the patient continues without cough and has dyspnea only with high efforts,without fatigue,and her weight is 52 kg and BMI 21.1 kg/m^(2).She also reduced her conventional treatment for COPD,using only a bronchodilator on-demand,without topical corticoids.Conclusions:This case illustrates an interesting case of a patient with presarcopenia and chronic cough,refractory to conventional approach,successfully treated with a combination of nutraceuticals and physical exercises. | Jozélio Freire de Carvalho Aaron Lerner | 2021 | Discussion of Clinical Cases2021,8,4: | 0 |
